Effect of Mannitol plus Vitamins B in the management of patients with piriformis syndrome.

BACKGROUND: Piriformis syndrome (PS) is an entrapment of the sciatic nerve by the piriformis muscle, or myofascial pain from the piriformis muscle. OBJECTIVE: The aim of this study was to investigate the effectiveness of Mannitol plus Vitamins B regime in the management of PS. METHODS: Twenty two patients were included in this study and received 250 ml of mannitol 20% intravenous infusion for 5 days + Vitamins B (vitamin B1 10 mg + vitamin B2 10 mg + vitamin B12 50 μg PO) for 6 weeks. Clinical outcomes were assessed systematically by clinical tests (tenderness, FAIR test, Beatty's, Freiberg'

Sciatica of nondisc origin and piriformis syndrome: diagnosis by magnetic resonance neurography and interventional magnetic resonance imaging with outcome study of resulting treatment.

OBJECT: Because lumbar magnetic resonance (MR) imaging fails to identify a treatable cause of chronic sciatica in nearly 1 million patients annually, the authors conducted MR neurography and interventional MR imaging in 239 consecutive patients with sciatica in whom standard diagnosis and treatment failed to effect improvement. METHODS: After performing MR neurography and interventional MR imaging, the final rediagnoses included the following: piriformis syndrome (67.8%), distal foraminal nerve root entrapment (6%), ischial tunnel syndrome (4.7%), discogenic pain with referred leg pain (3.4%)

Diagnosis and Management of Piriformis Syndrome: A Rare Anatomic Variant Analyzed by Magnetic Resonance Imaging.

Piriformis syndrome is an uncommon condition that causes significant pain in the posterior lower buttocks and leg due to entrapment of the sciatic nerve at the level of the piriformis muscle. In the typical anatomical presentation, the sciatic nerve exits directly ventral and inferior to the piriformis muscle and continues down the posterior leg. Several causes that have been linked to this condition include trauma, differences in leg length, hip arthroplasty, inflammation, neoplastic mass effect, and anatomic variations. A female presented with left-sided lower back and buttock pain with radi

Bilateral piriformis syndrome in two elite soccer players: Report of two cases.

Piriformis syndrome, a relatively rare condition, is described as entrapment of a sciatic nerve at the level of the piriformis muscle. There have been a few reports of bilateral piriformis syndrome in literature. In this study, we present bilateral piriformis syndrome in two professional soccer players from different teams who are symptom free at last follow-up after surgery. In both patients, resting EMG records were read normal, however EMG recording during the activity revealed prolonged H-reflexes. Both patients had no relief from conservative treatment and rehabilitation, therefore surgic

Piriformis syndrome: a cause of nondiscogenic sciatica.

Piriformis syndrome is a nondiscogenic cause of sciatica from compression of the sciatic nerve through or around the piriformis muscle. Patients typically have sciatica, buttocks pain, and worse pain with sitting. They usually have normal neurological examination results and negative straight leg raising test results. Flexion, adduction, and internal rotation of the hip, Freiberg sign, Pace sign, and direct palpation of the piriformis cause pain and may reproduce symptoms. Imaging and neurodiagnostic studies are typically normal and are used to rule out other etiologies for sciatica. Conservat

Piriformis Syndrome and Wallet Neuritis: Are They the Same?

Piriformis syndrome (PS) involves the piriformis muscle and adjacent sciatic nerve, producing features similar to true sciatica of lumbar spine origin, for example, lumbar disc prolapse, thereby confusing pain physicians about the diagnosis. Synonyms that are used frequently for PS are deep gluteal syndrome, extra-spinal sciatica, wallet neuritis, etc. Commonly presenting symptoms of PS include gluteal pain which increases with affected side sitting/per-rectal manoeuvre, and improves with ambulation/intra-lesional steroid, etc. Among various PS associations, wallet neuritis is one of them. Prevalence and Clinical Patterns of Piriformis Syndrome Among Actively Competing and Retired Elite Hockey Players.

Piriformis syndrome, a neuromuscular disorder caused by sciatic nerve compression by the piriformis muscle, remains understudied in athletic populations despite anecdotal reports of elevated prevalence in hockey players. This study investigated the prevalence of piriformis syndrome symptoms and potential risk factors in actively competing (current) and retired (former) high-level hockey players. A cross-sectional survey was conducted among 67 actively competing and retired professional, collegiate, and junior hockey players (58 males, 9 females; mean age 25.6 ± 4.0 years; mean playing experien

Sciatic nerve palsy caused by haematoma from iliac bone graft donor site.

A 27-year-old woman developed a haematoma-related compressive neuropathy of the right sciatic nerve following the harvesting of a bone graft from the posterior aspect of the right iliac crest. The nerve was compressed in an enclosed compartment between the sciatic notch and the piriformis muscle, which is thought to be involved in the piriformis syndrome. The symptoms improved once the haematoma was evacuated, and this complication stresses the importance of adequate haemostasis of the area from which the bone graft is taken.

Report of a Novel Bilateral Variation of Sciatic and Inferior Gluteal Nerve: A Case Study.

INTRODUCTION: The sciatic nerve is the thickest nerve of the sacral plexus which innervates many muscles and vast areas of the skin of the lower limb. It leaves the pelvis via the greater sciatic foramen, emerges into the gluteal region by passing under the piriformis muscle, and descends beneath the gluteus maximus to divide into its terminal branches; the tibial and common peroneal nerve at the superior angle of the popliteal fossa. In some cases, the sciatic nerve divides into the tibial and common peroneal nerves at a higher level and one of them or both passes through or over the piriform

Confirmation of needle placement within the piriformis muscle of a cadaveric specimen using anatomic landmarks and fluoroscopic guidance.

Of patients presenting to pain clinics, complaints are of low back or buttock pain with or without radicular leg symptoms is one of the most common. Piriformis syndrome may be a contributor in up to 8% of these patients. The mainstay of treatment is conservative management with physical therapy, anti-inflammatory medications, muscle relaxants, and correction of biomechanical abnormalities. However, in recalcitrant cases, a piriformis injection of anesthetic and/or corticosteroids may be considered. Because of its small size, proximity to neurovascular structures, and deep location, the pirifor

Piriformis syndrome: a simple technique for US-guided infiltration of the perisciatic nerve. Preliminary results.

The piriformis syndrome is an uncommon cause of sciatica, buttock or thigh pain. Because of the deep location of the muscle and sciatic nerve, infiltration has traditionally been guided by electromyography, fluoroscopy, computed tomography or magnetic resonance imaging. The aim of the present study is to describe a simple technique for ultrasound (US)-guided perisciatic infiltration of corticosteroids and anaesthetic using the inferior gluteal artery as a landmark. This technique was used satisfactorily in ten patients. Although the series in the study is limited, the procedure for US-guided i

Little needle-scalpel for piriformis syndrome: A protocol for systematic review and meta-analysis.

BACKGROUND: Piriformis syndrome (PS) is a condition in which the sciatic nerve is compressed when passing through the inferior mouth of the piriformis muscle, mainly caused by pain in one hip and leg. In severe cases, patients may experience severe buttock and lower limb pain, discomfort, difficulty walking, and claudication. It is estimated that the annual incidence of low back pain and sciatica is about 40 million cases, and the annual incidence of piriformis syndrome is about 2.4 million cases. The aim of this systematic review is to assess the effectiveness and safety of Little needle-scal

The internal obturator muscle may cause sciatic pain.

Six patients suspected to have piriformis syndrome were operated in the hip region in an attempt to relieve pressure on the sciatic nerve. The piriformis muscle and tendon as well as their relationship to the sciatic nerve were found to be normal. However, the internal obturator muscle was found to be very tense, slightly hyperaemic and pressing the sciatic nerve. During Lasegue's testing on the operating table the internal obturator and not the piriformis muscle impinged on the nerve at an early stage in the hip flexion movement. A sectioning of the tendon to the internal obturator muscle nea
