Hypopituitarism is a major manifestation of pituitary adenoma apoplexy. We postulated that the acute deterioration in pituitary function may be caused by compression of portal vessels and the pituitary stalk, secondary to a sudden increase in intrasellar contents. If this were the case, one would predict improvement in pituitary function to occur after surgical decompression. We studied pituitary function in eight patients presenting with the clinical syndrome of pituitary adenoma apoplexy before and on multiple occasions after urgent surgical decompression. Partial or complete hypopituitarism

A young woman developed intermittent headaches and progressive hyperpigmentation after bilateral adrenalectomy for Cushing's disease. Results of sellar polytomography were abnormal. Her plasma ACTH levels increased to 4750-7340 pg/ml and did not rise with insulin-induced hypoglycemia. Although she experienced no clinical features associated with spontaneous infarction of a pituitary tumor, plasma ACTH levels fell to 474-575 pg/ml, and hemorrhagic necrosis was found in a 5-mm chromophobe adenoma at transsphenoidal surgery. Postoperatively, ACTH levels returned to normal (51-88 pg/ml), with the

Pituitary tumor apoplexy is one of a few life-threatening conditions in patients with pituitary adenomas. The very low incidence of this complication hinders formulation of widespread guidelines on diagnostic and therapeutic management. This article presents current opinions on epidemiology, risk factors, clinical signs and symptoms, also the difficulties in selecting appropriate therapeutic management. The paper has focused particularly on making decisions regarding surgical or conservative treatment in pituitary tumor apoplexy. Also the consequences of these decisions based on the key litera

Melanoma metastases to the pituitary gland are extremely rare, with only a few reported cases. We report an unusual case of pituitary metastatic melanoma in which the patient presented with pituitary apoplexy. A 68-year-old man presented general fatigue and anorexia following sudden headache. Neurological examination disclosed bitemporal hemianopsia. Computed tomography (CT) scans revealed a suprasellar mass including intratumoral hematoma. Magnetic resonance (MR) images demonstrated a circumscribed mass lesion in the intra- and suprasellar regions, compressing the optic chiasm. Surgical explo

BACKGROUND: Rathke cleft cyst (RCC) can cause acute symptoms mimicking pituitary adenoma (PA) apoplexy. We evaluated the clinicoradiologic features for distinguishing RCC from PA apoplexy.
METHODS: We retrospectively evaluated 22 patients with RCC and 24 patients with PA with apoplexy-like symptoms who underwent surgery via a transsphenoidal approach between November 1999 and December 2016. We compared the clinical data and magnetic resonance (MR) images between the 2 groups.
RESULTS: The RCC group was younger and had smaller tumors compared with the PA group (P = 0.02 and 0.001, respectivel

Pituitary apoplexy is a rare and potentially life-threatening condition resulting from a sudden hemorrhage or infarction of the pituitary gland, often within a pre-existing adenoma. This report describes the case of a 45-year-old male patient who presented with an acute onset of a severe headache, visual disturbances, and cranial nerve palsy. Laboratory investigations revealed hypopituitarism with adrenal insufficiency, and magnetic resonance imaging confirmed a hemorrhagic pituitary macroadenoma compressing the optic chiasm. Prompt initiation of corticosteroid therapy and urgent transsphenoid

Pituitary metastasis (PM) is an uncommon manifestation of systemic malignant tumours. It is the least common site of intracranial metastases. As PM has no clinical or radiological pathognomonic features, their diagnosis is challenging. Herein, we present a rare case of a PM unveiling lung cancer. A 60-year-old male with no medical history of malignancy was admitted with a sudden headache, retro-orbital pain, and a severe loss of both eyes' visual acuity. After proper investigations and endoscopic resection of the sellar mass, the diagnosis was confirmed to be pituitary metastasis of lung carci

AIMS AND METHODS: Prolactinomas are a common cause of sexual dysfunction and infertility. We aimed, through this case report, to illustrate the difficulties of management of women with giant prolactinoma, especially in cases of desire of pregnancy.
RESULTS: A 30-year-old woman was referred to our department for secondary amenorrhea. Investigations showed a prolactin level of 5168 ng/mL and giant pituitary adenoma of 4 cm in diameter. Cytoreductive surgery was performed after failure to normalize prolactin levels during three years with medical treatment by cabergoline. After seven months, men

OBJECTIVE: To describe an unusual presentation of a patient with recurrent pituitary apoplexy of an adenoma that switched phenotypes from a nonfunctioning, or silent gonadotroph adenoma (SGA), to a silent corticotroph adenoma (SCA). We discuss the potential etiologies of both recurrent pituitary apoplexy and phenotype switching of pituitary tumors.
METHODS: The presented case includes clinical and biochemical findings, surgical outcomes, and pathologic reports related to the treatment of our patient who presented with recurrent pituitary apoplexy.
RESULTS: A 56-year-old man presented for eva

Pituitary apoplexy is a rare clinical presentation caused by infarction of the pituitary gland or adenoma with or without hemorrhage. Although pituitary apoplexy is usually spontaneous, one of the predisposing factors is treatment with dopamine agonists, especially bromocriptine. The occurrence of apoplexy during cabergoline therapy is reported much less frequently. In this article, we report a 34-year-old man with macroprolactinomas who developed sudden visual deterioration due to pituitary apoplexy 1 year after initiation of cabergoline therapy. He was treated via endoscopic trans-sphenoidal

Pituitary apoplexy (PA) is a rare and potentially fatal clinical condition presenting acute headache, vomiting, visual impairment, ophthalmoplegia, altered mental state and possible panhypopituitarism. It mostly occurs in patients with haemorrhagic infarction of the pituitary gland due to a pre-existing macroadenoma. Although there are pathological and physiological conditions that may share similar imaging characteristics, both clinical and imaging features can guide the radiologist towards the correct diagnosis, especially using magnetic resonance imaging (MRI). In this review, we will descr

PURPOSE: Pituitary apoplexy can be a life threatening and vision compromising event. Antiplatelet and anticoagulation use has been reported as a contributing factor in pituitary apoplexy (PA). Utilizing one of the largest cohorts in the literature, this study aims to determine the risk of PA in patients on antiplatelet/anticoagulation (AP/AC) therapy.
METHODS: A single center, retrospective study was conducted on 342 pituitary adenoma patients, of which 77 patients presented with PA (23%). Several potential risk factors for PA were assessed, including: patient demographics, tumor characterist

BACKGROUND: Pituitary apoplexy is rare clinical entity resulting from hemorrhage or infarction of a pituitary tumor, typically presenting with headaches, endocrinopathy, and variable neurological manifestations. Recurrent apoplexy, particularly multiply recurrent cases, is exceedingly rare and associated with clinical equipoise in many cases.
OBSERVATIONS: The authors report the case of an 85-year-old female who presented with three distinct episodes of pituitary apoplexy over 6 years, each presenting with acute headache in the absence of neurological deficits. Given the patient's age, lack o

BACKGROUND: Pituitary apoplexy is a rare, life-threatening complication of pituitary macroadenomas which is often triggered by physiological stressors. It typically occurs spontaneously or may be precipitated by surgical procedures. However, occurrence after cataract surgery is exceedingly rare.
CASE PRESENTATION: A 61-year-old Pakistani hypertensive male presented with sudden-onset severe headache and projectile vomiting, 3 day post-cataract surgery. Imaging revealed a hemorrhagic pituitary macroadenoma consistent with apoplexy. Neurologic examination was normal. The patient was managed cons
