Mid-trimester maternal serum AFP levels in predicting adverse pregnancy outcome.

OBJECTIVE: In this prospective study, we investigated the association between mid-trimester maternal serum alpha-fetoprotein AFP (MSAFP) levels and adverse pregnancy outcome in a South-Western Greek population. MATERIALS AND METHODS: 110 healthy Greek women with spontaneous pregnancies, investigated for MSAFP levels between the 13th and 24th week of gestation and followed for adverse pregnancy outcome. AFP levels > 2.0 multiples of the median value for gestation were considered abnormal. Statistical analysis was performed by Pearson's chi-square test. RESULTS: Elevated MSAFP levels were dete

Thyroid antibody positivity in pregnancy is associated with altered reproductive hormone concentrations and placental growth in Lewis rats.

Thyroid antibody positivity (TAb+) is common among reproductive-aged women, and TAb+ in pregnancy is associated with complications including miscarriage and gestational diabetes. These associations persist in women with normal thyroid status, but there are minimal studies examining the underlying mechanisms. This study was designed to investigate the effects of TAb+ on maternal, placental and fetal health, with a focus on metabolism, using a Lewis rat model of TAb+ induced before pregnancy. Aspects of maternal fertility, cardiovascular adaptations and placental function were also assessed. Oes

Choosing placental hypoxic-ischemic measures that have clinical implications in child development and diseases.

PURPOSE: Placental hypoxic-ischemic pathology is one of the common causes for adverse outcomes. But there is no commonly accepted evaluation system on specific morphological and histopathological measures of the placenta. OBJECTIVE: This study aims to systematically select several core placental hypoxic-ischemic measures that have a high prognostic relevance to child health. METHODS: We used data from the Collaborative Perinatal Project, a multicenter prospective cohort study that recruited over 55,000 pregnant women and followed their offspring to 7 years old. Women who had information on p

Value of multimodal ultrasound assessment of placental dysfunction in gestational diabetes mellitus: a prospective study.

BACKGROUND: Gestational diabetes mellitus (GDM) induces progressive placental structural and functional abnormalities that are often undetectable by conventional ultrasound. Accurately assessing these changes is crucial for preventing adverse perinatal outcomes. This study aimed to evaluate the diagnostic value of multimodal ultrasound in assessing placental structural and functional alterations in pregnancies complicated by GDM, and to identify reliable sonographic biomarkers for evaluating placental abnormalities and clinical risk stratification. METHODS: In this prospective study, 177 preg

Further investigation on the predictive value of human placental lactogen in high-risk pregnancies.

Blood samples were taken from 254 women with pregnancies with various complications and 119 completely normal pregnant women for measurement of serum human placental lactogen (hPL) during the third trimester. The value of this test in the management of these pregnancies was retrospectively evaluated through details of outcome. Serum hPL and urinary estriol were compared as tools for assessment of fetal condition. Serum hPL was found to be very efficient in the prenatal diagnosis of intrauterine growth retardation associated with maternal hypertension. Low hPL levels were recorded in all severe

Microparasites and Placental Invasiveness in Eutherian Mammals.

Placental invasiveness-the number of maternal tissue layers separating fetal tissues from maternal blood-is variable across mammalian species. Although this diversity is likely to be functionally important, variation in placental invasiveness remains unexplained. Here we test the hypothesis that increased risk of transplacental transmission of pathogens from the mother to the fetus promotes the evolution of non-invasive placentation, the most likely derived condition in eutherian mammals. Specifically, we predict that non-invasive placentation is associated with increased microparasite species

No correlation between ultrasound placental grading at 31-34 weeks of gestation and a surrogate estimate of organ function at term obtained by stereological analysis.

We test the experimental hypothesis that early changes in the ultrasound appearance of the placenta reflect poor or reduced placental function. The sonographic (Grannum) grade of placental maturity was compared to placental function as expressed by the morphometric oxygen diffusive conductance of the villous membrane. Ultrasonography was used to assess the Grannum grade of 32 placentas at 31-34 weeks of gestation. Indications for the scans included a history of previous fetal abnormalities, previous fetal growth problems or suspicion of IUGR. Placentas were classified from grade 0 (most immatu

[Endocrino-pharmacological study of reproduction: Role and biosynthesis of steroid hormones in the feto-placental unit].

Although considerable information is available concerning steroidogenesis in the human fetus, the function of the different steroids formed during pregnancy and the factors regulating this delicate hormones balance are poorly understood. During human pregnancy, the placenta synthesizes large quantities of progesterone, estradiol, estrone and estriol and secretes these hormones into both the maternal and fetal circulations; progesterone from maternal lipoprotein-cholesterol, estradiol and estrone from maternal and fetal dehydroepiandrosterone sulfate (DHAS), and estriol largely from fetal 16 al

[Antepartum monitoring and peripartal findings in mono- and dichorial twin pregnancy].

In a group of 65 twin pregnancies the difference of perinatal findings and antepartal test results was evaluated in relation to amnionicity and chorionicity. Monochorionic placentation was found in 33% of the pregnancies. The rate of foetal malformation (11%), neuromuscular dysfunction (6%), perinatal mortality (11%) and duration of neonatal intensive care was increased in those cases. The most useful diagnostic tool was B-Mode-ultrasound (first detection and surveillance of multiple pregnancy, especially diagnosis of inter-twin growth discordancy). Non stress test and Doppler sonography were

[Feto-placental system in diabetes mellitus and hydramnios].

The fetoplacental system of 132 women with diabetes mellitus and 85 healthy women was studied during the third pregnancy trimester. Blood estradiol and urinary estriol levels were found-reduced in the diabetics, particularly in those with insulin-dependent diabetes. Blood progesterone and oxytocinase levels were found unchanged. These parameters lowered in diabetic glomerulosclerosis. If pregnant diabetics develop hydramnion, their blood estradiol, progesterone, prolactin, and oxytocinase levels increased.

Placental and Doppler ultrasound findings in pregnant women with SARS-CoV-2 infection.

BACKGROUND: Several viral infections cause changes in the placenta. Cytomegalovirus, herpes viruses, and HIV cause increased placental thickness; Zika virus induces focal regions of necrosis; parvovirus B19 causes a structural injury. Umbilical flow can be considered a direct measurement of vascular placental function. OBJECTIVE: This study aimed to compare placental ultrasound and umbilical Doppler findings in pregnant women who tested positive or negative for SARS-CoV-2. Our work aimed to confirm the suspicion of placental infection and the consequence in fetal physiopathology. STUDY DESIG

Evaluating the association between placenta DNA methylation and cognitive functions in the offspring.

The placenta plays a crucial role in protecting the fetus from environmental harm and supports the development of its brain. In fact, compromised placental function could predispose an individual to neurodevelopmental disorders. Placental epigenetic modifications, including DNA methylation, could be considered a proxy of placental function and thus plausible mediators of the association between intrauterine environmental exposures and genetics, and childhood and adult mental health. Although neurodevelopmental disorders such as autism spectrum disorder have been investigated in relation to pla

Circulating placental small extracellular vesicles miR-2110 depletion drives maternal-fetal endothelial injury in preeclampsia.

BACKGROUND: Preeclampsia (PE) is a leading cause of maternal and perinatal mortality. Placental dysfunction drives the onset of the condition through inadequate spiral artery remodeling and ischemia-hypoxia, triggering endothelial cell injury mediated by small extracellular vesicles (sEVs), which can increase long-term cardiovascular risk in both mothers and offspring. However, the mechanisms underlying this process remain unclear and unpredictable. Although placental sEVs carry dysfunctional miRNAs associated with endothelial injury, traditional methods of separation from plasma/explantations

Effects of placentation on selected Type A behaviors in adult males in the National Heart, Lung, and Blood Institute (NHLBI) twin study.

A dermatoglyphic index derived from monozygotic (MZ) twins of known placental type was used to estimate placentation retrospectively in a sample of adult male MZ twins. Examination of behavioral test scores with respect to placentation showed that the within-pair difference of most measures of type A behavior was smaller in presumed monochorionic than presumed dichorionic pairs. Measures of cognitive function and hostility were not related to placental type. Intraclass correlations in the monochorionic subgroup of MZ twins were greater than the correlations reported for the full sample of MZ t

Patterns of placental injury. Correlations with gestational age, placental weight, and clinical diagnoses.

Given the dependence of the fetus on adequate placental function, one might predict that specific placental lesions would be associated with distinct pregnancy outcomes. To test this hypothesis, we examined 609 nontwin placentas and explored the relationship between histologic diagnosis and the following variables: gestational age at delivery, placental size, and selected categories of clinical disease. Five separate patterns of placental injury were examined. Two lesions were particularly frequent prior to 37 weeks' gestation: increased intervillous fibrin, which was most common in early pret

Placental development in a mouse model of spinal muscular atrophy.

Spinal Muscular Atrophy (SMA) is an autosomal recessive disorder, leading to fatal loss of motor neurons. It is caused by loss of function of the SMN gene, which is expressed throughout the body, and there is increasing evidence of dysfunction in non-neuronal tissues. Birthweight is one of most powerful prognostic factors for infants born with SMA, and intrauterine growth restriction is common. In the SMNΔ7 mouse model of SMA, pups with the disease lived 25% longer when their mothers were fed a higher fat, "breeder" diet. The placenta is responsible for transport of nutrients from mother to fe

Disparity Between Functional and Structural Recovery of Placental Mitochondria After Exposure to Hypoxia.

Intrauterine growth restriction (IUGR) affects 5-10% of pregnancies with placental hypoxia, playing a key role as a common pathophysiological pathway of different etiologies. Despite the high metabolic rate of the placenta and its "gatekeeper" role in protecting the fetus from hypoxia, the response of placental mitochondria to hypoxic stress is not well understood. This study tested the hypothesis that transient exposure to hypoxia leads to a loss of placental mitochondria and affects their function. Human villous trophoblastic (JEG-3) cells were cultured under normoxic and hypoxic conditions

Pregnancy monitoring in mares: Ultrasonographic and endocrine approaches.

Methods to diagnose and monitor equine pregnancy continue to advance with improved instrumentation enabling the development of novel, non-invasive approaches to assess fetal well-being and viability using ultrasound and endocrine testing. From early embryonic loss to placentitis, that is typically encountered later in gestation, fetal viability and development as well as placental function can be evaluated using two fundamentally different, structural and functional, approaches. Ultrasound provides structural information on embryonic and fetal growth using such parameters as combined thickness

Effect of morphological and functional changes in the secundines on biometric parameters of newborns from dichorionic twin pregnancies.

OBJECTIVES: The aim of the study was to determine if, and to what extent, structural and functional changes of the secundines influence biometric parameters of neonates from dichorionic twin pregnancies. MATERIAL AND METHODS: The study included neonates from dichorionic, diamniotic twin pregnancies, along with their secundines. Based on histopathological examination of the secundines, the mass and dimensions of the placenta, length and condition of the umbilical cord, chorionicity, focal lesions, and microscopic placental abnormalities were determined for 445 pairs of twins. Placental T2* estimated by magnetic resonance imaging and fetal weight estimated by ultrasound in the prediction of birthweight differences in dichorionic twin pairs.

INTRODUCTION: Intertwin birthweight (BW) difference is associated with an increased risk of adverse outcome. Ultrasound estimated fetal weight (EFW) is the current method to predict intertwin BW difference, however, the sensitivity is poor. Therefore, new methods are needed. Placental T2* estimated by magnetic resonance imaging (MRI) provides non-invasive information about the placental function. This study aimed to investigate placental T2* difference as a new predictor of BW difference, and to compare it to the EFW. METHODS: We included 25 dichorionic twin pairs at 19-38 weeks' gestation.
