Ophthalmologic findings in patients with nonsyndromic plagiocephaly.

The purpose of this article was to study the prevalence of strabismus and astigmatism in children with nonsyndromic plagiocephaly. Children with deformational plagiocephaly do not have an increased prevalence of strabismus compared with the general population but do have an increased prevalence of astigmatism, whereas children with nonsyndromic craniosynostotic plagiocephaly have an increased prevalence of strabismus and astigmatism. This observational cohort study retrospectively reviewed patients in a multidisciplinary craniofacial clinic between 1993 and 2001. Patients were referred for eva

Study of asymmetries of the cranial vault in plagiocephaly.

Organic plagiocephaly is caused by premature synostosis of the coronal suture. The subject is dealt with in the context of a desire to categorize these types of asymmetries and to find asymmetry indices using computed tomography scan images. From a cephalometric point of view, the authors looked for a reproducible reference, independent of the structures to be studied-the vestibular orientation, which is based on the definition of a mediolabyrinthic plane, making it possible to assess the symmetry. One of the problems encountered in this study of the cranial vault is related to the difficulty

An assistive device for the treatment of positional plagiocephaly.

An assistive device (AD) was used to treat 122 infants with moderate-to-severe positional plagiocephaly. Clinical evaluations indicate that the AD provided the most benefit when applied to 2- to 8-month-old children. Our results suggest that the AD may join molding helmets and physical therapy as a treatment for moderate-to-severe positional plagiocephaly in infants under the age of 1 year.

Occipital plagiocephaly: a critical review of the literature.

OBJECT: The literature on occipital plagiocephaly (OP) was critically reviewed to determine the feasibility of establishing treatment recommendations. METHODS: Using standard computerized search techniques, medical literature databases containing peer-review articles dating from 1966 were queried for key words related to OP. The titles of all articles were scanned for relevance, and copies of potentially relevant articles published in English were reviewed. Articles in which treatment was discussed were categorized according to their weight of evidence as Class I (prospective randomized contr

Refractive error and ocular motility in plagiocephaly.

Plagiocephaly, which is caused by premature closure of one of the coronal sutures, leads to fronto-orbital asymmetry. The aim of this work was to study the repercussions of orbital deformation on the visual system. Twenty-one patients presenting with plagiocephaly at birth and operated on by the same craniofacial technique (bilateral approach with translation and advancement of the entire involved orbits) were included in the study. All of the patients were examined by clinical anthropometry with three-dimensional reconstruction and underwent complete eye examination by the same observer. Foll

Craniofacial growth in plagiocephaly and Crouzon syndrome.

Very little information is available in the literature regarding the morphology and growth of the cranial base, the orbits and the jaws, in patients with premature craniosynostosis. The present report deals with the craniofacial morphology and growth in plagiocephaly and Crouzon syndrome. For each anomaly a dry skull was compared with a clinical case followed longitudinally with roentgencephalometry. Metallic implants in the jaws were employed in one case. Cranial base involvement was demonstrated for both types of premature craniosynostosis. In Crouzon syndrome the cartilagenous cranial base

Positional posterior plagiocephaly: a single-center review.

OBJECTIVE: The authors sought to assess the prevalence and severity of positional posterior plagiocephaly (PPP) in the pediatric population at a tertiary care center. METHODS: The authors conducted a retrospective review of 1429 consecutive patients aged 2 months to 18 years who presented with head trauma and a negative CT scan in 2018. The cohort was stratified by age. The cranial vault asymmetry index (CVAI) was calculated at the superior orbital rim. Asymmetry was categorized according to the following CVAI scores: mild (3.5%-7%), moderate (7%-12%), and severe (> 12%). Patients were groupe

Unicoronal synostotic plagiocephaly: surgical correction: Lille's technique.

INTRODUCTION: For 35 years, we have a tight neuro-plastic surgical cooperation for the surgical correction and long-term systematic follow-up of 125 cases of unicoronal synostotic plagiocephaly. METHODS: We have tried to understand why some patients had kept an asymmetrical facial growth pattern in spite of a good fronto orbital correction. Analysis in vestibular orientation which was available from 1993 has demonstrated a discrepancy between the ocular and the vestibular verticality referential system. So we have designed a surgical procedure to try to fix that problem. RESULTS AND CONCLUSI

Postoperative cognitive outcome for synostotic frontal plagiocephaly.

OBJECT: The goals of this study were to evaluate the cognitive outcome after surgery for synostotic frontal plagiocephaly and to compare the effects of early (< 1 year of age) and late (> or = 1 year of age) surgical correction on intellectual development. METHODS: In a prospective study involving 220 patients, both preoperative and postoperative cognitive function were measured using developmental quotient (DQ) or intelligence quotient (IQ) determination. Preoperative intracranial pressure (ICP) was also measured, and the presence of the fibroblast growth factor receptor 3 (FGFR3) P250R muta

Visual field defects in deformational posterior plagiocephaly.

PURPOSE: We sought to determine whether visual field abnormalities occur in infants with deformational posterior plagiocephaly and to assess whether there is a relationship between the severity and laterality of visual field abnormalities with the severity and laterality of skull deformity. METHODS: A retrospective chart review was performed on 40 consecutive infants with deformational posterior plagiocephaly. Each was tested with standardized binocular arc perimetry in the horizontal plane. Sixteen patients also had 3-dimensional computed tomography of the skull. Hemifield asymmetry of >/=20

Analysis of posterior plagiocephaly: deformational versus synostotic.

Appropriate management of posterior plagiocephaly requires differentiation of occipitoparietal flattening caused by lambdoid synostosis from that caused by deformational forces. In a 2 1/2-year prospective study of 115 infants presenting with unilateral posterior cranial flattening, only one child had synostotic posterior plagiocephaly (lambdoid synostosis), whereas 114 infants had deformational posterior plagiocephaly. Deformational occipitoparietal flattening was more common on the right (61 percent) than on the left (30 percent), and minor contralateral frontal flattening was not unusual (5

Plagiocephaly: differential diagnosis based on endocranial morphology.

Plagiocephaly is a descriptive term that connotes an asymmetrically oblique or twisted head. Such cranial dysmorphology has a number of etiologies, the most common of which are unicoronal synostosis, unilambdoid synostosis, and plagiocephaly without synostosis. Use of the term plagiocephaly in the literature is often ambiguous in that at times it is used inclusively for all etiologies while at other times it is used exclusively as a synonym for unicoronal synostosis. Although differentiation by physical examination among unicoronal synostosis, unilambdoid synostosis, and plagiocephaly without

[Plagiocephaly. Craniofacial asymmetry with variable curling-up].

The plagiocephaly syndrome is an asymmetrical cranial deformity due to premature synostosis of a coronal hemi-suture. The predominant alteration is at the spheno-frontal suture. The classification is attempted between the isolated plagiocephaly (P) an plagiocephaly with hypertelorism, on one side, that are to be distinguished from complex asymmetrical cranio-facial deformities. The frontal P. are differentiated from occipital P (pachycephaly) and from mixed hemi-cranial P. The deformity affects the fronto-orbital region situated anterior to the synostosis. The coronal suture incurves around a

Revisiting a Rare Cause of Plagiocephaly: Frontosphenoidal Craniosynostosis.

Frontosphenoidal craniosynostosis remains a rare cause of anterior plagiocephaly, with only 49 cases reported worldwide. Delayed diagnosis may lead to significant aesthetic and functional consequences. This case series presents 2 recently treated patients, highlighting clinical features, imaging findings, and surgical management. A retrospective review of 2 patients diagnosed and treated at our multidisciplinary head shape clinic was conducted, with parental consent obtained. Both children presented with unilateral brow flattening and preserved brow height. Imaging confirmed frontosphenoidal s

Cost and Satisfaction Implications of Using Telehealth for Plagiocephaly.

UNLABELLED: Patients with deformational plagiocephaly are often referred for evaluation by a plastic surgeon. During the early COVID-19 pandemic, visits were performed predominantly via telehealth. This study compares costs, satisfaction, and technological considerations for telehealth and in-person consultations for plagiocephaly. METHODS: This prospective study evaluated telehealth and in-person consultation for plagiocephaly between August 2020 and January 2021. Costs were estimated using time-driven activity-based costing (TDABC) and included personnel and facility costs. Patient-borne ex

Perceptions and Preferences of Laypersons in the Management of Positional Plagiocephaly.

BACKGROUND: Uncertain clinical evidence for treating positional plagiocephaly, especially with helmet therapy, creates difficulties in counseling parents of patients. This study investigates layperson perceptions and treatment preferences for positional plagiocephaly to provide patient-oriented evidence for management. METHODS: Adult laypersons were recruited through crowdsourcing to view digitally-modified images of normal, mildly, moderately, or severely plagiocephalic infant heads. Participants provided demographic information and rated the infant's head shape and potential related social
