Characterizing the interindividual postexercise hypotension response for two order groups of concurrent training in patients with morbid obesity.

Background: Postexercise hypotension (PEH) is a common physiological phenomenon occurring immediately after endurance training (ET), resistance training (RT), and ET plus RT, also termed concurrent training (CT); however, there is little knowledge about the interindividual and magnitude response of PEH in morbidly obese patients. Aim: The aims of this study were (1) to investigate the effect of CT order (ET + RT vs. RT + ET) on the blood pressure responses; 2) characterize these responses in responders and nonresponders, and 3) identify potential baseline outcomes for predicting blood pressure

Acute normovolaemic haemodilution during anaesthesia in a polycythaemic French Bulldog presented for transvalvular pulmonic stent angioplasty.

A 2-year-old, female entire French Bulldog was presented for exercise intolerance, tachypnoea and cyanosis. Severe pulmonic stenosis and hypoplasia with a right-to-left shunting patent foramen ovale was identified on echocardiography. Polycythaemia was present on initial haematology. Minimally invasive transvalvular pulmonic stent angioplasty (TPSA) was scheduled to improve blood flow to the pulmonary circulation and reduce shunt fraction. Anaesthesia was induced with fentanyl, midazolam and propofol administered intravenously to effect. Anaesthesia was maintained with propofol, fentanyl and l

Sex differences between postmenopausal women and similar-age men in response to orthostatic stress following 2 wk of bed rest.

Reduced orthostatic tolerance is common following periods of bed rest that are associated with illness or surgery, putting individuals at higher risk for syncope and falls following hospitalization. Following menopause, mechanisms of female cardiovascular regulation change, which may be associated with sex-specific responses to orthostatic stress following bed rest. The purpose of our experiment was to investigate sex differences between healthy postmenopausal women and similar-age men (age: 55-65 yr) for their orthostatic tolerance and cerebrovascular responses to standing following bed rest.

First-in-man safety evaluation of renal denervation for chronic systolic heart failure: primary outcome from REACH-Pilot study.

BACKGROUND: Sympathetic overactivation, is reduced by renal denervation in drug-resistant hypertension. A similar role for renal denervation in heart failure remains unstudied, partly due to the concern about potential concomitant deleterious blood pressure reductions. This pilot study evaluated the safety of renal denervation for heart failure using an intensive follow-up protocol. METHOD: 7 patients (mean age 69 years) with chronic systolic heart failure (mean BP on referral 112/65 mmHg) on maximal tolerated heart failure therapy underwent bilateral renal denervation May-July 2011. Patients

Recovery pulse rate and cardiovascular function indices in young female adults following orthostasis.

BACKGROUND: Recovery pulse rate (RPR) and other cardiovascular indices - such as heart rate variability and blood pressure recovery - are underutilized tools in assessing autonomic and cardiovascular adaptability to orthostasis. While orthostatic hypotension is well-documented, the prognostic significance of delayed heart rate recovery and impaired autonomic compensation remains insufficiently explored. Emerging evidence suggests that abnormal RPR may predict cardiovascular morbidity and autonomic dysfunction; however, standardized clinical guidelines for its interpretation are lacking. Bridgi

The effect of mobilization on blood pressure for stroke survivors with moderate or severe injury: a rapid review.

PURPOSE: Severe stroke survivors respond to mobilization rehabilitation interventions with more varied outcomes than those with less severe stroke. They may face additional risks for impaired blood pressure regulation and extended vulnerability to blood pressure changes, potentially contributing to poor recovery outcomes. The primary aim of this rapid review was to identify and synthesize current evidence investigating the effects of mobilization on blood pressure, for stroke survivors in the acute and early stage of rehabilitation. The secondary aim was to identify potential risks associated w

Pharmacological stress testing.

Pharmacological stress in conjunction with radionuclide myocardial perfusion imaging has become a widely used noninvasive method of assessing patients with known or suspected coronary artery disease. In the United States, over one third of perfusion imaging studies are performed with pharmacological stress. Pharmacological stress agents fall into two categories: coronary vasodilating agents such as dipyridamole and adenosine, and cardiac positive inotropic agents such as dobutamine and arbutamine. For both, in the presence of coronary artery disease (CAD), perfusion image abnormalities result

Autonomic neural functions in space.

Autonomic neural functions are important to regulate vital functions in the living body. There are different methods to evaluate indirectly and directly autonomic, sympathetic and parasympathetic, neural functions of human body. Among various methods, microneurography is a technique to evaluate directly sympathetic neural functions in humans. Using this technique sympathetic neural traffic leading to skeletal muscles (muscle sympathetic nerve activity; MSNA) can be recorded from human peripheral nerves in situ. MSNA plays essentially important roles to maintain blood pressure homeostasis again

Nirmatrelvir-ritonavir targeting viral persistence in post-COVID-19 condition (long COVID) in the USA (RECOVER-VITAL): a randomised, double-blind, placebo-controlled, phase 2 trial.

BACKGROUND: Post-acute sequelae of SARS-CoV-2 infection, more commonly known as long COVID, has emerged as a major health problem. The pathogenesis of long COVID is unknown, but among the leading hypotheses is viral persistence. We aimed to investigate whether the use of the SARS-CoV-2 antiviral nirmatrelvir-ritonavir improved long COVID symptoms. METHODS: We conducted a double-blind, placebo-controlled, randomised trial involving adults who had developed persistent symptoms (≥12 weeks) associated with three major symptom phenotypes (cognitive, autonomic, or exercise) after acute SARS-CoV-2 i

Shared autonomic phenotype of long COVID and myalgic encephalomyelitis/chronic fatigue syndrome.

INTRODUCTION: Long COVID and myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) are relatively common and disabling multisystem disorders that share overlapping features, including post-infectious onset and similar clinical manifestations such as brain fog, fatigue, muscle pain, and dysautonomia with orthostatic intolerance. These similarities suggest that Long COVID and ME/CFS may share common pathophysiological mechanisms, though the underlying mechanisms remain poorly understood, partly due to the difficulty in quantifying many of the symptoms. MATERIALS AND METHODS: This retrospe

Pharmacological interventions for the treatment of depression in chronic obstructive pulmonary disease.

BACKGROUND: Studies report that up to 80% of individuals with chronic obstructive pulmonary disease (COPD) may struggle with symptoms of depression. However, this major comorbidity in COPD is rarely managed effectively. A number of recent studies indicate that left untreated, COPD-related depression is associated with worse quality of life, worse compliance with COPD treatment plan, increased exacerbations, hospital admissions, and healthcare costs when compared to individuals with COPD without depression. Regrettably, COPD practice guidelines do not provide conclusive treatment recommendation

Superset Versus Traditional Resistance Training Prescriptions: A Systematic Review and Meta-analysis Exploring Acute and Chronic Effects on Mechanical, Metabolic, and Perceptual Variables.

BACKGROUND: Supersets are a time-efficient resistance training (RT) method that involve the sequencing of two exercises with little or no rest between them. However, despite their common implementation during RT, a comprehensive and quantitative review is still lacking. OBJECTIVES: The primary aim of this systematic review and meta-analysis was to compare the acute and chronic effects of superset and traditional set prescriptions on mechanical, metabolic, and perceptual variables. Evolution of diuretics and ACE inhibitors, their renal and antihypertensive actions--parallels and contrasts.

The emergence of diuretic drugs and angiotensin converting enzyme (ACE) inhibitors ranks amongst the major therapeutic advances of modern medicine. The discovery of these drug groups arose largely by chance, yet each has dramatically influenced the treatment of congestive cardiac failure and arterial hypertension. The central role which diuretics have had in the management of both oedema and hypertension hinges on their ability to induce a net renal excretion of solute and water by selective interference with either active or passive ion transport processes in different segments of the nephron

The EMazing Race: A Novel Gamified Board and Clinical Practice Review for Emergency Medicine Residents.

AUDIENCE: Emergency medicine residents and medical students. INTRODUCTION: Many emergency medicine residency programs have their residents take an in-training exam (ITE) in which performance has been linked to certifying exam pass rates, so solid preparation is important.1 The typical clinical schedule of an emergency medicine resident is incredibly varied and time-consuming, which limits time to dedicate to preparation. All emergency medicine programs have synchronous and asynchronous protected time for resident education which provides an excellent opportunity to prepare learners for in-tra

An Appy That Needs Epi: An Atypical Presentation of Anaphylaxis.

AUDIENCE: This simulation is intended for 4th year medical students. BACKGROUND: Shock is the result of inadequate circulation and failure to perfuse tissues, leading to cellular and organ dysfunction.1 Anaphylactic shock specifically is a type of distributive shock secondary to an IgE (immunoglobulin E) dependent reaction, which can result in respiratory compromise and cardiovascular collapse. The National Institute of Allergy and Infectious Diseases/Food Allergy and Anaphylaxis Network (NIAID/FAAN) laid out three diagnostic criteria for the diagnosis of anaphylaxis. Treating Lows: Management of Orthostatic Hypotension.

AbstractOrthostatic hypotension is a prevalent clinical condition, caused by heterogenous etiologies and associated with significant morbidity and mortality. Management is particularly challenging in patients with uncontrolled hypertension. A thorough assessment is needed to draw an appropriate management plan. The treatment aims to improve postural symptoms while minimizing side effects and reducing iatrogenic exacerbation of supine hypertension. A personalized management plan including rationalizing medications, patient education, identification, and avoidance of triggers, as well as nonphar
