Post-cardiac injury syndrome (PCIS) encompasses inflammatory pericardial syndromes occurring after cardiac injury, including post-pericardiotomy syndrome (PPS) following cardiac surgery, post-myocardial infarction (Dressler syndrome), and post-traumatic pericarditis. With the expanding use of cardiac surgical and interventional procedures, PCIS has become increasingly prevalent despite a marked decline in Dressler syndrome in the contemporary reperfusion era. The syndrome is believed to result from an immune-mediated inflammatory response to myocardial and pericardial injury, leading to perica

A 64-year-old woman presented eight days following pericardiectomy with a 2-day history of progressive dyspnea and new crushing chest pain. Chest x-ray revealed development of interval "cardiomegaly." Bedside echocardiography revealed a large accumulation of pericardial fluid despite absence of pericardium. Diagnostic and therapeutic mediastinocentesis was performed and confirmed the diagnosis of mediastinal hemorrhage. Subsequent to pericardiectomy, mediastinal hemorrhage can masquerade as cardiomegaly and may require urgent intervention.

Postoperative edema and effusion (POEE) following cardiopulmonary bypass (CPB) surgery in children retards recovery and may aggravate postpericardiotomy (PPS), capillary leak syndrome (CLS), or multiorgan failure (MOF). Compared with complication-free children, POEE affected children have different preoperative serum levels of circulating cytokines and adhesion molecules. These levels may be used preoperatively to assess POEE, but their determination is time consuming, costly, and a substantial blood volume is required. Altered serum levels of cytokines and adhesion molecules also may be refle

BACKGROUND: The blood retained in posterior pericardium can trigger an inflammatory response that increases postoperative atrial fibrillations (POAFs), and it can complicate postoperative course. We retrospectively investigated the impact of a posterior pericardial drain (PPD) in reducing late postoperative pericardial effusion (pPE) and POAFs during the first 30 postoperative days.
METHODS: Two hundred and fifty coronary artery bypass grafting patients were divided into two groups according to the presence of a PPD in addition to the anterior one. Perioperative data and the incidence of POAF

PURPOSE OF REVIEW: Benign pleural effusions comprise a heterogeneous group of disorders, but recent advances have been uneven across different etiologies. This review summarizes the developments most likely to influence current clinical practice, focusing on heart failure-related pleural effusions, hepatic hydrothorax, and postcoronary artery bypass graft (CABG) pleural effusions.
RECENT FINDINGS: Heart failure-related pleural effusions have experienced the greatest advances recently. A blood-free pleural fluid triple combination (protein, lactate dehydrogenase, and cholesterol) provides diag

Recurrent tunnel stenosis of the left ventricular outflow tract following operation for subaortic stenosis and hypoplastic aortic annulus remain a challenge for pediatric cardiac surgeons. We have recently applied a new technique of extended aortic root replacement using an aortic allograft to treat three patients who had previously been operated upon for subaortic stenosis and three who had aortic stenosis with a hypoplastic aortic annulus. This new procedure combines the concept of aortoventriculoplasty with allograft aortic root replacement and coronary artery reimplantation. The valved aor

Surgical correction of pectus excavatum in children has gained new momentum since the introduction of the new minimally invasive repair by Nuss. To date, no studies directly evaluate the outcome of the new technique versus that of the conventional technique. From 2000 to 2002, 28 patients underwent pectus excavatum correction in the authors' hospital. Twenty-one were treated by minimally invasive repair of pectus excavatum and seven patients had open correction. Intraoperative and postoperative complications, clinical outcome, and patient satisfaction were evaluated. In the minimally invasive

OBJECTIVES: This study sought to compare the safety and efficacy of the HELEX septal occluder (HSO) with surgical repair of atrial septal defect (ASD).
BACKGROUND: The HSO is a low-profile, double-disk occluder device for percutaneous closure of secundum ASD.
METHODS: Patients were enrolled (HSO arm prospectively, surgery arm prospectively/retrospectively) from 14 U.S. sites and followed up for 12 months postprocedure. Investigator-reported outcomes were evaluated, including closure success (no or clinically insignificant residual shunt) and the incidence of adverse events. The first 3 HSO p

Exercise testing has a definite role in pediatrics today. Different methods are presented, and the value of maximal exercise with determination of oxygen uptake and blood lactate is stressed. In children with heart disease, exercise testing with precordial electrocardiogram can be of both diagnostic and prognostic value. The cardiovascular function at different intensities of exercise is evaluated, serious dysrhythmias may be revealed, hypertension judged and the effect of drug therapy can be checked by exercise testing. It is an important way in assessing the child's functional capacity after

Postpericardiotomy syndrome (PPS) may be associated with tamponade and pericardial constriction that may require procedural intervention. The aim of this study was to identify clinical features associated with adverse events requiring procedural intervention in patients with PPS. A total of 239 patients who developed PPS after cardiac surgery were monitored for 12 months. PPS was diagnosed if 2 of the 5 following findings were present: fever without infection, pleuritic pain, friction rub, pleural effusion, and pericardial effusion (<60 days after surgery). The primary end point was the develo

OBJECTIVES: To evaluate feasibility, efficacy, and safety of an approach to atrial septal defect (ASD) occlusion in children ≤20 kg that eliminates cardiac wall erosion risk.
BACKGROUND: Cardiac wall erosion is a potentially catastrophic complication of ASD device closure. The HELEX Septal Occluder (HSO) is a compliant device with no reports of erosion. The HSO is technically difficult to deploy in smaller children and cannot be used to close larger defects. To eliminate wall erosion risk, we use the HSO when feasible and surgery for larger defects.
METHODS: Retrospective review of ASD proce

The etiologic evaluation of pericardial effusion is frequently unsuccessful when noninvasive methods are used. To determine the cause of the current episode, all patients with echographically identified pericardial effusion from May 1998 to December 2002 underwent noninvasive diagnostic testing of blood, throat, and stool samples. Patients with postpericardiotomy syndrome were excluded. To analyze the value of our tests, we tested randomly selected blood donors as negative controls. Among 204 included patients, 107 (52.4%) had a final etiologic diagnosis: the etiology of 52 was highly suspecte

Post-pericardiotomy syndrome (PPS) is an inflammatory process involving the pleura, pericardium, or both and occurs after cardiothoracic surgery. Surgical atrial septal defect (ASD) closure is associated with higher incidence of PPS post-operatively as compared to other operations. Reported incidence of PPS varies from 1 to 40%. NSAIDs are often used to treat PPS and in our center, some practitioners have prescribed ibuprofen prophylactically. This study sought to investigate the impact of prophylactic treatment with ibuprofen on the development and severity of PPS following surgical ASD closu

Post-pericardiotomy syndrome (PPS) occurs in a subgroup of patients who have undergone cardiothoracic surgery and is characterized by fever, pleuritic pain, pleural effusion, and pericardial effusion. It is associated with significant morbidity, and the leading complications include tamponade and constrictive pericarditis. Epidemiologic studies have found that PPS often occurs among younger patients; however, there is a lack of comprehensive risk stratification. It is therefore important to be able to identify patients who are at high risk for developing this disease. The diagnosis is made if
