The author suggests a new modification of operation for formation of subfascial lymphovenous anastomoses (LVA) on the dorsum of the foot in treatment of edema of the lower limbs. Most patients with chronic venous ulceration or severe pre-ulcer damaged skin have incompetent popliteal valves and a high ambulant venous pressure (AVP). Competency can be restored by a vein valve transplant taken from the arm and placed in the popliteal fossa, with subjective and objective improvement. PURPOSE: To determine the hereditary and nonhereditary risk factors contributing to the development of noncatheter-related thrombosis in children.
PATIENTS AND METHODS: Ninety-five children with noncatheter-related thrombosis from a single center were analyzed for clinical manifestations and outcome during a period of 3.5 years. Patients with thrombosis were evaluated for congenital and acquired risk factors, and patients were screened for factor V Leiden, prothrombin 20210A mutations, protein C, protein S, antithrombin III deficiencies, and antiphospholipid antibodies.

Under both inpatient and outpatient conditions several skin grafting techniques (eg. split-skin meshgraft, postage stampgrafts) can be useful in treating already phlebosurgically operated venous ulcers. 82 already phlebosurgically longterm treated venous ulcers were skin grafted under clinical conditions in the time from 1983 to 1992. A series of 213 consecutive patients suffering from acute iliofemoral vein thrombosis who underwent venous thrombectomy is analyzed. Surgery was performed from an inguinal approach with a Fogarty-balloon-catheter from the proximal vessel and by bimanual exprimation from the femoral and popliteal veins. Late post-thrombotic complaints after subclavian vein thrombosis are reported with highly varying frequencies (8-80% severe disability). The therapeutic approach depends partly on this frequency. In a prospective study we performed color duplex ultrasonography to evaluate patency and valvular function of previous thrombosed veins 12 years after the acute thrombosis. Normal clinical findings were found in 64% of the patients, mild postthrombotic skin changes in 28%, and marked trophic changes in 5%; only 1 venous ulcer occurred.
