Prazosin for treatment of post-traumatic stress disorder: a systematic review and meta-analysis.

BACKGROUND: Prazosin has been an accepted treatment for patients with post-traumatic stress disorder (PTSD) who experience sleep disturbances, including nightmares. Results of a recent large randomized control trial did not find benefit of prazosin vs placebo in improving such outcomes. A meta-analysis that includes this most recent trial was conducted to examine the pooled effect of prazosin vs placebo on sleep disturbances and overall PTSD symptoms in patients with PTSD. METHODS: A systematic review of the published literature on trials comparing prazosin vs placebo for improvement of overa

The efficacy of prazosin for the treatment of posttraumatic stress disorder nightmares in U.S. military veterans.

BACKGROUND AND PURPOSE: Nightmares associated with posttraumatic stress disorder (PTSD) are a hallmark symptom among U.S. military veterans who have seen combat. Management of combat-related nightmares can be difficult and current pharmacologic options are limited and tend to have adverse side effects. The aim of this review is to explore recent literature regarding the efficacy of prazosin for the treatment of nightmare disorder in the veteran population. METHODS: Recent literature consisting of three systematic reviews was reviewed, as well as current clinical guidelines published by The De

Preliminary findings concerning the use of prazosin for the treatment of posttraumatic nightmares in a refugee population.

Prazosin, a centrally active alpha-1 adrenergic receptor antagonist, has reduced nightmares and sleep disturbances in placebo-controlled studies involving patients with combat and civilian related posttraumatic stress disorder (PTSD). In this retrospective chart review, we analyzed data from 23 refugees diagnosed with chronic PTSD who were treated with prazosin. The recurrent distressing dreams item of the Clinician Administered PTSD Scale (CAPS) was used to quantify nightmare severity. A Clinical Global Impressions-Change (CGI-C) score assessed change in overall PTSD severity exclusive of nig

Severe envenoming by the Indian red scorpion Mesobuthus tamulus: the use of prazosin therapy.

We describe the clinical course and outcome in 46 victims of severe scorpion envenoming treated with prazosin (P), and compare them with earlier patients treated with conventional therapy (C) (n = 45) and nifepidine (N) (n = 28). The incidence of complicating left ventricular failure was 29% for C, 35% for N and 6.5% for P; that of acute pulmonary oedema was 46% for C, 14% for N and zero for P; mortality was 25% for C, 3.5% for N and zero for P. Although this is a historical study, prazosin appears to significantly reduce morbidity and shorten recovery time. Experience in other countries sugge

Acute tolerance to furosemide. Pretreatment with captopril or prazosin does not influence diuresis and natriuresis.

To investigate whether the development of acute tolerance to furosemide in human subjects could be prevented or delayed by angiotensin converting enzyme inhibition or alpha 1-receptor blockade, a study was conducted on healthy volunteers. The protocol on the experimental days was identical except for pre-treatment with placebo, captopril or prazosin. During continuous furosemide infusion with urinary furosemide excretion at a constant rate, the subjects became progressively dehydrated, with a maximal decrease in plasma volume of 9-11%. The diuretic/natriuretic response to furosemide was simila

[Effects of terazosin on the blood pressure responses to tilting in conscious animals: comparison with prazosin].

Inhibitory effects of terazosin on the compensatory blood pressure responses to tilting were studied in conscious rabbits and spontaneously hypertensive rats (SHR). In rabbits, doses which reduced the mean blood pressure by 15 mmHg were 330 micrograms/kg, i.v., for terazosin and 42 micrograms/kg, i.v., for prazosin, while those which depressed the blood pressure responses to tilting by 30 mmHg were 180 micrograms/kg, i.v., for terazosin and 54 micrograms/kg, i.v., for prazosin. In SHR, almost equal decreases in the mean blood pressure (about 30%) were observed by 1 mg/kg prazosin, p.o., 20 mg/

Effects of prazosin on isolated human urethra and in patients with lower motor neurons lesions.

In isolated human urethra, prazosin competitively inhibited contractions induced by noradrenaline, but had no effect on contractions elicited by potassium (127 mM), suggesting an action solely on alpha-adrenoceptors. Seven patients with lower motor neuron lesions and micturition disturbances were treated with 2 mg of prazosin twice daily. Five were investigated by simultaneous urethrocystometry before and during treatment. Prazosin reduced the intraurethral pressure, the intravesical pressure during bladder filling, and autonomous bladder waves. Voiding and incontinence improved in five patien

Prazosin Monotherapy for Adolescent Posttraumatic Stress Disorder: A Case Report and Narrative Review.

Pediatric posttraumatic stress disorder (PTSD) is a debilitating condition that is associated with significant functional impairment, diagnostic complexity, and limited evidence-based treatment options. Among its core features, trauma-related sleep disturbances-particularly nightmares-are highly prevalent, profoundly impairing, and contribute to the chronicity and severity of the disorder. Prazosin, a selective alpha1-adrenergic antagonist, has demonstrated efficacy in adults for reducing nightmares and improving sleep, but pediatric data remain limited. This case report describes an adolescen

The α1 adrenoceptor antagonist prazosin potentiates morphine induced conditioned place preference in rats.

The norepinephrine (NE) system is involved in pathways that regulate morphine addiction. Here, we investigated the role of α1 adrenoceptor in the ventrolateral orbital cortex (VLO) of rats with repeated morphine treatment and underlying molecular mechanisms. The rewarding properties of morphine were assessed by the conditioned place preference (CPP) paradigm. Prazosin, an α1 adrenoceptor antagonist, was microinjected into the VLO. The expression of α1 adrenoceptor, p-CaMKII/CaMKII, CRTC1, BDNF and PSD95 in the VLO were determined by immunohistochemistry or western blotting. Neurotransmitter NE

Enduring vulnerability to transient reinstatement of hemiplegia by prazosin after traumatic brain injury.

A single dose of an alpha1-noradrenergic antagonist transiently reinstates hemiplegia after recovery from brain injury, which suggests that noradrenaline (NA) is required to maintain recovery. No systematic studies have determined the postinjury duration of this vulnerability. This study used a within-subject, dose-response design to determine whether prazosin (PRAZ), an alpha1-NA antagonist, or propranolol (PROP), a beta-NA antagonist, would continue to reinstate hemiplegia over time after recovery from weight-drop traumatic brain injury (TBI). PRAZ transiently reinstated hemiplegia as measur

[Treatment of obstruction caused by prostatic hypertrophy with prazosin. 1-year follow-up].

Since Caine demonstrated the presence of alpha-adrenergic receptors in prostate tissue in 1975, an alpha-blocker capable of producing maximum relaxation of prostatic tissue with minimum side effects has been sought. Prazosin, a selective alpha-1 blocking agent, must meet these requirements. We report the results of a study undertaken in 28 patients with benign hyperplasia of the prostate demonstrated by flowmetry, clinically and by ultrasound. A significant improvement was observed in all patients except for the ultrasound measured prostate size, which remained unchanged. Undesired effects wer

Dose-response study of prazosin in Raynaud's phenomenon: clinical effectiveness versus side effects.

In a dose-response study of prazosin in 24 patients with Raynaud's phenomenon, the clinical effectiveness and side effects of 3, 6, and 12 mg prazosin administered daily were compared. Diary analysis and measurement of finger skin blood flow and temperature before, during, and after cold challenge showed no major differences among these three dose regimens. Standing systolic blood pressure decreased significantly during the 12-mg period in comparison with the 6-mg (P less than .05) and with the 3-mg period (P less than .02), whereas heart rate increased in the 6-mg and 3-mg periods (P less tha

Prazosin for post-traumatic nightmares: A clinical audit in acute public psychiatric unit patients.

ObjectivePost-traumatic nightmares (PTNs) are a debilitating symptom of post-traumatic stress disorder (PTSD), yet evidence for pharmacological treatment remains inconsistent. This audit examined the effectiveness and tolerability of prazosin for PTNs in an Australian inpatient psychiatric population.MethodA clinical audit with pre-test post-test analysis was conducted using records from Monash Health wards. Adult patients (n = 50) prescribed prazosin for PTNs for ≥7 days were included. Symptom severity was measured using the Clinical Global Impression-Severity (CGI-S) scale at treatment initi

Re-emergence of posttraumatic stress disorder nightmares with nursing home admission: treatment with prazosin.

Seniors with a history of emotional trauma decades earlier can experience a recurrence of posttraumatic stress disorder symptoms when transitioning to a nursing home. We present the case of an 86-year-old male Holocaust survivor admitted to a nursing home for physical therapy and rehabilitation 6 weeks after the death of his wife; the patient was expressing a persistent death wish. Despite the multiple risk factors for depression, his distress was specifically related to the reemergence of nightly posttraumatic nightmares. Over the course of 1 week of treatment with 1 mg prazosin at bedtime, h

Structure-activity relationships for prazosin and WB 4101 analogues as alpha 1-adrenoreceptor antagonists.

Several alpha-adrenoreceptor antagonists were prepared by coupling one of the two moieties of WB 4101 (1) with one of the two moieties of prazosin (2). Their blocking activity and relative selectivity on alpha 1- and alpha 2-adrenoreceptors were evaluated in the isolated rat vas deferens. Although retaining a significant selectivity toward alpha 1-adrenoreceptors, all the drugs were weaker antagonists than the parent compounds 1 and 2. Opening the piperazine ring of 2 gave 3, which displayed a very high activity and selectivity toward alpha 1-adrenoreceptors (alpha 1/alpha 2 = 3890). This may

The Relative Effects of Prazosin on Individual PTSD Symptoms: Evidence for Pathophysiologically-Related Clustering.

BACKGROUND: The α1-adrenoreceptor antagonist prazosin has in many but not all studies been found to be effective for PTSD associated nightmares, hyperarousal symptoms, and total symptom severity. The particular efficacy of prazosin for nightmares and hyperarousal symptoms suggests there may be a subset of PTSD symptoms that are more tightly associated with an α1-adrenoreceptor mediated noradrenergic mechanism, but cross traditional diagnostic symptom clusters. However, the efficacy of prazosin for individual symptoms other than nightmares and sleep disruption has not previously been examined.

Demographic Variation in the Use of Prazosin for Treatment of Sleep Disturbance in Combat Veterans with PTSD.

OBJECTIVES: Off-label use of prazosin for posttraumatic stress disorder-related sleep disturbances (PTSD-SD) is widespread in Veterans Health Administration (VA) settings, but clinical trials have had mixed results. Trial criteria may exclude significant sub-groups, and therefore a 2018 study may not provide the final evidence of prazosin efficacy for PTSD-SD. This study analyzed correlates of prazosin use in Vietnam era (VNE) and Operations Enduring Freedom/Iraqi Freedom (OEF/OIF) cohorts to illustrate patterns of usage in this heterogeneous population. METHOD: Data extracted on patients wit

Prazosin initiation and dose titration in a patient with posttraumatic stress disorder on concurrent carvedilol.

One mechanism involved in the pathophysiology of posttraumatic stress disorder (PTSD) is increased noradrenergic stimulation. Prazosin, a commonly utilized treatment for PTSD nightmares, works to block noradrenergic stimulation of the alpha-1 adrenoreceptor. Dual antagonism of this receptor would be expected to increase risk of adverse effects. Carvedilol has both alpha-1 adrenergic and nonselective beta-adrenoreceptor antagonist activity. To our knowledge, there is no clinical guidance on use of prazosin in patients concomitantly prescribed carvedilol for hypertension. This case describes the
