[Exercise test in isolated anomalies of auriculo-ventricular conduction in children and adolescents. Value in idiopathic auriculo-ventricular blocks and ventricular pre-excitation syndromes].

Abnormal atrioventricular conduction in children and adolescents without obvious underlying cardiac disease poses the problem of long term prognosis. This study of 45 patients aged 8 to 18 years with congenital atrioventricular block (30 cases) or ventricular pre-excitation (15 cases), was undertaken to determine the value of exercise testing, a physiological adrenergic stress test, in the assessment of these patients. Our results show that exercise testing was valuable in determining the site of atrioventricular block: during exercise the degree of block increased in two patients, justifying

[PQ interval and pre-excitation syndrome in direct fetal electrocardiogram--fetal Lown-Ganong-Levine (LGL) syndrome].

Preexcitation syndromes are rare disturbances of the atrioventricular excitation transmission in direct fetal electrocardiograms. As example we report on a fetal Lown-Ganong-Levine (LGL)-syndrome. This is to differentiate from the Wolff-Parkinson-White (WPW)-syndrome, which is possible by typical changes in the QRS-complex. Possibly the short PQ-interval in the fetal electrocardiogram is functional, but needs postnatal control.

Does the age of evaluation change the long-term follow-up of untreated pre-excitation syndrome?

BACKGROUND: Due to high rates of ablation at the time of diagnostic EP study, follow-up of the natural history of untreated pre-excitation syndrome has become difficult. We present patients in which such data is available and study the effect of initial age on the evolution. METHODS: In this retrospective review, 126 patients, 47 aged ≤19 years, 79 aged more than>19 underwent 2 similar electrophysiological studies (EPS) within 1 to 25 years of one another (8.8 ± 6.8) for occurrence of symptoms or new evaluation. First EPS was indicated for syncope (10), atrioventricular re-entrant tachycardia

Fifteen-year follow-up of a heterozygous Fabry's disease patient associated with pre-excitation syndrome.

A 47-year-old woman with heterozygous Fabry's disease with pre-excitation syndrome has been followed up for 15 years. Diagnosis was confirmed by the typical electron microscopic feature of the endomyocardial specimen and a decreased plasma alpha-galactosidase activity. As the disease progressed, the interventricular septum thickened from 11 to 17 mm as measured by echocardiography, while the AH interval was prolonged from 80 to 140 msec. In Fabry's disease, the PR interval has been reported to be variable from short PR to AV block. Therefore, this case may be helpful to understand the time cou

Ventricular fibrillation in a patient with Wolff-Parkinson-White syndrome unrelated to pre-excited atrial fibrillation.

A 52-year-old man was admitted due to out-hospital cardiac arrest. Recurrent ventricular fibrillation (VF) occurred under therapeutic hypothermia thereafter. Previously inadverted full pre-excitation was documented exclusively and immediately prior to 4 out of the 5 VF relapses. Coronary vasospasm and early repolarization were also documented. An electrophysiological study demonstrated poor anterograde conduction over a left-sided accessory pathway. We theorize that maximum pre-excitation favored in-hospital VF by augmenting the repolarization vulnerability induced by therapeutic hypothermia,

[Disorders of cardiac rhythm in combined ventricular pre-excitation syndrome and primary mitral valve prolapse].

Primary prolapse of the mitral valve was diagnosed in 27 (11.7%) out of 230 patients with preexcitation syndrome: 10 of them presented with Wolff-Parkinson-White syndrome, 15 with Clerc-Lévy-Critesco syndrome, and two patients with combination of both syndromes. Preexcitation syndrome combined with mitral valve prolapse manifested itself by different disorders of the cardiac rhythm, mainly by the supraventricular forms: by PST in 17, nonparoxysmal tachycardia in 1, atrial fibrillation paroxysms in 7, sinus tachycardia in 2, supraventricular extrasystole in 3, pacemaker migration in the atria i

Pre-Excited Atrial Fibrillation in Wolff-Parkinson-White (WPW) Syndrome: A Case Report and a Review of the Literature.

Wolff-Parkinson-White (WPW) syndrome is defined by specific electrocardiogram (ECG) changes resulting in ventricular pre-excitation (the so-called WPW pattern), related to the presence of an accessory pathway (AP), combined with recurrent tachyarrhythmias. WPW syndrome is characterized by different supraventricular tachyarrhythmias (SVT), including atrioventricular re-entry tachycardia (AVRT) and atrial fibrillation (AF) with rapid ventricular response, with AVRT being the most common arrhythmia associated with WPW, and AF occurring in up to 50% of patients with WPW. Several mechanisms might b

[The prospective evolution of the efficacy of antiarrhythmia therapy in paroxysmal supraventricular tachycardia in patients with the ventricular pre-excitation syndrome].

Evolution of the effectiveness of antiarrhythmic drugs used in 240 patients with preexcitation syndrome to arrest episodes of paroxysmal supraventricular tachycardia was analysed prospectively. The first intravenous administration was effective in 97, 93.3, 80.9, 75, 70.3, 60, 48% of cases for novocainamid, verapamil, cordarone, disopyramide, ajmaline, ethacizine, mexitil and inderal, respectively. When observed in prospective evolution for 10-14 years, overall group efficacy dropped from 81.3% at first administration to 21.4%.

Rapid and low-cost method to prove the nature of no documented tachycardia in children and teenagers without pre-excitation syndrome.

Aims Symptoms in children are often difficult to interpret. The purpose of this study was to report the results of transoesophageal electrophysiological study (EPS) performed in children complaining of sudden onset tachycardia with normal non-invasive studies. Methods and results Eighty-two children and teenagers (mean age 15 +/- 3 years) presented with suspected but no documented paroxysmal supraventricular tachycardia (SVT). ECG was normal. Non-invasive studies were negative; 23 children had syncope with tachycardias. They underwent transoesophageal EPS in our out-patient clinic. The mean du

Comparison of Electrophysiologic Profiles in Pediatric Patients with Incidentally Identified Pre-Excitation Compared with Wolff-Parkinson-White Syndrome.

The rising utilization of screening electrocardiograms has resulted in increased incidental identification of ventricular pre-excitation in pediatric patients. We compared accessory pathways of incidentally identified pre-excitation to Wolff-Parkinson-White Syndrome (WPW) with the aim to identify factors important in preprocedural counseling and planning. This single-center, retrospective study of patients ≤18 years without congenital heart disease identified 227 patients diagnosed with pre-excitation and referred for invasive electrophysiology study between 2008 and 2017. WPW Syndrome was dia

Left ventricular dyssynchrony in pre-excitation syndrome: effect of accessory pathway location and reversibility after ablation therapy.

We sought to evaluate the effects and reversibility of different locations of accessory pathways (AP) on left ventricular dyssynchrony (LVdys). The acute and chronic effects of AP were evaluated in a canine model (n = 11) and in patients with pre-excitation syndrome (n = 25). Pre-excitation was simulated in the canine model by applying VDD-type epicardial ventricular pacing near the atrioventricular (AV) groove with 50-ms AV interval after median thoracotomy, at five different sites in each animal. For the simulation of pre-excitation through the septal accessory pathway, right basal septal pa

[The functional characteristics of the prostanoid system in patients with supraventricular paroxysmal tachyarrhythmias in ischemic heart disease and the pre-excitation syndrome].

Different variants of the prostanoid system response under paroxysms of tachycardia in patients with ischemic heart disease presenting with preexcitation syndrome are believed to be not only a manifestation of etiological and pathogenetic differences--they also reflect particular features of the state of the neurohumoral control on the whole in the nosological forms studied.
