Prediabetes and chronic kidney disease: a comprehensive review of association and clinical implications.

Prediabetes affects nearly one-third of adults in the United States and is increasingly recognized as a contributor to early chronic kidney disease (CKD). This review summarizes current evidence linking prediabetes with early kidney changes, including albuminuria, glomerular hyperfiltration, and mild renal impairment, as well as its association with increased cardiovascular risk. Large observational cohorts, such as the Chronic Renal Insufficiency Cohort, suggest an association between prediabetes and progression of proteinuria, although its independent contribution to advanced CKD remains var

Glucose ingestion in cystic fibrosis induces severe redox imbalance: A potential role in diabetes.

BACKGROUND: Cystic fibrosis related diabetes (CFRD) is the most common co-morbidity associated with cystic fibrosis (CF). Individuals with CF demonstrate airway and systemic oxidation compared to people without CF. Furthermore, systemic oxidation precipitated by hyperglycemia in non-CF diabetes has been shown to lead to enhanced inflammation. We hypothesized that the presence of both CF and diabetes in an individual would result in hyperglycemia-induced redox imbalance to an oxidative state. This in turn would result in enhanced production of pro-inflammatory cytokines. METHODS: Systemic redo

Acute Nutritional Ketosis and Its Implications for Plasma Glucose and Glucoregulatory Peptides in Adults with Prediabetes: A Crossover Placebo-Controlled Randomized Trial.

BACKGROUND: The potential of a ketone monoester (β-hydroxybutyrate; KEβHB) supplement to rapidly mimic a state of nutritional ketosis offers a new therapeutic possibility for diabetes prevention and management. While KEβHB supplementation has a glucose-lowering effect in adults with obesity, its impact on glucose control in other insulin-resistant states is unknown. OBJECTIVES: The primary objective was to investigate the effect of KEβHB-supplemented drink on plasma glucose in adults with prediabetes. The secondary objective was to determine its impact on plasma glucoregulatory peptides. MET

Undiagnosed Diabetes and Prediabetes in Yemen: A Growing Public Health Crisis in the Shadow of Conflict.

Background and Objectives: Type 2 diabetes mellitus (T2DM) is increasing in prevalence worldwide, placing a substantial burden on healthcare systems, particularly in resource-limited settings. In Yemen, limited screening and diagnostic capacity contribute to delayed detection and management. Prediabetes, a reversible state of dysglycemia, carries significant cardiovascular risk and frequently progresses to diabetes. Early identification of both conditions is vital for prevention and public health planning. Materials and Methods: This cross-sectional study, conducted from July 2024 to May 2025

Enduring improvements in hepatic insulin sensitivity predict sustained remission of prediabetes during a 3-year lifestyle intervention: results from the PREVIEW multinational diabetes prevention trial.

BACKGROUND: Recent investigation advocates the use of prediabetes remission as a goal of diabetes prevention. We aimed to compare changes in metabolic markers in participants with and without sustained remission of prediabetes during a 3-year lifestyle intervention. METHODS: This post-hoc analysis used data from the PREVIEW trial, a 3-year, multinational, multicenter, randomized controlled trial aiming to examine the effects of lifestyle interventions on prevention of type 2 diabetes among high-risk adults. Adult participants with prediabetes and overweight/obesity underwent 8-weeks of rapid

Early effective intervention can significantly reduce all-cause mortality in prediabetic patients: a systematic review and meta-analysis based on high-quality clinical studies.

BACKGROUND: Reducing the occurrence of diabetes is considered a primary criterion for evaluating the effectiveness of interventions for prediabetes. There is existing evidence that early lifestyle-based interventions can significantly decrease the incidence of diabetes. However, whether effective interventions can reduce long-term outcomes in patients, including all-cause mortality, cardiovascular risks, and the occurrence of microvascular complications, which are the most concerning issues for both patients and clinicians, remains a subject of inconsistent research findings. And there is no d

Therapy insight: neurological complications of prediabetes.

Stroke and peripheral neuropathy are recognized neurological complications of diabetes. Increasing epidemiological evidence also implicates the prediabetic state of impaired glucose tolerance (IGT) as a risk factor for cerebrovascular events and peripheral neuropathy. Data linking IGT to cognitive decline or deficits, however, are less robust. IGT is one component of metabolic syndrome, together with central obesity, hypertension, hypertriglyceridemia and reduced HDL. Each component of metabolic syndrome is an independent risk factor for stroke, but hyperglycemia might be more important than o

Prediabetes as a critical stage for risk of dementia and stroke: evidence from the UK Biobank and Mendelian Randomization.

BACKGROUND AND AIMS: Type 2 diabetes (T2D) is a recognized risk factor for dementia and stroke, but whether risks begin during prediabetes remains unclear. We aimed to determine the risks of these neurological conditions among individuals with prediabetes and the glycemic thresholds at which risks emerge. METHODS: We studied 432,887 adults (mean age, 57 years; 55% women) from the UK Biobank who were free of dementia or stroke at baseline, categorized by glycemic status (normoglycemia, prediabetes, and T2D). Incident dementia, stroke, and Magnetic Resonance Imaging (MRI)-derived brain markers,

Acute effect of prednisone and deflazacort on glucose tolerance in prediabetic subjects.

The diabetogenic effect of deflazacort (DF), an oxazolinic synthetic corticosteroid, was studied in 12 healthy adult subjects with a positive family history of diabetes mellitus. Three oral glucose tests (oGTT) were performed at 9.00 a.m., after a 12 h fast, following randomized administration of Placebo (PL), or Deflazacort (DF 36 +36 mg) or Prednisone (PN 30 + 30 mg) 12 and 2 h before the test. Plasma glucose (BG), insulin (IRI), non-esterified fatty acids, (NEFA), total cholesterol (CL), HDL-cholesterol (HDL-CL) and triglycerides (TG) were measured at time 0, and BG, IRI, NEFA were again me

The diabetic paradox differs in patients with prediabetes and normal glucose regulation after colorectal surgery.

BACKGROUND: Patients without diabetes, composed of those with normal glucose regulation or prediabetes, experience a higher complication rate than patients with diabetes in response to surgical stress. It is unknown if excess complications occur equally among all patients without diabetes and if hyperglycemia or inflammatory indices are the best markers of postoperative complications. METHODS: In this retrospective review of medical records, postoperative infections, acute kidney injury (AKI), and composite in-hospital complications were measured after colorectal surgery in patients with varyi

A Review of Fetal Development in Pregnancies with Maternal Type 2 Diabetes Mellitus (T2DM)-Associated Hypothalamic-Pituitary-Adrenal (HPA) Axis Dysregulation: Possible Links to Pregestational Prediabetes.

Research has identified fetal risk factors for adult diseases, forming the basis for the Developmental Origins of Health and Disease (DOHaD) hypothesis. DOHaD suggests that maternal insults during pregnancy cause structural and functional changes in fetal organs, increasing the risk of chronic diseases like type 2 diabetes mellitus (T2DM) in adulthood. It is proposed that altered maternal physiology, such as increased glucocorticoid (GC) levels associated with a dysregulated hypothalamic-pituitary-adrenal (HPA) axis in maternal stress and T2DM during pregnancy, exposes the fetus to excess GC.

Medical Management of the Diabetic Patient.

Diabetes prevalence is increasing in the United States. As of 2017, 30.3 million people in the United States have diabetes (9.4% of population). Of these, an estimated 7.2 million persons are undiagnosed. Prediabetes, a condition that is associated with peripheral neuropathy and is a precurser to diabetes, affects 83.1 million or 33.9% of adults in the United States. Diabetes is most commonly diagnosed by fasting plasma glucose or by an A1C test. An oral glucose tolerance test can also be used, but its use is primarily restricted to research settings.

Association between periodontitis and the prevalence and prognosis of prediabetes: a population-based study.

BACKGROUND: Diagnosis and intervention of prediabetes is an emerging method for preventing diabetic progression and complications. Periodontitis has been reported to strongly correlate with the dysregulation of glucose metabolism. Nonetheless, the relationship between periodontal status and the prevalence of prediabetes as well as its prognosis remains elusive. This study aimed to investigate the association of periodontitis with the prevalence of prediabetes and furtherly explore the all-cause mortality of different periodontal status among patients with prediabetes. METHODS: The dateset fro

Is there any relationship between recurrent oral aphthous stomatitis and prediabetes?

Periodontal disease has been associated with glycaemia. Recurrent aphthous stomatitis (RAS) is one of the most common oral lesions and is characterized by painful recurrent oral ulcers. However, the exact cause and treatment of RAS is not yet well-known. Furthermore, there is still unknown the relationship between RAS and glycaemia. Prediabetes has shown to have role on worse metabolic profile. However, there is still no data on the relationship between prediabetes and clinical RAS. Therefore, the purpose of this hypothesis is to assess whether the prediabetes aggravate RAS.

The impact of exercise on skeletal muscle proteome of prediabetic subjects analyzed with data independent mass spectrometry.

Physical exercise of even a moderate intensity is beneficial in both the prevention of prediabetes and management of Type 2 diabetes mellitus, as skeletal muscle is a primary tissue responsible for glucose uptake. Exercise-evoked proteomic alterations in muscle of subjects with prediabetes are of great importance for the study of relationships between insulin resistance and exercise. Although data-dependent (DDA) proteomic analysis is a cornerstone of deep proteome profiling employed in the elucidation of skeletal muscle biology, data-independent (DIA) approaches gain popularity in the studies

[Characteristics of the hormonal regulation of glycemia in rats with varying alcoholic motivation].

Rats preferring ethanol differ from those preferring water in a lowered blood serum IRI level in the presence of the same level of glycemia. Ethanol preferring animals are characterized by raised function of the adrenal cortex revealed in an elevated II-oxycordicosteroid content in these glands and a lowered cholesterol level. The glucose tolerance test (4 g/kg by intragastric administration) shows a faster depletion of beta-cells of langerhans islets in ethanol preferring animals which is suggestive of a prediabetic state. A different sensitivity of the insular apparatus to glucose is noted i

Cost-effectiveness of implementing the National Health Service England Diabetes Prevention Programme in Canada: a modelling study.

UNLABELLED: The National Health Service (NHS) England's Diabetes Prevention Programme (DPP) is a cost-effective, publicly funded program of structured dietary and physical activity counselling that has been shown to reduce the incidence of progression from prediabetes to type 2 diabetes. We sought to examine the cost-effectiveness of implementing an NHS England-style DPP in Quebec and Ontario. METHODS: We compared usual care for people with prediabetes with a DPP using a cohort-state transition Markov model of 4 mutually exclusive health states (normal glucose tolerance, prediabetes, type 2 d
