Anesthesia and outcome after partial hepatectomy for adult-to-adult donor transplantation.

PURPOSE OF REVIEW: The worldwide adoption of live liver donors as a source of donor organs for transplantation has been curtailed by the very real risk of complications in otherwise healthy people. Our objective in this review is to outline the perioperative management of the live liver donor for adult-to-adult transplantation. RECENT FINDINGS: The incidence and severity of complications following live liver donation is extremely variable, and reporting needs to be standardized if we are to improve the perioperative management and outcomes. Agreed definitions would clarify the incidence and s

Will performance measurement lead to better patient outcomes? What are the roles of the National Quality Forum and Medical Specialty Societies?

Performance measures (PMs) are specified metrics by which a health-care provider's care can be compared with national benchmarks. The use of PMs is a key component of efforts to improve the quality and value of health care. The National Quality Forum (NQF) is the federally recognized endorser of PMs. From 2006 to 2009, the Quality Improvement Committee (QIC) of the American College of Chest Physicians engaged in the review of proposed PMs as a member of the NQF. This article provides a review of the QIC's experience with PMs and NQF membership and the lessons learned, an overview of the enhanc

Predictors of the application of exposure in vivo in the treatment of agoraphobia in an outpatient clinic: An exploratory approach.

OBJECTIVE: Although exposure in vivo is considered to be the most effective therapy component in the treatment of agoraphobia (AG), there is a remarkable lack of its application in psychotherapeutic routine care. We examined the severity of anxiety, psychological distress/comorbidity, therapeutic process/alliance, and sociodemographic status as potential predictors of in vivo exposure. METHOD: We applied correlational analyses and logistic regression analyses in a sample of N = 92 patients (main diagnosis AG) in an outpatient setting. RESULTS: Logistic regression analyses did not yield any s

Establishing a definition for a nurse-led clinic: structure, process, and outcome.

AIM: This paper reports a study to define a nurse-led clinic by exploring the domains of structure, process and outcome. BACKGROUND: Nurse clinics have been introduced as a measure to support intermediate care after the acute phase of disease. Previous studies have been mainly descriptive, or have only addressed a particular aspect of clinic service. METHODS: This exploratory study was conducted in two phases. In the first phase, nurses from 34 clinics were interviewed. In the second phase, 162 clinic sessions were observed, and 162 patients and 16 physicians were interviewed. The data were

Application of a theoretical model to evaluate COPD disease management.

BACKGROUND: Disease management programmes are heterogeneous in nature and often lack a theoretical basis. An evaluation model has been developed in which theoretically driven inquiries link disease management interventions to outcomes. The aim of this study is to methodically evaluate the impact of a disease management programme for patients with chronic obstructive pulmonary disease (COPD) on process, intermediate and final outcomes of care in a general practice setting. METHODS: A quasi-experimental research was performed with 12-months follow-up of 189 COPD patients in primary care in the

Improved cardiac arrest outcomes: as time goes by?

In a recent issue of Critical Care, Mally and colleagues reported outcomes from an observational study of out-of-hospital cardiac arrests in Slovenia. Multivariable analysis identified independent predictors for hospital discharge, including higher end-tidal carbon dioxide (ETCO2) levels, higher mean arterial pressure (MAP) and the recency (years) of the arrest. ETCO2 has been previously demonstrated to correlate with cardiac index, and predict successful resuscitation. Initial ETCO2 reflects the initial adequacy of resuscitation, and the ETCO2 on admission to hospital reflects a number of fac

A methodology for preparing 'ideal' treatment outlines in psychiatry. The Quality Assurance Project.

The methodology of preparing treatment process criteria for agoraphobia and the depressive disorders is described. To avoid bias three sources of information were used: the results of a meta-analysis of the treatment outcome literature, the views of a sample of practising psychiatrists, and the views of a small representative expert committee. When criteria for schizophrenia and the neuroses have been completed the quality assurance project should serve two main purposes: first as an aid to continuing and postgraduate education and second as a basis for clinical review criteria required by pee

Public service announcements: their effect on smoking.

The reasons for choosing the two-stage hierarchy of effects model in Table 5 and the process model of commitment in Table 6 to test are twofold. First, the two-stage hierarchy of effects model allows for reconsideration of the decision once it is made. Secondly, the process model of commitment seemed to be most appropriate in this context, since few commitment models have been specifically applied to marketing and the process model suggested offered the most complete description of commitment of those offered. Other models, such as the EKB model, would be more difficult to operationalize and w

Quality indicators for integrated care of dysphagia in hospital settings.

This article proposes a panel of quality indicators for the management of swallowing rehabilitation (SR) therapy in a hospital setting. There were four stages in developing these indicators: identifying procedures to be managed; generating indicators and standardizing data collection; identifying the correlation among indicators; and formulating the panel of indicators. The following 12 quality indicators were developed: swallowing evaluation index; individual care index; speech-language pathologist (SLP) care index; number of assisted patients index; severity rate; swallowing diagnosis rate p

Validity and the OSCE.

In preparation for a celebration of '30 years of OSCEs' held during the 2002 meeting of the Association for Medical Education in Europe (AMEE), I was asked to discuss the question, 'Are OSCEs valid to assess competence?". My first instinct was to review work undertaken in ay countries by famous researchers such as Harden, Colliver, Rothman, van der Vleuten, Stillman, Tamblyn and others who have studied and written about the validity of OSCEs. I could then have reviewed the extensive literature produced in Canada by the Medical Council of Canada and in the United States by the Education Commiss

Laboratory-related outcomes in healthcare.

Outcomes studies, long common on the therapeutic side of medicine, are appearing in the diagnostic arena. Outcomes can be defined as results of medical interventions (therapies or tests) in terms of health or cost. The studies of outcomes are important because funding for medical interventions increasingly depends on them; a major accrediting agency even defines "quality" entirely in terms of outcomes. The study of laboratory-related outcomes is complex. Multiple steps occur between testing and outcomes, physicians act unpredictably on test results, and outcomes studies have high costs relativ

Why, why, why?: Reason-giving and rumination as predictors of response to activation- and insight-oriented treatment rationales.

This study examines the relationships among the reasons a person offers for depression, the tendency to ruminate in response to depression, and reactions to activation-oriented (AO) or insight-oriented (IO) treatment rationales. Adults from the community (N=51) completed self-report measures of reason-giving and rumination and rated the credibility of, and personal reactions to, AO and IO rationales presented in written and videotape formats. Participants who gave more reasons for depression also tended to ruminate more in response to depressed mood. Reason-giving and rumination predicted lowe

Comprehensive medical care for victims of sexual assault: a survey of Illinois hospital emergency departments.

BACKGROUND: Sexual assault is a major public health problem affecting 17 million women in the United States. Hospital emergency departments (EDs) are often the primary point of care for these women. We propose the concept of "comprehensive medical care management" (CMCM) at the primary point of care for victims of sexual assault. The objectives of this study were (a) to describe medical care services provided to sexual assault victims presenting at the EDs in Illinois hospitals, and (b) to identify the percentage of hospitals that always provide all 10 elements of CMCM. STUDY DESIGN: This is

The challenge of adopting the health assessment when implementing antenatal and postnatal Group Care: identifying and understanding cross-country modifications and corresponding strategies to enable its adoption.

BACKGROUND: Antenatal and postnatal Group Care, based on the Centering Healthcare model, relies on three core components: health assessment, interactive learning and community building. The health assessment consists of self-assessments conducted by the participants and one-to-one medical check-ups conducted by the healthcare provider. Research shows that this component can be challenging within existing health care systems. This study aimed to investigate the modifications and corresponding strategies applied to adopt the health assessment during Group Care implementation. METHODS: A qualita

[Quality of hospital care in Germany: analysis of the trend between 2004 and 2008 from external quality assurance data].

BACKGROUND AND OBJECTIVES: Introducing fixed fees for inpatient hospital care has raised fears of a negative influence on treatment quality. To address these concerns, an external quality assurance was implemented. The results of the quality indicators should indicate changes in the quality of inpatient care between 2004 and 2008. METHODS: On the basis of the evaluation data delivered by the Federal Quality Assurance Agency, quality indicators with similar results of at least 3 subsequent years were included. A trend was identified with linear regression as well as with a constant change in t

Satisfaction and anxiety for women during investigation of an abnormal screening mammogram.

Five community-designed pilot projects were undertaken to reduce the time to diagnosis following an abnormal mammogram. One thousand five hundred and seventy-eight women with abnormal mammograms completed a self-administered questionnaire (71% response) which inquired about experiences and satisfaction with time delay from screening to diagnosis, the information received and support given by health professionals, and measures of anxiety and stress during this time interval. Four of the five pilot project initiatives shortened the time interval from screening to diagnosis, the greatest improvem

An ethnographic study of the meaning of continuity of care in mental health services.

OBJECTIVE: As a step toward developing a standardized measure of continuity of care for mental health services research, the study sought to identify the interpersonal processes of giving and receiving day-to-day services through which individual providers create experiences of continuity for consumers. METHODS: Ethnographic methods of field observation and open-ended interviewing were used to investigate the meaning of continuity of care. Observations were carried out at two community mental health centers and a psychiatric emergency evaluation unit in Boston. Sixteen recipients and 16 provi
