Index of contractile asymmetry improves patient selection for CRT: a proof-of-concept study.

BACKGROUND: Nearly one-third of heart failure (HF) patients do not respond to cardiac resynchronization therapy (CRT) despite having left bundle branch block (LBBB). The aim of the study was to investigate a novel method of quantifying left ventricular (LV) contractile asymmetry in HF. METHODS: Patients with HF and LBBB undergoing CRT (n = 89, 37.1% females, 68 ± 9 years, ischemic etiology in 61%, LV ejection fraction 27.1 ± 7.1%) were analyzed. LV longitudinal systolic strain rate values were extracted from curved anatomical M-mode plots of standard long-axis 2D-echocardiography images and c

Gastrointestinal safety of triflusal solution in healthy volunteers: a proof of concept endoscopic study.

PURPOSE: Triflusal is an antiplatelet agent that irreversibly acetylates cyclooxygenase isoform 1 (COX-1) and therefore inhibits thromboxane biosynthesis. It was initially marketed as capsules containing 300 mg of active substance. In 2006 a new 600 mg (10 ml) oral solution form of triflusal was authorized in Spain. The primary aim of this study was to compare the gastrointestinal safety of the new triflusal oral solution with triflusal capsules in healthy volunteers. METHODS: Sixty healthy subjects were randomly assigned, in a 2.5:2.5: 1 ratio, into one of three groups, with 25 subjects rece

Histopathological characterization of primary cutaneous melanoma using infrared microimaging: a proof-of-concept study.

BACKGROUND: The diagnosis of malignant melanoma is based upon the histological evaluation of the lesion. As such, the morphological interpretation relies on the expertise of a dermatopathologist. Infrared microimaging is emerging as a new powerful tool to investigate tissue biochemistry. Infrared spectra probe the biochemical constitution of the sample and are real tissue-specific spectroscopic fingerprints. OBJECTIVES: To assess the potential of infrared microimaging to aid in the analysis of tissue sections from primary cutaneous melanomas. METHODS: Ten samples of melanoma sections from th

Proof-of-concept study: longitudinal immune biomarker signatures in treatment-naïve IgG4-related disease.

Immunoglobulin G4-related disease (IgG4-RD) is a chronic, systemic, immune-mediated fibroinflammatory disorder characterized by heterogeneous organ involvement and variable clinical expression. Reliable biomarkers are needed to quantify inflammatory activity, distinguish active inflammation from irreversible damage, and monitor treatment response. While serum IgG4 is a recognized marker of disease burden in proliferative phenotypes, its diagnostic specificity is limited and its correlation with disease activity is inconsistent across the clinical spectrum, particularly in predominantly fibroti

NKG2D-based CAR-T cells for synovial sarcoma: A preclinical proof-of-concept study.

The prognosis for advanced synovial sarcoma remains poor, with limited therapeutic options. Recently, cloned T cell receptor (TCR)-engineered T cell therapy has shown promising results in patients with advanced synovial sarcoma; however, the number of patients eligible for this therapy is limited owing to human leukocyte antigen (HLA) restriction. To solve this problem, we focused on chimeric antigen receptor (CAR)-T cells that target ligands of NKG2D, an activating receptor of natural killer cells. Because NKG2D recognizes a family of eight ligands, we hypothesized that NKG2D-based CARs could

Walking football for people with chronic breathlessness: A mixed-methods proof of concept study.

IntroductionPulmonary rehabilitation (PR) provides substantial benefits, but these are rarely sustained. Walking football (WF), with its slower pace and widespread availability, may offer an exercise-maintenance option. This proof-of-concept study aimed to understand the potential of WF as an exercise-maintenance intervention following PR.MethodsA mixed-methods, prospective observational design was used. Patients completing PR at two NHS trusts were invited to attend weekly 2-hour WF sessions for 6 weeks. Feasibility outcomes included recruitment, adherence, and safety. An ethnographic approac

Reframing physical activity barriers in individuals with multiple sclerosis: a proof-of-concept study.

PURPOSE: To explore how a brief reframing intervention influences physical activity (PA) behaviors and cognitive biases in individuals with Multiple Sclerosis (MS). MATERIALS AND METHODS: In this mixed-method, single-arm study, 22 individuals with MS received a one-on-one, ∼15-min reframing session targeting negative exercise-related thinking patterns. Participants completed surveys at four time points (pre-, post-, 1-week, and 4-weeks post-intervention) assessing cognitive errors and PA. A follow-up interview at 1 week explored individual experiences and perceived changes. Interview data wer

Proof-of-concept study: Remote capillary blood collection for hCG analysis in early pregnancy.

INTRODUCTION: Capillary blood collection, a technique traditionally used in diabetes care, shows promise for many applications including pregnancy monitoring. Serial measurement of serum human Chorionic Gonadotrophin (hCG) is frequently necessary for managing early pregnancy, including molar pregnancy, requiring multiple visits to a maternity hospital for blood collection by venepuncture. This proof-of-concept study aimed to assess the clinical performance and user acceptability of capillary blood samples collected remotely, as an alternative to venous blood for hCG measurement. METHODS: Wome

Auditory spatial cueing reduces neglect after right-hemispheric stroke: A proof of concept study.

Spatial neglect after right-hemispheric stroke, characterized by the failure to attend or respond to the contralesional space, is a strong negative outcome predictor. Neglect is a supramodal syndrome affecting not only the visual but also the auditory modality. Preliminary studies used this audio-visual cross-modal effect to show short-lasting effects on attention towards the neglected space. The aim of the present study was to introduce a new technique of auditory stimulation combining the unspecific effect of music (i.e., patients choose their preferred music) with the effects of auditory sp

Brain fog and non-coeliac gluten sensitivity: Proof of concept brain MRI pilot study.

AIMS: Non-Coeliac Gluten Sensitivity (NCGS) is poorly understood, particularly in terms of its neurological outcomes. We initially conducted a prospective postal survey to investigate its neurological presentation and symptom course. Results from this then motivated a follow-up pilot study utilising brain MRI to characterise potential diagnostic biomarkers for future research. METHODS: Patients with NCGS were recruited from a specialist centre and completed a prospective postal questionnaire (N = 125). This summarised symptoms experienced, their severity and their course. Onset time was compa

Automated pupillometry to detect command following in neurological patients: a proof-of-concept study.

BACKGROUND: Levels of consciousness in patients with acute and chronic brain injury are notoriously underestimated. Paradigms based on electroencephalography (EEG) and functional magnetic resonance imaging (fMRI) may detect covert consciousness in clinically unresponsive patients but are subject to logistical challenges and the need for advanced statistical analysis. METHODS: To assess the feasibility of automated pupillometry for the detection of command following, we enrolled 20 healthy volunteers and 48 patients with a wide range of neurological disorders, including seven patients in the i

Cardiogoniometry can predict positive response to cardiac resynchronization therapy - A proof of concept study.

BACKGROUND: According to American Heart Association guidelines, QRS duration and morphology are used to select patients for cardiac resynchronization therapy (CRT). But still there are some patients who are not responding to this device. We investigated whether the Cardiogoniometry (CGM) as a three-dimensional vectorcardiogram method can improve patient selection. METHODS: Echocardiography and CGM were performed for 25 consecutive patients with Left bundle branch morphology who were candidate for CRT implantation and were in sinus rhythm. Patients re-evaluated by echocardiography after 6 mont

Imagery rescripting in individuals with binge-eating behavior: An experimental proof-of-concept study.

OBJECTIVE: Mental imagery is more strongly related to emotions than verbal cognitions. Binge eating is associated with dysfunctional emotional regulation. However, cognitive therapy techniques have focused on verbal cognitions. This proof-of-concept study compares a traditional cognitive therapy technique, cognitive restructuring (CR), with imagery rescripting (IR) in individuals with binge-eating disorder or bulimia nervosa. METHOD: Thirty-six participants were asked to recall an idiosyncratic mental image of social rejection. They were then randomly assigned to a single session of CR or IR.

Triple-Gene Therapy for Stroke: A Proof-of-Concept in Vivo Study in Rats.

Natural brain repair after stroke is extremely limited, and current therapeutic options are even more scarce with no clinical break-through in sight. Despite restricted regeneration in the central nervous system, we have previously proved that human umbilical cord blood mono-nuclear cells (UCB-MC) transduced with adenoviral vectors carrying genes encoding vascular endothelial growth factor (VEGF), glial cell-derived neurotrophic factor (GDNF), and neural cell adhesion molecule (NCAM) successfully rescued neurons in amyotrophic lateral sclerosis and spinal cord injury.
