Infections of electrophysiologic cardiac devices.

Infections related to electrophysiologic cardiac device placement are increasing dramatically and are associated with adverse outcomes. With the US population aging and living longer, cardiac devices are utilized in an older population with significant comorbidities, making the threat from infectious complications significant. Electrophysiologic cardiac devices typically include an implanted pulse generator and an intravascular lead. Infection may occur at any component of the device, resulting in varied presentations. Bacteremia in cardiac device recipients may reflect primary generator pocke

Staphylococcus aureus bacteremia in patients with prosthetic devices: costs and outcomes.

PURPOSE: Although Staphylococcus aureus is a leading cause of nosocomial infection, little is known about the impact of S. aureus bacteremia on patients with prosthetic devices. This investigation sought to define the clinical outcome, health care resource use, and infection-associated costs of S. aureus bacteremia in patients with prostheses. SUBJECTS AND METHODS: All hospitalized patients with a prosthetic device and S. aureus bacteremia during the 96-month study period were identified prospectively. Clinical data were collected at the time of hospitalization. Data regarding infection-relat

Debridement, antibiotics and implant retention: are all approaches to periprosthetic joint infection equal? Same procedures, different outcomes.

BACKGROUND: The reported success rate of debridement, antibiotics, and implant retention (DAIR) for periprosthetic joint infection varies considerably between studies, despite institutions reporting use of the same procedure. In this narrative review, we aim to delineate the differences between the various DAIR approaches and highlight why they influence the outcome. OBJECTIVES: We performed a PubMed and internet search investigating the different approaches for DAIR and their associated outcomes. SOURCES: Twenty-two studies with defined infection criteria, consisting of 50 or more periprost

Role of ureteral stent material and coating to prevent ureteral stent related issue: A systematic review and meta analysis.

INTRODUCTION: Ureteral stents require materials that balance bulk and surface properties. Achieving both can be challenging, as ideal bulk properties may not align with optimal surface properties. Thus, researching coatings and biomanufacturing methods for ideal materials is essential. METHODS: A systematic review and meta-analysis, following PRISMA Guidelines, involved literature searches across five databases: PubMed, Scopus, Embase, ClinicalKey, and Cochrane. From 417 screened articles, eight studies were deemed eligible for qualitative and quantitative analysis. The selected articles unde

Corynebacterium freneyi as a cause of early prosthetic valve endocarditis.

Infective endocarditis (IE) is associated with severe complications and a high mortality rate. Identification of the causative pathogen is crucial to optimise treatment. We present a case of prosthetic valve endocarditis caused by Corynebacterium freneyi, a very rare cause of human infection and not previously reported as a cause of IE. Despite proper antibiotic therapy, the patient eventually needed surgery after progression of the infection. After surgery, he quickly recovered without evidence of relapse during an 8-month follow-up period. This report highlights critical decision making in a

Risk factors for periprosthetic joint infection after total joint arthroplasty: a systematic review and meta-analysis.

Many of the mooted risk factors associated with periprosthetic joint infection (PJI) after total joint arthroplasty (TJA) remain controversial and are not well characterized. Online and manual searches were performed using Medline, Embase, Chinese National Knowledge Infrastructure and the Cochrane Central Database from January 1980 to March 2014). For inclusion, studies had to meet the quality assessment criteria of the CONSORT statement, and be concerned with evaluation of risk factors for PJI after TJA. Two reviewers extracted the relevant data independently and any disagreements were resolv

Prolonged survival and risk factors for reintervention after inflatable penile prosthesis implantation.

BACKGROUND: Inflatable Penile Prosthesis (IPP) is the treatment for erectile dysfunction (ED) refractory to pharmacological therapies. Long-term data on factors associated with prosthesis survival remain unclear. This study aimed to analyze the long-term survival of penile prostheses and identify risk factors associated with survival without reintervention. METHODS: This is a retrospective, single-center study of patients who underwent IPP implantation between January 2014 and December 2022. Preoperative data related to the patient and the etiology of ED, as well as perioperative data, were c

[Delay in accurate diagnostics during the Covid-19 pandemic: a case of fatal MSSA disseminated infection in an elder patient].

Periprosthetic joint infections at an advanced age can lead to significant diagnostic and therapeutic challenges, mostly related to an atypical form of presentation in this population. We describe in this case the delayed diagnostic process of a disseminated invasive Staphylococcus aureus infection in a frail patient with a total hip prosthesis. The detection of Staphylococcus aureus in a urine sample was a supplementary hint towards the possible existence of a Staphylococcus aureus bacteraemia. The course of this case shows that teleconsultation and ambulatory based diagnostics are not approp

One- vs. Two-Stage Exchange for Chronic Hip Periprosthetic Joint Infection: A Framework Based on Patient Treatment Burden and Institutional Capability.

» Recent evidence demonstrates equivalent infection-eradication rates between one-stage revision (OSR) and two-stage revision (TSR) in chronic hip periprosthetic infection at experienced centers. » The decision between OSR and TSR should account for patient treatment burden, which includes functional limitation, spacer complications, medical morbidity, psychological burden, and cost. » Traditional OSR selection criteria may identify patients with greater overall surgical complexity rather than reliably unfavorably biasing reinfection risk. » Standardized institutional protocols are critical fo

Periprosthetic joint infection in patients with multiple arthroplasties.

Despite numerous studies focusing on periprosthetic joint infections (PJIs), there are no robust data on the risk factors and timing of metachronous infections. Metachronous PJIs are PJIs that can arise in the same or other artificial joints after a period of time, in patients who have previously had PJI. Between January 2010 and December 2018, 661 patients with multiple joint prostheses in situ were treated for PJI at our institution. Of these, 73 patients (11%) developed a metachronous PJI (periprosthetic infection in patients who have previously had PJI in another joint, after a lag period)

Distal Femoral Replacement and Periprosthetic Joint Infection After Non-Oncological Reconstruction: A Retrospective Analysis.

BACKGROUND: Distal femoral replacement (DFR) is commonly used to manage massive bone loss around the knee arising from aseptic loosening, periprosthetic joint infection (PJI), and distal femoral fractures. A number of studies report the outcome of DFR with considerable variation in long-term survivorship. This study investigated the outcome of DFR for patients with aseptic failures, fractures, and PJI. METHODS: A retrospective review of 182 patients who underwent DFR for non-oncological indications between 2002 and 2018 was conducted. Data collected included the following: indication, postope

An alternative molecular approach for rapid and specific detection of clinically relevant bacteria causing prosthetic joint infections with bacteriophage K.

Prosthetic joint infections (PJI) represent the most serious cause of prosthetic joint loosening, with high impact on patient life and health economics. Although not entirely reliable, the cultivation of intraoperative prosthetic tissue or synovial fluid remains the gold standard for determining the cause of PJI. Therefore, molecular methods are increasingly being introduced. The aim of this study was to optimize and assess an alternative molecular approach with the use of bacteriophage K for more rapid and specific detection of staphylococci in sonicate fluid (SF) of PJI. The best results wit

[Research advances of complications regarding temporomandibular joint prosthesis replacement].

Replacement of the temporomandibular joint (RTJ) has undergone decades of technological improvements and is now an important treatment for end-stage temporomandibular joint (TMJ) disease. The main complications of RTJ include aseptic loosening, heterotopic ossification, postsurgical infection, hypersensitivity reaction, prosthesis loosening or malposition, nerve injury and severe bleeding. Improvement in clinical technique is now the key to resolving complications. In the future, improvements in prosthetic materials may be an important development for newer iterations. With the development and

Epidemiology and microbiology of prosthetic joint infections: a nine-year, single-center experience in Pavia, Northern Italy.

BACKGROUND: Prosthetic joint infections (PJIs) are a growing matter of concern due to their economic and social burden on health systems. In Italy, surgical data on PJIs are available in a national registry, but microbiological data are still scarce. MATERIALS AND METHODS: We performed a retrospective study at a single center with records of patients treated for primary PJIs of knee or hip from January 1, 2011, to May 30, 2018. Patients with infections of osteosynthesis means and external devices were excluded, as well as PJI recurrences and polytrauma patients. Infections were diagnosed acco
