Which is better? One or two? A randomized clinical trial of single-plate versus double-plate Molteno implantation for glaucomas in aphakia and pseudophakia.

PURPOSE: Previous studies have suggested that primary double-plate Molteno implantation may be beneficial. Therefore, the authors performed a randomized clinical trial to evaluate the relative effectiveness and safety of single- versus double-plate Molteno implantation. METHODS: From March 1988 to February 1990, 132 patients who underwent Molteno implantation for medically uncontrollable non-neovascular glaucomas in aphakia or pseudophakia were randomly assigned to receive either single- or double-plate implants. RESULTS: The 1- and 2-year life-table success rates (success [survival] defined

A comparison of grating visual acuity, strabismus, and reoperation outcomes among children with aphakia and pseudophakia after unilateral cataract surgery during the first six months of life.

PURPOSE: The method of correcting aphakia after unilateral cataract extraction during infancy is controversial. Some authorities advocate correction with an intraocular lens (IOL) whereas others advocate correction with a contact lens (CL). We compared grating visual acuity, alignment, and reoperative outcomes in age-matched children treated with these 2 modalities at 5 clinical centers. METHODS: Twenty-five infants born in 1997 or 1998 with a dense unilateral congenital cataract who had cataract surgery coupled with (IOL group, n = 12) or without (CL group, n = 13) primary IOL implantation w

Minimal myopic shift in pseudophakic versus aphakic pediatric cataract patients.

PURPOSE: To evaluate postoperative refractive changes in aphakic and pseudophakic children in a comparative case series. METHODS: The medical records of pediatric patients with aphakia and pseudophakia were reviewed retrospectively. The mean change in refractive error from one postoperative examination to the next was calculated at each age to give aggregate curves of postoperative refractive change for each group. RESULTS: A total of 233 aphakic and 92 pseudophakic eyes were studied. The median age at the time of surgery was 0.8 years (range, 0.0-16 years) for patients with aphakia and 7.3

The Effect of Age-Related Macular Degeneration on Polarization Pattern Perception.

PURPOSE: The purpose of this study was to determine if a battery of polarization-modulated stimuli, quantified as a single metric, is effective in identifying macular disease in the presence/absence of cataract or pseudophakia. METHODS: Using a modified liquid crystal display, polarization pattern perception (PPP) for a formulated battery of geometric and logMAR stimuli was evaluated in participants that had either no eye pathology (healthy participants) or were grouped according to the presence of cataract, pseudophakia, and/or age-related macular degeneration (AMD). PPP was quantified as re

Rate of axial growth after congenital cataract surgery.

PURPOSE: To evaluate the rate of axial growth after congenital cataract surgery. DESIGN: Prospective observational case series. METHODS: Rate of axial growth of 158 eyes (79 children < 10 years) undergoing surgery was correlated with age at surgery, laterality, and visual axis obscuration. After measuring axial length (AL) at each follow-up, the mean AL was calculated, adding the AL of all eyes divided by their total number. Rate of axial growth is the percentage difference between preoperative mean AL and mean AL at last follow up. The temporal profile of RAG is the difference between two c

ESCRS Binkhorst lecture 2002: Pseudophakic preservative maculopathy.

Many antiglaucoma eyedrops are reported to cause cystoid macular edema (CME) in aphakia and pseudophakia. We review 4 clinical and laboratory studies that compare the incidence of CME in early postoperative pseudophakia in eyes that received preserved latanoprost and timolol, nonpreserved timolol, and the preserved and nonpreserved vehicle for these drugs and looked at the morphological damage to cells and the changes in the indicators of cytokine and prostaglandin (PG) synthesis caused by latanoprost and timolol and the preservative benzalkonium chloride. Based on the findings of these studie

[Complications and after care in aphakic and pseudophakic patients].

The aim of every cataract operation is optical rehabilitation. With the greater social demands made today by aphakic and pseudophakic patients, the recognition and treatment of diseases with early or late onset after cataract extraction assumes greater importance. The early aims are rapid mobilization, complete absence of irritation, and normalization of pressure. Regular administration of corticosteroids, essential in the post-operative treatment of pseudophakia, may mask a bacterial or mycotic infection. The sole indication for removal of an implant is the uveitis-glaucoma-hyphema syndrome.
