Emotional Labour in Psychotherapy: A Scoping Review of How Therapists Regulate Emotions in Clinical Practice.

Emotional labour is an integral yet conceptually underexamined feature of psychotherapeutic work. This scoping review mapped how emotional labour has been conceptualized and operationalized among psychotherapists, along with the associated outcomes and factors linked to greater emotional burden, and potential protective resources. Following JBI and PRISMA-ScR guidance, PsycINFO, Web of Science and Scopus were searched in August 2025 and updated in April 2026 for peer-reviewed studies on emotional labour among professionals delivering psychotherapy published in English or Spanish. Two reviewers

Cooperation in psychotherapy increases metacognitive abilities: a single-case study.

INTRODUCTION: Both clinical observations and empirical data suggest that the ability to think about the mental states of themselves and others (i.e., metacognition) is a crucial factor strongly associated to the outcome of individual psychotherapies. Although it has been hypothesized that the activation of cooperation between patient and psychotherapist within psychotherapy sessions may increase metacognitive abilities, few data is still available to support this hypothesis. METHODS: We explore the association between cooperation of patient and psychotherapist and the modifications of metacog

[Psychotherapeutically Competent Child Guidance-General Framework and Legal Foundations].

Assistances in education are pedagogical and therapeutic benefits connected with it. For child guidance as an assistance in education therefore psychotherapeutic competence is constitutive. The article describes the professional and legal framework of a psychotherapy in child guidance. It specifies on the one hand the composition of the multi disciplinary team (including child and youth psychotherapists and psychological psychotherapists) and the additional therapeutic qualifications acquired by the skilled employees. On the other hand it traces the judicial border between a medical psychother

Routine Data Analyses to Compare Outpatient Depression Treatment Regimens.

BACKGROUND: Routine data analyses are becoming increasingly important for health policy decision making. However such databases often vary in data quality, completeness and accessibility. The aim of this study is to describe the quality of a large outpatient healthcare database, the process of data extraction and to give a brief overview of data-structure with focusing on provider-type and disease severity in an example of the treatment of depressive disorders. METHOD: The quality of the database is described and diagnosis rates of depression in outpatient care (ICD-10 diagnoses F32/33) in re

"Normal" vs. "difficult" cases with eating disorders: the therapists' perspective.

INTRODUCTION: Patients with an eating disorder (ED) are considered difficult to treat. We aimed to identify the characteristics of patients with an ED, which are considered especially challenging to treat from the therapist's perspective. METHODS: Psychotherapists with experience in ED treatment were asked to describe a "normal" as well as a "difficult" case they treated lately, using an online-survey. They described their patients by filling in a list of symptoms and their severity, on personality traits and functioning as well as a list of possible therapeutic challenges experienced over th

Using natural language processing to assess proxy measures of therapeutic alliance across suicide risk tiers.

Therapeutic alliance is a core predictor of psychotherapeutic outcome, in general, and suicide prevention, specifically. Although clinical literature suggests that maintaining positive alliance with high-suicide-risk patients can be particularly challenging, few studies evaluate this phenomenon, primarily due to alliance measurement difficulties. Our study derives proxy alliance metrics using natural language processing and models differences across classified suicide risk strata and suicide decedent status. We first identified all United States Department of Veterans Affairs (VA) psychotherap

A Multimethod Psychometric Approach to Analyse the Profile of Mental Functioning ‒ M Axis of the Psychodynamic Diagnostic Manual, 2nd Edition (Pdm-2): A Focus on Cluster B Personality Disorders.

OBJECTIVE: Within the realm of international diagnostic systems, the Psychodynamic Diagnostic Manual - 2nd edition (PDM-2) stands out as a particularly effective instrument for evaluating mental functioning based on a psychodynamic perspective. This research aimed to explore the psychometric properties of the M Axis and to investigate the dynamics among its 12 capacities for individuals with Cluster B personality disorders. METHOD: A sample of 285 psychotherapists who regularly use the PDM-2 in their professional practice was involved in this research. The collected data were analysed using a

Integrating Perspectives of Patients, Their Significant Others, and Therapists on Recovery From Personality Disorders: A Mixed-Methods Study.

Patients with personality disorders (PDs) often struggle in life after treatment, even when symptomatic remission is achieved. The question arises of whether current outcome measurements are relevant for patients and people around them. To address this, we used concept mapping, a mixed-method approach, to construct a framework of recovery from the perspectives of patients, their significant others, and therapists (n = 39). Participants brainstormed what recovery involved and then prioritied and clustered these ideas (n = 61). Data were analyzed using multidimensional scaling and hierarchical c

Understanding therapists' use of routine outcome monitoring in a low-embedded context. A qualitative study in Italian public health services.

OBJECTIVE: Routine Outcome Monitoring (ROM) improves psychotherapy outcomes, yet its integration into routine clinical practice remains limited in settings where it is not structurally embedded. This study examines how psychotherapists engage with and make sense of ROM in such low-implementation contexts, using Italian public mental health services as an empirical case. METHODS: Fifteen psychotherapists who used ROM for 12 to 36 months within a larger clinical trial were interviewed about their experiences. Interviews were audio-recorded, transcribed, and analyzed using Constructivist Grounde

Psychotherapeutic interventions in melanoma patients.

OBJECTIVES: The need for and the effectiveness of psychotherapy in the treatment of patients with skin cancer have been shown. However, only insufficient data are available to answer the questions as to what kind of psychotherapy is useful and how psychotherapeutic interventions should be designed. METHODS: During the past 7 years we performed crisis intervention, focal therapy and longer-lasting psychodynamically orientated psychotherapy within a consultation liaison service with the Department of Dermatology of Innsbruck University Hospital. We investigated the type of interventions using q

Identifying and integrating helpful and harmful religious beliefs into psychotherapy.

The 2 main roles of the psychotherapist involve identifying and understanding the client's problems/strengths and treating problems. Suggestions are offered to guide addressing or avoiding religious beliefs in both roles. Types of religious beliefs that contribute to distress, particularly for youth, are identified and treatment options are offered.

Between sessions: How challenging and satisfying moments shape therapists' inner worlds-A comparative interpretative phenomenological analysis study of novice and experienced therapists.

The primary aim of this study was to provide an in-depth understanding of psychotherapists' experiences between sessions, following both challenging and satisfying sessions. A qualitative research design, interpretative phenomenological analysis, was employed. The study included seven novice and seven experienced psychotherapists. Semistructured interviews were conducted with all participants, and the data were analyzed according to interpretative phenomenological analysis guidelines. The findings are organized around two overarching domains: challenging and satisfying moments as experienced b

Therapists' experiences with non-verbal communication in videopsychotherapy: a qualitative interview study.

BACKGROUND: Videopsychotherapy (VPT) has gained in importance since the Corona pandemic. As non-verbal communication (NVC) differs from the face-to-face (FtF) therapy, its practical assessment is crucial for long-term quality improvement. OBJECTIVE: The aim of the study is to investigate how psychotherapists perceive specific aspects of NVC in VPT, in order to derive practical insights and lay the groundwork for future research approaches to improve treatment quality. METHODS: Semi-structured expert interviews were conducted with twelve psychotherapists in Germany. The interviews took place

An empirical study of countertransference reactions toward patients with personality disorders.

OBJECTIVE: The study examined to what extent patients with cluster A + B personality disorders (PDs) evoked other countertransference reactions among psychotherapists compared with patients with cluster C PDs as well as the relationship between the different countertransference reactions and outcome. METHODS: A total of 11 therapists at the Department for Personality Psychiatry, Ullevaal University Hospital, Oslo, Norway, filled out the Feeling Word Checklist-58 (FWC-58), 2 weeks after admission and 2 weeks before discharge, for 71 patients admitted to the day treatment program. [The concept of behavioral addiction and limits of the term addiction].

The numbers of persons with a prevalence for behavioral addiction are rising especially among the young. Psychiatrists and psychotherapists are still awaiting indications for diagnostic classification and treatment approaches. We discuss the nosological aspects and suggest categorizing gambling and excessive computer and internet use as behavioral addictions. In specific cases the addiction model can also be applied for excessive sexual behavior, compulsive buying and obesity.

[Individual therapy as a special form of complex psychotherapy].

A combination of several psychotherapeutic methods will be meaningfull only if they are tuned to, and supplement, each other. Indiscriminate combinations may produce harmful effects. The choice of psychotherapeutic methods depends upon the objective that the psychotherapist desires to accomplish. In psychotherapy it is essential that all actions taken be governed by plans worked out in accordance with the particular objectives to be accomplished.

The Therapist Evaluation Inventory: evidence for an unidimensional scale.

The internal structure of the Therapist Evaluation Inventory (TEI) was investigated with data gathered from therapy sessions conducted by 16 individual psychotherapists with 292 psychiatrically disturbed adolescent inpatients. A principal components analysis (Varimax rotation) indicated a single dimension that accounted for 70.39% of scale variance. The findings did not corroborate a factor structure consistent with the a priori scales of the TEI.

Clinical decisions on diagnosis and indication for treatment in stereotypical vs. non-stereotypical cases of eating disorders.

BACKGROUND: Prevalence of non-stereotypical Eating Disorders (EDs; e.g., characterized by normal weight, subjective binge eating) is much higher compared with stereotypical EDs (e.g., characterized by under- or overweight, objective binge eating), but treatment rates are much lower. Despite the similar subjective burden, individuals with non-stereotypical symptom presentation are often misperceived and do not receive adequate treatment. METHODS: Using case vignettes, we investigate not only whether non-stereotypical cases of ED are judged differently by a sample of N = 308 psychotherapists in
