Finding a voice: a journey towards being assertive.

This is a case study of two depressed women who were able to benefit from a 16-week course of Interpersonal Psychotherapy. This well-established brief psychotherapy has become increasingly available throughout the UK. The focus of this biopsychosocial approach that is on change within the patients interpersonal world means a more active goal-focused approach that addresses symptomatic and emotional concerns using the individuals narrative. In both cases, there was a significant reported change in their depression, initial presenting problems, understanding of their difficulties and interperson

Distinct positive and negative dimensions of psychotherapy treatment expectations across three independent samples.

Treatment expectations are central to psychotherapy outcomes but are typically assessed as a unidimensional construct focusing on anticipated improvement. This study investigated whether expectations of improvement, worsening, and side effects, as measured by the Generic Rating Scale for Previous Treatment Experiences, Treatment Expectations, and Treatment Effects (GEEE), represent distinct dimensions in psychotherapy. We examined three samples differing in treatment context and symptom burden: patients currently in psychotherapy (N = 102), prescreened patients awaiting psychotherapy (N = 83),

Follow-up effects of a brief Good Psychiatric Management treatment for borderline personality disorder.

BACKGROUND: Borderline personality disorder (BPD) is among the most frequent, debilitating, and lethal mental conditions. Treatment for patients with BPD involves structured psychotherapy, but such treatment is often not readily available due to imbalances in supply and demand for care. Brief treatments that are available as first-line interventions may address this delay in care. No controlled study has assessed the effectiveness of brief psychiatric interventions, such as 10 sessions of Good Psychiatric Management (GPM) over 12 months follow-up, in comparison with treatment as usual. Whereas

Effectiveness of brief group transdiagnostic therapy for emotional disorders in primary care: A randomized controlled trial identifying predictors of outcome.

AbstractBrief transdiagnostic psychotherapy is a possible approach for emotional disorders in primary care. The objective of the present randomized controlled trial was to determine its effectiveness compared with the treatment as usual based on pharmacological interventions in patients with mild/moderate symptoms. In addition, emotional regulation strategies and cognitive factors were studied as potential predictors. Participants (N = 105) were assigned to brief group therapy based on the Unified Protocol (n = 53) or treatment as usual (n = 52). They were assessed before and after the interve

[Psychodynamic Intervention in Severe Obsessive-Compulsive Disorder].

Obsessive-compulsive disorder (OCD) is among the most common psychiatric conditions and is associated with substantial distress and functional impairment. Although Cognitive Behavioral Therapy (CBT) and pharmacotherapy are considered first-line treatments, not all patients respond sufficiently. The case of a young woman demonstrates that a psychodynamic short-term therapy over five weeks can lead to marked symptom reduction (Y-BOCS: 39.5 → 19). This improvement is attributed to addressing intrapsychic conflicts and structural deficits.

Session-level emotional processing and subsequent somatic symptom change in online ISTDP for treatment-resistant somatic symptom disorder: A lagged within-person secondary analysis.

OBJECTIVE: Difficulties in emotional processing are implicated in the development and maintenance of Somatic Symptom Disorder (SSD) and may represent a target for therapeutic change. This study examined whether session-level emotional processing, operationalized as "rise in complex feelings", predicts subsequent reductions in somatic symptoms during online Intensive Short-Term Dynamic Psychotherapy (ISTDP) for treatment-resistant SSD. METHODS: Twenty-five participants with moderate to severe SSD and non-response across two prior intervention phases received up to 16 sessions of online ISTDP (

Examining Who Benefits from Short-Term Psychodynamic Psychotherapy for Depression: Insights from a Series of Individual Participant Data Meta-analyses.

Because little was known about who benefits specifically from short-term psychodynamic psychotherapy (STPP) for depression, we conducted an individual participant data meta-analysis project, collecting and combining raw datasets from 43 trials (N = 4,155) assessing its efficacy and examining participant baseline characteristics as moderators of outcome. Findings supported STPP's efficacy compared to control conditions and antidepressant medication, and when added to antidepressants. Episode duration and baseline severity moderated treatment outcome. Drawing on these findings and a decade of da

The Impact of a Brief Psycho-Education and Skills Intervention on Trauma Symptoms in a Prison Setting: Proof of Concept Using Group and Case Analysis.

The links between trauma and poorer physical health and psychological well-being in adulthood are now well established. Research shows levels of trauma are especially high amongst those who are incarcerated although evidence on what works to help this group is limited. To examine the effectiveness and acceptability of participation in a new brief, structured group intervention in fostering stabilization through reducing the symptoms of post-traumatic stress disorder (PTSD) and improving mental health in male inmates who have experienced trauma. Participants completed questionnaire measures bef

Dosage effects of psychodynamic and schema therapy in people with comorbid depression and personality disorder: four-arm pragmatic randomised controlled trial.

BACKGROUND: Higher intensity of psychotherapy might improve treatment outcome in depression, especially in those with comorbid personality disorder. AIMS: To compare the effects of 25 individual sessions (weekly) of two forms of psychotherapy - short-term psychoanalytic supportive psychotherapy (SPSP) and schema therapy - with the same treatments given for 50 sessions (twice weekly) in people with depression and personality disorder. Trial registration: NTR5941. METHOD: We conducted a pragmatic, double-randomised clinical trial and, over 37 months, recruited 246 adult out-patients with comor

Dispositional optimism as predictor of outcome in short- and long-term psychotherapy.

OBJECTIVES: Dispositional optimism predicts various beneficial outcomes in somatic health and treatment, but has been little studied in psychotherapy. This study investigated whether an optimistic disposition differentially predicts patients' ability to benefit from short-term versus long-term psychotherapy. DESIGN: A total of 326 adult outpatients with mood and/or anxiety disorder were randomized into short-term (solution-focused or short-term psychodynamic) or long-term psychodynamic therapy and followed up for 3 years. METHODS: Dispositional optimism was assessed by patients at baseline w

School-based, short-term group treatment for behaviorally disturbed young adolescent males: a pilot intervention.

Behaviorally maladjusted students represent a serious problem for schools. A short-term group counseling program was initiated as an alternative to out-of-school suspensions for male students in a middle school (grades six-eight). Thirteen adolescents were identified for group treatment. All received a standard psychiatric interview, and standard measures of self-concept, anxiety, cognitive complexity, and sexual maturity. Eleven students referred to the program had conduct disorders supported by Devereux Behavior profiles. Compared to 48 males of similar age (m = 12.5 years) without behavior

Psychotherapy of dysthymia.

OBJECTIVE: The author reviews empirical research on the psychotherapy of dysthymia. Dysthymia, a prevalent mood disorder, has been shown frequently to respond to antidepressant medication. The need for a treatment for dysthymic subjects unable or unwilling to take, or unresponsive to, medication still remains. METHODS: Studies were located by computerized search and the author's knowledge of the literature. All reports of studies on psychotherapy outcome for dysthymic patients, except studies of late-life chronic major depression, were included. RESULTS: Psychotherapy research on dysthymia h

The working alliance predicts session-by-session change in problems and functioning in short-term psychodynamic psychotherapy.

The working alliance is hypothesized to be crucial for facilitating change during psychodynamic therapy. However, few studies have examined the session-by-session effects of the alliance on outcomes within this therapeutic approach. We aimed to test the effect of the client- and therapist-reported alliance on next-session problems/functioning in short-term psychodynamic therapy conducted in a university training clinic. A secondary aim was to test whether baseline personality functioning and adverse childhood experiences moderate the association between alliance and outcome. A total of 152 cli

Cohesion and outcome in short-term psychodynamic groups for complicated grief.

This study used two measures of cohesion for the process analysis of 12 short-term, time-limited groups for complicated grief. The measures had similar theoretical definitions but differed in terms of rater source (member vs. observer), measurement score (mean of items vs. global rating), and rating unit (individual vs. group). We examined the relationship between the measures, assessed the development of cohesion over the life of the group, and evaluated each measure's relationship to outcome. A principal components analysis with each measure yielded one cohesion component, which supported a

Therapeutic approaches for survivors of disaster.

Common psychiatric responses to disasters include depression, PTSD, generalized anxiety disorder, substance-abuse disorder, and somatization disorder. These symptom complexes may arise because of the various types of trauma experienced, including terror or horror, bereavement, and disruption of lifestyle. Because different types of disaster produce different patterns of trauma, clinical response should address the special characteristics of those affected. Traumatized individuals are typically resistant to seeking treatment, so treatment must be taken to the survivors, at locations within thei

Randomized controlled trial comparing brief dynamic and supportive therapy with waiting list condition in minor depressive disorders.

BACKGROUND: Although many evidences suggest the presence of specific therapeutic factors in brief dynamic therapy (BDT), few studies have investigated its efficacy in the treatment of depressive disorders in comparison to other psychotherapies. The aim of this study was to determine whether BDT is more effective than brief supportive psychotherapy (BSP) and waiting list condition in the treatment of minor depressive disorders. METHOD: Thirty patients with primary DSM-IV dysthymic disorder, depressive disorder not otherwise specified or adjustment disorder with depressed mood completed a rando

Heart rate variability predicts therapy outcome in pain-predominant multisomatoform disorder.

BACKGROUND: Autonomic imbalance establishes an important model to understand organically unexplained physical complaints. Our study aimed to investigate whether the functioning of the autonomous nervous system corresponds with somatoform symptoms and predicts the outcome of brief psychotherapy in these patients. METHOD: As a part of multicenter study assessing effects of brief psychodynamic interpersonal therapy on multisomatoform disorder (PISO-study), 106 patients participated in a stress experiment investigating autonomic reactivity during application of the Stroop-test. Brief training in mindfulness meditation reduces symptoms in patients with a chronic or recurrent lifetime history of depression: A randomized controlled study.

BACKGROUND: Training in mindfulness has been introduced to the treatment of depression as a means of relapse prevention. However, given its buffering effects on maladaptive responses to negative mood, mindfulness training would be expected to be particularly helpful in those who are currently suffering from symptoms. This study investigated whether a brief and targeted mindfulness-based intervention can reduce symptoms in acutely depressed patients. METHODS: Seventy-four patients with a chronic or recurrent lifetime history were randomly allocated to receive either a brief mindfulness-based i
