Surgical anatomy and nuances of the expanded transpterygoid approach to the pterygopalatine fossa and upper parapharyngeal space: a stepwise cadaveric dissection.

BACKGROUND: Superb knowledge of anatomy and techniques to remove the natural barriers preventing full access to the most lateral aspect of the skull base determines the ease of using the transpterygoid approach (ETPA) as the main gateway for all the coronal planes during endonasal surgeries. METHODS: Throughout stepwise image-guided cadaveric dissections, we describe the surgical anatomy and nuances of the ETPA to the pterygopalatine fossa (PPF) and upper parapharyngeal space (UPPS). CONCLUSION: The ETPA represents a lateral extension of the midline corridor and provides a valuable route to

Clinically relevant morphometric analysis of pterygopalatine fossa and its volumetric relationship with adjacent paranasal sinuses: a CT-based study.

OBJECTIVES: This study aimed to perform morphometric measurements of the pterygopalatine fossa (PPF), the transition zone to critical neurovascular structures. The second aim was to investigate the relationship between the volumes of the PPF and the paranasal sinuses and the effect of nasal septum deviation (NSD) types on all these measurements. METHODS: We performed PPF's morphometry and all volume measurements on the CT images of 260 patients (130 male and 130 female, age range 18-79). RESULTS: All volumetric measurements and the angle between foramen rotundum (FR) and pterygomaxillary fis

Ultrasound-guided trigeminal nerve block via the pterygopalatine fossa: an effective treatment for trigeminal neuralgia and atypical facial pain.

BACKGROUND: Patients presenting with facial pain often have ineffective pain relief with medical therapy. Cases refractory to medical management are frequently treated with surgical or minimally invasive procedures with variable success rates. We report on the use of ultrasound-guided trigeminal nerve block via the pterygopalatine fossa in patients following refractory medical and surgical treatment. OBJECTIVE: To present the immediate and long-term efficacy of ultrasound-guided injections of local anesthetic and steroids in the pterygopalatine fossa in patients with unilateral facial pain th

Infraorbital nerve: a surgically relevant landmark for the pterygopalatine fossa, cavernous sinus, and anterolateral skull base in endoscopic transmaxillary approaches.

OBJECTIVE Endoscopic transmaxillary approaches (ETMAs) address pathology of the anterolateral skull base, including the cavernous sinus, pterygopalatine fossa, and infratemporal fossa. This anatomically complex region contains branches of the trigeminal nerve and external carotid artery and is in proximity to the internal carotid artery. The authors postulated, on the basis of intraoperative observations, that the infraorbital nerve (ION) is a useful surgical landmark for navigating this region; therefore, they studied the anatomy of the ION and its relationships to critical neurovascular stru

Endoscopic Endonasal Removal of Stray Bullets in the Fossa Pterygopalatine in Innocent Young Bystanders of Conflicts in Somalia in a Period of Six Months.

In Somalia, which is located in the horn of Africa, a fragile and insecure state structure allowed the strengthening of terrorist groups provoking armed conflicts. Stray bullet injuries can be defined as an accidental bullet wound caused by an anonymous attacker and are usually associated with celebratory gunfire or urban violence. The anatomy of the pterygopalatine fossa (PPF) is complex and penetrating foreign body injuries pose even a greater challenge for the surgeon to operate in this area. Endoscopic approaches facilitate the removal of foreign bodies from the paranasal sinuses, orbital

Trigeminal Nerve Blockade in the Pterygopalatine Fossa for the Management of Postoperative Pain in Three Adults Undergoing Tonsillectomy: A Proof-of-Concept Report.

SETTING: Post-tonsillectomy pain in adults can be severe and is often poorly controlled. Pain can lead to decreased oral intake, bleeding, longer hospital stays, emergency department visits, dehydration, and weight loss. Due to persistent pain despite scheduled medications, other methods for pain control are needed. Local/regional anesthetic options have been previously studied in this population. Unfortunately, neither the injection of local anesthetics into the tonsillar fossa nor the postoperative topical application of local anesthetics to the tonsillar bed has demonstrated efficacy in lar

Ultrasound-guided percutaneous versus trans-nasal pterygopalatine fossa block in endoscopic trans-sphenoidal pituitary gland surgery: a randomized controlled trial.

BACKGROUND: Various techniques are being used to create a relatively bloodless surgical field during endoscopic trans-sphenoidal pituitary gland surgery (ETS). Pterygopalatine fossa (PPF) block helps to improve the field of surgery and control haemodynamic fluctuations. METHODS: Eligible patients with the American Society of Anaesthesiologists class I or II were randomly classified by closed envelope method. Group TN received bilateral transnasal PPFB with 4 mL of 0.25% bupivacaine after induction of general anaesthesia. Group PC received bilateral ultrasound-guided infrazygomatic percutaneous

Endoscopic transnasal approach to pterygopalatine fossa and infratemporal fossa: case series utilizing endoscopic modified medial maxillectomy (EMMM)/direct approach to the anterior and lateral part of the maxillary sinus (DALMA).

BACKGROUND: Endoscopic transnasal surgery (ETS) has become an established treatment for skull base lesions; however, its lateral reach to the pterygopalatine fossa (PPF) and infratemporal fossa (ITF) remains limited due to restricted maneuverability within the nasal corridor. To address this limitation, the endoscopic modified medial maxillectomy (EMMM) and the direct approach to the anterior and lateral part of the maxillary sinus (DALMA) provides lateral access through the maxillary sinus. This study evaluated the technical feasibility and outcomes of ETS combining EMMM and DALMA. METHOD: A

Morphometric evaluation and clinical implications of the greater palatine foramen, greater palatine canal and pterygopalatine fossa on CBCT images and review of literature.

INTRODUCTION: The pterygopalatine fossa (PPF) infiltration is performed to reduce blood flow during endoscopic sinus surgery and septorhinoplasty, as well as to control posterior epistaxis and provide regional anesthesia in dental procedures. PPF infiltration performed with consideration of the morphometrics of greater palatine foramen (GPF), greater palatine canal (GPC) and PPF would increase the success of the procedure and reduce the risk of complications. The aim of this study is to investigate the GPF, GPC, lesser palatine foramen (LPF), lesser palatine canal (LPC) and PPF morphology via

Neuroendoscopic Resection of Trigeminal Schwannoma in the Pterygopalatine/Infratemporal Fossa via the Transnasal Perpendicular Plate Palatine Bone or Transnasal Maxillary Sinus Approach.

BACKGROUND: Both the pterygopalatine fossa (PPF) and the infratemporal fossa (ITF) lie outside the midline of the skull base. Lesions in the PPF or ITF include trigeminal schwannoma (trigeminal schwannoma, TS), which originates from the second or third branch of the trigeminal nerve (maxillary nerve or mandibular nerve). Due to their typically deep anatomic location, lesions in the PPF or ITF can be difficult to treat using traditional surgical approaches. In recent years, because of their advantages, which include the fact that they allow the problem to be observed close up, neuroendoscopic t

Surgery of the Anterior Infratemporal Fossa (AITF) and the Pterygopalatine Fossa (PPF) lesions: critical assessment of operative exposure, loss of functioning tissue and morbidity between Extradural Subtemporal Infratemporal Approach (ESITA) and Endonasal Endoscopic Transmaxillary Transpterygoid Approach (EEMP).

BACKGROUND: Surgery of the anterior infratemporal fossa (AITF) and pterygopalatine fossa (PPF) is technically demanding. The Extradural Subtemporal Infratemporal Approach (ESITA) and the Endonasal Endoscopic Transmaxillary Transpterygoid (EEMP) Approach are established routes for accessing these regions. This study quantitatively compares the surgical exposure, surgical freedom, angles of attack, and clinical outcomes achieved with these approaches. METHODS: ESITA and EEMP were performed on ten sides of five cadaveric heads. Resection volumes were calculated using 3D reconstructions from thin
