Mucosal perforation during Fredet-Ramstedt pyloromyotomy is the cause of unnecessary morbidity and death if unrecognized. Previously, a variety of closure have been advocated. In 1,777 cases there has been 42 (2.3%) intraoperative perforations. One unrecognized perforation led to a postoperative death. Closure of a mucosal tear is accomplished safely by a simplified suture technique without prolonged convalescence or death.

INTRODUCTION: Pyloromyotomy is the standard care for hypertrophic pyloric stenosis. The traditional approach for this procedure is a right upper quadrant transverse incision, although other "open" approaches, such as circumumbilical or periumbilical incision have been described. The more recent approach used is laparoscopic pyloromyotomy (LP), but experience feedback is still debated and its benefits remain unproven. The aim of this study was to make a review of all our LP procedures with an objective evaluation according to the literature.
METHODS: A retrospective analysis of all the LPs per

BACKGROUND: Open transumbilical pyloromyotomy (UMBP) and laparoscopic pyloromyotomy (LAP) have been compared on different outcomes, but postoperative pain as a primary end point had never been assessed. The aim of this study was to compare the use of analgesia in UMBP and LAP patients.
METHODS: Infants with hypertrophic pyloric stenosis treated by UMBP in 2008-2009 were matched with LAP-treated infants. Demographics, type and use of analgesia, and length of stay were recorded. Statistical analysis was performed using the Fisher exact test.
RESULTS: Each group contained 19 patients (N = 38) w

PURPOSE: High volume tubing is used to deliver carbon dioxide during laparoscopic procedures. Failure to prime the tubing with carbon dioxide prior to abdominal insufflation may result in the delivery of nitrogen-containing air to the abdominal cavity. We report a case in which initial insufflation of laparoscopic gas resulted in immediate cardiovascular collapse requiring prolonged resuscitation. Persistent intracranial emboli following the arrest may have resulted from nitrogen contamination of the delivered gas.
CLINICAL FEATURES: A 12-day-old female underwent laparoscopy for pyloric steno

INTRODUCTION: The purpose of this study was to compare laparoscopic and open pyloromyotomy using data from multiple centers in the United States.
METHODS: Children's hospitals that have a predominant surgical approach to pyloromyotomy were identified in the Pediatric Health Information System database. Using 2005 data, institutions were stratified into open (OPEN) and laparoscopic (LAP) groups. Patients with significant comorbid conditions were excluded. Group differences were compared using t tests and Mann-Whitney nonparametric tests for continuous variables and exact tests for categorical

Thirty-seven infants with hypertrophic pyloric stenosis had a modified Ramstedt pyloromyotomy performed. The incidence of postoperative vomiting was low (16%), with no infant suffering sustained vomiting requiring re-exploration for incomplete pyloromyotomy. The modified Ramstedt's pyloromyotomy is a simple procedure and may confer an advantage in the postoperative course of the recovering infant.

An unusual case of prolonged retention of a swallowed, round-shaped, magnet tack in the stomach of a patient is reported. The patient had had a previous pyloromyotomy for pyloric stenosis. Patients with a history of pyloromyotomy for pyloric stenosis may present special problems for the passage of foreign bodies reaching the stomach and may be more likely to require surgical intervention.

INTRODUCTION: Enhanced recovery after surgery protocols following pediatric pyloromyotomy have been published; however, literature evaluating same-day discharge (SDD) is lacking. This analysis aimed to evaluate SDD outcomes following laparoscopic pyloromyotomy.
MATERIALS AND METHODS: The American College of Surgeons National Surgical Quality Improvement Program Pediatrics database (2017-2021) was queried for patients with pyloric stenosis who underwent laparoscopic pyloromyotomy. Preterm infants, patients aged <4 wk or >6 mo, and open operations were excluded. Patients discharged same day pos

BACKGROUND: Per-oral pyloromyotomy (POP), also known as gastric per-oral endoscopic myotomy, is the first-line endoscopic intervention for medically refractory gastroparesis. This study aimed to assess the value of pyloric impedance planimetry using a functional lumen imaging probe (FLIP) during POP.
METHODS: Patients who underwent POP between October 2019 and February 2024 were retrospectively reviewed. FLIP measurements, symptoms measured using the Gastroparesis Cardinal Symptom Index (GCSI), and gastric emptying scintigraphy (GES) were evaluated before and after POP.
RESULTS: Of 35 patien

INTRODUCTION: Pyloric stenosis can now be treated effectively with laparoscopic pyloromyotomy (LP). Few large outcome studies have been published regarding the laparoscopic technique. In this paper, we describe our experience with the first 185 consecutive LPs.
MATERIALS AND METHODS: An institutional review board-approved retrospective outcomes analysis was performed on our first 185 consecutive LPs. Previous publications comparing open pyloromyotomy (OP) and LP are reviewed. Our hypothesis is that, with experience, the outcome of LP will equal or surpass that of OP.
RESULTS: A total 185 inf

BACKGROUND: Several authors have reported on laparoscopic pyloromyotomy (LP) since the technique was originally described in 1990, but its benefits remain unproven. We performed a randomized controlled trial comparing LP to open circumumbilical pyloromyotomy (OP) for hypertrophic pyloric stenosis.
METHODS: In a prospective study, 102 infants with pyloric stenosis were randomly assigned to either LP (n = 50) or OP (n = 52). The primary outcome measure was the incidence of postoperative vomiting; the secondary parameters were the durations of surgery and anesthesia, surgical complications, and

BACKGROUND: Gastric peroral pyloromyotomy (G-POP) is a minimally invasive endoscopic technique for treating refractory gastroparesis (GP). The functional luminal imaging probe (FLIP) is an endoscopic tool for measuring the physiologic parameters of the gastrointestinal sphincters. Here, FLIP was used to investigate the association between physiologic measurements of the pylorus and clinical outcomes after G-POP using the Gastroparesis Cardinal Symptom Index (GCSI) to monitor clinical response.
METHODS: This was a single-center prospective study of 48 patients with GP who underwent G-POP for t

BACKGROUND: Surgical treatments of refractory gastroparesis include pyloromyotomy and gastric electrical stimulator (GES). It is unclear if patients may benefit from a combined approach with concomitant GES and pyloromyotomy.
METHODS: Retrospective cohort analysis of all patients with refractory gastroparesis treated with GES implantation with and without concomitant pyloromyotomy at Cleveland Clinic Florida from January 2003 to January 2023. Primary endpoint was efficacy (clinical response duration and success rate) and secondary endpoints included safety (postoperative morbidity) and length

PURPOSE: Pyloromyotomy for hypertrophic pyloric stenosis (HPS) is one of the most common non-elective operations performed in the neonatal period. This project aims to explore outcomes of pyloromyotomy and compare differences between laparoscopic versus open pyloromyotomies in newborns diagnosed with HPS.
METHODS: The Nationwide Readmissions Database (NRD) from 2010 to 2014 was queried to identify patients diagnosed with HPS that underwent repair.
RESULTS: In total, 30,915 children (18% female) underwent pyloromyotomy for HPS. Median length of stay for index admission was 2 days. A total of

BACKGROUND: Gastroparesis patients may undergo pyloromyotomy/pyloroplasty for chronic refractory symptoms. However, some patients have persistent symptoms. It is unknown if balloon dilation may improve their symptoms.
AIMS: We aimed to (1) assess if pyloric through-the-scope (TTS) balloon dilation results in symptom improvement in gastroparesis patients with suboptimal response to pyloromyotomy/pyloroplasty and (2) determine endoscopic functional luminal imaging probe (EndoFLIP) characteristics of these patients before dilation.
METHODS: Patients with severe gastroparesis refractory to pylor

INTRODUCTION: Hypertrophic pyloric stenosis is a commonly encountered pediatric surgical issue. Initially treated with open surgical techniques, many pediatric surgeons have adopted the minimally invasive approach using laparoscopy. However, some concerns exist that the rate of incomplete pyloromyotomy is elevated in laparoscopy. We propose a new technique to assess the adequacy of laparoscopic pyloromyotomy.
METHODS: Adequacy of laparoscopic pyloromyotomy was assessed by confirming that the top of the serosa on one side of the pylorus has adequate freedom to reach the bottom of the muscle on

We retrospectively reviewed 223 infants who underwent pyloromyotomy for hypertrophic pyloric stenosis (HPS) at our institution from January 1984 to May 1990. Each patient's postoperative feeding regimen was determined by the attending surgeon. The four distinct regimens used were as follows: A (n = 66): NPO overnight (> 10 h) with cautious feeding advancement every 4 hours x 2, then every 2 hours x 2, then every 1 1/2 hours x 8, then ad lib; B (n = 46): NPO until 6 to 8 hours postoperatively with the same cautious feeding advancement as in A; C (n = 42): NPO until 6 hours postoperatively with

INTRODUCTION: Congenital hypertrophic pyloric stenosis (CHPS), the most common infantile disease requiring surgical intervention, is routinely treated with open or laparoscopic pyloromyotomy. Recently, gastric peroral endoscopic pyloromyotomy (G-POEM) has been used for adult gastroparesis. We aimed to evaluate the efficacy and safety of G-POEM in treating infantile CHPS.
METHODS: We reviewed data from 21 G-POEM-treated patients at 3 tertiary children's endoscopic centers in China between January 2019 and December 2020. Clinical characteristics, procedure-related parameters, perioperative mana
