Quinine induced simvastatin toxicity through cytochrome inhibition - a case report.

BACKGROUND: Nocturnal leg cramps are painful, involuntary muscle contractions commonly seen in elderly. While mostly harmless, they can severely impair quality of life and often disrupt sleep. Adverse drug effects may be responsible for a fraction of nocturnal leg cramps but often go unrecognized, resulting in additional prescribing intended to deal with adverse effects that might be better addressed by reduction, substitution, or discontinuation of the offending agent. CASE PRESENTATION: An 87 year old female presented as outpatient in family medicine with nocturnal leg cramps which had been

Efficacy of quinine against ichthyophthiriasis in common carp Cyprinus carpio.

Ichthyophthiriasis, caused by Ichthyophthirius multifiliis, is an economically important worldwide parasitic disease that infects all freshwater fish. Since the banning of malachite green for use in food fish, there has been a great need for alternative therapeutants. The goal of this study was to investigate the efficacy of quinine against I. multifiliis. Parasite developmental stages from our laboratory-established life cycle in rainbow trout Oncorhynchus mykiss were exposed to quinine in vitro, and a dual fluorescent staining technique was used to allow a clear distinction between viable an

[Acral necroses after therapy with quinine sulfate for calf cramps].

The most common cause of acral cyanosis is vascular spasm which can be induced by several drugs. An 87-year-old woman developed red and livid skin lesions on the fingers of both hands and several toes one month after beginning treatment with quinine sulfate 200 mg daily. The skin lesions progressed to necrosis in some areas. Quinine sulfate is a widely prescribed drug for nocturnal cramps. The following side effects may develop, particularly in the elderly: exanthems, pruritus, urticaria, erythema multiforme, purpura and photosensitivity. Our case points to the possibility of acral necrosis an

Plasma quinine concentrations in falciparum malaria with acute renal failure.

Plasma quinine (Qn) monitoring was performed in 32 patients with severe falciparum malaria (10 with acute renal failure (ARF) and 22 with other severe manifestations) who were treated with the standard regimen of 10 mg/kg body weight Qn dihydrochloride, with a loading dose of 20 mg/kg body weight. Median plasma Qn concentrations prior to the first dose on each day were approximately 10-30% higher in ARF patients than in non-ARF patients during acute infection. Seven patients underwent haemodialysis; 2 died after 2 cycles. There were no significant changes in plasma Qn concentrations in patient

Phototoxic properties of quinine and quinidine: two quinoline methanol isomers.

Clinical photoreactions have been reported for quinine and quinidine after systemic and topical administration. We have investigated the phototoxic properties of these two quinoline methanol isomers in vitro using the Candida albicans inhibition test and photohemolysis, and in vivo with the mouse tail phototoxicity test. Both isomers were phototoxic in the hemolysis model, quinine being the more potent compound. In the Candida test only quinidine was phototoxically active. In the mouse tail model, measuring edema, the phototoxic activity of quinidine was comparatively low, causing a 7.3% wet w

Quinine overdose. Prevention of visual damage by stellate ganglion block.

A case of quinine poisoning is described. Stellate ganglion block was performed immediately on the basis of the clinical history of visual disturbance without waiting for physical signs to develop. There was no residual field defect despite the presence of toxic levels of the drug. It is suggested that stellate ganglion block may prevent development of visual field defects.

Identification of genetic markers of quinine partial resistance in Plasmodium falciparum.

The genetic basis of Plasmodium falciparum resistance to quinine, a drug used to treat severe malaria, has long been unclear. To investigate this, here we used a human liver-chimaeric mouse model to conduct a P. falciparum genetic cross between quinine-partially resistant and quinine-sensitive parasites. Drug profiling and quantitative trait loci analyses of 120 unique recombinant progeny mapped resistance to segments on chromosomes 7 and 12, indicating a polygenic basis. The chloroquine resistance transporter PfCRT and a structurally similar putative drug/metabolite transporter, DMT1, were id

Quinine and chloroquine: Potential preclinical candidates for the treatment of tegumentary Leishmaniasis.

Leishmaniasis is an endemic disease in more than 90 countries, constituting a relevant public health problem. Limited treatment options, increase in resistance, and therapeutic failure are important aspects for the discovery of new treatment options. Drug repurposing may accelerate the discovery of antiLeishmanial drugs. Recent tests indicating the in vitro potential of antimalarials Leishmania resulted in the design of this study. This study aimed at evaluating the susceptibility of Leishmania (L.) amazonensis to chloroquine (CQ) and quinine (QN), alone or in combination with amphotericin B (

Cinchona bark and quinine in the Portuguese official pharmacopoeias (1794-2001).

Cinchona bark (bark from plants of the genus Cinchona with antimalarial activity) and its alkaloid quinine were widely used to treat intermittent fevers. This paper aims to quantitatively analyze the presence of Cinchona bark, quinine and other Cinchona bark-derived substances in the Portuguese official pharmacopoeias published between 1794 and 2001. The analysis showed that the Pharmacopêa Portugueza (1876) is the Portuguese official pharmacopeia with the highest percentage of medicines containing Cinchona bark (2.61%). The Farmacopeia Portuguesa IV (1935) is the official pharmacopeia with th

Phosphorylation as a method of tuning the enantiodiscrimination potency of quinine--an NMR study.

Quinines phosphorylated at the C-9 hydroxyl group (diphenyl and diethyl phosphates) were synthesized and validated as novel effective chiral solvating agents in two alternative methods based on (1)H and (31)P NMR spectroscopy. Tested with a representative set of racemic analytes, the title compounds induced shift nonequivalence effects in (1)H NMR signals with values up to 0.1-0.2 ppm for 3,5-dinitrobenzoyl-substituted amino acids. In terms of enantiodifferentiation extent and application range, introduction of a phosphate group was proven to be superior compared to the action of nonmodified q

The effects of quinine and chloroquine antimalarial treatments in the first trimester of pregnancy.

Quinine (n = 246) was used to treat uncomplicated Plasmodium falciparum and chloroquine (n = 130) was used to treat P. vivax, in a total of 376 episodes of malaria in the first trimester of pregnancy, in 300 Karen women (Thailand, 1995-2000). Parasites were still present on day 6 or 7 in 4.7% (11/234) of episodes treated with quinine. The overall 28 day parasite reappearance rate following quinine was 28.7% (60/209) for primary treatments and 44% (11/25) for re-treatments. Quinine treatment resulted in a high rate of gametocyte carriage: person-gametocyte-weeks = 42.5 (95% CI 27.8-62.1) per 10

The effect of quinine on outer hair cell shape, compliance and force.

Quinine intoxication causes a well-described syndrome that includes tinnitus, sensorineural hearing loss and vertigo. The pathophysiology of quinine's effects on hearing is unknown, but may include a peripheral component. The cochlear outer hair cell is known to be motile and to contribute force to amplify the vibration pattern of the organ of Corti. The outer hair cell is also a target of diseases involving tinnitus and sensorineural hearing loss, including salicylate intoxication. These effects may be mediated through changes either in motile force or in mechanical properties. Quinine's effe

Establishing oral preference for quinine, phencyclidine and caffeine solutions in rats.

Rats were given fixed-time, 1-min food-pellet delivery sessions, for 3 h every day, which resulted in over drinking (schedule-induced polydipsia). In previous research, groups of animals came to prefer solutions of cocaine or lidocaine to concurrently presented water, if the drugs were first offered in a glucose/saccharin vehicle, which was then gradually eliminated, so that the choice became a drug solution in water vehicle versus water. In the present experiment, the same procedure produced a preference for 0.025 mg/ml quinine solution to water, indicating that a bitter solution that was not

Allopurinol as an additive to quinine in the treatment of acute complicated falciparum malaria.

The emergence of chloroquine resistance, and a world-wide scarcity of quinine, have resulted in a search for newer antimalarial drugs directed against falciparum malaria. Allopurinol causes virtually complete inhibition of purine biosynthesis of malaria parasites, which may prove lethal to the parasites. This study was designed to examine if allopurinol is additive to quinine in the treatment of acute falciparum malaria. Forty-seven Asian-Indian adults with acute complicated falciparum malaria were assigned to a treatment period of five days. They were randomly assigned to receive either oral

Effects of quinine on neural activity in cat primary auditory cortex.

The effect of systemically applied quinine on single-unit firing activity in primary auditory cortex was investigated in seven cats. A dose of 100 or 200 mg/kg of quinine hydrochloride was administered intramuscularly and recordings from the same units were performed prior to application and continuously up to on average 5.5 h after administration. All animals showed 10-40 dB of threshold shift about 30 min after administration and some animals showed recovery during the course of the investigation. Significant increases were found in spontaneous firing rates for low-firing-rate units (initial

Verapamil vs quinine in recumbent nocturnal leg cramps in the elderly.

In an open-labeled trial with eight elderly patients (aged 62 to 87 years) suffering from nocturnal leg cramps refractory to treatment with quinine sulfate, we ruled out other active disease processes and substituted verapamil hydrochloride therapy (120 mg at bedtime). Response to treatment was assessed by biweekly observations by the primary care physician and nightly by the research registered nurse for the entire duration of the trial, lasting eight weeks. Observations made and clinical conditions reported were indicative of improvement and disappearance of cramping when therapy was changed

Quinine and Quinidine Derivatives as Photosensitizers for Photodynamic Inactivation of Bacterial Pathogens.

This study reports the synthesis and characterization of 5-azoniabenzo[no]tetraphene derivatives of Cinchona alkaloids with potential applications in antimicrobial photodynamic therapy (aPDT). These compounds exhibit absorption maxima at 430-441 nm, as well as in the UV-A (340-342 nm) and UV-C (270-275 nm) regions, with fluorescence emission peaks ranging from 519 to 534 nm. At nontoxic concentrations of 2 μg mL-1 and upon irradiation with blue light (418 nm), these compounds demonstrated potent bactericidal activity depending on the light dose. Staphylococcus aureus was eradicated after 5 min

Cinchona bark and quinine as psychiatric treatments in Portugal, 1700-1900.

Cinchona bark and its main medicinal derivative - quinine - are known antimalarials. Until the 20th century, medicines with Cinchona bark or quinine were the only effective treatment for malaria used on a large-scale. However, Cinchona bark and quinine were also used in other diseases. Regarding psychiatric uses, in Britain, Cinchona bark was recommended in mania (17 th century) and nervous diseases (18 th century). Nevertheless, the full scope of Cinchona bark and quinine's proposed activity in psychiatric disorders has not been assessed. We aimed to investigate references to Cinchona bark an
