There are marked racial/ethnic disparities in breast cancer mortality. Cultural racism, expressed through societal norms, stereotypes, and implicit biases, may contribute. We utilized a previously validated state-level multidomain latent measure, the cultural racism factor (CRF). Using data from the Surveillance, Epidemiology, and End Results Program (2018-2020), we evaluated associations between CRF and risk of death using Cox proportional hazards (PH) model with state as random effect. We examined time-varying effects and conducted race- and ethnicity-stratified analyses. Among 225 991 patie

We investigate whether there is pluralistic ignorance-a group-level shared misperception of what other group members are doing, thinking, or feeling-around COVID-19 vaccination and medical racism. We focus on two distinct anti-vaccination movements grounded in distinct racial groups: Black Americans and White Americans. Black American vaccine hesitancy is uniquely grounded in the history and continued reality of medical racism. White American vaccine hesitancy is influenced by factors including social media use, institutional mistrust, and psychological reactance. A Qualtrics panel sample (N =

Investigators from minoritized backgrounds are underrepresented in psychiatric research. That underrepresentation contributes to disparities in outcomes of access to mental health care. Drawing on lived experience, scholarly qualitative reports, and empirical data, the authors review how the underrepresentation of minoritized researchers arises from interlocking, self-reinforcing effects of structural biases in our research training and funding institutions. Minoritized researchers experience diminished early access to advanced training and opportunities, stereotype threats and microaggression

OBJECTIVES: Structural racism (SR) is viewed as a root cause of racial and ethnic disparities in maternal health outcomes. However, evidence linking SR to increased odds of severe adverse maternal outcomes (SAMO) is scant. This study assessed the association between state-level indicators of SR and SAMO during childbirth.
METHODS: Data for non-Hispanic Black and non-Hispanic white women came from the US Natality file, 2017-2018. The exposures were state-level Black-to-white inequity ratios for lower education level, unemployment, and prison incarceration. The outcome was patient-level SAMO, i

Since early 2020 COVID-19 has swept across the United States, exposing shortcomings in the current healthcare delivery system. Although some interim efforts have been made to mitigate the spread of infection and maintain access to treatment for opioid use disorder, more permanent changes are needed to combat the ongoing opioid crisis. In this commentary, we describe the regulatory barriers to methadone maintenance treatment that disproportionately impact communities of color. We then discuss strategies supporting more equitable access to this proven treatment for opioid use disorder.

BACKGROUND: Structural racism, the interconnected policies, institutions, and norms that systematically disadvantage communities of color, remains a core determinant of health inequities in the United States. Despite growing recognition of its impact, public health lacks standardized, validated measurement tools across contexts, limiting intervention effectiveness and policy change.
METHODS: Following PRISMA guidelines, we systematically reviewed peer-reviewed studies published between January 2000 and March 2025 that quantified structural racism in relation to health outcomes in the U.S. We

INTRODUCTION: People who identify as Black, Indigenous, and People of Color (BIPOC) represent an increasing proportion of patients with cystic fibrosis (CF). Despite the transformative potential of highly effective modulator therapies, BIPOC people with CF (pwCF) experience disproportionately worse outcomes, including lower lung function and increased risk of hospitalization. Social and structural conditions exacerbate these inequities. To better understand these dynamics, this study used photovoice, a community-based participatory research method, to explore how BIPOC pwCF describe the influe

BACKGROUND: Structural racism remains a pervasive and deeply embedded determinant of healthcare inequity globally. Despite strengthened professional standards and policy commitments to equity, racialised populations continue to experience substantial disparities in healthcare access, quality, safety and outcomes. These inequities are sustained not only through interpersonal encounters but also through structural conditions embedded within healthcare systems, workforce arrangements, governance processes and organisational cultures. Structural racism additionally affects nurses' wellbeing, workf

Structural racism is a fundamental cause of racial health inequities; however, it is a complex construct that is difficult to quantitatively analyze due to its multi-dimensionality. We classified fifty US states into three typologies of structural racism using a latent profile analysis. Five domains of structural racism were included in the analysis: Black-White inequities in educational attainment, employment, homeownership, incarceration, and income. In separate analyses, we specified two, four, and five typologies and describe how states' groupings changed when different numbers of typologi

Addressing the grave racial inequities in maternal health requires a new generation of research that better operationalizes root causes of these outcomes. Recent frameworks improving the conceptualization of structural racism have illuminated the need for better conceptual clarity when investigating neighborhoods as a site of structural marginalization for Black birthing populations as well. In particular, better conceptualization of dynamic feedback in how neighborhoods are constructed and experienced, especially as they embed vicious cycles of place-based racialization, is integral to produc

IMPORTANCE: Racial and ethnic inequities in the criminal-legal system are an important manifestation of structural racism. However, how these inequities may influence the risk of severe maternal morbidity (SMM) and its persistent racial and ethnic disparities remains underinvestigated.
OBJECTIVE: To examine the association between county-level inequity in jail incarceration rates comparing Black and White individuals and SMM risk in California.
DESIGN, SETTING, AND PARTICIPANTS: This population-based cross-sectional study used state-wide data from California on all live hospital births at 20

INTRODUCTION: Disadvantaged populations have higher rates of chronic disease, including heart disease, cancer, and diabetes. Race, ethnicity, lower socioeconomic status, and poverty all contribute to these disproportionate rates. Other factors, including systemic racism, climate change, poor diet, lack of food access, and epigenetic influences, that are distributed and experienced differently across vulnerable populations also play a significant role in the development of chronic diseases. This comprehensive review of contributors to chronic diseases emphasizes a unique focus on these identifi

OBJECTIVES: The present study utilized an intersectional framework to examine if two forms of gendered racism, psychological emasculation and messages about Asian American men being undesirable partners, were associated with Asian American men's nicotine use. We also examined the potential mediating roles of two racial identity statuses, racial conformity and racial immersion.
METHODS: A sample of 356 Asian American men living in the United States of America (USA) completed a cross-sectional survey via Qualtrics containing measures assessing the aforementioned constructs of interest. The prim

Several recent studies have investigated the mental health consequences of structural racism. Structural racism has been defined as "macro-level societal conditions that limit opportunities, resources, and well-being of less privileged groups on the basis of race/ethnicity and/or other statuses, including but not limited to, gender identity, sexual orientation, disability status, social class or socioeconomic status (SES), religion, geographic residence, national origin, immigration status, limited English proficiency, physical characteristics, or health conditions."1 Researchers have hypothes

BACKGROUND: Non-Hispanic Black (NHB) women face a 50% increased risk of delivering preterm compared to non-Hispanic White (NHW) women in the United States. Sociodemographic and pregnancy risk factors do not fully explain this inequity. This inequity exists even among women with a college education, although recent empirical analysis on racial inequities in preterm delivery (PTD) among college-educated women is lacking. Furthermore, the contribution of preconception risk factors to the racial inequity in PTD has not been examined.
OBJECTIVES: To determine whether: (i) there is a NHB-NHW inequi

Racism-related stress is thought to contribute to widespread race/ethnic health inequities via negative emotion and allostatic stress process up-regulation. Although prior studies document race-related stress and health correlations, due to methodological and technical limitations, they have been unable to directly test the stress-reactivity hypothesis in situ. Guided by theories of constructed emotion and allostasis, we developed a protocol using wearable sensors and daily surveys that allowed us to operationalize and time-couple self-reported racism-related experiences, negative emotions, an

Black women are underrepresented in interpersonal violence research and studies often overlook the impact of racism on mental health. The current study used person-centered analytic methods to identify latent profiles of Black women's experiences of child maltreatment, adult sexual assault (ASA), racism, and gendered racism. Differences in depression, alcohol use, and discrimination coping strategies were examined across profiles. Data from 425 Black women in the United States revealed a three-profile solution. Profile 1 (Lower Adversity; n = 267) had the lowest scores on all measures. Profile

BACKGROUND: Racialized disparities in hypertensive disorders of pregnancy persist, yet the role of interconnected structural determinants has not been fully explored. We evaluated the association between county-level indicators of structural racial discrimination (SRD) and Black-White disparities in HDP in Georgia.
METHODS: We conducted exploratory factor analysis to identify latent factors of county-level SRD from 12 indicators representing historic racial violence, criminal justice, residential segregation, and political representation. We linked factor scores to 2020-2024 Georgia natality

BACKGROUND: Intersectionality, or the multidimensional influence of social identity and systems of power, may drive increased morbidity and mortality for adults of color with Down syndrome. We documented racial and ethnic differences in death and hospitalizations among Medicaid-enrolled adults with Down syndrome and assessed the interaction of racial-ethnic group and Down syndrome.
METHODS: Our sample consisted of 119,325 adults with Down syndrome and >3.2 million adults without intellectual disability enrolled in Medicare at any point from 2011 to 2019. We calculated age-adjusted mortality a

CONTEXT: Historically, Black women strategically employed silence to endure enslavement to the U.S., and other forms of racial violence. The current study aimed to understand contemporary perspectives on self-silencing.
OBJECTIVE: To explore young adult Black women's experiences of self-silencing and its potential impact on their physical and mental well-being.
METHODS: Data are from 16 semi-structured interviews with Black women ages 18 to 39 in southwest Pennsylvania conducted between October 2021 - May 2022. We analyzed the interviews using inductive thematic analysis.
