Variable cellular and radiobiological effects of [177Lu]Lu-PSMA-I&T in patient-derived models of prostate cancer.

BACKGROUND: [177Lu]Lu-PSMA radioligand therapy targets metastatic castration-resistant prostate cancer by delivering radiation to cells expressing prostate-specific membrane antigen (PSMA). While some patients show remarkable responses, up to 50% show little to no benefit, and disease progression inevitably occurs. Eligibility is mainly based on PSMA avidity by PET imaging, yet responses remain highly variable, highlighting a disconnect between PSMA expression and clinical efficacy. This study used PSMA-positive patient-derived xenografts (PDXs) from the Melbourne Urological Research Alliance

Face the Pain: Radiobiological and Clinical Considerations of Re-radiosurgery to the Trigeminal Nerve Following Irradiation of an Abutting Petroclival Meningioma.

Trigeminal neuralgia is a common symptom of benign tumors compressing the trigeminal nerve, leading to debilitating pain and a devastating impact on quality of life. Stereotactic radiosurgery is a validated option for intracranial meningiomas with excellent tumor control rates that correlate with symptomatic improvement. Select cases with refractory or recurrent trigeminal neuralgia can benefit from a second treatment targeting the trigeminal nerve. We present a case of refractory trigeminal neuralgia secondary to compression of a previously irradiated petroclival meningioma successfully treat

Dosimetric and radiobiological analyses of a de-escalation strategy for elective nodal regions in human papillomavirus-associated oropharyngeal cancer.

INTRODUCTION: In this simulation study, we examined the effects of a de-escalation strategy with a reduced dose to subclinical nodal regions in patients with human papillomavirus (HPV)-associated oropharyngeal carcinoma (OPC). METHODS: We created two patterns of intensity-modulated radiotherapy for 16 patients with HPV-associated OPC. In the standard and de-escalation plans, the initial field including elective nodal regions received 46 and 30 Gy, followed by 20 and 36 Gy to the cutdown field, respectively. Comparison metrics were set for each organ at risk (OAR). We compared these metric val

A computational tool for patient specific dosimetry and radiobiological modeling of selective internal radiation therapy with (90)Y microspheres.

In recent years we have witnessed tremendous progress in selective internal radiation therapy. In clinical practice, quite often, radionuclide therapy is planned using simple models based on standard activity values or activity administered per unit body weight or surface area in spite of the admission that radiation-dose methods provide more accurate dosimetric results. To address that issue, the authors developed a Matlab-based computational software, named Patient Specific Yttrium-90 Dosimetry Toolkit (PSYDT). PSYDT was designed for patient specific voxel-based dosimetric calculations and r

Emission of fluorescent x-radiation from non-lead based shielding materials of protective clothing: a radiobiological problem?

The aim of this study was to investigate the effectiveness of different shielding materials in protective clothing using dicentric frequency in human peripheral lymphocytes as a marker of radiation-induced damage. Blood samples from a healthy donor were exposed to 70 kV x-rays behind shielding materials lead (Pb), tin/antimony (Sn + Sb) and bismuth barrier/tin/tungsten (Bi + Sn + W) with the same nominal lead equivalent value of 0.35 mm lead. Irradiation was performed either in contact (exposure position A, containing secondary radiation) or at a distance of 19 cm behind the shielding material

Radiobiological effects of 131I and 125I on the DNA of the rat thyroid. I. Comparative study with emphasis on the postradiation hypothyroidism occurrence.

One of the major disadvantages of the use of 131I in the treatment of thyrotoxicosis is the development of hypothyroidism. Alternatively, 125I has been proposed for thyrotoxicosis therapy, and was thought to be preferable to 131I because of the short range of its emitted soft electrons. Several studies have shown 125I to be as effective as 131I in the treatment of thyrotoxicosis, and equally likely to produce hypothyroidism. This work compared the radiobiological effects of 131I and 125I given in doses to deliver the same amount of radiation to the rat thyroid gland. These effects were studied

Home-made low-cost dosemeter for photon dose measurements in radiobiological experiments and for education in the field of radiation sciences.

Reliable dosimetry systems are crucial for radiobiological experiments either to quantify the biological consequences of ionizing radiation or to reproduce results by other laboratories. Also, they are essential for didactic purposes in the field of radiation research. Professional dosemeters are expensive and difficult to use in exposure facilities with closed exposure chambers. Consequently, a simple, inexpensive, battery-driven dosemeter was developed that can be easily built using readily available components. Measurements were performed to validate its readout with photons of different en

Radiosurgical options in neuro-oncology: a review on current tenets and future opportunities. Part II: adjuvant radiobiological tools.

Stereotactic radiosurgery (SRS) is currently a well-established, minimally invasive treatment for many primary and secondary tumors, especially deep-sited lesions for which traditional neurosurgical procedures were poorly satisfactory or not effective at all. The initial evolution of SRS was cautious, relying on more than 30 years of experimental and clinical work that preceded its introduction into the worldwide medical community. This path enabled a brilliant present, and the continuous pace of technological advancement holds promise for a brighter future. Part II of this review article will

From CHART to CHARTWEL in non-small cell lung cancer: clinical radiobiological modelling of the expected change in outcome.

CHART (Continuous Hyperfractionated Accelerated Radiotherapy) has been shown to improve the tumour control probability and survival relative to conventional radiotherapy in patients with inoperable non-small cell lung cancer (NSCLC). CHARTWEL (CHART Weekend-less) is a further development of this schedule escalating the physical dose to 60 Gy while maintaining the low dose per fraction of 1.5 Gy. In this schedule, three fractions, with a minimum interval of 6 h between fractions, are delivered 5 days per week. This extends overall treatment time from the 12 days of CHART to 18 days. Pulsed Low-Dose-Rate Radiotherapy (PLDR): A Comprehensive Review of Radiobiological Rationale and Clinical Applications in Modern Cancer Care.

Pulsed low-dose-rate radiotherapy (PLDR) has emerged as a promising retreatment strategy that delivers radiation in a series of low-dose pulses, potentially increasing sublethal damage repair in normal tissues while exploiting low-dose hyper-radiosensitivity and impaired DNA repair mechanisms in tumor cells. This review synthesizes current evidence on PLDR by outlining its historical development, radiobiological foundations, and evolving clinical applications across tumor sites, with particular attention to re-irradiation settings. Preclinical studies suggest enhanced normal-tissue sparing and
