[Corrective radius operations. Indications and results].

Posttraumatic malunion usually results from incorrect treatment of Colles or Smith fracture. Chronic pains, limited motion and impaired power can be avoided and cosmetic appearance of the hand improved by restoration of the functional anatomy of the wrist. The malalignment is corrected by open wedge osteotomy of the distal end of the radius. A small iliae cortico-cancellus bone graft is inserted at the osteotomy site. By using this technique it is usually possible to avoid additional resection of the distal ulna. The surgical approach dorsal or volar is determined by the type of dislocation. T

[Late results of radius head resection].

The results after excision of the radial head were good and fair in every case. In our 15 re-examined patients we saw no poor results. Indications for excision were comminuted fractures and fractures of the radial neck without possibility of reduction. Two patients had subluxations of the distal radio-ulnar joint. The subluxations were no more than three millimeters of proximal displacement of the radius after injury. At follow-up the subluxations were no more than 5 millimeters. There was no correlation between the degree of subluxation and the presence of symptoms. Valgus deformity we saw in

ESTES recommendations regarding distal radius fractures.

Distal radius fractures (DRFs) account for 15% of all fractures in adults. Optimal treatment of DRFs depend on patient and fracture characteristics. Non- or minimally displaced DRFs can be treated nonoperatively with a cast. Although surgical treatment is becoming more popular, the majority of patients with a DRF are still treated nonoperatively. In addition, these considerations may be different for elderly patients. Conflicting evidence exists on the different aspects of treatment of DRFs. These recommendations regarding distal radius fractures describe considerations regarding diagnostic mo

Distal Radius Fracture with Dorsal Angulation.

Distal radial fractures (DRFs) are prevalent injuries and represent one of the most common fractures seen in the Emergency Department (ED). DRFs often result from falls on an outstretched hand (FOOSH). This case study details a 64-year-old female who sustained a distal radial fracture with dorsal angulations (DRFDA) from ice skating, confirmed via radiographic evaluation. Initial treatment involved immobilization, pain management, and follow-up with orthopedic specialists. Proper diagnosis and management are essential to prevent long-term functional impairments, with treatment ranging from cas

[Conservative management of distal radius fractures].

Distal radius fractures are the third most frequent fractures in Germany. The decision for conservative treatment and the weighing up of surgical treatment need an exact consideration of the indications based on instability criteria and the extent of possible articular involvement. Indications for an emergency operation must be excluded. In cases of stable fractures or multimorbid patients in a poor general condition conservative treatment is indicated. The principles for a successful treatment are the precise reduction and stable retention in a plaster splint. In the further course, fractures

[Distal radius fracture-tactic and approach].

OBJECTIVE: The aim of surgical treatment is fracture healing with restored alignment, rotation, and joint surface. Stable fixation allows for functional postoperative aftercare. INDICATIONS: Displaced intra- and extra-articular fractures which either could not be adequately reduced or in which a secondary displacement is to expected due to instability criteria. The following factors are considered instability criteria: age > 60 years, female, initial dorsal displacement > 20°, dorsal comminution, radial shortening > 5 mm, palmar displacement. CONTRAINDICATIONS: The only absolute contraindica

Avoiding Complications in Distal Radius Fractures.

Managing fractures of the distal radius is a fundamental skill for orthopaedic surgeons. Given the prevalence of these fractures, complications following operative management are well described and frequently encountered. Surgeons should be observant in the acute phase for emergent conditions such as acute carpal tunnel syndrome. Careful radiographic examination intra-operatively can help prevent delayed complications by identifying surgical errors such as hardware malposition or malreduction. Many problems that arise during the treatment of distal radius fractures are the result of technical

Column-Specific Distal Radius Fracture Fixation.

Despite the popularity and success of volar fixed angle plating, variations in distal radius fracture presentation underscore the importance of understanding different surgical treatment options to maximize patient outcomes. The 3-column theory of wrist mechanics provided the foundation for using a column-specific fixation approach. Implant placement within the ulnar column can be challenging because of anatomical constraints. Some have described "safe zones" for implant position to decrease the potential for tendon impingement. A thorough understanding of the surrounding anatomy can allow fix

Perioperative management of distal radius fractures.

Fractures of the distal radius are the most common upper limb fracture and account for over a sixth of all fractures seen in emergency departments. Although most of these fractures are managed non-operatively, a significant number of complex injuries undergo operative management. This educational review of up to date guidelines discusses the perioperative management of distal radius fractures and provides readers with continuing professional development activities.

[Distal radius fractures : Update on imaging].

Distal radius fractures are associated with high individual and socioeconomic relevance. Optimized radiography and computed tomography (CT) imaging are essential for individualized therapy planning. Plain CT imaging with 3D reconstruction and digital removal of the carpus and ulna is helpful for choosing the surgical approach in the presence of intra-articular radius fractures. If the fracture line leads towards the scapholunate compartment, ligamentous injury requiring treatment can be detected or ruled out before surgery with the help of direct CT arthrography (CTA). Acquisition of imaging d

Classification systems for distal radius fractures.

Background and purpose - The reliability of conventional radiography when classifying distal radius fractures (DRF) is fair to moderate. We investigated whether reliability increases when additional computed tomography scans (CT) are used. Patients and methods - In this prospective study, we performed pre- and postreduction posterior-anterior and lateral radiographs of 51 patients presenting with a displaced DRF. The case was included when there was a (questionable) indication for surgical treatment and an additional CT was conducted within 5 days. 4 observers assessed the cases using the Fryk

Distal Radius Fracture Outcomes and Rehabilitation.

Distal radius fractures account for nearly 1 of every 5 fractures in individuals aged 65 or older. Moreover, increased susceptibility to vertebral and hip fractures has been documented in patients a year after suffering a distal radius fracture. Although women are more susceptible to hip fractures, men experience a higher mortality rate in the 7 years following a distal radius fracture. Traditional approaches to distal radius fractures have included both surgical and nonsurgical treatments, with predominant complaints involving weakness, stiffness, and pain. Nonsurgical approaches include immo

[Carpal malalignment following distal radius fracture].

Adaptive carpal malalignment is the consequence of malunion of the distal radius. Since the radial metaphysis and capitate have to be aligned, any disorientation of the radial epiphysis will force the proximal carpal row to adapt, as it is the only mobile element. There are two types of adaptation depending where the compensative occurs: (1) midcarpal - leading to flexion between the lunate and capitate, with the lunate maintaining a normal relationship with the radial epiphysis axis; (2) radiocarpal - combining flexion and dorsal displacement of the lunate relative to the axis of the radial e

Concomitant hip and distal radius fractures.

BACKGROUND: Concomitant ipsilateral hip and distal radius fractures are uncommon, and little research has been published about these injuries. Our aim was to evaluate the characteristics and results of treatment for these injuries. METHODS: Between 2006 and 2012, 35 concomitant hip and distal radius fractures were identified, comprising the study group. The characteristics and results of treatment for these injuries were evaluated and analyzed. Another matched control group with isolated hip fractures was collected for comparison of patient characteristics, fall mechanism, fracture pattern, b

DVR plating of distal radius fractures.

Volar plating has become the standard of care for most distal radius fractures. When done for the right indication and with adequate mastering of the technique complication ratio is low. The concept of subchondral support is key in this technique. Osteoporotic patients will especially benefit from this type of fixation which allows early immobilization, quick return to activities of daily living and early good outcome.

Distal radius fractures: an epidemiological review.

OBJECTIVE: Distal radius fractures are among the most common fractures encountered in clinical practice. Although multiple epidemiological studies have been done in Western populations, there is a lack of data within Asia. Our study aims to fill this knowledge gap. METHODS: This study included all patients aged 16 years and above with distal radius fractures seen between November 2008 and May 2009 by the Department of Hand and Reconstructive Microsurgery at the Singapore National University Hospital. A retrospective analysis was done. Information captured included clinical data, hand dominanc

[Current treatment of distal radius fractures].

Most fractures of the distal radius are treated with closed reduction and casting. Locking plates are the most common method of fixation. In young persons and in elderly persons with functional activity above that of their age level, a <10 degrees dorsal or <20 degrees volar angle of tilt, a radial shortening of <2 mm, a radial inclination of >15 degrees and an intra-articular step-off or gap of <1 to 2 mm are regarded as an acceptable limit of position of the radius. In persons over 65 years of age the functioning of the hand can usually be restored with casting.
