Sublingual gland excision for the surgical management of plunging ranula.

OBJECTIVE: A plunging ranula is a pseudocystic collection of mucin extravasated from the sublingual gland into the floor of mouth and through the mylohyoid muscle into the neck. While the lining of a ranula is non-secreting and resection of the sublingual gland is adequate for simple sublingual mucoceles, many surgeons attempt to address plunging ranulas with extensive transoral and transcervical dissections. We review our experience managing plunging ranulas with intraoral sublingual gland excision and ranula drainage alone. METHODS: This is a case series of patients with plunging ranulas wh

Plunging Ranula Occurring Without its Oral Counterpart: A Case Report.

A ranula is a lesion that arises from mucin spillage from the sublingual or submandibular gland ducts, beneath the mucosa of the oral floor. A plunging or cervical ranula dissects the mylohyoid muscle and appears as a submandibular swelling. Rarely, a plunging ranula is encountered without its oral counterpart. Here, we report a rare case of a plunging ranula that occurred without its oral counterpart in a 11-year-old male patient. We have also discussed the pathogenesis and treatment options for the ranula.

Sublingual-plunging ranula as a complication of supraomohyoid neck dissection.

Ranulas are rare cystic lesions resulting from damage or rupture of one or more of the ducts of the sublingual gland, that lead to mucus extravasation or dilatation of the gland's duct. Extravasation cysts are more common than retention cysts. We present a case of a 45-year-old male with a squamous cell carcinoma of the ventral surface of the tongue that was treated with excision of the oral lesion and bilateral supraomohyoid neck dissection without supplementary radiotherapy. A left myocutaneous platysma flap was raised for defect closure. Ten months postoperatively he presented complaining o

Treatment of ranula using carbon dioxide laser--case series report.

Ranulas are mucus extravasation phenomenon formed after trauma to the sublingual gland or mucus retention from the obstruction of the sublingual ducts. There are various methods for treating ranulas, including marsupialization with or without open packing, excision of ranula with or without removal of sublingual gland, and laser excision and vaporization of ranula. The authors present a case series report on the use of carbon dioxide laser treatment for ranula and a literature review of cases treated using carbon dioxide laser. The authors' experience and reports in the literature indicate tha

Treatment of ranula--excision of the sublingual gland versus marsupialization.

The purpose of this work is to estimate optimum surgical treatment of ranula according to the type of the lesion. Nine patients with ranula surgically treated between 1989 and 2000 were investigated retrospectively. Six patients had sublingual type ranula and three had submandibular type. In five cases including recurrence cases, the sublingual gland was excised. Marsupialization was performed for four cases, which were superficial, protruded and within 2 cm of diameter. In all cases, histopathological diagnoses were pseudocysts without epithelial lining and there was no recurrence. Almost all

A clinicopathological study on the associations between ranulas and autoimmune diseases.

The study aimed to investigate autoimmune disease (AD) prevalence in ranula patients, analyse their clinicopathological features, and explore the associations between ADs and ranulas. This cross-sectional study enrolled surgically treated ranula patients between 2021 and 2023. AD diagnosis was established. Clinical, serological, and histopathological features were compared. Among the 102 patients, 36.3% had confirmed ADs (predominantly primary Sjögren syndrome and undifferentiated connective tissue disease), 42.2% had no ADs, and 21.6% were suspected of ADs. In the confirmed AD group, 83.8% de

Oral Rehabilitation of a Child Superimposed with Tooth Impaction and Ranula.

INTRODUCTION: Dental impaction may be due to several etiological factors, including genetic conditions, lack of space, failure of primary tooth resorption and prolonged retention of these teeth, presence of intraosseous cysts, and trauma in the region. The ranula is a mucus-filled cyst-like mass that usually originates from extravasation of mucus from the sublingual gland. CASE DESCRIPTION: A 7-year-old Saudi female presented to the pediatric dental clinic at Prince Mohammed Bin Abdulaziz Hospital (PMBAH), Al-Madinah al-Munawwarah, Saudi Arabia, with complicated orodental problems that were i

A Giant Epidermoid Cyst in the Floor of Mouth Mimicking Ranula.

Dermoid cysts are benign ectodermal-derived epithelial cysts rarely found on the floor of the neck. They may be congenital or acquired. They comprise three histological variants according to their contents and include dermoid, epidermoid, and teratoma. Epidermoid cysts are lined by epithelium but do not contain skin appendages like hair follicles and sebaceous glands, as seen in dermoid cysts. Teratoma on the other hand contain mesodermal element. They reveal either a supra-myelohyiod or infra-myelohyiod floor-of-mouth location and can be clinically confused with various close differentials in

Comparison of outcomes of treatment for ranula: a proportion meta-analysis.

Treatment for ranula is classified into three categories depending on how the leaking saliva is managed: removal of the leaking site by intraoral or transcervical resection of the sublingual gland; formation of a drainage tract through the wall of the pseudocyst by micromarsupialisation or marsupialisation; or sealing of the site of the leak by inducing fibrosis with a sclerosing agent. Resection of the sublingual gland is probably the option most likely to be curative for both oral and plunging ranula considering their pathophysiology. Although alternative treatments have been introduced to a

Mucoepidermoid carcinoma mimicking a mucocele (ranula) in the floor of the mouth.

Mucoepidermoid carcinoma (MEC) is the most common salivary gland malignancy. The submandibular gland is rarely involved, about less than 10% of all other major salivary glands. We report a case of a 49-year-old female who presented with a mass on the left side of the floor of the mouth. She was complaining of neck pain for the last 2-3 days and had noticed the mass growing in size and becoming tender to palpation. There was no exacerbation of pain during eating, drinking, or opening of mouth. A multidetector CT scan was made. The radiographic examination revealed a mostly well-defined hypodens

Pediatric epidermoid cysts masquerading as ranulas: A case series.

Pediatric neck masses represent a variety of differential diagnoses. A common pathology in pediatric cystic neck tumors include ranulas, mucus retention cysts due to salivary gland obstruction. Epidermoid cysts are lesions infrequently encountered in the pediatric population and may appear similarly to ranulas on computed tomography imaging. MRI more easily differentiates these masses, and should therefore be the preferred imaging modality. Due to their distinct intraoperative management, ranulas and epidermoid cysts should be distinguished preoperatively through proper workup.

Successful management of oral ranula: a rare case in an infant.

An oral ranula is a retention cyst that arises from the sublingual gland as a result of ductal obstruction and fluid retention. This report describes the successful management of a rare case of oral ranula in an infant. A 4-month-old male infant was referred for emergency treatment due to a 2-month history of a swelling in the right sublingual region. The examination revealed a lesion of approximately 3.5 cm in diameter, which was jeopardizing the infant's breastfeeding. The lesion's dimensions suggested a ranula. Surgical specimens were sent for histopathological analysis to confirm the diagn

Marsupialization for treatment of oral ranula: a second look at the procedure.

Simple marsupialization to manage oral ranula has fallen into disfavor because of excessive failures and the high incidence of iatrogenically caused cervical ranula that may follow this procedure. With the simple addition of packing the entire pseudocystic cavity with gauze after its unroofing, the rate of recurrence is minimized. It is recommended that oral ranula be treated initially by marsupialization with packing and, if recurrence occurs, then the offending sublingual gland should be excised.

Ranulas--mucoceles of the oral cavity: experience in 26 children.

Ranulas or mucoceles of the floor of the mouth are uncommon but do occur in approximately 5% of patients undergoing submandibular duct relocation for the management of uncontrollable sialorrhea. Review of our experience with 26 patients over 12 years ending in 1986 substantiated a concept previously advocated by other authors that ranulas are usually extravasation pseudocysts developing after disruption of sublingual gland elements, and that excision of the ipsilateral sublingual gland is the management approach of choice.

Plunging ranula: a report of three cases and review of the literature.

Three cases of plunging ranula are reported and the literature reviewed. Extravasation of saliva from the sublingual gland due to trauma or obstruction of its ducts appears to be the most likely cause of plunging ranula. Available data suggest that the submandibular gland is usually not involved, although at the time of surgery it may be extremely difficult to exclude a submandibular origin of the cyst in the neck. Communication between the oral and cervical components of the plunging ranula probably occur via a hiatus in the mylohyoid muscle. Such communication passing directly into the subma

Clinical Outcomes of Polidocanol Foam Sclerotherapy in Treatment of Sublingual Ranula.

INTRODUCTION AND AIMS: Ranula is a mucus retention cyst originating from the sublingual gland, and surgical intervention has traditionally been the main treatment. However, surgery is often associated with recurrence, risk of nerve injury, and patient discomfort. Sclerotherapy using polidocanol, a low-toxicity agent, has emerged as a promising minimally invasive alternative. This study aimed to evaluate the clinical outcomes and safety of polidocanol injection therapy in patients with sublingual ranula. METHODS: This retrospective study included 34 patients with sublingual ranula who underwen

Unilateral Submandibular Gland Aplasia with Ipsilateral Sublingual Ranula - A Case Report.

BACKGROUND: Congenital aplasia of major salivary glands is a very rare entity, especially if it concerns an ipsilateral aplasia in a nonsyndromic patient. KEY POINTS FROM THE CASE: The aim of this report is to present a case of an aplasia of the left submandibular gland, which was incidentally diagnosed during presurgical imaging for an ipsilateral sublingual ranula. Histopathological evidence of the lack of sublingual gland tissue in the excised specimen of the ranula is discussed. MAIN LESSONS TO BE LEARNED FROM THIS CASE REPORT: Unilateral submandibular aplasia has unknown etiology. Clini
