Conversion to on pump from OPCAB is associated with increased mortality: results from a randomized controlled trial.

OBJECTIVE: Recent developments in cardiac stabilization devices, have made OPCAB feasible. However, there is conflicting evidence to date regarding the rate and adverse events associated with conversion from OPCAB to CABG with CPB. The objective of the present study was to review all conversions in the setting of a randomized controlled trial comparing CABG with or without CPB. METHODS: Three hundred patients requiring CABG surgery at a single institution were prospectively randomized to have the procedure performed with CPB (n=150) or OPCAB (n=150). Patients that could not be performed OPCAB

[Transfer of single embryo as a method to reduce twins pregnancy rate in in-vitro fertilization treatment].

INTRODUCTION: The high success rate in IVF treatments is followed by a high rate of multiple pregnancies. Over 30% of IVF pregnancies are multiples and carrying higher risk to the neonates compared with singleton pregnancy. Twin pregnancy is less dangerous then high order multiple pregnancy but it has a significantly higher risk factor compared with singleton pregnancy. Therefore, it is crucial to find methods to reduce twin pregnancy rate. The goal of this review is to present the peril of twin pregnancy and to evaluate the alternative of selective single embryo transfer (SET) in order to red

[Assessment of Tricuspid Insufficiency and the Function of Right Ventricle Using Cardiac Magnetic Resonance Imaging Combined with Echocardiography].

Right-sided cardiac valvular diseases have traditionally been considered less important than disease of mitral or aortic valve. However, severe tricuspid regurgitation could lead to right ventricle dysfunction and reduce patients' survival rate. In clinic setting, tricuspid valve disease should be paid more attention for patients with secondary tricuspid regurgitation caused by left-sided valvular surgery combined with irreversible annular dilatation increasing the risk of reoperation. In this review, we summarize the epidemiology, anatomy, pathology, diagnosis, ultrasound and cardiac magnetic

Pterygium recurrence rate at the Princess Alexandra Hospital.

Over two separate time periods, 136 consecutive pterygium operations in 103 patients were performed at the Princess Alexandra Hospital. These patients were evaluated for recurrence of their pterygia in a retrospective study by chart review and re-examination. In 136 eyes there was an overall recurrence rate of 46% using a number of therapies. This study highlights the relatively unsatisfactory result of pterygium removal in a day surgery and eye outpatient department surgical setting by a number of trainee ophthalmologists, and stresses the need for a reassessment of pterygium and its treatmen

Cerebrovascular disease in the era of left ventricular assist devices with continuous flow: risk factors, diagnosis, and treatment.

Cerebral infarction and hemorrhage are among the principal sources of morbidity and mortality associated with mechanical circulatory support. There has been particular concern with the recently reported increased rate of device thrombosis in continuous-flow left ventricular assist devices. The neurologic management of stroke among these patients has limited data to inform current clinical practices. In this investigation we review the available literature on stroke in patients with continuous-flow left ventricular assist devices, with a focus on treatment algorithms in the acute setting and fo

Success of junctional anterior cervical discectomy and fusion.

BACKGROUND CONTEXT: Junctional breakdown has long been a consideration for surgeons when performing an arthrodesis in the cervical spine. Numerous authors have reported symptomatic junctional disease after fusion with varying degrees of reoperation. To our knowledge, there are no large series recording the fusion rate using instrumentation as an adjuvant to bone grafting. PURPOSE: To determine the fusion rate when arthrodesis is performed in the setting of junctional stenosis using iliac crest autograft and instrumentation. STUDY DESIGN/SETTING: This is a retrospective review performed on pa

Incidence of Infectious Mononucleosis in Universities and U.S. Military Settings.

OBJECTIVE: The reported incidence rates for Infectious Mononucleosis (IM) within universities and military settings vary widely from study to study. Several factors may have contributed to the discrepancy in these incidence rates include misdiagnosis, ambiguity in the reported sample populations, and number of students who visited and were diagnosed at their campus's health service centers. The current review examines previously reported literature on the incidence rate in universities and military settings of infectious mononucleosis taking into account these possible confounding factors. Comparison of intrapartum fetal heart rate tracings in patients with neonatal seizures vs. no seizures: what are the differences?

OBJECTIVE: To determine which intrapartum fetal heart rate parameters in the presence of severe neonatal acidosis (pH < 7.0) appropriately predicts the development of neonatal seizures in the context of hypoxic ischemic encephalopathy (HIE). METHODS: The intrapartum fetal heart rate tracings of 25 neonates who developed neonatal seizures secondary to HIE were compared with 25 matched neonates with similar pH and gestational age who did not develop seizures. All patients had at least 2 hours of intrapartum fetal heart rate patterns available for review. We compared the fetal heart rate paramet
