Update on orbital trauma.

PURPOSE OF REVIEW: To evaluate the recent developments in the evaluation and treatment of orbital trauma from April 2003 to March 2004. The scope of orbital trauma included a MEDLINE search under the keywords of zygoma and orbital fracture, and traumatic optic neuropathy. RECENT FINDINGS: Preoperative volume assessment with computed tomography of orbital fractures can help surgical planning of volume replacement. Ultrasonography can visualize anterior orbital fractures, including rim and zygoma trauma. Orbital trauma has a high incidence of concurrent ocular trauma. Fractures in the presence

Primary oculomotor nerve malignant peripheral nerve sheath tumor in an infant: reflex-mediated biopsy failure and tubular port-assisted diagnostic escalation.

BACKGROUND: Intracranial malignant peripheral nerve sheath tumors (MPNSTs) are exceptionally rare, and primary origin from cranial nerves is particularly uncommon. Involvement of the oculomotor nerve represents an extraordinarily rare entity. Their deep mesencephalic location, nonspecific imaging characteristics, and proximity to critical neurovascular structures pose major diagnostic challenges. In addition, surgical manipulation near the third cranial nerve may trigger trigeminocardiac or oculocardiac reflex (TCR/OCR), resulting in severe hemodynamic instability and potentially limiting mini

Trigeminocardiac reflex: a unique case of recurrent asystole during bilateral trigeminal sensory root rhizotomy.

BACKGROUND: The trigeminocardiac reflex is the sudden-onset of dysrhythmia and hypotension during manipulation of any of the branches of the trigeminal nerve. The trigeminal nerve and cardioinhibitory vagus nerve constitute the afferent and efferent pathways in the reflex arc. The trigeminocardiac reflex has been reported to occur during craniofacial surgery, balloon-compression rhizolysis of the trigeminal ganglion, and tumour resection in the cerebellopontine angle. PATIENT & METHOD: A 2-year-old male patient with haemangioma near the sella turcica underwent rhizotomies of both sides of the

[Ocular hemodynamics following suction cup oculopression].

In the study reported here the authors investigated the influence on ocular perfusion pressures of suction-cup oculopression, a procedure which permits oculopression without orbital compression. Patients over 50 years of age were examined before cataract extraction. Ulrich's method of ocular oscillodynamography was used to determine ocular perfusion pressures. A Mikuni and Yoneyama suction-cup dynamometer was applied for suction-cup oculopression. Ocular hypotonia following suction-cup oculopression was adequate and comparable to that achieved by other oculopression methods. After oculopressio

Adequacy of Anesthesia Guidance Combined with Peribulbar Blocks Shows Potential Benefit in High-Risk PONV Patients Undergoing Vitreoretinal Surgeries.

Background/Objectives: Postoperative nausea and vomiting (PONV) are common after general anesthesia (GA) and, in patients undergoing vitreoretinal surgery, may be triggered by the oculocardiac reflex (OCR) leading to the oculoemetic reflex (OER). Inadequate dosing of intravenous rescue opioid analgesics may further provoke OCR. Adequacy of Anesthesia (AoA) monitoring enables optimized titration of intravenous rescue opioid analgesics, while preemptive intravenous or peribulbar analgesia may reduce opioid use. This study evaluated the impact of preemptive paracetamol or peribulbar block (PBB) c

Motion sickness occurrence does not correlate with nystagmus characteristics.

The purpose of this study was to test the hypothesis whereby eye movements as such may be an important factor in the development of motion sickness (MS). The horizontal eye movements of 27 subjects were measured during earth vertical axis rotation (EVAR) and during off vertical axis rotation (OVAR). Two groups were set up, one including subjects who suffered severe MS during the test, and the other including subjects with no MS symptoms. We found no differences in nystagmus parameters (EVAR: gain and time constant; OVAR: eye velocity modulation and eye position modulation) between the MS and t

Current trends in ophthalmic anesthesia. The Second Walter S. Atkinson Lecture.

General anesthesia permits surgery involving large operative fields without limit to time. It is essential in children and uncooperative adults and provides an immobile, quiet patient who does not recall the operative procedure. Local and regional anesthesia re indicated when the surgical field is limited in size and extent. Neuroleptanesthesia in ophthalmology usually involves the administration of droperidol followed by fentanyl followed in intubation, sometimes using a muscle relaxant, and maintenance of anesthesia with nitrous oxide. The terrifying dreams produced by ketamine may be minimi

Anesthetic management for pediatric strabismus surgery: Continuing professional development.

PURPOSE: Strabismus surgery is one of the most common pediatric ophthalmic procedures. The purpose of this continuing professional development module is to update physicians on the anesthetic considerations of pediatric patients undergoing strabismus surgery. PRINCIPAL FINDINGS: The preoperative assessment is important, as patients undergoing strabismus surgery may have an associated neuromuscular disorder, congenital syndrome, or cardiac disease. Malignant hyperthermia is no longer considered as being an issue associated with strabismus. The laryngeal mask airway is used frequently and has b

[Angelman syndrome and severe vagal hypertonia. Three pediatric case reports].

Angelman's syndrome is an association of severe mental retardation with absence of language, ataxia, convulsions and hyperactive, joyful behaviour with frequent bouts of laughing. Genetic diagnosis is possible in about 80% of cases. No cardiovascular abnormalities have been described in this syndrome to date. The authors report the cases of three children with Angelman's syndrome who presented with severe malaise due to increased vagal tone. The age of onset of symptoms was between 20 months and 8 years. One of the children had malaises triggered by bouts of laughing. The diagnosis was confirm

Correlations between urinary excretion of catecholamines and electrocardiographic parameters of vagal hyperreactivity in infants with fainting spells. Implication of sympathetic hypotonia?

BACKGROUND: Vagal hyperreactivity (VHR) is a frequent etiology of infant fainting spells; but it is sometimes difficult to diagnose. A biochemical test would therefore be useful, especially as the oculocardiac reflex (OCR) test innocuity is not absolute. AIMS: To evaluate urinary excretions of norepinephrine, epinephrine and dopamine as markers for vagal hyperreactivity. METHODS: During check-up of 55 infants from 0.5 to 11 months of age, for discomfort episodes, including OCR and Holter recording, 24h urinary assays of total norepinephrine, epinephrine and dopamine were carried out to evalu

Life-threatening complications in ophthalmic surgery: a systematic review.

Ophthalmic surgical procedures are widely acknowledged for their safety and efficacy. Undoubtedly, advances in ophthalmic surgery, along with the improvement of anaesthetic techniques, have contributed to the reduction in the occurrence of these events. However, although uncommon, systemic severe and life-threatening adverse events can still occur and it is imperative for an ophthalmologic surgeon to have a comprehensive understanding of them to act in terms of proactive prevention, prompt recognition, and optimal treatment, thus maximizing patients' outcomes. Among life-threatening complicati

A Comparison between the Effects of Propofol and Sevoflurane in Pediatric Strabismus Surgery on the Quality and Depth of Anesthesia.

BACKGROUND: Strabismus surgery may be associated with several undesirable complications as increased incidence of the oculocardiac reflex (OCR), hemodynamic changes, emergency agitation (EA), postoperative pain, nausea, and vomiting. Previous studies suggested that deeper anesthesia monitored by bispectral index (BIS) protects against OCR. This study aims to evaluate the effect of the type of anesthesia on the quality of anesthesia in pediatric patients. Clinical analysis of internal orbital fractures in children.

In order to describe the demographics, etiologic and clinical factors, and outcomes of orbital fractures in children, we have reviewed a case series of 17 patients under 18 years of age with internal orbital fractures (i.e., without involvement of the orbital rim) presenting to the Ghil hospital between March 2000 and June 2001. For 15 of the patients, we performed orbital wall reconstruction with Medpor barrier sheet implantation (thickness 1mm) through transconjunctival approach under endoscopic guidance, while maintaining mere observation on the other 2 patients. There were 14 male and 3 fe
