Hemodynamic instability during treatment of intracranial dural arteriovenous fistula and carotid cavernous fistula with Onyx: preliminary results and anesthesia considerations.

BACKGROUND AND AIM: 9 patients with 10 arteriovenous fistulas were treated with Onyx at our institution over a period of 19 months: 4 direct and indirect carotid-cavernous fistulas (CCFs) and 6 dural arteriovenous fistula (DAVFs). Complete occlusion was achieved with no recurrences or permanent complications in our small series. We report hemodynamic instability, including severe bradycardia and asystole, during embolization of DAVF and CCF with Onyx in several patients. These changes were reversible with interruption of Onyx injection and administration of atropine. No recurrence of symptoms

Is the Occurrence of Bradycardia During Percutaneous Balloon Compression for Trigeminal Neuralgia a Clinical Prognostic Indicator?

BACKGROUND: Trigeminal ganglion percutaneous balloon compression (PBC) for treating trigeminal neuralgia (TN) is favored for its high success rate and minimal complications. During PBC, significant hemodynamic changes can occur, leading to sudden bradycardia, asystole, or rapid blood pressure increase, collectively known as the trigeminocardiac reflex (TCR). Isolated bradycardia during PBC is common. Despite this, mortality risk is very low, with bradycardia becoming critical in few patients. This unknown mechanism might indicate effective compression on Gasser's ganglion, crucial for clinical

The effect of a face mask for respiratory support on breathing in preterm infants at birth.

OBJECTIVE: Applying a mask on the face for respiratory support could induce a trigeminocardiac reflex leading to apnoea and bradycardia. We have examined the effect of applying a face mask on breathing and heart rate in preterm infants at birth. METHODS: Resuscitation videos of infants ≤ 32 weeks gestation recorded from 2010 until 2018 at the Leiden University Medical Centre and the General University Hospital in Prague were reviewed. All infants received respiratory support via face mask. Breathing and heart rate were noted before and after application of the face mask and over the first 5 m

Perioperative adverse cardiac events in maxillofacial surgery: A systematic review and meta-analysis.

BACKGROUND AND AIMS: Maxillofacial surgeries, including procedures to the face, oral cavity, jaw, and head and neck, are common in adults. However, they impose a risk of adverse cardiac events (ACEs). While ACEs are well understood for other non-cardiac surgeries, there is a paucity of data about maxillofacial surgeries. This systematic review and meta-analysis report the incidence and presentation of perioperative ACEs during maxillofacial surgery. METHODS: We included primary studies that reported on perioperative ACEs in adults. To standardise reporting, ACEs were categorised as 1. heart r

Effect of initial and subsequent mask applications on breathing and heart rate in preterm infants at birth.

OBJECTIVE: Application of a face mask may provoke the trigeminocardiac reflex, leading to apnoea and bradycardia. This study investigates whether re-application of a face mask in preterm infants at birth alters the risk of apnoea compared with the initial application, and identify factors that influence this risk. METHODS: Resuscitation videos and respiratory function monitor data collected from preterm infants <30 weeks gestation between 2018 and 2020 were reviewed. Breathing and heart rate before and after the initial and subsequent mask applications were analysed. RESULTS: In total, 111 i

N-hexyl cyanoacrylate (Magic Glue) for middle meningeal artery embolization: a retrospective multicenter analysis.

BACKGROUND: Chronic subdural hematoma (cSDH) is common, with an incidence of up to 5/100,000, increasing with age. Surgery carries a recurrence risk of around 20%. Three randomized controlled trials demonstrated the benefit of middle meningeal artery embolization (MMAE) using ethylene vinyl alcohol (EVOH)-based embolics like Onyx (Medtronic) or Squid (Balt Group). Other agents, including particles and N-butyl cyanoacrylate (NBCA), are under investigation. N-hexyl cyanoacrylate (Magic Glue, Balt Group) is a newer NBCA-based embolic with slower precipitation, enabling potentially safer and more

Recurrent Bronchospasm During Intra-arterial Chemotherapy via IMAX Access for Retinoblastoma: A Case Report.

Intra-arterial chemotherapy (IAC) has become an increasingly popular approach for treating advanced retinoblastoma. It offers targeted drug delivery and reduced systemic toxicity when compared to systemic chemotherapy. Respiratory complications are rare, but this case presents a patient who experienced recurrent episodes of intraoperative bronchospasm possibly linked to an anatomical variation. A six-month-old female with Group D retinoblastoma in her right eye underwent five cycles of IAC within six months of diagnosis. Due to failed visualization of the right ophthalmic artery during angiogr

The Effect of Depth of Anesthesia on Hemodynamic Changes Induced by Therapeutic Compression of the Trigeminal Ganglion.

BACKGROUND: Percutaneous compression of the trigeminal ganglion (PCTG) has been used to treat trigeminal neuralgia since 1983. A PCTG-related trigeminocardiac reflex (TCR) can induce dramatic hemodynamic disturbances. This study investigates the effects of depth of propofol anesthesia on hemodynamic changes during PCTG. MATERIALS AND METHODS: A total of 120 patients who underwent PTCG for trigeminal neuralgia were randomly assigned to control group-intravenous saline pretreatment before PCTG puncture and anesthesia targeted to bispectral index (BIS) 40 to 60 throughout, and study group-intrav

Comparing the effect of two different interfaces on breathing of preterm infants at birth: A matched-pairs analysis.

OBJECTIVE: Applying a face mask could provoke a trigeminocardiac reflex. We compared the effect of applying bi-nasal prongs with a face mask on breathing and heart rate of preterm infants at birth. METHODS: In a retrospective matched-pairs study of infants <32 weeks of gestation, the use of bi-nasal prongs for respiratory support at birth was compared to the use of a face mask. Infants who were initially breathing at birth and subsequently received respiratory support were matched for gestational age (±4 days), birth weight (±300 g), general anaesthesia and gender. Breathing, heart rate and o

Continuous Eye Movement Monitoring via Superior Orbital Fissure Electrode Placement in Orbital and Cranio-orbital Lesion Surgery: An Initial Consecutive Case Series.

Orbital and cranio-orbital lesions are often benign, but postoperative ocular misalignment or diplopia can impair the quality of life. Eye movement monitoring has mainly been used for intermittent mapping for localization; however, serial, stimulus-evoked surveillance with prompt alerts remains challenging. We report an initial consecutive case series evaluating feasibility, safety, and early outcomes of continuous, stimulus-evoked electro-oculography monitoring using repetitive direct stimulation through an electrode placed adjacent to the superior orbital fissure. We retrospectively reviewed

Second Impact Syndrome. Myth or reality?

INTRODUCTION: Second impact syndrome (SIS) is a devastating condition occurring in sport-induced mild brain injury. SIS is drastically defined by anamnestic, clinical and radiological criteria, which is unusual in the field of cranial traumatology. The purpose of this study was to provide a literature review of this syndrome. MATERIAL AND METHODS: We conducted a literature review of all published studies on PubMed. The keywords were "second impact syndrome and catastrophic head injury", "second impact syndrome and sport", "repeat concussion and catastrophic brain injury", "catastrophic head i

The Styloid-Pterygoid Line: A Reproducible Anatomic Landmark for V3 Perineural Spread Biopsy.

Perineural spread (PNS) along the mandibular division (V3) of the trigeminal nerve is an important prognostic finding in head and neck malignancies that leads to disease upstaging and treatment intensification. Histologic confirmation traditionally requires invasive open biopsy or challenging CT/fluoroscopically guided access through foramen ovale, which carries risks of hemorrhage, infection, and trigeminocardiac reflex activation. We describe a CT-guided transcondylar approach using the styloid-pterygoid line as an anatomic landmark to safely access the mandibular nerve within the infratempo
