Vestibulo-ocular reflex dynamics with head-impulses discriminates Usher patients type 1 and 2.

Usher Syndrome classification takes into account the absence of vestibular function but its correlation with genotype is not well characterized. We intend to investigate whether video Head Impulse Test (vHIT) is useful in screening and to differentiate Usher Syndrome types. 29 Usher patients (USH) with a genetically confirmed diagnosis and 30 healthy controls were studied with vHIT and dizziness handicap inventory questionnaire (DHI). Statistical significant differences between USH1, USH2 and controls were found in the vestibulo-ocular-reflex (VOR) gain of all SCCs, with USH1 patients consiste

Associations Between the Vestibulo-Ocular Reflex and Cognitive Function in de novo Parkinson's Disease.

OBJECTIVE: Vestibulo-ocular reflex (VOR) impairment has been reported in Parkinson's disease (PD). However, its clinical implications, particularly with respect to cognition, remain unclear. We investigated canal-specific VOR changes and their associations with cognitive function, motor symptoms, gaits, and dopamine transporter (DAT) uptake in de novo PD patients. METHODS: We prospectively enrolled 127 patients with de novo PD who underwent video head-impulse tests (video-HITs), comprehensive neuropsychological assessments, gait analysis, and 18F-N-(3-fluoropropyl)-2β-carbon ethoxy-3β-(4-iodo

Differences in Left-Right Vestibulo-Ocular Reflex Gain in the Video Head Impulse Test.

Objectives: The objective of this study was to evaluate left-right symmetry of vestibulo-ocular reflex (VOR) gain on the video head impulse test (vHIT) and its diagnostic implications relative to caloric test results. Methods: Patients presenting with dizziness who underwent a bithermal caloric test and vHIT were retrospectively analyzed. Of 764 patients identified, nine with bilateral vestibular hypofunction were excluded, leaving 755 patients for analysis (451 showing symmetric caloric responses; 304 with unilateral vestibular hypofunction (UVH)). Horizontal VOR gain was compared between dir

Transiently worse postural effects after vestibulo-ocular reflex gain-down adaptation in healthy adults.

Suffering an acute asymmetry in vestibular function (i.e., vestibular neuritis) causes increased sway. Non-causal studies report associations between lateral semicircular canal function and balance ability, but direct links remain controversial. We investigate the immediate effect on body sway after unilateral vestibulo-ocular reflex (VOR) gain down adaptation simulating acute peripheral vestibular hypofunction. Eighteen healthy adults, mean age 27.4 (± 12.4), stood wearing an inertial measurement device with their eyes closed on foam before and after incremental VOR gain down adaptation to si

Vestibulo-ocular reflex tests may reflect unilateral Ménière disease progression: A cross-sectional study.

Vestibular disorders can manifest several patterns of horizontal vestibulo-ocular reflex (VOR) impairment, which can be detected by caloric test and video head impulse test (vHIT). Several studies have examined the patterns of caloric-vHIT response in Ménière disease (MD). The purpose of this study was to investigate the diversity of caloric-vHIT response and its related factors in unilateral MD patients. We also explore the possibility of assessing the progression of unilateral MD by using the horizontal VOR tests.Ninety-eight patients with unilateral MD were enrolled and underwent a battery

The vestibulo-ocular reflex assessment in patients with Ménière's disease: examining all semicircular canals.

UNLABELLED: Abstract Conclusion: The distribution of abnormal results is not uniform between different canals in each patient; the most frequent gain reduction is obtained for the posterior canal. Gain reduction reflects the disease duration and amount of hearing loss. OBJECTIVE: To test the hypothesis that the vestibulo-ocular reflex (VOR) evoked after stimulation of each semicircular canal behaves in a different manner in patients with unilateral definite Ménière's disease. METHODS: We studied the VOR evoked by rapid head-impulses in the plane of the 6 semicircular canals in 36 patients. I

Vertical vestibulo-ocular reflex, smooth pursuit and eye-head tracking dysfunction in internuclear ophthalmoplegia.

Vertical smooth eye movements were quantified by magnetic search coil oculography in 7 patients with internuclear ophthalmoplegia (INO), 4 bilateral and 3 unilateral, and compared with age-matched normal subjects. The upward and downward vestibulo-ocular reflex (VOR) had reduced gain and abnormal phase lag in both unilateral and bilateral INO during active head motion at frequencies from 0.25 to 2 Hz. The phase lag was attributed to disruption of vestibular eye velocity signals and relative preservation of eye position signals. Fixation of a stationary target enhanced smooth eye movement gain

Cervico-Ocular and Vestibulo-Ocular Reflexes in Subclinical Neck Pain and Healthy Individuals: A Cross-Sectional Study.

Alterations in neck sensory input from recurrent neck pain (known as subclinical neck pain (SCNP)) result in disordered sensorimotor integration (SMI). The cervico-ocular (COR) and vestibulo-ocular (VOR) reflexes involve various neural substrates but are coordinated by the cerebellum and reliant upon proprioceptive feedback. Given that proprioception and cerebellar processing are impaired in SCNP, we sought to determine if COR or VOR gain is also altered. COR and VOR were assessed using an eye-tracking device in 20 SCNP (9 M and 11 F; 21.8 (SD = 2.35) years) and 17 control (7 M and 10 F; 22.40

Abolished tilt suppression of the vestibulo-ocular reflex caused by a selective uvulo-nodular lesion.

Within the cerebellum several structures are considered to modulate the function of the vestibulo-ocular reflex (VOR). The nodulus and uvula have been implicated with the low-frequency components of the VOR, which are mediated by the velocity storage system. We report a patient with a selective lesion of the nodulus and ventral uvula. The findings suggest that nodulus and uvula normally exert an inhibitory effect on the velocity storage mechanism.

Vestibulo-Ocular Reflex Results in Patients with Intralabyrinthine Schwannomas: Case Series with a Literature Review.

Background and Clinical Significance: Intralabyrinthine schwannoma (ILS) is a rare benign tumor of the inner ear, often presenting with nonspecific symptoms such as hearing loss, tinnitus and vertigo. Vestibular function in ILS patients remains underexplored. This study aims to evaluate vestibulo-ocular reflex (VOR) function and inner ear magnetic resonance imaging (MRI) signal changes in ILS, and to provide insights into potential mechanisms underlying vestibular dysfunction. Case Presentation: We report four cases of MRI confirmed ILS, including two intravestibular and two intravestibulococh

Enhanced Vestibulo-Ocular Reflex Responses on vHIT. Is It a Casual Finding or a Sign of Vestibular Dysfunction?

In current clinical practice, when in response to vHIT testing the observed slow-phase eye-velocity responses are significantly higher than head velocity, the most probable cause is considered to be an inadequate collection method or a recording artifact. We present two cases with clinical diagnoses of Menière's Disease: for both cases, enhanced eye velocity responses were measured with a rigorous vHIT testing protocol. In the first case we measured these enhanced responses on each test performed during a 5 year time series; in the second case multiple measurements were obtained from a patient

The nodulus and uvula: source of cerebellar control of spatial orientation of the angular vestibulo-ocular reflex.

The nodulus and rostral-ventral uvula of the vestibulo-cerebellum play a critical role in orienting eye velocity of the slow component of the angular vestibulo-ocular reflex (aVOR) to gravito-inertial acceleration (GIA). This is done by altering the time constants of "velocity storage" in the vestibular system and by generating "cross-coupled" eye velocities that shift the eye velocity vector from along the body yaw axis to the yaw axis in a spatial frame. In this report, we show that eye velocity generated through the aVOR by constant velocity centrifugation in the monkey orients to the GIA i

Neurotoxic mechanism of cinnabar and mercuric sulfide on the vestibulo-ocular reflex system of guinea pigs.

Cinnabar, a naturally occurring mercuric sulfide (HgS), has been combined with Chinese herbal medicine as a sedative for more than 2000 years. To date, its neurotoxic effect on the vestibulo-ocular reflex (VOR) system has not been reported. By means of a caloric test coupled with electronystagmographic recordings, the effect of commercial HgS and cinnabar on the VOR system of guinea pigs was studied. HgS or cinnabar was administered orally (1.0 g/kg) to Hartley-strain guinea pigs once daily for 7 consecutive days. A battery of electrophysiological, biochemical, and histopathological examinatio

Evaluation of the human vestibulo-ocular reflex at high frequencies with a helmet, driven by reactive torque.

A new technique was developed to test the VOR in humans. A tightly but comfortably fitting helmet was provided with an electronically controlled torque motor that rotated a mass around a vertical axis. Acceleration of the motor caused reactive torque of the helmet in the opposite direction which was transmitted to the (freely movable) head. Sinusoidal head oscillations in the frequency range 2-20 Hz (peak-to-peak amplitudes about 10 degrees and 0.1 degrees respectively) were easily achieved, as well as step displacements of the head with accelerations on the order of 1000 degrees/s2. Limitatio

An Integrated System for Comprehensive Mouse Peripheral Vestibular Function Evaluation Based on Vestibulo-ocular Reflex.

PURPOSE: In the fields of both vestibular and auditory research, reliable vestibular function tests are essential. However, unlike the auditory function tests, which use standard Auditory Brainstem Response (ABR) equipment, there is no equivalent widely adopted apparatus for vestibular tests. Vestibulo-ocular reflexes (VORs) are the compensatory ocular reflexes that ensure stable vision during head motion. VORs are widely used in clinics to diagnose vestibular deficits. In the research, VORs have been used by various groups to evaluate the mouse vestibular function. However, the effectiveness
