Control of deaths from diarrheal disease in rural communities. I. Design of an intervention study and effects on child mortality.

From May through October 1980, the "Strengthening Rural Health Delivery" project (SRHD) under the Rural Health Department of the Ministry of Health of Egypt had conducted an investigation into prevention of child mortality from diarrheal disease through testing various modules of Oral Rehydration Therapy delivery mechanisms. In a six-cell design counting a total of almost 29,000 children, ORT was provided both as hypotonic sucrose/salt solution prepared and administered by mothers and normotonic, balanced electrolyte solution in the hands of both mothers and health care providers and the effec

Case management of cholera.

Cholera still affects about three million people a year and kills approximately 100,000. Cholera can be effectively managed in the majority of cases with oral rehydration solution alone. Up to one third of patients present with severe dehydration, which can be diagnosed clinically, and will require rapid intravenous rehydration with Ringers Lactate or other appropriate fluid before being managed with oral rehydration solution. Antibiotics reduce the duration of illness and should be used in patients with severe dehydration. Resistance is common and local sensitivities should guide the choice o

[Therapy of hypertonic dehydration].

Hypertonic dehydration developed in 30 patients (age ranged from 17 days to 28 months) with acute gastroenteritis. Rehydration was achieved with i.v. solutions, early potassium replacement, and especially with oral solutions (WHO-solution: 90 mval/l Na+ content). The average amount of sodium and fluids was 23.2 mval/kg/d and 270 ml/kg/d respectively to cause an average drop in sodium of 0.6 mval/h. There were no CNS-complications or deaths.

Oral hydration therapy for children in Florida.

Eighty-six children in Florida died of complications associated with diarrhea between 1985 and 1990, deaths which constituted an important preventable fraction of infant mortality. The state will support health professionals in reducing the number of hospitalizations and deaths due to diarrheal complications, Governor Lawton Chiles announced in September 1991, and the Florida Department of Health and Rehabilitative Services is being awarded a $100,000 grant from the Centers for Disease Control for a three-year study on the effectiveness and utilization of oral rehydration therapy. Crystalloids versus colloids for resuscitation in shock.

The optimal composition of fluid for volume resuscitation in critically ill patients has been the subject of controversy for decades. Clinicians are faced with several options, including crystalloid solutions of varying tonicity, several colloid preparations (albumin and others), and blood products. Some of these solutions may be differentially distributed between the intra- and extravascular, and intra- and extracellular compartments, accounting for a variety of physiological effects. Two recently published meta-analyses concluded that colloids afford no survival benefit in critically ill pat

[Hypotension and hydration during spinal anesthesia for caesarean section].

Spinal anesthesia is the technique of choice for elective cesarean section. It is easily achievable, fast and reliable technique. Maternal hypotension is a complication of spinal anesthesia, which is associated with adverse effects on both the mother and the fetus. Thus, the aim of the anesthesiologists should be to treat maternal hypotension quickly and efficaciously, or better yet, to actively prevent it. In current practice the application of vasopressors to maintain arteriolar tone is the most widely used strategy for the prevention of hypotension induced by spinal anesthesia. Other method

Performance parity in cartilage repair: SPMK-g-PEEK versus cartilage-cartilage interfaces.

Effective fluid exudation and rehydration are essential for the low-friction function of healthy articular cartilage, facilitating interstitial fluid pressurisation, solute transport, and aqueous lubrication. However, current metallic biomaterials used in focal cartilage repair or hemiarthroplasty compromise this fluid-pressure dependent load support, leading to the erosion of the interfacing cartilage. This study investigates bioinspired hydrophilic 3-sulfopropyl methacrylate potassium salt (SPMK) polymer grafted onto a PEEK substrate (SPMK-g-PEEK) as a potential solution. SPMK-g-PEEK aims to

Gastroenteritis Rehydration Of children with Severe Acute Malnutrition (GASTROSAM): A Phase II Randomised Controlled trial: Trial Protocol.

BACKGROUND: Children hospitalised with severe acute malnutrition (SAM) are frequently complicated (>50%) by diarrhoea ( ≥3 watery stools/day) which is accompanied by poor outcomes. Rehydration guidelines for SAM are exceptionally conservative and controversial, based upon expert opinion. The guidelines only permit use of intravenous fluids for cases with advanced shock and exclusive use of low sodium intravenous and oral rehydration solutions (ORS) for fear of fluid and/or sodium overload. Children managed in accordance to these guidelines have a very high mortality. The proposed GASTROSAM tri

Optimisation of HPMC ophthalmic inserts with sustained release properties as a carrier for thermolabile therapeutics.

A methodology was developed and optimised for the preparation of a new drug delivery system (DDS) with sustained release properties to allow ocular protein delivery and to limit destructive production steps during manufacturing. Elevated temperatures, shear forces and an oxidative environment should be avoided in order to prevent denaturation or oxidation of proteins. An aqueous HPMC solution was prepared using heat and casted into small semi-rod-shaped PVC blisters. The polymer solution was allowed to cool down and was partially dehydrated at room temperature. A drug solution containing glyce

Oral rehydration of malnourished children with diarrhoea and dehydration: A systematic review.

Background: Diarrhoea complicates over half of admissions to hospital with severe acute malnutrition (SAM). World Health Organization (WHO) guidelines for the management of dehydration recommend the use of oral rehydration with ReSoMal (an oral rehydration solution (ORS) for SAM), which has lower sodium (45mmols/l) and higher potassium (40mmols/l) content than old WHO ORS. The composition of ReSoMal was designed specifically to address theoretical risks of sodium overload and potential under-treatment of severe hypokalaemia with rehydration using standard ORS. In African children, severe hypon

Carbohydrate-electrolyte rehydration protects against intravascular volume contraction during colonic cleansing with orally administered sodium phosphate.

BACKGROUND: Colonic cleansing with sodium phosphate causes intravascular volume contraction in some patients. This study tested the hypothesis that carbohydrate-electrolyte oral rehydration would attenuate the hypovolemic changes associated with administration of sodium phosphate. METHODS: Adult outpatients were randomized to ingest either regular clear fluids or a carbohydrate-electrolyte rehydration solution during precolonoscopy purgation by ingestion of aqueous sodium phosphate. Investigators were blinded to the treatment assignment of patients. Clinical hemodynamic measurements and bioch

Rehydration with fructose worsens dehydration-induced renal damage.

BACKGROUND: Increasing evidence suggests heat stress induced chronic kidney disease (CKD) may be mediated by endogenous fructose generation and may be exacerbated by rehydration by fructose-containing solutions. We have recently reported a model of CKD induced by heat stress. Here we test the hypothesis that rehydration with fructose may induce worse kidney injury than rehydration with equal amounts of water, and we also test if this fructose-induced injury is associated with activation of inflammasomes in the kidney. METHODS: Mice were recurrently exposed to heat (39.5 C0 for 30 min/h, 5 tim

[Oral rehydration in newborns with dehydration caused by diarrhea].

The clinical experience obtained while treating 43 dehydrated newborns due to diarrhea with oral rehydration solution (ORS) using the formula recommended by the World Health Organization is reported. Of the 43 patients, 26 were severely dehydrated (greater than equal to 10% of weight recovery once rehydrated). The averaged time need to correct the dehydration was 4.7 +/- 2.7 hours, with a average intake of ORS of 26.5 +/- 7.5 mL/kg/hour. Children who were being breastfed continued so during the rehydration period. Two of the patients were hospitalized for intravenous treatment, one was due to

Treatment of cholera-like diarrhoea with oral rehydration.

Cholera diarrhoea remains a major global health problem that has caused seven pandemics. The pathogenesis of cholera is attributable to the production of cholera toxin by the causative pathogen, Vibrio cholerae. The toxin causes increased production of cyclic adenosine monophosphate and this results in massive water and electrolyte secretion into the intestinal lumen. These changes manifest clinically as the painless defecation of voluminous stools that resemble 'rice water', leading to severe dehydration. The cornerstone in the management of cholera diarrhoea is the use of oral rehydration so
