Primary care management of the liver transplant patient.

The patient referred for liver transplantation typically has complications from a progressive, irreversible liver injury. Less traditional complications of end-stage liver disease, such as bone disease and some hepatobiliary malignancies, may also prompt referral. However, there are contraindications to liver transplantation, such as metastatic malignancy and persistent substance abuse. Each patient should be referred as early as possible. The evaluation process includes a complete physical examination and social and psychologic evaluations. If transplantation is agreed upon, the patient is li

Increasing Self-Compassion: Review of the Literature and Recommendations.

Depression is a common symptom among college students and is often accompanied by negative thoughts about oneself. Self-compassion is a technique students can use to combat those negative thoughts. Self-compassion is an emotion-regulation strategy in which the individual engages in self-kindness, particularly after a mistake, failure, and/or rejection. This paper reviews the concept of self-compassion, assessment of self-compassion, and interventions that have been shown to increase self-compassion. Self-compassion is associated with lower levels of psychopathology and higher levels of well-be

Depressed Adolescents' Pupillary Response to Peer Acceptance and Rejection: The Role of Rumination.

Heightened emotional reactivity to peer feedback is predictive of adolescents' depression risk. Examining variation in emotional reactivity within currently depressed adolescents may identify subgroups that struggle the most with these daily interactions. We tested whether trait rumination, which amplifies emotional reactions, explained variance in depressed adolescents' physiological reactivity to peer feedback, hypothesizing that rumination would be associated with greater pupillary response to peer rejection and diminished response to peer acceptance. Twenty currently depressed adolescents

Group marginalization: extending research on interpersonal rejection to small groups.

An extensive research literature has examined the reactions of individuals facing interpersonal rejection. Small groups can also be rejected, but current research tells us little about the experiences of groups and their members directly. We integrate findings from various literatures to gain insight into shared rejection experiences and their outcomes. Of most practical importance, we argue that groups can be expected to react with more hostility than individuals when rejected. Four existing models that account for how group processes might alter such reactions are examined: a need-threat mod

Why rejection hurts: a common neural alarm system for physical and social pain.

Numerous languages characterize 'social pain', the feelings resulting from social estrangement, with words typically reserved for describing physical pain ('broken heart', 'broken bones') and perhaps for good reason. It has been suggested that, in mammalian species, the social-attachment system borrowed the computations of the pain system to prevent the potentially harmful consequences of social separation. Mounting evidence from the animal lesion and human neuroimaging literatures suggests that physical and social pain overlap in their underlying neural circuitry and computational processes.

[Psychological aspects of ulcerative rectocolitis (author's transl)].

Ulcerative rectocolitis has been known for quite some time as a psychosomatic disease, it is an organic disease occurring in people with a particular personality rendering them susceptible to certain conflicts or stress which would be able to play a causing role in the appearance or exacerbations of the disease. Insofar as the psychological study is concerned, there are two important points to keep in mind: the role of psychological trauma as a stimulating factor and the basic personality. In 90% of the cases observed, psychological factors are found as provoking the hemorrage, as found by Gro

A qualitative exploration of the lived experiences of youth living with HIV/AIDS at Jimma University Medical Center and Jimma Health Center, Southwest Ethiopia.

BACKGROUND: In Ethiopia, young people are often underserved in HIV programs, with limited access to counseling, treatment, and care. Most research on youth and HIV has focused on epidemiology and prevention among 15–24-year-olds, while little attention has been paid to the lived experiences of youth living with HIV. OBJECTIVES: This study explored the lived experiences of youth living with HIV who receive services from Jimma Health Center and Jimma University Medical Center. METHODS: A descriptive phenomenological design was employed to explore the experiences of purposively selected youth liv

Near-total human face transplantation for a severely disfigured patient in the USA.

BACKGROUND: Multiple reconstructive procedures are common for the reconstruction of complex facial deformities of skin, soft tissues, bony structures, and functional subunits, such as the nose, lips, and eyelids. However, the results have been unsatisfactory. An innovative approach entailing a single surgical procedure of face allograft transplantation is a viable alternative and gives improved results. METHODS: On Dec 9, 2008, a 45-year-old woman with a history of severe midface trauma underwent near-total face transplantation in which 80% of her face was replaced with a tailored composite t

Freud's first article in French in 1893, the year of Charcot's death.

This article examines Sigmund Freud's first publication in French, written in 1893, the year of Jean-Martin Charcot's death. Following his stay at the Salpêtrière (1885-1886), Freud published an article on hysterical paralysis in the Archives of Neurology, appearing alongside Charcot's obituary. This publication marked a pivotal epistemological shift in Freud's intellectual trajectory from neurology to psychoanalysis. Influenced by Charcot's clinical demonstrations, Freud began distancing himself from strictly neurological explanations of hysteria in favor of psychological interpretations. His

Spiritist views of mental disorders in Brazil.

The Spiritist perspective on mental disorders exerts a great influence in Brazil. Spiritist theory supports the survival of the spirit after death with an exchange of knowledge between the incarnated and disincarnated spirits. This article reviews the texts on mental disorders and Spiritism written by four leading Spiritist authors: Allan Kardec, Bezerra de Menezes, Inácio Ferreira and Joanna de Angelis. These authors advocated a model of spiritual etiology without rejecting the biological, psychological, and social causes of mental disorders. The Spiritist etiologic model for mental disorders

Recurrent miscarriage. II: Clinical associations, causes, and management.

Firm evidence on the causes of recurrent miscarriage is scant. The true rate is probably artificially heightened by a reproductive compensation effect. The commonest direct cause is probably repeated sporadic chromosome abnormalities, which occur consecutively merely by chance. Congenital and acquired anatomical defects of the uterine fundus and cervix, parental chromosomal rearrangements, gene mutations, antibodies to cardiolipin, and luteal phase defects each make a small contribution. Other causes, such as polycystic ovaries and immune rejection, may play some part but the evidence is not c

Adolescent personality factors in self-ratings and peer nominations and their prediction of peer acceptance and peer rejection.

In this study, the robustness of the Big Five personality factors in adolescents' self-ratings and peer nominations was investigated. Data were obtained on 2,001 adolescents attending secondary school (885 girls; 1,116 boys; M age = 14.5 years). Exploratory and confirmatory factor analyses on the self-ratings confirmed the Big Five personality factors. In contrast, exploratory analysis on the peer nominations revealed five different factors: Aggression-Inattentiveness, Achievement-Withdrawal, Self-Confidence, Sociability, and Emotionality-Nervousness. It is suggested that peers evaluate group

Patients' experiences in hospital following a liver transplantation.

Research is scarce regarding patients' experiences, feelings and thoughts the first 4 weeks after liver transplantation. Most research involving patients with a liver transplant are conducted several months, or even years, after the transplantation. The aim of this study is to present results from research interviews that took place post-transplant while patients still were in hospital. The design is explorative and hermeneutic. Fifteen patients were interviewed 3-5 weeks after transplantation. The results are presented in the following themes: (i) general contentment with the hospital stay, (

Contextual determinants of psychopathology. The singularity of attachment as a predictor of mental dysfunction.

The general aim of this research is to evaluate the singular weight of attachment relationships in psychopathology, to determine the point to which these relationships cannot be subsumed by other variables that are famously related to attachment, such as experience of stressful life events, social support, and coping styles. 172 people treated in mental health centers provided data related to these four aspects and about manifestations of psychopathology. Multiple linear regression analyses determined that 51% of the variance in the level of symptomatology is explained with six variables: two

The interpersonally sensitive disposition and health: an integrative review.

This article reviews studies that have examined the association between constructs related to interpersonal sensitivity (IS) and morbidity and mortality from major medical illnesses. We define IS as a stable trait characterized by ongoing concerns about negative social evaluation. This disposition makes people vigilant for as well as sensitive to others' evaluations of them. To avoid negative social evaluation, they adopt defensive behaviors such as submission and inhibition. Aspects of IS are captured by various constructs, including introversion, rejection sensitivity, social inhibition, soc

[Body and subjectivity: about anorexia nervosa].

To start with the clinical description of anorexia nervosa is indicative of the difficulty involved not only with regard to its etiology (whether it is an endocrine, neurological or psychological disorder) but also with regard to its diagnosis (whether or not it is a hysterical disorder, whether or not it is a disease as such) and with regard to its treatment (the stubbornness of the symptom). The early-onset differential criterion is rejection. The subject's eating-related problem must be placed within the framework of the radical dependency the human organism has with regard to the mother's

Instruments and Taxonomy of Workplace Bullying in Health Care Organizations.

PURPOSE: This study was aimed to evaluate the methodological issues and comprehensiveness of workplace bullying instruments and to suggest a taxonomy of psychological abuse. METHODS: Nineteen instruments applied in health care organizations and 469 questionnaire items mainly regarding psychological abuse were collected through a literature review. Three researchers classified the questionnaire items according to a "taxonomy of psychological abuse in the workplace." RESULTS: Many instruments of workplace bullying were developed in the 2000s using a reflective measurement model, but their psyc

Childhood emotional invalidation and adult psychological distress: the mediating role of emotional inhibition.

OBJECTIVE: To evaluate a model in which chronic emotional inhibition mediates the relationship between a history of childhood emotional invalidation or abuse and adult psychological distress. METHOD: One hundred and twenty-seven participants completed a series of self-report questionnaires, and a subset of this group (n=88) completed an additional measure of current avoidant coping in response to a laboratory stressor. Structural equation modeling was used to evaluate and compare a full and partial mediational model. RESULTS: Findings strongly supported a model in which a history of childhoo
