Residential Structural Racism and Prevalence of Chronic Health Conditions.

IMPORTANCE: Studies elucidating determinants of residential neighborhood-level health inequities are needed. OBJECTIVE: To quantify associations of structural racism indicators with neighborhood prevalence of chronic kidney disease (CKD), diabetes, and hypertension. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study used public data (2012-2018) and deidentified electronic health records (2017-2018) to describe the burden of structural racism and the prevalence of CKD, diabetes, and hypertension in 150 residential neighborhoods in Durham County, North Carolina, from US census block

Social determinants of health as predictors of latent symptom clusters in patients undergoing maintenance hemodialysis: A secondary analysis of the HOPE consortium randomized trial.

The aims of this study were to characterize symptom clusters and examine the influence of social determinants of health (SDOH) on symptom cluster membership among people receiving maintenance hemodialysis and experiencing chronic pain. This study utilized baseline data collected from the 643 patients enrolled in the HOPE Consortium Trial designed to test the efficacy of an intervention to reduce chronic pain. Latent profile analyses were performed to characterize symptom clusters. SDOH variables were collected using self-report and the Census Geocoder. Five distinct clusters of symptoms were i

"I'm a Work in Progress": Rethinking Posttraumatic Growth Among Low-Income Black Men.

Low-income Black men in the United States experience cumulative trauma shaped by structural violence, including poverty, residential segregation, over-policing, mass incarceration, and community violence. These systemic harms disrupt identity, relationships, and opportunity, constraining both exposure to trauma and pathways for recovery and growth. Drawing on Tedeschi and Calhoun's posttraumatic growth (PTG) framework, integrated with socioecological and culturally grounded perspectives on resilience, this study examines how low-income Black men make meaning of trauma and pursue transformation

Pushing the boundary of child well-being: A spatial examination of child mortality in transition zones of extreme economic inequality and material hardship.

How do patterns of socioeconomic inequality shape the risk of child fatality in urban areas? Research has shown that both intentional and unintentional child deaths are concentrated in areas of social disadvantage. Yet in densely populated cities, child fatality risk may not follow smooth spatial patterns and instead exhibit abrupt shifts across neighborhood boundaries. This study applies a dissimilarity-based Bayesian spatial conditional autoregressive (CAR) model to detect localized discontinuities in child mortality risk associated with structural inequality. Using a continuous dissimilarit

Supporting young adults with hemiplegia: services and costs.

This paper lays out the service and cost consequences of supporting a nationally representative sample of young adults with hemiplegic cerebral palsy. The total costs burden and the additional costs that accrue to the public and independent sector due to their disabilities are calculated. Most of the young adults live at home. Many lead "ordinary" lives and attend further or higher education establishments or go to work. A small number of study members, however, make intensive use of expensive, often segregated facilities such as residential homes or schools and make considerable use of other

Stakeholder conceptualisation of multi-level HIV and AIDS determinants in a Black epicentre.

HIV has reached epidemic proportions among African Americans in the USA but certain urban contexts appear to experience a disproportionate disease burden. Geographic information systems mapping in Philadelphia indicates increased HIV incidence and prevalence in predominantly Black census tracts, with major differences across adjacent communities. What factors shape these geographic HIV disparities among Black Philadelphians? This descriptive study was designed to refine and validate a conceptual model developed to better understand multi-level determinants of HIV-related risk among Black Phila

Assessing sediment recontamination from metals in stormwater.

Recontamination of sediments by stormwater is a major concern when evaluating the potential effectiveness of sediment remediation. Stormwater and sediment sampling were conducted in a mixed-use watershed at Paleta Creek in San Diego, CA to evaluate methods for assessing sediment recontamination by metals. Size-segregated stormwater contaminant loads with simultaneous receiving water and sediment measurements were used to identify dominant sources and contaminants with respect to their impact on sediment recontamination. Most of the stormwater contaminant loads of Cd, Cu, Pb, and Zn were associ

Associations between historical redlining and BMI: Potential indirect pathways through neighborhood socioeconomic and environment factors and their associated lifestyle behaviors in CARDIA.

It is unknown whether living in neighborhoods historically rated as "high-risk" mortgage by lenders (i.e., "redlining") defined by maps of the Home Owners' Loan Corporation (HOLC) neighborhood ratings may associate with obesity through neighborhood factors and health behaviors, or how such associations might differ by race and gender. We used data from the 1985-86 Coronary Artery Risk Development in Young Adults (CARDIA) study to test whether retrospective cohort associations between redlining and body mass index (BMI) were mediated by paths from census-derived social and economic neighborhood

The Impact of Social Determinants of Health on Ocular Diseases in Western New York: A Comparative Ecological Study of Two U.S. Counties.

Background/Objectives: As the world becomes more connected, it is becoming critical for clinicians to understand other cultures, races, and ethnicities to provide the most effective therapy. A comprehensive understanding of all communities necessitates an examination of the social determinants of health (SDH). Eye diseases and many other conditions are influenced by SDH. To elucidate the impact of SDH on eye health, a comparative ecological analysis of Monroe and Erie Counties in New York State was conducted to identify any differences in SDH and glaucoma, age-related macular degeneration (AMD

Does State-Level Structural Racism Impact Risk for Suicide Ideation and Attempts Among US Adolescents Across Race and Ethnicity?

OBJECTIVE: Our study examined the association between state-level structural racism and past year rates of suicide ideation (SI) and suicide attempts (SA) among non-Hispanic Black, Hispanic, and non-Hispanic White adolescents. METHOD: This secondary analysis used state- and individual-level linked data combining multiple years (ie, 2011-2019) of the Youth Risk Behavior Surveys, the 2019 American Community Survey, and the 2019 state-level Structural Racism Index (SRI), a composite measuring state-level Black-White (B-W) and Hispanic-White (H-W) racial inequities across 5 domains: residential s

The Relationship Between Zoning Policies, Racialized Economic Segregation and Life Expectancy Across Census Tracts in Chicago, 2010-2015.

INTRODUCTION: Racial segregation is a recognized cause of racial health disparities. One factor that has contributed to racial segregation is municipal zoning policies that exclude people of color and those of low income from advantaged residential neighborhoods. Few studies have explicitly explored the relationship between zoning policies and health. Using data on zoning policies in Chicago in 1923 and 2012, we examined inequities in life expectancy across census tracts in relation to the proportion of land in each tract zoned for specific purposes. METHODS: For each Chicago census tract, we

Neighborhood gentrification, income inequality, and cancer incidence among Hispanic/Latino adults.

BACKGROUND: Neighborhood gentrification leads to displacement of people, networks, and resources, and to differences in how income is distributed in the community. Few studies, however, have examined the roles of neighborhood gentrification and income inequality in relation to cancer incidence, particularly among Hispanic/Latino populations. We examined measures of neighborhood gentrification and income distribution in association with cancer risk among Hispanic/Latino adults. METHODS: We included 14 303 adults (ages 18-75 years) from the Hispanic Community Health Study/Study of Latinos with

Social structure, race, and gonorrhea rates in the southeastern United States.

OBJECTIVES: We sought to identify characteristics of counties in the southeastern United States associated with endemically high rates of gonorrhea. In particular, we were interested in aspects of race other than the proportion of Blacks in a population, including the potential influence of under-reporting of infections. DESIGN: The associations between the characteristics of counties in 1990, and the rates of reported gonorrhea from 1986 to 1995, were estimated with multivariable logistic regression. SETTING: 14 states in the southeastern region of the United States. PARTICIPANTS: 835 coun

Health Care Utilization among Young Adults: Associations with Historic Redlining.

INTRODUCTION: This study explored the association between historic redlining and health-related outcomes. METHODS: Respondents from Waves III-V of the National Longitudinal Survey of Adolescent to Adult Health were mapped to districts coded red by the Home Owners' Loan Corporation (HOLC). Logistic regression evaluated the association between distance from a redlined district and having insurance, visiting a private physician, receiving medical care, and perceiving good health. RESULTS: About 71% of White, 53.5% of Black, and 41.9% of Hispanic individuals lived five miles or further outside a

Understanding the Intersection Between Racial Segregation, Social Isolation and Safety Perceptions on Health for an Economically Disadvantaged Urban Community.

Safety concerns continue to be challenging amid racially segregated communities of urban concentrated disadvantage. Although contributing factors to crime in such neighborhoods have been described, there is a lack of knowledge regarding how social isolation and perceptions of safety impact community health. This qualitative study explored how minoritized residents (N = 23) from an urban community of economic disadvantage perceived the influence of environmental and residential conditions on their health. Use of the grounded theory method produced conceptual insights into the process of how saf

Racial and ethnic disparities in percutaneous coronary intervention: contemporary evidence and pathways to equity.

Racial and ethnic disparities in percutaneous coronary intervention (PCI) persist despite major advances in contemporary acute coronary syndrome (ACS) care and represent an important challenge in achieving equitable cardiovascular outcomes. In this State-of-the-Art Review, we propose a PCI care-continuum framework demonstrating how inequities emerge sequentially across multiple stages of care; including baseline cardiovascular risk, prehospital triage, invasive coronary angiography, PCI decision-making, procedural optimization, and post-procedural outcomes. Contemporary evidence from randomize

Association of Neighborhood Social Determinants of Health with Acute Kidney Injury during Hospitalization.

KEY POINTS: AKI is common among hospitalized patients. However, the contribution of neighborhood social determinants of health to AKI risk is not known. We found that among 26,769 hospitalized patients, 26% developed AKI. Patients who lived in the most disadvantaged areas (highest tertile of Area Deprivation Index) had a 10% greater odds of developing AKI than counterparts in the lowest Area Deprivation Index tertile. Patients who lived in rural areas had 25% greater odds of not recovering from AKI by hospital discharge. This study demonstrates an association between neighborhood disadvantage

Multilevel contributors to racial and ethnic inequities in the resolution of abnormal mammography results.

PURPOSE: Multiple ecological levels influence racial inequities in the completion of diagnostic testing after receiving abnormal mammography results (diagnostic resolution). Yet, few studies examine more than two ecological levels. We investigated the contributions of county, imaging facility, and patient characteristics on our primary and secondary outcomes, the achievement of diagnostic resolution by (1)Black women and Latinas, and (2) the entire sample. We hypothesized that women of color would be less likely to achieve resolution than their White counterparts, and this relationship would b

Obesity Disparities in California Vary by School Ethnic Segregation and School Neighbourhood Income.

OBJECTIVE: This study examines how ethnic disparities in childhood obesity differ across combinations of school ethnic composition and school neighbourhood income in California, to identify the contexts in which disparities are greatest. Prior studies have been unable to study this due to the high correlation between segregation and income. METHODS: This observational study focuses on overweight/obesity data objectively measured among California public school children in 2018/2019 and school neighbourhood data from the American Community Survey, using a 'within versus between' differences ana

Historical Redlining and Spatiotemporal Patterns in Breast Cancer Screening.

IMPORTANCE: Geographic variation in breast cancer screening may reflect persistent structural inequities in access to preventive care. Understanding whether historical redlining remains associated with screening, independent of contemporary social vulnerability, neighborhood conditions, and geographic access, is critical for targeting interventions within cancer center catchment areas. OBJECTIVES: To examine the association between historical redlining and breast cancer screening prevalence, accounting for social vulnerability, neighborhood characteristics, and geographic access, and to chara
