Pre-stroke undiagnosed dysphagia lusoria as a rare cause of aspiration pneumonia with respiratory failure in a stroke patient.

INTRODUCTION: Dysphagia is a risk factor for aspiration pneumonia and acute respiratory failure in acute stroke patients. Dysphagia lusoria is caused by compression on the esophagus from artery lusoria, when the aberrant right subclavian artery arises from the descending aortic arch. We present a rare case report of pre-stroke undiagnosed dysphagia lusoria as a cause of aspiration pneumonia with acute respiratory failure in a 67-year-old female patient admitted with a minor left intracerebral hemorrhage in the left basal ganglia. On admission to the stroke unit, she had Glasgow Coma Scale of 1

The effects of shallow versus deep endotracheal suctioning on the cytological components of respiratory aspirates in high-risk infants.

BACKGROUND: The harmful effects of mechanical ventilation and suctioning are compounded if endotracheal suctioning (ETS) is inappropriately performed. Deep ETS involves catheter insertion into the endotracheal tube until resistance is met. Shallow ETS may be beneficial in lessening mechanical irritation to the first bronchial layers. However, clinical observation reveals wide variation in the length of the suction catheter for ETS in high-risk infants. OBJECTIVE: The study was conducted to examine the effects of deep and shallow ETS on the cytological components of respiratory aspirates from

Life-Saving Cryoextraction for Acute Severe Respiratory Failure Caused by Cheese-Filled Fish Cake Aspiration: A Case Report.

A 79-year-old man with chronic obstructive pulmonary disease (COPD) aspirated cheese-filled Kamaboko, a Japanese fish cake, accidentally while having his lunch. He developed acute respiratory failure and presented to a referral hospital. Chest computed tomography (CT) revealed an aspirated fish cake in the right middle bronchus. After endotracheal intubation in the intensive care unit, the fish cake was successfully removed using a cryoprobe. Although foreign body (FB) aspirations are rare in adults, they can cause life-threatening respiratory dysfunction. Prompt removal of the FB using an app

Successful Management Foreign Body Aspiration Associated with Severe Respiratory Distress and Subcutaneous Emphysema: Case Report and Literature Review.

The presence of a foreign body in the airway is a potentially life-threatening clinical condition that requires urgent medical attention. We present a case of a 12-year-old boy who presented in the emergency room with a history of an episode of choking after aspiration of a foreign body, followed by severe respiratory distress and subcutaneous emphysema. Chest radiography revealed hyperinflation data, pneumothorax, and subcutaneous emphysema data. The flexible bronchoscope examination showed the presence of an inorganic foreign body impacted on the carina with tracheal lesions and laryngeal ed

Development of an Accurate Bedside Swallowing Evaluation Decision Tree Algorithm for Detecting Aspiration in Acute Respiratory Failure Survivors.

BACKGROUND: The bedside swallowing evaluation (BSE) is an assessment of swallowing function and airway safety during swallowing. After extubation, the BSE often is used to identify the risk of aspiration in acute respiratory failure (ARF) survivors. RESEARCH QUESTION: We conducted a multicenter prospective study of ARF survivors to determine the accuracy of the BSE and to develop a decision tree algorithm to identify aspiration risk. STUDY DESIGN AND METHODS: Patients extubated after ≥ 48 hours of mechanical ventilation were eligible. Study procedures included the BSE followed by a gold stan

Diagnostic accuracy of digital RNA quantification versus real-time PCR for the detection of respiratory syncytial virus in nasopharyngeal aspirates from children with acute respiratory infection.

BACKGROUND: Virus-specific molecular assays such as real-time polymerase chain reaction (RT-PCR) are regularly used as the gold standard to diagnose viral respiratory tract infections, but simultaneous detection of multiple different pathogens is often challenging. A multiplex digital method of RNA quantification, nCounter (NanoString Technologies), can overcome this disadvantage and identify, in a single reaction, the presence of different respiratory viruses. OBJECTIVES: To evaluate the accuracy of nCounter to identify and quantify RSV-A and RSV-B in nasopharyngeal aspirates (NPA) of childr

Suspected Bulbar-Predominant Myasthenia Gravis Exacerbation in an Older Adult With Respiratory Deterioration and Silent Aspiration: A Case Report.

Myasthenia gravis (MG) is an autoimmune disorder of the neuromuscular junction characterized by fluctuating skeletal muscle weakness. Bulbar-predominant exacerbations may be difficult to recognize in older adults with multiple comorbidities, particularly when respiratory deterioration is initially attributed to cardiopulmonary disease. We report the case of an 83-year-old woman with generalized acetylcholine receptor antibody-positive MG, dementia with behavioral disturbance, moderate chronic obstructive pulmonary disease (COPD), chronic kidney disease, and previous deep vein thrombosis who wa

Open dataset and deep learning model for intelligent diagnosis of neonatal respiratory distress syndrome and aspiration syndrome in newborns.

Neonatal pulmonary diseases such as aspiration syndrome (AS) and respiratory distress syndrome (NRDS) require timely diagnosis, yet manual interpretation of neonatal X-rays is labor-intensive and subjective. To support AI-assisted diagnosis, we constructed the first Chinese neonatal pulmonary ailment dataset (NPA), covering both normal and diseased cases of varying severity. However, the NPA dataset exhibits severe class imbalance, and traditional augmentations randomly mix lesions, often corrupting pathological semantics. To address this, we propose a Polluted CutMix framework that selectivel

Global Burden and Forecast of Fall-Related Respiratory Foreign Body Aspiration in Older Adults (1990-2040): A Systematic Analysis.

Foreign body in the respiratory system caused by falls (F-RFBA) among individuals aged 70 years and older has been a growing concern globally, yet comprehensive global epidemiological data on this issue remain sparse. Data from the Global Burden of Disease (GBD) study from 1990 to 2021 are systematically reviewed to assess incidence, prevalence, and years lived with disability (YLDs) associated with F-RFBA. Projections are made for the period up to 2040 using Bayesian age-period-cohort (BAPC) models. From 1990 to 2021, the incidence, prevalence, and YLDs associated with F-RFBA among individual

A Novel Type of Aspiration Presenting With Acute Respiratory Failure Caused by Upper Airway Obstruction Due to Diaper Pica.

We herein report an extremely rare case of acute respiratory failure due to pica of the high-molecular-weight polymers in a diaper (HMWPD). An 81-year-old woman was admitted to our hospital because of dyspnea. Several hours earlier, the facility staff saw her tearing the diaper and eating it. And several hours later, the staff found her complaining of dyspnea. Her chest computed tomography revealed airway foreign bodies. Immediately, flexible bronchoscopy was performed. The trachea and the bilateral bronchi were filled with the HMWPD. We tried to grasp the HMWPD with various common instruments

Time-dependent changes in pulmonary surfactant function and composition in acute respiratory distress syndrome due to pneumonia or aspiration.

BACKGROUND: Alterations to pulmonary surfactant composition have been encountered in the Acute Respiratory Distress Syndrome (ARDS). However, only few data are available regarding the time-course and duration of surfactant changes in ARDS patients, although this information may largely influence the optimum design of clinical trials addressing surfactant replacement therapy. We therefore examined the time-course of surfactant changes in 15 patients with direct ARDS (pneumonia, aspiration) over the first 8 days after onset of mechanical ventilation. METHODS: Three consecutive bronchoalveolar l

Respiratory failure with hilar mass: Role of endobronchial ultrasound-guided transbronchial needle aspiration in the medical intensive care unit.

We report the case of a 58-year-old man on chronic steroid therapy, who developed a rapidly progressive right upper lobe infiltrate/mass that extended into the right hilum. Respiratory failure necessitated endotracheal intubation. Broad spectrum antibiotics were initiated without clinical improvement and because of his immunosuppressive therapy opportunistic pathogens were considered. Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) was performed in the Medical Intensive Care Unit (MICU) with rapid onsite evaluation. Specimens obtained from the right hilar mass reve

Therapeutic bronchoscopy in a child with sand aspiration and respiratory failure from near drowning--case report and literature review.

Foreign matter aspiration occurs relatively commonly in drowning and near-drowning events. In most cases, stomach contents are aspirated. Sand aspiration rarely occurs and there are no reported cases in children with near drowning. Limited data are available on clinical presentation and management of sand aspiration with accidental burial. We report a 3-year-old boy who nearly drowned while swimming in brackish waters and was found face down in sand. Sand aspiration was suspected when the child continued to have persistent wheezing and high ventilatory requirement despite intensive bronchodila

A Hard Lump to Chew: Delayed Diagnosis of Foreign Body Aspiration in an Elderly Man Leading to Severe Respiratory Failure.

Foreign body aspiration (FBA) in adults is uncommon and often associated with risk factors such as advanced age and chronic alcohol abuse. It can constitute a serious emergency with acute symptoms secondary to airway obstruction, but there are cases in which it goes unnoticed, causing late complications with insidious symptoms and recurrent respiratory infections. We present the case of a 77-year-old male patient with a history of hypertension, type 2 diabetes mellitus, dyslipidemia, hyperuricemia, probable chronic obstructive pulmonary disease, chronic alcohol abuse, and gonarthrosis, who was

Massive Amniotic Fluid Aspiration in a Case of Sudden Neonatal Death With Severe Hypoplasia of the Retrotrapezoid/Parafacial Respiratory Group.

We report a case of a baby, who, after pregnancy complicated by maternal Addison's disease and Hashimoto's thyroiditis and natural delivery, unexpectedly presented a cardiorespiratory collapse and died 1 hour after birth without responding to prolonged neonatal resuscitation maneuvers. The cause of death was reliably established by carrying out a forensic postmortem examination. More specifically, the histological examination of the lungs showed the presence of abundant endoalveolar and endobronchial cornea scales caused by absorption of amniotic fluid. The neuropathological examination of the
