Aspirin hypersensitivity in patients with chronic rhinosinusitis and nasal polyposis: frequency and contributing factors.

BACKGROUND: Aspirin-exacerbated respiratory disease (AERD) is a syndrome frequently seen in patients with chronic rhinosinusitis and nasal polyposis (CRSwNP). However, there are few studies on evaluating the prevalence of aspirin (acetylsalicylic acid [ASA]) hypersensitivity in patients with CRSwNP using the oral aspirin challenge (OAC) test. This cross-sectional study was designed to determine the frequency of ASA hypersensitivity and factors associated with it in patients with CRSwNP in Tehran, Iran. METHODS: Adult patients with CRSwNP who were presented to the asthma and allergy clinic wer

Histone Modifications in a Mouse Model of Early Adversities and Panic Disorder: Role for Asic1 and Neurodevelopmental Genes.

Hyperventilation following transient, CO2-induced acidosis is ubiquitous in mammals and heritable. In humans, respiratory and emotional hypersensitivity to CO2 marks separation anxiety and panic disorders, and is enhanced by early-life adversities. Mice exposed to the repeated cross-fostering paradigm (RCF) of interference with maternal environment show heightened separation anxiety and hyperventilation to 6% CO2-enriched air. Gene-environment interactions affect CO2 hypersensitivity in both humans and mice. We therefore hypothesised that epigenetic modifications and increased expression of ge

Immediate Fluoxetine Hypersensitivity With Predominant Airway and Oropharyngeal Symptoms in an Adolescent.

Fluoxetine is a widely prescribed selective serotonin reuptake inhibitor in children and adolescents. Hypersensitivity reactions are rare and poorly characterized in this population. We report the case of a 17-year-old female who developed acute throat tightness with perceived oropharyngeal swelling and respiratory symptoms within 30 minutes of the first dose of fluoxetine, in the absence of cutaneous manifestations. Symptoms improved with bronchodilator therapy. Basophil activation testing demonstrated drug-induced activation, and an oral drug provocation test reproduced symptoms, confirming

Tourette's syndrome in patients referred for allergy evaluation.

BACKGROUND: Tourette's syndrome is a relatively common disorder that may present as a spectrum that can include both motor and behavioral features. METHODS: We report four patients referred to allergists to rule out atopy as a reason fro their presumed respiratory symptoms who were subsequently found to have Tourette's syndrome. A review of the literature was also performed using Medline. RESULTS: The symptoms of Tourette's syndrome may overlap those associated with allergy involving the respiratory tract. There is little published information on the association of Tourette's syndrome with a

Asthma and related atopic disorders in outpatients attending an urban HIV clinic.

BACKGROUND: Drug hypersensitivity and pruritic cutaneous disorders are common in human immunodeficiency virus (HIV)-infected patients. Systematic cross-sectional studies, detailing the pattern of atopic disease, however, have not been performed in patients with HIV infection. OBJECTIVE: To analyze the relationships between atopic disease and factors relating to lifestyle, environment, gender, genetics, and HIV disease-associated immune cell depletion. METHODS: CD4 cell counts and interview data were recorded in a survey of 136 adult patients attending a community hospital-based HIV outpatien

[Behavior of bronchial reactivity in 50 children before and after exclusion of food allergens].

In this study the author has purposed to demonstrated that the food allergens increase the bronchoreactivity. This studied group was formed by 50 boys with chronic rhinitis and/or nasal perennial and/or laryngeal and bronchial obstruction, unresponsive to aeroallergens but responsive to food allergens determined with FBST test. This test is performed by making the patient go on an anallergic diet (formed by tapioca's flour, rabbit, olive oil, beets, lettuce, pear, sugar, salt for seven days). The first week are controlled the spirometric values and every week is introduced a new food with ambu

NSAIDs-hypersensitivity often induces a blended reaction pattern involving multiple organs.

Non-steroidal anti-inflammatory drugs (NSAIDs)-induced hypersensitivity reactions are classified by the European Network on Drug Allergy (ENDA) as either cross-reactive or selective. The former is the most frequent type and includes patients with exclusively respiratory symptoms (NSAIDs-exacerbated respiratory disease, NERD) or exclusively cutaneous symptoms: NSAIDs-induced urticaria/angioedema (NIUA); and NSAIDs-exacerbated cutaneous disease (NECD). However, although not reflected in the current classification scheme (ENDA), in clinical practice a combination of both skin and respiratory symp

Acetyl Salicylic Acid Challenge in Children with Hypersensitivity Reactions to Nonsteroidal Anti-Inflammatory Drugs Differentiates Between Cross-Intolerant and Selective Responders.

BACKGROUND: Hypersensitivity reactions to nonsteroidal anti-inflammatory drugs (NSAIDs) in children are becoming a great concern. Most studies have focused on adults, with noted discrepancies observed in the classification of hypersensitivity reactions to NSAIDs in children when compared with adults. OBJECTIVE: To phenotype a group of children with hypersensitivity reactions to NSAIDs, including paracetamol, and analyze the degree of agreement with the entities reported in adults and how they fit the proposed classifications. METHODS: The study comprised 116 children aged 0.5 to 14 years, wi

[Hypersensitivity pneumonitis and abscess reaction to nontuberculous mycobacteria acquired form jacuzzi aerosol].

INTRODUCTION: Mycobacterium avium complex can be responsible for a number of different radio-clinical presentations, ranging from invasive infections to hypersensitivity pneumonitis due to repeated inhalation of antigens. The diagnosis of hypersensitivity pneumonitis is clinical, radiological, biological and microbiological. CASE REPORT: A 61-year-old male developed a hypersensitivity pneumonitis reaction to non-tuberculous mycobacteria, following the repeated use of his own spa, which later evolved into chronic respiratory failure. The diagnosis was made via an environmental analysis. Immuno

Food and drug reactions, wheezing, and eczema in preterm infants.

Allergic reactions were investigated in 777 preterm infants who were randomly assigned to early diet and followed up to 18 months post term. Wheezing or asthma was common (incidence 23%); it was associated with neonatal ventilation, maternal smoking, and a family history of atopy and was unexpectedly reduced in babies born by caesarean section. Even in non-ventilated infants, the incidence of subsequent wheezing was 18%, rising to an estimated 44% (using logistic regression) when the foregoing risk factors (excluding ventilation) were present. Eczema occurred in 151 infants (19%) and was stron

Management of infantile asthma.

Infantile asthama is an important pediatric problem and may cause substantial morbidity and mortality in this age group. The pathophysiology of allergic asthma involves a type I hypersensitivity reaction that is mediated by reaginic antibodies of the IgE class. Various factors predisposing to infantile asthma have been suggested but not confirmed. The differential diagnosis of infantile wheezing is of particular importance in this very young age group. An appreciation of the natural history and clinical characteristics of the disease, and of the important causative factors (foods, environmenta

The role of high resolution oesophageal manometry in occult respiratory symptoms.

PURPOSE: Cough Hypersensitivity Syndrome is the urge to cough following minimal stimulation, but its mechanism and method of provocation remain unexplained. 121 patients (44 males, 77 females; age range 18-81 years) were evaluated. PROCEDURES: High resolution manometry was performed on consecutive patients presenting with unexplained respiratory symptoms (Respiratory Group 61). Data were compared with matched controls, i.e. dyspepsia without respiratory symptoms (Dyspepsia Group 60). MAIN FINDINGS: Results showed increased inspiratory gastro-oesophageal pressure gradient due to significantly

How can we better classify NSAID hypersensitivity reactions?--validation from a large database.

BACKGROUND: Hypersensitivity to nonsteroidal anti-inflammatory drugs (NSAIDs) is one of the most common drug hypersensitivities. Several clinical subtypes have been distinguished depending on symptomatology (respiratory, cutaneous, anaphylaxis), timing (immediate, delayed), underlying chronic disease (asthma, chronic urticaria) or putative mechanism of the reaction (allergic, nonallergic mediated). The aim of the present study was to better classify the many hypersensitivity reactions to NSAIDs. METHODS: In the present retrospective study, during an 11-year study period, we collected data fro

Cough-Predominant Laryngeal Hypersensitivity Syndrome in a Middle-Aged Woman: A Case Report.

A middle-aged woman presented with a three-month history of sudden-onset persistent dry cough, with no preceding respiratory illness or significant medical history. She was an active smoker with a prior heavy smoking history. Initial investigations, including chest imaging, computed tomography of the neck and thorax, pulmonary function testing, and otolaryngological assessment, were normal. Speech and language therapy evaluation did not reveal any abnormalities of breathing or voice function. Flexible bronchoscopy demonstrated normal lower airway anatomy but revealed marked laryngeal hypersens

Immune responses in bats following intranasal infection with Histoplasma capsulatum.

Groups of bats (Artibeus lituratus) were infected by the intranasal instillation of a suspension of mycelia and spore particles of Histoplasma capsulatum containing either 10(4) or 10(6) viable units. Bats infected with the high dose had viable H. capsulatum in the lungs, liver, spleen and gut as early as 2 weeks post-infection. Complement-fixing antibodies to the organism were detectable 3 weeks after infection, whereas precipitating antibodies were not persent until 5 weeks. Significantly delayed hypersensitivity to histoplasmin was noted at 2 and 4 weeks after infection. By 9 weeks, delayed
