BACKGROUND: Physical restraints are used in the emergency department (ED) for agitated patients to prevent self-harm and protect staff. Prior studies identified associations between sociodemographic factors and ED physical restraints use.
OBJECTIVES: The primary objective was to compare characteristics of ED patients receiving physical restraints for violent and nonviolent indications vs. patients who were not restrained. The secondary objective was to compare rates of restraint use among ED providers.
METHODS: This was a single-center cross-sectional study of adult ED patients from March 20

OBJECTIVE: Emergency holds are commonly used in mental health crisis care, yet pediatric-specific data on their application and downstream consequences for emergency department (ED) care remain limited. We sought to describe sociodemographic and clinical factors associated with emergency hold status, including their relation to ED physical restraint use, among youth presenting to the ED for mental or behavioral health (MBH) concerns.
METHODS: We conducted a retrospective cross-sectional study of patients aged 6 to 18 years presenting with an MBH concern to an urban, tertiary pediatric ED from

OBJECTIVES: Older adults may present to the emergency department (ED) with agitation, a symptom often resulting in chemical sedation and physical restraint use which carry significant risks and side effects for the geriatric population. To date, limited literature describes the patterns of differential restraint use in this population.
DESIGN, SETTING, PARTICIPANTS, AND MEASUREMENTS: This retrospective cross-sectional study used electronic health records data from ED visits by older adults (age ≥65 years) ranging 2015-2022 across nine hospital sites in a regional hospital network. Logistic re

The optomotor reflex (OMR) provides a behavioral assessment of an animal's contrast sensitivity and visual acuity. Mice or rats are typically placed directly onto a small circular platform by hand; however, handling animals like this can stimulate stress and anxiety, which introduce confounding factors when interpreting data. It has been shown that non-aversive handling methods, such as picking up mice or rats in a familiar tunnel/tube, can reduce anxiety. This is of particular interest in studies where animals display heightened stress, overactivity, or motor dysfunction, resulting in an inab

OBJECTIVE: To determine the relationships, if any, between use of seclusion and restraint and factors such as demographic parameters, diagnosis, legal admission status (voluntary or involuntary), symptoms, cognitive function, global functioning, therapeutic alliance, attitudes toward medication, and insight, among psychiatry inpatients in Ireland.
METHODS: We used validated tools to perform detailed assessments of 107 adult psychiatry inpatients admitted to acute psychiatry units at 2 general hospitals in Dublin, Ireland over a 30-month period, between September 2017 and February 2020.

INTRODUCTION: Behavioral symptoms in hospitalized older adults with cognitive impairment often lead to physical and chemical restraint use, despite associated harms. Patient-centered care models show promise in reducing restraint use but are rarely implemented in routine practice. This project implemented CoCare-CI, a clinical innovation to address behavioral symptoms in hospitalized older adults with cognitive impairment.
METHODS: CoCare-CI was implemented on a 24-bed ACE unit in a 128-bed community hospital from January 2023 to August 2024 by a multidisciplinary team led by a geriatric nurs

Little is known about the impact of environmental restraint, characterized by limited space and a lack of stimulating elements, on adult mental and physical health. We examined the influence of environmental restraint on depression-like behaviors and physical status in rats, and the potential mitigating effects of Lactococcus cremoris H61 as a beneficial bacterium. Rats subjected to environmental restraint exhibited prolonged maladaptive immobility in the forced swim test without showing changes in body weight, locomotion, or social motivation. Daily activities remained unaffected. These findi

Stress responses are triggered by diverse stressors and typically involve acute common physiological changes including increased heart rate and adrenocortical hormone secretion. In contrast, core body temperature (Tc) may either increase or decrease depending on the stressor. Although restraint is widely used to induce stress responses in laboratory animals, heterogeneous effects on Tc have been reported. To elucidate the central and peripheral mechanisms underlying restraint-induced hypothermia, a reliable model that consistently induces hypothermia is first required. In this study, we aimed

BACKGROUND: It is important to protect patients' rights by preventing the misuse and abuse of physical restraints through societal and ethical discussions and education.
AIM: This study aimed to develop and implement a simulation-based education program on older adult safety and ethics for nursing students; determine the acceptability of the simulation design; examine pretest-posttest changes in attitudes toward the use of restraints, ethical sensitivity, and person-centered nursing; and explore nursing students' learning experiences during the simulation.
DESIGN: This convergent mixed-metho

Researchers always encounter the problem that animals are difficult to control in acupuncture-related animal experiments. There are already some methods to prevent animals from escaping treatment sites. However, the effect of these fixation methods is different from the human treatment state. Therefore, we have invented a new fixation method to keep rats prone, quiet, and awake during treatment. The equipment consists of a porous plastic restraint platform, a square cotton cloth, and several restraint straps for fixing rats. First, thread the restraint straps into the restraint platform. Medical students (NSB, NM, JDW) spearheaded revision of the policy and clinical practice for shackling incarcerated patients at Boston Medical Center (BMC), the largest safety net hospital in New England. In American hospitals, routine shackling of incarcerated patients with metal restraints is widespread-except for perinatal patients-regardless of consciousness, mobility, illness severity, or age. The modified policy includes individualized assessments and allows incarcerated patients to be unshackled if they meet defined criteria. The students also formed the Stop Shackling Patients Coalitio

WHAT IS KNOWN ON THE SUBJECT?: There is body of knowledge available about the harms associated with restrictive interventions used in behaviour management, service users' perceptions of the use of restraints, and staff competence in behaviour management. The staff perspective has been studied in terms of staff exposure, responses to and prevention of aggression, staff-related factors associated with service user aggression, and staff attitudes and perceptions towards violence. The definitions of competence in behaviour management provided in training interventions are fragmented and based on u

In many surgical procedures, there is a need to secure the animal's skull in a particular orientation. Described here is a surgical headholder for the rat that is inexpensive and easily constructed. The main component of the headholder is a screw type tubing clamp that is modified to allow the hard palate of the rat to be rigidly secured to the base plate of the clamp. This headholder has proven to be a highly satisfactory alternative to commercially available devices.

Two stainless steel bolts were implanted and fixed parallel in the frontal sinus of cats with dental acrylic and screws. When unanaesthetized, the head was fixed to a frame by securing the bolts to an acrylic plate. This arrangement provides a quick, rigid and safe fixation of the head without any signs of discomfort.

Rat pups that are separated early from their mothers, at postnatal day 15, become hypothermic when subjected to physical restraint on postnatal day 30. Restraint of separated pups also elicits an unusually high incidence of gastric erosions, as well as insomnia and an increase in quiet wakefulness. If hypothermia during restraint is prevented, neither the erosions nor the behavioral responses occur. Rat pups separated at the customary age (postnatal day 22) do not become hypothermic during restraint, and the restraint of such pups is not associated with either gastric erosion or insomnia.

The current study examines the use of physical control (seclusion and restraint) in a sample of patients hospitalized in a public mental hospital during 1989. A total of 243 patient records involving physical control were examined; of these, 124 had been civilly committed and the remaining 119 were forensic (Incompetent to Stand Trial or Not Guilty by Reason of Insanity). Some 870 physical control events occurred in the sample, a rate that is intermediate when compared with other facilities described in the literature. Seclusion was employed most often (46% of events), with the combination of

We report on 7 nursing home or hospital patients who died suddenly and unexpectedly during physical restraint. Four of the patients were found dead hanging beside their beds, with their waist restraints displaced to the thorax. In spite of a variety of preexisting diseases, asphyxia by thorax compression was the most probable cause of death. Three other patients, when falling out of their beds, were strangulated by the head opening of a nursing bedcover fixed to the bed. In all instances the fatal accidents resulted from improper handling of the restraint devices, namely from the omission of b

INTRODUCTION: Managing agitation in the emergency department (ED) is challenging and frequently involves decisions about physical restraints and emergent medications. While institutional protocols and behavioral response teams exist, little is known about how individual clinicians make decisions about restraint use-decisions with significant implications for safety, ethics, and equity. The objective of the study was to explore how emergency physicians, residents, and nurses make decisions about physical restraint and emergent medication use during episodes of patient agitation in the ED.
