Occlusive retinal vasculitis in a patient with West Nile virus.

BACKGROUND: West Nile virus (WNV) was first identified in the United States in 1999. In addition to a spectrum of systemic manifestations, several ocular conditions secondary to the virus have been reported, including chorioretinitis, uveitis and optic neuritis. Age and diabetes mellitus (DM) have been reported to be associated risk factors for the more severe forms of the systemic disease. Only seven cases of occlusive retinal vasculitis have been reported in patients with WNV infection. CASE HISTORY: A 60-year-old Asian male presented with complaints of decreased vision in his left eye. He

Retinal vasculitis associated with autoantibodies to Sjögren's syndrome A antigen.

Severe retinal vasculitis caused by a systemic lupus-like illness developed in two patients with distinctive clinical and immunologic characteristics. Both patients were young women with mild systemic disease and autoantibodies directed against a protein-ribonucleic acid complex termed the Sjögren's syndrome A antigen (SSA). One patient had no antibodies to nuclear antigens on conventional testing, and the other had a low-titer antinuclear antibody test. At the time of onset of retinal arteriolitis, neither patient had an exacerbation of multisystem disease or serologic activity. Despite oral

Use of ultra-wide-field retinal imaging in the management of active Behçet retinal vasculitis.

BACKGROUND: Retinal vasculitis (RV) is an important component of Behçet disease. It may be difficult to detect either clinically or with conventional retinal imaging. The role of ultra-wide-field (UWF) retinal imaging was assessed in the diagnosis and management of RV associated with Behçet disease. METHODS: A total of 38 eyes of 20 patients with active RV associated to Behçet disease underwent UWF imaging with the Optos scanning laser ophthalmoscope, fundus autofluorescence, and/or fluorescein angiography. This study determined the UWF findings and percentage of patients in whom this imaging

Retinal vasculitis associated with intravitreal bevacizumab injection: A case report.

Anti-vascular endothelial growth factor drugs are the treatment of choice for macular edema due to venous occlusions. While rare, they have been associated with some uncommon adverse effects. We present a case of retinal vasculitis associated with bevacizumab in a72-year-old woman who presented to our clinic with sudden visual acuity loss in her left eye due to macular edema following central vein occlusion. She was treated with bevacizumab, with a minor inflammatory response that resolved with topical steroids. After 6 weeks, the macular edema recurred and a 2nd dose of bevacizumab was indica

Occlusive retinal vasculitis: novel insights into causes, pathogenesis and treatment.

PURPOSE OF REVIEW: Occlusive retinal vasculitis (ORV) has a large differential diagnosis and varied therapeutic approaches. This review highlights existing and novel causes and treatment options for ORV. RECENT FINDINGS: Mutations in CAPN5, TREX1, and TNFAIP3 have been associated with dominantly inherited forms of ORV. Various intraocular therapeutics have been shown to occasionally cause ORV; the most recent medications associated with ORV are vancomycin and brolucizumab. In cases of ORV linked to Behçet's disease, clinical trials support the use of tumor necrosis factor alpha inhibitors. S

Idiopathic retinal vasculitis, aneurysms, and neuroretinitis (IRVAN) syndrome: clinical perspectives.

Idiopathic retinal vasculitis, aneurysms, and neuroretinitis (IRVAN) syndrome typically affects young, healthy individuals. Despite the dramatic fundus appearance seen in this syndrome, these patients are usually asymptomatic. The syndrome includes peculiar vascular abnormalities in the form of multiple aneurysmal dilatations seen along retinal arterioles and optic nerve-head arterioles, which are best appreciated on fluorescein angiography. Neuroretinitis and retinal vasculitis are seen in all patients, and manifested by staining of the optic nerve head and diffuse leakage from vessels, mainl
