Chest wall regional volume in heart failure patients during inspiratory loaded breathing.

Were evaluated individuals divided into two groups: we studied chronic heart failure (CHF) (19 patients with CHF plus cardiomegaly) and control (12 healthy volunteers) during performance of inspiratory loaded breathing (ILB). We evaluated: spirometry, functional capacity through the six-minute walk test (6MWT), and distribution of thoracoabdominal volumes via optoelectronic plethysmography (OEP), namely volume variations of pulmonary rib cage (Vrc,p), abdominal rib cage (Vrc,a), and abdomen (Vab). In each compartment, the percentage contributions of right and left sides were also calculated. D

Fitting unanchored puzzle pieces in the skeleton: appropriate 3D scapular positions for the quadrupedal support in tetrapods.

Deducing the scapular positions of extinct tetrapod skeletons remains difficult, because the scapulae and rib cage are connected with each other not directly by skeletal joint, but by thoracic muscles. In extant non-testudine quadrupedal tetrapods, the top positions of the scapulae/suprascapulae occur at the anterior portion of the rib cage, above the vertebral column and near the median plane. The adequacy of this position was tested using three-dimensional mechanical models of Felis, Rattus and Chamaeleo that assumed stances on a forelimb on a single side and the hindlimbs. The net moment ab

Factors affecting postoperative pulmonary function deterioration in adolescent idiopathic scoliosis: A prospective study using 3-dimensional image reconstruction by biplanar stereoradiography.

This study aimed both to evaluate rib cage parameters in patients with adolescent idiopathic scoliosis (AIS) by three-dimensional (3D) image reconstruction using biplanar stereoradiography (EOS) (EOS Imaging, Paris, France) and identify factors associated with postoperative pulmonary function deterioration (PFD). A total of 67 patients with Lenke type 1 or 2 AIS (59 females and 8 males; mean age, 14.4 years) undergoing posterior corrective fusion with a rod rotation maneuver based on segmental pedicle screw fixation were recruited. 3D images and pulmonary function test results were analyzed pr

Internal rib structure can be predicted using mathematical models: An anatomic study comparing the chest to a shell dome with application to understanding fractures.

The human rib cage resembles a masonry dome in shape. Masonry domes have a particular construction that mimics stress distribution. Rib cortical thickness and bone density were analyzed to determine whether the morphology of the rib cage is sufficiently similar to a shell dome for internal rib structure to be predicted mathematically. A finite element analysis (FEA) simulation was used to measure stresses on the internal and external surfaces of a chest-shaped dome. Inner and outer rib cortical thickness and bone density were measured in the mid-axillary lines of seven cadaveric rib cages usin

Relative strengths of the chest wall muscles.

We hypothesized that during maximal respiratory efforts involving the simultaneous activation of two or more chest wall muscles (or muscle groups), differences in muscle strength require that the activity of the stronger muscle be submaximal to prevent changes in thoracoabdominal configuration. Furthermore we predicted that maximal respiratory pressures are limited by the strength of the weaker muscle involved. To test these hypotheses, we measured the pleural pressure, abdominal pressure (Pab), and transdiaphragmatic pressure (Pdi) generated during maximal inspiratory, open-glottis and closed

Does in-brace correction affect coronal spinal and thoracic cage parameters in individuals with idiopathic scoliosis? A retrospective cohort study.

The aim of the study is to identify the effects of in-brace correction on coronal spinal and thoracic cage parameters in individuals with idiopathic scoliosis (IS). The coronal spinal parameters [Cobb angle, apical vertebral rotation (AVR), lateral trunk shift, coronal alignment, biacromial slope and pelvic asymmetry] and the thoracic cage parameters [T1- 12 height, T1-S1 height, thoracic transverse diameter, and apical vertebral body-rib ratio (AVB-R)] of 89 child and adolescent patients were measured on posterior-anterior full-spine radiographs at pre-brace and in-brace conditions using Surg

Ribcage Morphology in Native South American Populations From Different Altitudes: Insights From a Global Comparative Framework.

OBJECTIVES: Altitude shapes human morphology as highland populations must cope with cold and hypoxic environments. Although Andean highlanders have been proposed to exhibit larger and deeper ribcages, this idea is mainly based on research using disarticulated skeletal elements or non-South American controls. The objective of this research is to study 3D ribcage configuration of native South American populations considering altitude and worldwide ribcage variation. METHODS: Ribcages of 37 adult South Americans (17 highlanders, 20 lowlanders) were reconstructed and analyzed using 3D geometric m

Effects of Derotational Breathing Exercises on Thoracic Cage Asymmetry in Adolescents with Thoracic Scoliosis.

Thoracic scoliosis is commonly associated with rib cage deformity and impaired respiratory mechanics. Derotational breathing exercises aim to correct three-dimensional trunk asymmetry by selectively expanding the concave hemithorax. This prospective quasi-experimental study investigated the effects of a 4-week supervised Schroth-based rotational angular breathing (RAB) program on intercostal distance (ICD), vertebral axial rotation, and Cobb angle in 20 female adolescents (age 14.30 ± 1.84 years; mean follow-up 33.45 ± 4.26 days) with thoracic idiopathic scoliosis. Radiographic measurements we

Traumatic Impact Assessment of CPR Load on a Human Ribcage.

Chest compression is a parameter of injury criteria assessment for human beings. Additionally, it is used to find the external compression response as a result of vehicle crashes, falls, or sporting impacts. This behavioral feature is described by many deterministic models related to specific experimental tests, hindering distinct scenarios. The present study evaluates the energy absorbed as a function of rib compression. The proposed model was obtained from three different computed tomography (CT) studies. The anthropometric values are interpolated to obtain a parametric curve for a human rib

Effect of hyperinflation on the diaphragm.

Acute hyperinflation causes the inspiratory muscles to operate at shorter than normal lengths. The ability of these muscles, in particular the diaphragm, to lower intrathoracic pressure is therefore reduced. Skeletal muscles, however, adapt to chronic shortening, and animals models of emphysema have shown that with chronic hyperinflation, the diaphragmatic muscle fibres lose sacromeres. As a result, the force-generating ability of these fibres is relatively preserved. In patients with hyperinflation due to chronic obstructive pulmonary disease, the ability of the diaphragm to generate pressure

Management of congenital chest wall deformities.

Congenital chest wall deformities are considered to be anomalies in chest wall growth. These can be categorized as either rib cage overgrowth or deformities related to inadequate growth (aplasia or dysplasia). Rib cage overgrowth leads to depression of the sternum (pectus excavatum) or protuberance of the sternum (pectus carinatum) and accounts for greater than 90% of congenital chest wall deformities. The remaining deformities are a result of inadequate growth. Evolution in the management of congenital chest wall deformities has made significant progress over the past 25 years. This article w

Improved pulmonary function after concave rib resection and posterior instrumentation for idiopathic scoliosis.

The authors conducted a prospective study in order to evaluate the effect'of concave rib osteotomy on pulmonary function, in the context of posterior instrumentation for the treatment of idiopathic scoliosis. Concave rib osteotomy improves the flexibility of the spine, especially in very rigid curves above 70. Seventy-eight consecutive children with a Cobb angle above 70 degrees were included in the study. Pulmonary function was measured preoperatively and at 12 months postoperatively. After surgery, all patients took part at an intense pulmonary rehabilitation program. The mean preoperative C

Nineth Rib Syndrome after 10(th) Rib Resection.

The 12(th) rib syndrome is a disease that causes pain between the upper abdomen and the lower chest. It is assumed that the impinging on the nerves between the ribs causes pain in the lower chest, upper abdomen, and flank. A 74-year-old female patient visited a pain clinic complaining of pain in her back, and left chest wall at a 7 on the 0-10 Numeric Rating scale (NRS). She had a lateral fixation at T12-L2, 6 years earlier. After the operation, she had multiple osteoporotic compression fractures. When the spine was bent, the patient complained about a sharp pain in the left mid-axillary line

Chest wall motion during spontaneous breathing and mechanical ventilation in dogs.

We measured the volume change of the thoracic cavity (delta Vth) and the volumes displaced by the diaphragm (delta Vdi) and rib cage (delta Vrc) in six pentobarbital-anesthetized dogs lying supine. A high-speed X-ray scanner (dynamic spatial reconstructor) provided three-dimensional images of the thorax during spontaneous breathing and during mechanical ventilation with paralysis. Tidal volume (VT) was measured by integrating gas flow. Changes in thoracic liquid volume (delta Vliq, presumably caused by changes in thoracic blood volume) were calculated as delta Vth - VT. Absolute volume displac

Abnormal breathing patterns in patients with mitral stenosis: a possible compensatory role.

The breathing patterns of 37 patients with mitral stenosis were investigated in standing position by Konno-Mead analysis. Hemodynamic parameters were measured by cardiac catheterization, pulmonary function variables by pulmonary function testings and distribution of pulmonary perfusion by Tc-99m-MAA scintigraphy. Seventeen patients displayed rib cage dominant breathing patterns, and 6 patients displayed paradoxical breathing patterns, whereas 14 patients displayed normal breathing patterns. None of these patients exhibited an abdomen dominant breathing pattern. The patients with abnormal breat

Voluntary hyperventilation changes recruitment order of parasternal intercostal motor units.

The order of recruitment of single-motor units in parasternal intercostal muscles during inspiration was studied in normal human subjects during quiet breathing and voluntary hyperventilation. Electromyograms were recorded from the second and third intercostal spaces by means of bipolar fine wire electrodes. Flow at the mouth, volume, end-expired CO2, and rib cage and abdominal anterior-posterior diameters were monitored. Single-motor units were identified using criteria of amplitude and shape, and the time of first appearance of each unit in each inspiration was noted. Hyperventilation was pe

Idiopathic Scoliosis Progression: Presenting Rib and Segmental Rib Index as Predictors-A Literature Review.

This report provides a concise selective representative overview of the predictor factors for progression in Idiopathic Scoliosis (IS). The Cobb angle method, rib hump deformity, imaging and advanced techniques for assessing skeletal maturity serve as key elements in evaluating prognostic factors for IS progression based on the patient's age at diagnosis-particularly in Infantile Idiopathic Scoliosis (IIS), Juvenile Idiopathic Scoliosis (JIS), and Adolescent Idiopathic Scoliosis (AIS). The commonly used approaches for determining skeletal maturity include the assessment of the iliac apophysis
