Analysing the efficacy and tolerability of dolutegravir plus either rilpivirine or lamivudine in a multicentre cohort of virologically suppressed PLWHIV.

OBJECTIVES: We aimed to investigate and compare the efficacy and safety of two dolutegravir-based two-drug regimens: dolutegravir + lamivudine versus dolutegravir + rilpivirine. METHODS: We analysed a cohort of people living with HIV (PLWHIV) switching to dolutegravir + lamivudine or dolutegravir + rilpivirine. We excluded from the analysis PLWHIV with no available pre-switch genotypic test or with a known resistance mutation to one of the study drugs. We evaluated incidence of virological failure (VF) and treatment discontinuation (TD), as well as changes in immunological and metabolic param

Efficacy, durability, and tolerability of dolutegravir/lamivudine and dolutegravir/rilpivirine for the treatment of HIV in a real-world setting in Belgium.

OBJECTIVES: A paradigm shift from three-drug regimens to two-drug regimens (2DRs) is currently taking place in real-world clinical practice. This study aimed to describe the efficacy, durability, and tolerability of dolutegravir (DTG)/lamivudine (3TC) and DTG/rilpivirine (RPV) in a real-world setting. METHODS: This was a retrospective, observational, multicentre (ten centres in Belgium) study involving adult treatment-naïve and treatment-experienced people living with HIV on DTG/3TC or DTG/RPV between 1 January 2019 and 30 September 2020. The primary endpoint was rate of virological suppressi

Risk Factors and Therapeutic Consequences of Virologic Failure With Long-Acting Cabotegravir Plus Rilpivirine in the Dat'AIDS Cohort.

BACKGROUND: Long-acting cabotegravir plus rilpivirine (CAB + RPV LA) is highly effective as maintenance therapy in virologically suppressed people with HIV (PWH), but virologic failure (VF) may lead to resistance and compromise future antiretroviral options. METHODS: We conducted a multicenter observational study within the French Dat'AIDS cohort, including HIV-1 virologically suppressed adults who switched to CAB + RPV LA between December 2021 and December 2023. VF was defined as two consecutive HIV-1 RNA >50 copies/mL or one >200 copies/mL. Remaining options were interpreted using ANRS and

Trough concentrations of cabotegravir and rilpivirine and their association with detectable viral load in people with HIV on long-acting treatment.

BACKGROUND: Cabotegravir (CAB) and rilpivirine (RPV) constitute the first complete non-oral ART regimen for HIV-1 treatment. Due to virologic failure (VF) with resistance in clinical trials, concerns persist regarding broader use in clinical practice. In particular, the role of trough drug concentrations in relation to viremia and VF remains unclear. This study explored the association between CAB and RPV trough concentrations in a retrospective, single-center study. METHODS: We retrospectively analyzed data from the HIV research and clinical care center MVZ München am Goetheplatz, Germany. I

A prospective assessment of the efficacy and durability of long-acting cabotegravir and rilpivirine in individuals with HIV in Spain (RELATIVITY study).

BACKGROUND: Daily oral antiretroviral therapy (ART) has transformed HIV into a chronic, manageable condition, but it still poses psychological, logistical, and biological challenges. Long-acting injectable (LAI) cabotegravir plus rilpivirine provides an alternative for virologically suppressed individuals, reducing treatment fatigue, stigma, and daily dosing. While clinical trials have demonstrated efficacy and safety, real-world data remain limited. We assessed the effectiveness and safety of LAI cabotegravir plus rilpivirine in Spain. METHODS: RELATIVITY is a 5-year, multicentre, ambispecti

Viral Suppression Rates at 48 Weeks in People With HIV Starting Long-Acting Cabotegravir/Rilpivirine With Initial Viremia.

BACKGROUND: We previously demonstrated at the Ward 86 human immunodeficiency virus (HIV) clinic in San Francisco that long-acting (LA) cabotegravir (CAB)/rilpivirine (RPV) (LA-CAB/RPV) can rapidly lead to viral suppression in people with HIV (PWH) with viremia due to adherence challenges. We now evaluate the durability of viral suppression in this population. METHODS: We conducted a retrospective cohort study of PWH who started LA-CAB/RPV with viremia (HIV RNA viral load ≥50 copies/mL) before December 2022. Our primary outcome was viral suppression (viral load <50 copies/mL) with LA-CAB/RPV p

Necrotic skin reaction following the administration of intramuscular injections of long-acting cabotegravir/rilpivirine and alternatively-sourced testosterone.

In this case, we describe a 52-year-old man who was switched to long-acting cabotegravir/rilpivirine (CAB/RPV LA) injectable antiretroviral therapy (ART) on a background of well-controlled HIV. He subsequently developed a painful and swollen area on his right ventrogluteal region 10 days after his first CAB/RPV LA injections when he injected alternatively sourced testosterone in the same region. Examination findings showed a firm and defined lump with ecchymosis noted on the skin overlying. His initial blood results did not show any gross abnormalities and an ultrasound of the area was equivoc

Injectable Long-Acting Cabotegravir-Rilpivirine Therapy for People Living With HIV/AIDS: Addressing Implementation Barriers From the Start.

Injectable cabotegravir and rilpivirine (CAB/RPV), administered bimonthly by a medical provider, is convenient and improves privacy and medication management. One year after approval, myriad implementation barriers threaten the access and sustainability of this life-saving innovation: (1) eligibility issues (viral suppression, drug resistance, and failed oral regimens); (2) injection requires medical provider and transportation to facility; (3) strict medication adherence; (4) life challenges-mental health, homelessness, joblessness; and (5) lack of insurance and high cost. Universal implement
