The effect of dexmedetomidine on rocuronium-induced neuromuscular blockade and its reversal by sugammadex.

BACKGROUND: Dexmedetomidine (DEX) is increasingly used in the intensive care unit for sedation and also serves as an adjuvant in general anesthesia and in procedural sedations. We tested whether dexmedetomidine at different concentrations influences the activity of the neuromuscular junction at the diaphragm and whether DEX has an impact on the action of rocuronium at the diaphragm as well as the reversal of the neuromuscular block by sugammadex. METHODS: 20 male Wistar rat phrenic nerve-hemidiaphragm system was used for our experiments. The concentration-response characteristics of DEX and r

A Novel Anesthetic in Thomsen Myotonia Congenita With Rocuronium and Sugammadex: A Case Report.

This report describes the use of continuous rocuronium infusion for paralysis during 2 extensive spinal surgeries in a pediatric patient with Thomsen myotonia congenita. Although paralytics are typically avoided to allow motor-evoked potential monitoring, in this case, paralysis was required to reduce myotonic episodes and related complications. Rocuronium infusion with sugammadex reversal was used successfully, with only one presumed myotonic episode and no serious sequelae. This novel approach highlights the need for anesthesiologists to balance monitoring considerations with patient safety.

Sugammadex reversal of an accidental rocuronium overdose in a dog in the context of incivility in the workplace.

A 7-year-old, 10.6 kg female mixed-breed dog underwent anaesthesia for craniotomy to excise a brain mass. During anaesthetic preparation, repeated time pressure remarks resulted in rushed preparation and workflow disruption. After anaesthetic premedication with methadone, general anaesthesia was induced with lidocaine, midazolam, and propofol, and maintained with propofol and remifentanil intravenous (IV) infusions. Rocuronium was administered as a bolus (0.5 mg kg-1 IV) followed by continuous infusion at an intended rate of 0.2 mg kg-1 hour-1. After 120 minutes of rocuronium constant-rate inf

A Prospective Randomized Study Comparing Intubating Conditions With and Without Priming With Rocuronium.

BACKGROUND: Rocuronium is widely used as an alternative to succinylcholine for facilitating endotracheal intubation, but at conventional doses its clinical onset may be slower. The priming technique has been proposed as a means of shortening the time to clinically acceptable intubation with non-depolarizing neuromuscular blocking agents. METHODS: In this prospective, randomized, double-blind study, 60 American Society of Anesthesiologists I-II patients aged 18-60 years undergoing elective surgery under general anesthesia were allocated into two groups (n = 30 each). The priming group received

Resistance to Neuromuscular Blockade by Rocuronium in Surgical Patients with Spastic Cerebral Palsy.

Individuals with spastic cerebral palsy (CP) often exhibit altered sensitivities to neuromuscular blocking agents (NMBAs) used for surgical intubation. We assessed usage of the NMBA rocuronium in patients with spastic CP and evaluated potential modifiers of dosing including gross motor function classification system (GMFCS) level, birthweight, gestational age, and the use of anticonvulsant therapy. In a case-control study, surgical patients with spastic CP (n = 64) or with idiopathic or non-neuromuscular conditions (n = 73) were enrolled after informed consent/assent. Patient data, GMFCS level

Anesthesia in a patient with multiple-system atrophy using a combination of rocuronium and sugammadex.

Multiple-system atrophy is an adult-onset progressive neurodegenerative disease affecting the central nervous system, including the spinal cord. There has been no perioperative guideline of the muscle relaxants used in multiple-system atrophy, although a recent article recommends anesthesiologists to use shorter-acting drugs at the lowest possible doses in the patients. Here, we document the first case with multiple-system atrophy undergoing surgery managed with a combination of rocuronium and sugammadex. The recovery time to the train-of-four count 2 after intravenous rocuronium 0.6 mg / kg,

Treatment of Anaphylaxis to Rocuronium with Sugammadex: A Case Report with Bronchospasm as the Only Symptom.

Anaphylaxis during anaesthesia is a rare event occurring in up to 1:20,000 anaesthetics and in 33%-63% neuromuscular blocking agents are involved. Several case reports suggested the effectiveness of sugammadex in the treatment of rocuronium-induced anaphylactic shock refractory to conventional treatment. We report a case of anaphylactic reaction to rocuronium that caused isolated respiratory symptoms and showed no improvement in oxygen saturation after intravenous corticosteroids and intratracheal beta-2 agonists and that was successfully treated with sugammadex. The underlying pathophysiologi

Rocuronium and sugammadex used effectively for electroconvulsive therapy in a patient with Brugada syndrome.

We report the successful anesthetic management of a patient with Brugada syndrome who underwent electroconvulsive therapy to treat bipolar disorder. Suxamethonium and neostigmine were contraindicated to avoid the vagotonic effects that can precipitate ventricular fibrillation during anesthesia in patients with Brugada syndrome. The combination of 1.2 mg/kg rocuronium and 10 mg/kg sugammadex was effectively and safely used to induce and antagonize neuromuscular block for 8 consecutive electroconvulsive therapy sessions in this patient.

Determination of interstitial rocuronium concentrations in the muscle tissue of anesthetized dogs by microdialysis.

INTRODUCTION: The objective was to establish and validate a microdialysis technique for the quantification of interstitial concentrations of the neuromuscular blocker, rocuronium, in the muscle tissue of dogs under steady-state conditions. METHODS: The standard and combined retrodialysis approaches were used for in vivo microdialysis probe calibration. After induction of anesthesia with pentobarbital (30 mg/kg), the left femoral vein was cannulated and blood drawn for protein binding determination. Microdialysis probes were inserted in the muscle and calibrated in vivo, using vecuronium as th

Determination of rocuronium and its putative metabolites in body fluids and tissue homogenates.

A sensitive and selective HPLC method was developed for the quantification of the neuromuscular blocking agent rocuronium and its putative metabolites (the 17-desacetyl derivative and the N-desallyl derivative of rocuronium) in plasma, urine, bile, tissue homogenates and stoma fluid. Samples were prepared by extraction of the biological matrix with dichloromethane, after mixing with a KI-glycine buffer. After evaporation of the organic solvent the samples were chromatographed on a reversed-phase HPLC column, using an aqueous buffer-dioxane (84:16, v/v) as the mobile phase. The aqueous buffer c

Accidental Sugammadex Administration Before Rocuronium During Electroconvulsive Therapy: The Zero-Complexation Scenario.

Electroconvulsive therapy (ECT) performed with rocuronium-sugammadex requires both agents to be prepared before each session, creating a persistent syringe-swap risk that compounds with repetition. A 48-year-old woman scheduled for her sixth ECT session received sugammadex 200 mg instead of rocuronium due to syringe misidentification driven by task automaticity accumulated over 5 uneventful preceding sessions. Because the entire sugammadex dose remained unbound in plasma (a state we term "zero-complexation"), subsequently administered rocuronium 100 mg (1.41 mg/kg) failed to achieve adequate n

Two Cases of Rocuronium-Induced Anaphylaxis/Anaphylactic Shock Successfully Treated With Sugammadex.

While anaphylaxis can occur at any time during general anesthesia, 90% of cases occur at induction of anesthesia. As several drugs are administered simultaneously at this time, it is difficult to identify the causative agent. However, it has been found that rocuronium is the most common drug associated with perioperative anaphylaxis. We treated 2 cases of patients who were administered sugammadex for anaphylactic symptoms thought to be caused by rocuronium, after which the anaphylactic symptoms disappeared. One of the most important aspects of treating anaphylactic shock is improving hemodynam
