Mental disorders, participation, and trajectories in the Danish colorectal cancer programme: a population-based cohort study.

BACKGROUND: People with mental disorders exhibit increased mortality due to colorectal cancer, despite having a similar incidence to the general population. We aimed to evaluate the extent to which people with mental disorders participate in organised colorectal cancer screening. METHODS: We conducted a population-based cohort study of all Danish residents aged 50-74 years who were invited to undergo biennial faecal immunochemical testing between March 1, 2014, and Sept 30, 2018. We used national registry data from all first-time invitees. The primary endpoint was participation within 90 days

Measuring oral COVID-19 antiviral effectiveness using linked Australian data: Challenges and opportunities.

BACKGROUND: As SARS-CoV-2 continues to evolve in the context of emergence of new SARS-CoV-2 variants, increasing vaccination and hybrid immunity, timely monitoring of COVID-19 antiviral effectiveness remains critical, particularly in high-risk populations. This study evaluated the feasibility of using routinely collected health data in Victoria, Australia, to estimate the real-world effectiveness of oral antivirals (molnupiravir and nirmatrelvir-ritonavir) against severe COVID-19 outcomes. METHODS: We analysed record-linked, routinely collected health data for individuals aged ≥ 70 years with

Estimating small area population from health intervention campaign surveys and partially observed settlement data.

Effective governance requires timely and reliable small area population counts. Geospatial modelling approaches which utilise bespoke microcensus surveys and satellite-derived settlement maps and other spatial datasets have been developed to fill population data gaps in countries where censuses are outdated and incomplete. However, logistics and costs of microcensus surveys and tree canopy or cloud cover obscuring settlements in satellite images limit its wider applications in tropical rural settings. Here, we present a two-step Bayesian hierarchical modelling approach that can integrate routi

Adversity profiles among court-involved youth: Translating system data into trauma-responsive programming.

BACKGROUND: Court-involved youth have high levels of adverse childhood experiences (ACEs), which can impact functioning in adolescence and throughout adulthood. Yet there is limited research to help clinicians translate these histories into trauma-responsive programming guidelines. OBJECTIVE: This manuscript utilizes data that is routinely collected to inform practitioners about how to utilize trauma histories to inform program and practice decisions. METHODS: This study used administrative data with a diverse sample of medium- to high-risk youth on probation (N = 5,378) to examine how ACE c

Predicting first falls among older adults with chronic conditions and polypharmacy using routinely collected health records.

BACKGROUND: Falls represent a major clinical and financial challenge for healthcare systems. Accurately predicting first falls remains challenging, especially when using routinely collected data. OBJECTIVE: Develop and evaluate a predictive model to identify elderly people at risk of first fall-defined as a fall after a 90-day fall-free period-in the Basque Country using routinely collected health records. METHOD: A retrospective study included patients aged ≥65 with at least two chronic conditions among heart failure, chronic obstructive pulmonary disease (COPD), and diabetes. Data on demog

The OBS UK Dashboard: an interactive tool for representative trial site selection to facilitate equality and diversity in maternity research.

BACKGROUND: Obstetric Bleeding Study UK (OBS UK) (award ID: 152057) is a National Institute for Health and Care Research (NIHR)-funded stepped wedge cluster randomised controlled trial of a complex intervention for postpartum haemorrhage. This was developed in Wales and evaluated in a feasibility study, with improvements in maternal outcomes observed. Generalisability of the findings is limited by lack of control data and limited ethnic diversity in the Welsh obstetric patient population compared to the United Kingdom (UK): 94% of the Welsh population identifies as White, versus 82% in the UK.

Are routinely collected NHS administrative records suitable for endpoint identification in clinical trials? Evidence from the West of Scotland Coronary Prevention Study.

BACKGROUND: Routinely collected electronic patient records are already widely used in epidemiological research. In this work we investigated the potential for using them to identify endpoints in clinical trials. METHODS: The events recorded in the West of Scotland Coronary Prevention Study (WOSCOPS), a large clinical trial of pravastatin in middle-aged hypercholesterolaemic men in the 1990s, were compared with those in the record-linked deaths and hospitalisations records routinely collected in Scotland. RESULTS: We matched 99% of fatal study events by date. We showed excellent matching (97%

Death after surgery among patients with chronic disease: prospective study of routinely collected data in the English NHS.

BACKGROUND: Five million surgeries take place in the NHS each year. Little is known about the prevalence of chronic diseases among these patients, and the association with postoperative outcomes. METHODS: Analysis of routine data from all NHS hospitals in England including patients aged ≥18 yr undergoing non-obstetric surgery between January 1, 2010 and December 31, 2015. The primary outcome was death within 90 days after surgery. For each chronic disease, we adjusted for age, sex, presence of other diseases, emergency surgery, and year using logistic regression models. We defined high-risk d

Small for Gestational Age Coded Diagnoses in Aotearoa New Zealand's Administrative Health Datasets: A Validation Study.

BACKGROUND AND AIMS: Inaccurate coding of small for gestational age (SGA) infants in routinely collected health data has implications for research based on those data. We aimed to estimate the sensitivity and specificity of coded SGA diagnoses in New Zealand's routinely collected hospitalisation and mortality data, and determine whether sensitivity and specificity varied by infant, pregnancy, and maternal characteristics. METHODS: We estimated birthweight centiles of live and stillborn infants delivered in New Zealand between 2005 and 2020 using the Fenton Population Reference Calculator and

Supplementing routine hospital surveillance of malaria to capture excess mortality and epidemiological trends: a five-year observational study.

INTRODUCTION: Malaria surveillance in Africa is conducted largely through health facility-based health management information systems (HMIS) which provide aggregated data to malaria control programs. Supplementation of HMIS surveillance with other routinely collected hospital data can provide vital statistics on malaria control in regions of high burden. METHODS: To assess the utility of supplementing HMIS data, we implemented a pilot program of enhanced malaria surveillance in a district hospital in northern Zambia over a five-year period. Data were tabulated from existing nursing records, c

Trends in percentages of gestational diabetes mellitus attributable to overweight, obesity, and morbid obesity in regional Victoria: an eight-year population-based panel study.

BACKGROUND: Gestational diabetes mellitus (GDM) is the fastest growing type of diabetes in Australia with rates trebling over the past decades partially explained by rising obesity rates and maternal age among childbearing women. Percentage of GDM attributable to obesity has been documented, mostly focusing on metropolitan populations. In parts of regional (areas outside capital cities) and rural Australia where overweight, obesity and morbid obesity are more prevalent, intertwined with socioeconomic disadvantage and higher migrant communities, trends over time in adjusted percentages of GDM a

Machine Learning Algorithms Using Routinely Collected Data Do Not Adequately Predict Viremia to Inform Targeted Services in Postpartum Women Living With HIV.

BACKGROUND: Adherence to antiretroviral treatment (ART) among postpartum women with HIV is essential for optimal health and prevention of perinatal transmission. However, suboptimal adherence with subsequent viremia is common, and adherence challenges are often underreported. We aimed to predict viremia to facilitate targeted adherence support in sub-Saharan Africa during this critical period. METHODS: Data are from PROMISE 1077BF/FF, which enrolled perinatal women between 2011 and 2014. This analysis includes postpartum women receiving ART per study randomization or country-specific criteria

HIV testing among men who have sex with men: how often should one test?

HIV counseling and testing has been a cornerstone of AIDS prevention strategies, with men who have sex with men being specifically targeted for the counseling and testing. Unfortunately, it appears that exposure to HIV counseling and testing has little effect on the behaviors of those who test negative. The Centers for Disease Control and Prevention recently altered its recommendations on when and how often men who have sex with men should undergo testing for HIV and other sexually transmitted diseases, now recommending annual testing for sexually active men who have sex with men, with even mo

Using routinely collected data to understand and predict adverse outcomes in opioid agonist treatment: Protocol for the Opioid Agonist Treatment Safety (OATS) Study.

INTRODUCTION: North America is amid an opioid use epidemic. Opioid agonist treatment (OAT) effectively reduces extramedical opioid use and related harms. As with all pharmacological treatments, there are risks associated with OAT, including fatal overdose. There is a need to better understand risk for adverse outcomes during and after OAT, and for innovative approaches to identifying people at greatest risk of adverse outcomes. The Opioid Agonist Treatment and Safety study aims to address these questions so as to inform the expansion of OAT in the USA. METHODS AND ANALYSIS: This is a retrospe

Reliability of routinely collected hospital data for child maltreatment surveillance.

BACKGROUND: Internationally, research on child maltreatment-related injuries has been hampered by a lack of available routinely collected health data to identify cases, examine causes, identify risk factors and explore health outcomes. Routinely collected hospital separation data coded using the International Classification of Diseases and Related Health Problems (ICD) system provide an internationally standardised data source for classifying and aggregating diseases, injuries, causes of injuries and related health conditions for statistical purposes. However, there has been limited research t

Supervised machine learning-based prediction for in-hospital pressure injury development using electronic health records: A retrospective observational cohort study in a university hospital in Japan.

BACKGROUND: In hospitals, nurses are responsible for pressure injury risk assessment using several kinds of risk assessment scales. However, their predictive validity is insufficient to initiate targeted preventive strategy for each patient. The use of electronic health records with machine learning technique is a promising strategy to provide automated clinical decision-making aid. OBJECTIVE: The purpose of this study was to construct a predictive model for pressure injury development which included feature variables that can be collected on the first day of hospitalization by nurses who rou
