Barriers and facilitators to conducting human subjects research at a safety net institution from the perspective of researchers.

INTRODUCTION: The COVID-19 pandemic revealed glaring problems with clinical research enterprise. Faced with crisis, several trials opened rapidly but enrolled homogenous populations with few Black, Indigenous, and People of Color (BIPOC) individuals. Inclusive trial enrollment is important to inspire trust and confidence in BIPOC populations that have been historically excluded or harmed from research and to improve the generalizability of research findings. Safety-net hospitals and institutions often care for BIPOC populations, and thus it is essential to improve equitable participation in re

Supporting primary care clinicians in caring for patients with alcohol use disorder: study protocol for Records for Alcohol Care Enhancement (RACE), a factorial four-arm randomized trial.

BACKGROUND: Unhealthy alcohol use, a spectrum of use inclusive of risky consumption and alcohol use disorder (AUD), is a leading cause of preventable death in the United States. Most people with unhealthy alcohol use do not receive evidence-based treatment. This four-arm factorial design randomized trial will assess whether population health management (PHM) and clinical care management (CCM) support for primary care providers (PCPs) are associated with improved AUD treatment engagement among their patients, beyond electronic health record (EHR) prompting and decision support alone. METHODS:

The Role of a Certified Registered Nurse Anesthetist Led Acute Pain Service in Preventing Persistent Postoperative Opioid Use.

Given the present opioid crisis, the use of opioids in the hospital setting is an increasing concern among hospital administrators and healthcare professionals. A serious problem related to surgical care is persistent postoperative opioid use among previously opioid-naïve patients. Certified registered nurse anesthetists (CRNAs) are strategically positioned within the hospital setting to address these concerns. These individuals are actively involved in managing the pain of their patients and can therefore lead change in relation to the opioid crisis. This article profiles a multidisciplinary

Patient-Centered Communication and Intention to Recommend a Safety Net Primary Care Clinic in the United States.

Patient-centered communication (PCC) is critical to improved patient experience and outcomes. We used binary logistic regression to examine how PCC at safety-net providers was associated with patient intent to recommend the facility. We used 2021-2022 national data collected by the Health Resources and Services Administration (HRSA). Our predictors were Consumer Assessment of Healthcare Providers and Systems (CAHPS) items to measure patient assessment of provider communication. We found that patients whose providers always listened carefully and knew important information about patients' medic

How Does Traumatic Hand Injury Impact Patients in a Safety-net Healthcare System? A Mixed Methods Study.

BACKGROUND: Traumatic hand injuries can result in substantial harms to patients' functional abilities and financial health, disproportionately affecting individuals who are of working age, low income, and uninsured. However, little is known about how patients and their families cope with the functional limitations and economic consequences that follow, and how these injuries affect patients' mental and emotional health. Exploring clinician and staff perceptions about the impact of traumatic hand injuries on patients may provide insight into how to help address the various burdens patients face

Youth Advisory Boards in Health Care Settings: Perspectives From Health Care Professionals.

BACKGROUND: There is a mutually beneficial, multidirectional relationship when young people are engaged in policy, programming, and research. Although the positive impact of youth engagement is evident, adultism and organizational readiness remain key barriers to youth participation in health care settings. OBJECTIVES: This study explored how adult health care professionals and hospital leadership at an urban safety-net hospital understand and conceptualize youth advisory boards (YABs) and assess their utility in health care settings. METHODS: Semistructured interviews (N = 10) were conducte

Characterizing Comorbidities in Total Joint Arthroplasty in a Safety-Net Hospital Using Age-Adjusted Charlson Comorbidity Index.

BACKGROUND: The joint arthroplasty population has a high number of comorbidities at the time of surgery, which increases the cost of care per episode. The goal of this study was to analyze the distribution of comorbid diseases in patients who have undergone total joint arthroplasty at our institution, which serves the poorest congressional district in the United States. We hypothesized that patients in an urban safety-net hospital would have higher comorbidity rates and higher Charlson Comorbidity Indices and that this would suggest the need for improved risk stratification in payment systems.

Collaborative Care for Anxiety: Differences in Patient and Clinic-Level Characteristics on Treatment Outcomes in a Safety-Net Hospital System.

The collaborative care model (CoCM) is an evidence-based model shown to improve treatment of depression and anxiety in the primary care setting. However, there is limited research on CoCM outcomes in underserved populations and which factors impact outcomes. We performed a retrospective chart review to examine GAD-7 scores for patients (N = 1,034) seen in primary care clinics within an urban, safety-net hospital system. The Wilcoxon signed-rank test and linear mixed models were used. Our population was 81% female and 89% self-identified Black or African American. There were significant reducti
