We report an atypical presentation of Toxoplasma retinochoroiditis with associated scleritis in a young and immunocompetent patient. The diagnosis was done on the basis of Polymerase chain reaction of vitreous sample, and the clinical response to specific treatment. This case highlights the unusual presentation of ocular toxoplasmosis as scleritis.

Scleritis and peripheral ulcerative keratitis (PUK) can present as isolated conditions or as part of a systemic inflammatory or infectious disorder. Both are serious ocular conditions that can result in vision loss and require early diagnosis and treatment. Nearly two thirds of patients with non-infectious scleritis require systemic glucocorticoid therapy and one fourth need a glucocorticoid-sparing agent as well. Essentially all patients with non-infectious PUK require systemic glucocorticoids. A detailed clinical history, thorough physical examination, and thoughtful laboratory evaluations a

The authors report a case of nodular scleritis in Sweet's syndrome. The patient was a 40-year-old Malay woman with a history of Sweet's syndrome presented with a 6-day history of a red and painful right eye associated with blurred vision. Clinical examination revealed nodular scleritis of the right eye which was deemed secondary to Sweet's syndrome after investigations for other systemic diseases were negative. The patient demonstrated good response to systemic corticosteroid therapy with complete resolution of scleritis in the right eye. However, her inflammatory skin condition worsened each

We present a case of severe unilateral posterior scleritis associated with zoledronic acid administration that was recognized and treated in a timely manner. Subsequent use of pamidronate was associated with milder inflammation in the same eye that eventually resolved with continued monthly infusions, indicating the possibility of immunologic tolerance. Physicians prescribing these drugs should be aware of this side effect, which, if not recognized and treated appropriately, is potentially devastating. We review the published data as well as the current treatment recommendations.

BACKGROUND: Stenotrophomonas maltophilia is a seldom-reported pathogen in ocular infections. The report describes six cases of Stenotrophomonas maltophilia (S. maltophilia) keratitis and scleritis. To our knowledge, this is the foremost report of S. maltophilia scleritis.
METHODS: Laboratory reports of patients diagnosed with S. maltophilia ocular infections were collected from the ophthalmic department of Chang-Gung memorial hospital from January 1, 2000, through December 31, 2003. On evaluation of risk factors, isolates, antibiotic sensitivities, and response to the treatment ensued.

In the submitted paper the authors present an account on the importance of ultrasonographic examination by the method of standardized echography for early and correct establishment of the diagnosis scleritis posterior. They mention the echographic image of posterior scleritis and the specific acoustic criteria which are important from the aspect of the differential diagnosis. They analyze differences of the echographic image in posterior scleritis, melanoma of the choroid, metastatic carcinoma of the choroid and haemangioma of the choroid. When knowing the specific acoustic criteria the succes

PURPOSE: To analyze the efficacy of transeptal corticosteroid injections in necrotizing scleritis as additional treatment to systemic immunosuppressors.
METHOD: Eleven patients with necrotizing scleritis treated with systemic immunosuppressors received transeptal corticosteroid injections during periods of active inflammation.
RESULTS: Transeptal corticosteroid injections in addition to systemic immunosuppressors halted active imflammation in 10 of 11 patients with necrotizing scleritis, showing few side effects. Because of that, there was no need to increase dosage nor to switch systemic im

BACKGROUND: Recurrent immune-mediated scleritis after adequate treatment of leprosy is not well documented in the literature. We describe an Australian resident with unilateral intra-ocular lepromatous leprosy who had persistent non-infectious scleritis.
METHODS: A man of Anglo-Indian ancestry initially presented with lepromatous leprosy and unilateral ocular involvement. The affected eye had an interstitial keratitis and a granulomatous anterior uveitis that responded to antileprotics and anti-inflammatory agents. Despite systemic cure with triple antileprotic therapy, he developed recurrent

BACKGROUND: Wegener's granulomatosis is a focal necrotizing granulomatous vasculitis of unknown etiology. It manifests initially as localized inflammation, usually of the lungs, and preceeds to the state of generalisation which can be life-threatening due to renal failure. Ocular and orbital involvement occurs in 28-58% of patients with Wegener's granulomatosis. More often, the anterior segments of the eye are involved whereas inflammation of the posterior segments is quite uncommon.
PATIENTS AND METHODS: We report a case of posterior scleritis as the initial manifestation of Wegener's granul

Scleritis is typically a non-infectious granulomatous inflammatory process. It may be found in conjunction with systemic disease, more commonly in those patients with necrotising anterior scleritis. We describe a patient with posterior scleritis and polymyalgia rhematica. This association has not previously been reported.

BACKGROUND: Scleritis can be a destructive disease frequently associated with autoimmune disorders. It is believed that primary vasculitis plays an important role in its pathogenesis, but little is known about the cellular effector mechanisms. The purpose of this study was to analyze the inflammatory cellular infiltrate in scleritis.
PATIENTS AND METHODS: Two enucleated eyes were studied. In one patient, enucleation was done after perforation occurred in anterior necrotizing scleritis and, in the other, after the misdiagnosis of posterior scleritis as an intraocular tumor. Morphological crite

A 36-year-old white man abruptly developed painless visual loss in his left eye. Fundoscopic evaluation revealed a shallow left superotemporal retinal detachment with an underlying yellowish choroidal infiltrate. Fluorescein angiography disclosed indistinct choroidal leakage, and both B-scan ultrasonography and computed tomography demonstrated superotemporal thickening of the choroid and the sclera. The patient was diagnosed clinically as having a scleritis and an associated exudative retinal detachment. He was placed on a several months' course of systemic prednisone, and despite this regimen
