Shoulder giant schwannoma - a diagnosis to be considered in painless shoulder masses.

Tumours formed from peripheral nerve sheaths are rare in the shoulder. We present the case of a 80-year-old female referred to an orthopaedic surgery clinic because of a large painless mass of the right shoulder, with one year of evolution. There was no history of trauma, involvement of any other joint or systemic complaints. Given the clinical and imagiological suspicion of a giant synovial cyst, the patient underwent complete surgical resection of the mass, which took place without any complications or functional impairment. Histological diagnosis - schwannoma - was unexpected given the lesi

The effectiveness of shoulder stretching and joint mobilizations on posterior shoulder tightness.

CLINICAL SCENARIO: It has been suggested that posterior shoulder tightness is a common contributor to shoulder impingement in overhead-throwing athletes. The incidence of shoulder pain in the general population has been reported to be as high as 27%, and as many as 74% of the patients who were seen for shoulder issues had signs of impingement. Particularly regarding physically active adults, shoulder impingement is frequent among overhead-throwing athletes and may lead to lost participation in sport, as well as other injuries including labral pathologies. Therefore, finding an effective mechan

Shoulder arthroscopy: its role in evaluating shoulder disorders in the athlete.

We retrospectively reviewed 100 initial shoulder arthroscopies from a general sports medicine orthopaedic practice to determine if arthroscopy is helpful diagnostically and to see if specific lesions correlate well with specific diagnoses. Seventy-two percent of the patients in the review had glenoid labral tears. Ninety-two percent of patients who had a diagnosis of either recurrent anterior dislocation or recurrent anterior subluxation demonstrated a tear of the anterior inferior margin of the glenoid labrum. Sixty-eight percent of patients with a diagnosis of impingement demonstrated tears

Posterior shoulder instability.

Posterior shoulder instability (PSI) is defined by dynamic, recurrent and symptomatic partial or total loss of posterior joint contact. Anatomic risk factors comprise ligament hyperlaxity, glenoid retroversion or dysplasia, and high horizontal acromial morphology. Associated anatomic lesions comprise labrum lesions, posterior glenoid erosion and/or fracture, and anterior humeral head notching. We distinguish two subcategories of PSI: functional and structural, respectively without and with anatomic lesions. In both categories, there may be anatomic risk factors. Clinically, functional PSI invo

Milwaukee Shoulder Syndrome.

Milwaukee shoulder syndrome (MSS) is a rare, rapidly destructive arthropathy associated with calcium hydroxyapatite crystal deposition. This condition is a combination of rotator cuff tear, osteoarthritic changes, noninflammatory joint effusion containing crystals, synovial hyperplasia, cartilage and subchondral bone destruction, and multiple osteochondral loose bodies. This article discusses pathophysiology, clinical presentation, differential considerations, and magnetic resonance imaging findings of MSS.

Shoulder outcomes measures.

General health as well as disease- or condition-specific outcome measures have long been used to assess patients with shoulder conditions. Currently, a variety of validated measures is available. Shoulder outcomes measures may be general (eg, American Shoulder and Elbow Surgeons; Constant; Disabilities of the Arm, Shoulder, and Hand), disease-specific (eg, Rotator Cuff Quality of Life, Western Ontario Rotator Cuff Index), or condition-specific (eg, Oxford Shoulder Instability Questionnaire). The results of shoulder arthroplasty and arthritis treatment can be assessed with the Hospital for Spec

Shoulder girdle lipomatosis.

Shoulder girdle lipomatosis is an extremely rare condition, with unique clinical and imaging features. We report shoulder girdle lipomatosis in a 46-year-old man who presented with a gradual soft tissue enlargement in his left shoulder. We discuss the magnetic resonance imaging features of this disease and the approach to accurate diagnosis. A review of the clinical and radiological features of shoulder girdle lipomatosis is also presented.

[Unstable shoulder dislocation].

Shoulder dislocations are a frequent event and the severity is often underestimated. A thorough clinical investigation and adapted imaging diagnostics are able to detect age-dependent injury patterns. A biphasic conservative treatment keeping the arm in a neutral to mild external rotation for 3 weeks has proven to be most effective and should be followed by an intensive physiotherapy. Nevertheless, conservative treatment protocols show unacceptable recurrence rates particularly in young men active in sport, therefore, surgical stabilisation is recommended. In the past open procedures were acce

[Shoulder calcifying tendinitis].

Calcifying tendinitis is a frequent shoulder disease but the surgical treatment is still debatable. The authors of this symposium reviewed retrospectively 450 patients treated by arthroscopal excision for calcifying tendinitis. Imaging were used to assess the cuff status in every case. The minimum follow-up was five years except for subscapularis and infraspinatus calcification (minimum two years). At the same time, we led a prospective study evaluating the prevalence of the calcifications on 1276 asymptomatic shoulders. The prevalence of rotator cuff calcification was 7.3%, with a female pred

Little Leaguer's shoulder.

A case of Little Leaguer's shoulder (LLS) in a 12-year-old male is presented. Classically, LLS is an overuse injury affecting adolescent pitchers. The diagnosis is the result of a thorough history, physical examination, and radiographic evaluation. Clinicians unfamiliar with LLS may fail to detect this injury and order a magnetic resonance imaging (MRI) study without radiographs. The objective of this case report is to help radiologists become more familiar with the MRI and radiographic findings of LLS.

Pediatric shoulder trauma.

Pediatric shoulder trauma is relatively uncommon. Injuries requiring surgical intervention are even rarer. However, it is important for the practicing orthopaedic surgeon to differentiate nonoperative injuries from the urgent and potentially operative injuries. Missing such an injury in the pediatric population could be potentially life threatening or lead to long-term disability. Posterior sternoclavicular dislocations should be differentiated from medial clavicular physeal injuries and promptly reduced. Open fractures or neurovascular threatening fractures should be attended to immediately.

Shoulder stiffness: diagnosis.

BACKGROUND: Pain and stiffness of the shoulder is a common complaint, particularly in the 40-80 years age group. There are several causes of painful, stiff shoulders, but the most common cause in the fifth decade of life is idiopathic capsulitis ('frozen shoulder'). OBJECTIVE: This article summarises the functional anatomy of the shoulder joint, the pathology of the conditions that lead to shoulder stiffness, and methods to differentiate them. DISCUSSION: Clinical history and examination is effective in differentiating rotator cuff tears, impingement and frozen shoulder. Restriction of gleno

Acute shoulder injuries.

The shoulder is the most mobile joint in the human body. The cost of such versatility is an increased risk of injury. It is important that family physicians understand the anatomy of the shoulder, mechanisms of injury, typical physical and radiologic findings, approach to management of injuries, and indications for referral. Clavicle fractures are among the most common acute shoulder injuries, and more than 80 percent of them can be managed conservatively. Humeral head fractures are less common and usually occur in elderly persons; 85 percent of them can be managed nonoperatively. Common acute

Shoulder stiffness: management.

BACKGROUND: There are several causes of painful, stiff shoulders--each with a different method of treatment. OBJECTIVE: This article summarises the methods available to treat the various causes of stiff shoulders. DISCUSSION: Impingement and partial thickness rotator cuff tears can be treated by the general practitioner with a subacromial steroid injection. Calcific tendinitis can be aspirated under ultrasound guidance when the deposit is immature. Acute full thickness tears are best managed surgically. We recommend arthroscopic capsular release with appropriate postoperative rehabilitation

Fetal shoulder dystocia.

Shoulder dystocia is an unpredictable obstetric complication with the incidence of 0.15% to 2%. An increase in the incidence of shoulder dystocia has been recorded over the last 20 years, probably just because it has now been regularly registered at maternity wards as an obstetric complication. The risk factors for shoulder dystocia include fetal macrosomia, fetal malformations and tumors, maternal adiposity, excessive weight gain during pregnancy, diabetes mellitus, pathologic pelvis, multiparity, short maternal stature, advanced maternal age, postterm pregnancy, so-called midforceps delivery

Beside electro-thermic procedures (ETACS, LACS) several suture-anchor system for labrum fixation have been developed in the last years. From 4/96 to 10/00 we performed 242 arthroscopic shoulder stabilizations with FASTak-(n = 159) Panalok-(n = 26) and Suretac suture anchors (n = 57) in our clinic. The patients were re-examined with a follow-up of at least 12 months. The best results were in the FASTak-group. After 2 years 4.7% suffered a redislocation. 28.6% (2 patients) needed a revision. The Rowe score was 83.1 +/- 20.9 points. There was a high satisfaction of the patients with the operative
