Complications related to wear-induced osteolysis, such as glenoid component loosening in anatomic total shoulder arthroplasty and scapular notching in reverse total shoulder arthroplasty continue to impact clinical outcomes following shoulder replacement. This review explores the evolution of prosthesis design, and how advancements in bearing surface technology may influence implant longevity and reduce complications related to wear, osteolysis, and loosening.

Resurfacing is a type of shoulder arthroplasty that involves replacing the humeral joint surface with a metal covering, or cap, thus preserving the bone of the proximal part of the humerus. If the glenoid is also replaced, a current conventional polyethylene glenoid replacement prosthesis or an interposed soft-tissue graft is used. The potential advantages of humeral resurfacing, as compared with conventional shoulder arthroplasty, are: (1) no osteotomy is performed (and thus the head-shaft angle does not have to be addressed); (2) minimal bone resection; (3) a short operative time; (4) a low

BACKGROUND: Shoulder function often is limited after tumor resection and endoprosthetic replacement of the proximal humerus. This is partly attributable to the inability to reliably reattach rotator cuff tendons to the prosthesis and achieve adequate shoulder capsule repair with a metallic prosthesis. An option to attain these goals is to use synthetic mesh for the reconstruction, although the value of this method has not been well documented in the literature.
QUESTIONS/PURPOSES: We asked whether patients who had shoulder reconstruction using synthetic mesh had (1) better shoulder function;

BACKGROUND: Late complications after humeral head replacement (HHR) for comminuted proximal humeral fractures are common and may necessitate revision surgery. This study evaluated the outcome of revision surgery of failed post-traumatic HHR with a less medialized reverse shoulder prosthesis.
METHODS: Thirty-three patients with failed post-traumatic HHR due to rotator-cuff insufficiency and glenoid erosion, but with sufficient preservation of the glenoid bone stock to permit primary stability of an inverted glenoid implant, underwent revision using the Bayley-Walker reverse shoulder prosthesis

BACKGROUND: Deep periprosthetic infection is a serious complication after total shoulder arthroplasty (TSA) and is associated with suboptimal clinical results. This retrospective study aimed to determine whether a difference in infection rate existed after primary anatomic TSA (aTSA) and primary reverse TSA (rTSA).
METHODS: A TSA database was retrospectively reviewed to identify primary TSAs performed from 2004 to 2012 and isolate cases of confirmed deep periprosthetic infection. Data collected included age, sex, preoperative diagnosis, previous nonarthroplasty operative history, implant type

Proximal humeral fractures occurring in elderly patients often lead to significant functional disability. The outcome of nonoperative management is disappointing. Treatment with open reduction and internal fixation may result in osteonecrosis of the humeral head, and pain. Hemiarthroplasty is often associated with tuberosity nonunion and poor function. Reverse shoulder arthroplasty is an attractive alternative because of the ability of the prosthesis to compensate for tuberosity complications. Early studies have shown promise in using the prosthesis to treat these difficult patients. The next

Reverse shoulder arthroplasty is an ideal treatment for glenohumeral dysfunction due to cuff tear arthropathy. As the number of patients treated with reverse shoulder arthroplasty is increasing, the incidence of complications after this procedure also is increasing. The rate of complications in reverse shoulder arthroplasty was reported to be 15%-24%. Recently, the following complications have been reported in order of frequency: periprosthetic infection, dislocation, periprosthetic fracture, neurologic injury, scapular notching, acromion or scapular spine fracture, and aseptic loosening of pr

The purpose of this prospective study was to determine the early subjective and functional result and outcome after primary implantation of humeral endoprosthesis in severely comminuted humeral head fractures in elderly patients. From 1993 to 1995, 27 humeral head fractures (27 patients) were treated by primary implantation of a Neer II modular shoulder prosthesis. Of these, 23 patients were women, 4 patients were men. The average age of patients was 76 (48-91) years. Postoperatively, one shoulder had to be mobilized under general anesthesia. Of the 27 patients 21 could be clinically examined

BACKGROUND: Reverse total shoulder arthroplasty (RSA) has been approved since 2014 in Japan, and the number of RSA cases has been accumulating. However, only short-to medium-term outcomes have been reported, with a small number of case series, because of its short history in Japan. This study aimed to evaluate complications after RSA in hospitals affiliated with our institute, with comparison to those in other countries.
METHODS: A multicenter retrospective study was performed at 6 hospitals. In total, 615 shoulders (mean age: 75.7 ± 6.2 years; mean follow-up: 45.2 ± 19.6 months) with at leas

Reverse total shoulder arthroplasty (RTSA) is increasingly used for a variety of complex shoulder abnormalities, particularly in patients with rotator cuff deficiency. Design improvements with better biomechanics have led to improved clinical outcomes, but the radiographic appearance and complication profile of contemporary RTSA implants differ from those of a decade ago. Plain radiographs can be used to identify whether the prosthesis is medialized (primarily older prostheses) or lateralized. A standardized imaging protocol and a reporting checklist are vital for the identification of complic

PURPOSE: Patients who sustain traumatic amputation of multiple fingers suffer both a functional and psychologic loss. Previous studies of prosthesis use for finger amputees have focused primarily on the psychologic benefits. Clinically our group noticed a functional improvement on hand function tests when patients with multiple digit amputations used a prosthesis. Given the expense of multiple finger prostheses we sought to determine if they led to a consistent functional improvement in these patients.
METHODS: Ten consecutive patients performed a battery of hand function tests and rated thei

The incidence of periprosthetic humerus fracture associated with shoulder arthroplasty is approximately 0.6% to 3%. Fractures of the humerus occur most often intraoperatively and are more common during total shoulder arthroplasty than hemiarthroplasty because of difficulties in gaining access to the glenoid. Osteopenia, advanced age, female sex, and rheumatoid arthritis are medical comorbid factors that may contribute to humerus fractures and associated delayed healing and poorer function. When the humeral prosthetic component is loose or the fracture line overlaps the majority of the length o

BACKGROUND: Rotator cuff tears following anatomic total shoulder arthroplasty increase with duration of follow-up. This study aimed to evaluate contact pressure between the rotator cuff tendons and prosthesis after anatomic total shoulder arthroplasty and compare these with the tendon-contact pressures in the native shoulder.
METHODS: Eight entire upper extremities were mounted onto a testing apparatus, and simulated muscle loading was applied to each rotator cuff tendon with the shoulder positioned in abduction, internal rotation, and external rotation. Pressure-sensitive film placed between

BACKGROUND: The Delta III total shoulder replacement is a reversed, semi-constrained prosthesis and is recommended for the management of rotator cuff arthropathy.
METHODS: We performed a retrospective mid-term review of the Delta shoulder replacements performed during 2000-2005 with a minimum 18-month follow-up. We recorded the pre-operative and post-operative constant scores.
RESULTS: The Constant's score improved from a pre-operative mean of 14.8 (95% CI +8.8 to +19.8) to a post-operative mean of 60.9 (95% CI +50 to +66.2) with a P valve of <0.0001.
CONCLUSION: Delta III prosthesis restor

For selected patients with flail upper extremities after brachial plexus injury, glenohumeral arthrodesis, above-the-elbow amputation, and fitting of an above-the-elbow prosthesis are effective reconstructive options. In such situations, soft-tissue thinning at the shoulder, especially deltoid atrophy, presents the potential problem of shoulder fusion hardware causing pain or even eroding overlying soft tissue. The authors have used a pedicled forearm musculofasciocutaneous flap, elevated just before above-the-elbow amputation and subsequently transposed to the shoulder, to provide high-qualit

We operatively treated, between 1978 and 1987, twenty-one shoulders in nineteen patients, fifty-four to eighty-four years old, who had disabling pain attributable to a massive tear of the rotator cuff, accompanied by loss of the surface of the glenohumeral joint. These patients were not candidates for total shoulder replacement because of the massive deficiency in the cuff and the fixed upward displacement of the humeral head. A prerequisite for hemiarthroplasty was a functionally intact coracoacromial arch to provide superior secondary stability for the prosthesis. One important aspect of the

Scapula and proximal humerus are the most frequent site of primary bone tumors of upper limb. Surgical reconstruction after resection, aimed to obtain a stable painful limb and active motion of the shoulder. Three major key-points can affect functional results: intra- or extra-articular resection, ability to offer a strength fixation of rotator cuff tendons, and remaining function of the deltoid muscle after resection. When most of the deltoid muscle is active associated with an intra-articular resection, recent results of reversed prosthesis are very promising in terms of active motion. For o
