Pediatric immune-mediated necrotizing myopathy.

Immune-mediated necrotizing myopathy (IMNM) is a type of inflammatory myopathy. Most patients with IMNM produce anti-3-hydroxy-3-methylglutaryl coenzyme A reductase or anti-signal-recognition particle autoantibodies. IMNM is much rarer in children than in adults. We conducted this mini review focusing on pediatric IMNM to present current evidence regarding its epidemiology, clinical characteristics, diagnosis, and treatment. Our findings indicate that pediatric IMNM often causes severe muscle weakness and is refractory to corticosteroids alone. Furthermore, delayed diagnosis is common because

Myositis autoantibodies.

PURPOSE OF REVIEW: To review recent advances in our understanding of autoantibodies associated with dermatomyositis and the autoimmune necrotizing myopathies. RECENT FINDINGS: Autoantibodies preferentially associated with dermatomyositis include those recognizing Mi-2, MDA5, TIF1γ, and NXP-2. Each of these is associated with a distinct clinical phenotype. Autoantibodies found in patients with autoimmune necrotizing myopathies recognize signal recognition particle and 3-hydroxy-3-methylglutaryl-coenzime A reductase (HMG-CoA) reductase. The latter are found in patients with statin-associated au

Characterisation of innate fungal recognition in the lung.

The innate recognition of fungi by leukocytes is mediated by pattern recognition receptors (PRR), such as Dectin-1, and is thought to occur at the cell surface triggering intracellular signalling cascades which lead to the induction of protective host responses. In the lung, this recognition is aided by surfactant which also serves to maintain the balance between inflammation and pulmonary function, although the underlying mechanisms are unknown. Here we have explored pulmonary innate recognition of a variety of fungal particles, including zymosan, Candida albicans and Aspergillus fumigatus, a

GTPase cycle and ER membrane regulate cargo handover during cotranslational protein targeting.

Signal recognition particle (SRP), a universally conserved protein-targeting machine, cotranslationally recognizes signal sequences of nascent endomembrane proteins and engages the SRP receptor (SR) at the endoplasmic reticulum (ER) membrane to unload the translating ribosome. The mechanism of membrane-associated steps in this pathway remains unclear. Here, we address this question using pre-steady-state and single-molecule fluorescence measurements. SR induces release of the signal sequence from SRP and detachment of the SRP guanosine triphosphatase (GTPase) domain from the ribosome. Despite

Anti-3-hydroxy-3-methylglutaryl-coenzyme a reductase necrotizing myopathy masquerading as a muscular dystrophy in a child.

INTRODUCTION: Immune-mediated necrotizing myopathies (IMNMs) are characterized by progressive weakness, elevated serum creatine kinase levels, and necrotizing myopathic features on muscle biopsy. Presence of highly specific autoantibodies against signal recognition particle (SRP) or 3-hydroxy-3-methylglutaryl- coenzyme A reductase (HMGCR) can aid in recognition and confirmation of IMNMs. METHODS: In this study we describe a boy with HMGCR-positive necrotizing myopathy and highlight the clinical features of the patient. RESULTS: In contrast to most adults, the patient described had a more ind

Headspace Particulate Measurements of Low Vapor Pressure Inorganic Oxidizers for Detection Canine Disciplines.

Fuel-oxidizer mixtures are popular energetics in homemade explosive devices composed of an oxidizer and a fuel. To date, detection canines remain one of the most successful tools for detecting the illicit usage of these materials. However, some popular inorganic oxidizers, such as potassium chlorate, have little to no anticipated vapors due to their negligible vapor pressures. There is subsequently an ongoing question in the detection canine discipline regarding what chemical(s) are enabling odor recognition. The goal of this study is to help improve our understanding of the chemical signature

Tonsillectomy Improved Therapeutic Response in Anti-SRP Myopathy With Chronic Tonsillitis.

Chronic tonsillitis has been attracted attention as a source of abnormal immune responses and a possible trigger of autoimmune diseases such as IgA nephritis, IgA vasculitis, palmoplantar pustulosis, psoriasis, rheumatoid arthritis, Behçet's disease, and myositis. Here we present the first report of anti-signal recognition particle antibody-associated necrotizing myopathy (anti-SRP myopathy) with IgA nephropathy and chronic tonsillitis in which the therapeutic response to intravenous immunoglobulin (IVIG) treatment was dramatically improved after tonsillectomy and accompanied by a rapid increa

Difficult case: rituximab in anti-SRP antibody myositis in pregnancy.

A 30-year-old nulliparous woman presented at 15-week gestation with severe skeletal and respiratory muscle weakness, having been diagnosed with anti-signal recognition particle antibody myositis 3 years before. Remission had previously been induced with rituximab (after failure of standard therapies). She had continued oral prednisolone and rituximab every 6 months but had stopped this when planning pregnancy. At 16-weeks gestation, she restarted corticosteroids and rituximab, with clinical and biochemical recovery and no complications. Rituximab should ideally be given in the first trimester;

APOBEC3G encapsidation into HIV-1 virions: which RNA is it?

APOBEC3G is a cytidine deaminase with potent antiviral activity. The protein deaminates single-stranded DNA but is known to bind cellular and viral RNAs. There is increasing evidence that RNA binding of APOBEC3G is important for packaging into viral particles. However, there is no consensus yet on the type of RNA involved.

Loose Particle Material Identification for Sealed Electronic Devices Using Pulse Endpoint Detection and CEEMDAN Feature Optimization.

Loose particles inside aerospace-sealed electronics cause circuit short circuits and contact faults. Particle Impact Noise Detection (PIND) relies on piezoelectric acoustic emission (AE) sensors to capture collision pulses, yet raw sensor signals are heavily contaminated by background noise, leading to severe time-frequency feature aliasing and low particle material recognition accuracy. This work proposes a sensing signal optimization method combining pulse endpoint detection and Complete Ensemble Empirical Mode Decomposition with Adaptive Noise (CEEMDAN) decomposition for PIND acoustic-sensi

A Flexible Hydrogel-Based Dual-Modal Sensor System for Intelligent Mechanical Gripping and Wearable Gesture Recognition.

The triboelectric nanogenerator (TENG) enables efficient mechanical-to-electrical energy conversion, offering green self-powered solutions for flexible electronics and microdevices. However, many TENG-based flexible sensors only support single-signal acquisition, failing to satisfy multimodal sensing requirements. In this work, a flexible sensor with a dual-sensing mechanism is developed, employing PVA/CS hydrogel as the triboelectric layer and PVA/CS/GL hydrogel as the electrode layer. By using TiO2-incorporated silicone rubber as the counter triboelectric layer, the output performance of the

[Clinical features and therapeutic response of our anti-SRP positive patients with myositis].

INTRODUCTION: Idiopathic inflammatory myopathies are a group of clinically heterogeneous diseases, which have been classified by myositis specific antibodies recently. The anti-SRP positive subset of this group is characterized by more severe clinical prognosis than other myositis specific antibody positive types. AIM: Our goal was to compare 16 anti-SRP positive patients in the Division of Clinical Immunology, Department of Internal Medicine, University of Debrecen with 16 antibody negative ones. METHOD: Muscle strength validated in both groups by the manual muscle test proved to be signifi

Identification of the SARS-CoV-2 genome packaging signal in the nsp12-coding region.

Selective genome packaging is a critical step for RNA viruses, which must distinguish genomic RNA from other abundant transcripts. For SARS-CoV-2, the cis-acting packaging signal is thought to be recognized by the nucleocapsid (N) protein, but its identity and mechanistic basis for selective recognition remain undefined. Here we identify the packaging signal within the nsp12 polymerase-coding region. CLIP-seq maps N-bound sites and, together with virus-like particle assays, pinpoints a conserved segment with strong packaging activity. An orthogonal defective-interfering RNA approach confirms i

SRP orchestrates protein biogenesis beyond initial ER membrane targeting.

The Signal Recognition Particle (SRP) targets nascent proteins to the Sec61 translocon for import into the endoplasmic reticulum (ER). However, its range of substrates, point of engagement during targeting, and hence full biological impact remain unclear. Here, we examined SRP interactions with the nascent proteome of S. cerevisiae during translation and membrane targeting. SRP binds effectively to transmembrane domains (TMDs) as they emerge from the ribosomal tunnel, but only to a minority of cleavable signal peptides. We identify nascent chain features that promote SRP binding, allowing to d

Overlap of Sjögren's Syndrome and Immune-Mediated Necrotizing Myopathy With Interstitial Lung Disease: A Case Report.

Immune-mediated necrotizing myopathy (IMNM), an inflammatory muscle disease, typically presents as severe muscle weakness due to immunologic mechanisms. Some cases also show cutaneous manifestations, interstitial lung disease, and sicca symptoms. In this report, we present a unique case of an elderly man with a history of mild and stable Sjögren's syndrome (SS) for over 10 years, who later developed interstitial lung disease and myositis, leading to a diagnosis of IMNM confirmed by the presence of anti-signal recognition particle antibodies. The coexistence of IMNM and SS is a rare occurrence,

Paraneoplastic Anti-3-hydroxy-3-methylglutary-coenzyme A Reductase Antibody-positive Immune-mediated Necrotizing Myopathy in a Patient with Uterine Cancer.

We report the case of a 69-year-old woman with proximal limb muscle weakness, who received post-operative chemotherapy for uterine cancer. Her serum creatinine kinase level was high (10,779 mg/dL) and a muscle biopsy from her left biceps revealed various sizes of muscle fibers accompanied by necrotic and regenerating fibers. She was positive for anti-3 hydroxy-3-methylglutary-coenzyme A reductase (anti-HMGCR) antibodies, but negative for anti-signal recognition particle (anti-SRP) antibodies. She was diagnosed with immune-mediated necrotizing myopathy (IMNM) and treated with prednisolone. Our

A Rare Association of Anti-SRP Antibody-Positive Immune-Mediated Necrotizing Myopathy and Sjögren Disease: Two Case Reports.

INTRODUCTION/AIMS: Immune-mediated necrotizing myopathy (IMNM) is an autoimmune myositis which is characterized by severe muscle fiber necrosis and refractoriness to immunotherapies. Overlap between myositis and other autoimmune disorders is well-recognized; however, the coexistence of IMNM and Sjögren disease (SjD) is rare. We investigated the clinicopathological features of anti-signal recognition particle antibody-positive IMNM associated with SjD. METHODS: We retrospectively reviewed previously reported cases and analyzed the clinical and pathological features of two patients: a 68-year-o
