Obstructive sleep apnea syndrome in children.

Obstructive sleep apnea syndrome (OSAS) in children is often caused by obstruction of the upper airway due to hypertrophy of the adenoids and palatine tonsils. Between October 1988 and December 1991, 50 children (34 males, 16 females) visited our department due to attacks of sleep apnea and underwent adenotomy or adeno-tonsillectomy. Respiratory monitoring during sleep was performed before and after operation, and the usefulness of the surgery was evaluated. Before operation, 27/ 50 children (54.0%) were diagnosed as having OSAS. Their age distribution showed peaks at the ages of 4 and 5 years

[Imaging in obstructive sleep apnea syndrome].

Obstructive sleep apneas result from pharyngeal collapse during sleep. Upper airways may be investigated using either static or dynamic techniques during wakefulness or sleep. In OSAS patients, imaging has different goals: to describe the anatomy of the pharynx and the sequence of the pharyngeal collapse, to try to select patients that will benefit from surgery, and to analyze the results of different treatments.

Papilledema and obstructive sleep apnea syndrome.

OBJECTIVES: To characterize the pathogenesis and clinical features of optic disc edema associated with obstructive sleep apnea syndrome (SAS). METHODS: A series of 4 patients with SAS and papilledema (PE) underwent complete neuro-ophthalmologic evaluation and lumbar puncture. In 1 patient, continuous 24-hour intracranial pressure (ICP) monitoring was also performed. RESULTS: All 4 patients had bilateral PE that was asymmetric in 2. Three patients had optic nerve dysfunction, asymmetric in 1, unilateral in 2. Daytime cerebrospinal fluid pressure measurements were within normal range. Nocturna

Clinical predictors of obstructive sleep apnea.

OBJECTIVE: To identify physical findings that can be standardized to predict the presence and the severity of obstructive sleep apnea (OSA). STUDY DESIGN: One hundred seventy-two patients who answered questionnaires with responses that suggested they might have OSA were included in this prospective study. METHODS: All patients underwent a physical examination and polysomnography. The physical examination included the measurement of four parameters used by anesthesiologists to identify patients likely to have difficult intubation to determine if these same parameters predict OSA. We recorded

Snoring, obstructive sleep apnea, and surgery.

Snoring and OSA syndrome are prevalent and important causes of sleep disturbance. Snoring, historically considered to be only a habitual annoyance, has significant physical and social consequences. OSA is now considered to be a major public health concern with significant morbidity and mortality. CPAP is considered the treatment of choice for OSA syndrome, but poor patient acceptance and compliance remain problematic. Surgical procedures have been developed to alter the offending anatomic abnormalities responsible for OSA. Identification of the offending anatomic site with application of the m

The nose and obstructive sleep apnea.

The role of the nasal passage in the pathophysiology of obstructive sleep apnea (OSA) is still controversial. To this end, induction of acute nasal obstruction in healthy volunteers is associated with sleep fragmentation and an increase in the number of obstructive and central apneas. Moreover, nasal topical anesthesia, which alters nasal reflexes, is associated with an increase in the number of obstructive and central apneas during sleep. Although nasal resistance is increased in patients with OSA, chronic nasal obstruction appears to play no significant role in the genesis, maintenance, or s

Pediatric adenotonsillectomy for obstructive sleep apnea.

Forty-four children who underwent adenotonsillectomy for obstructive sleep apnea (OSA) were studied. The diagnosis was confirmed polysomnographically. Patients with other medical problems or complications of OSA were excluded. The overall complication rate was 32%. Significant airway complications occurred in 16%. Factors associated with development of statistically significant airway complications were acute airway compromise, age < 3 years, thin body habitus, and both oxygen (O2) desaturation and carbon dioxide (CO2) retention seen polysomnographically. Although many OSA patients can safely

Cephalometric assessment in obstructive sleep apnea.

It is reported that some specific craniofacial characteristics are associated with obstructive sleep apnea syndrome (OSAS). To test this finding, the present study developed and assessed the feasibility of a craniofacial index score (CIS) in differentiating patients with OSAS from habitual snorers. Anthropometric measurements and lateral head radiographs were obtained on 24 male and 4 female patients with OSAS who had physician-diagnosed OSAS (respiratory disturbance index (RDI) >20), and 25 male and 5 female habitual snorers (RDI <20). Thirteen cephalometric and four anthropometric measure- m

Obstructive sleep apnea-induced supraventricular tachycardia.

Obstructive sleep apnea has been associated with various cardiac arrhythmias; however, supraventricular tachycardia has not been reported to occur in this disorder. This case report describes a patient who developed episodes of supraventricular tachycardia during periods of apnea and oxygen desaturation. With the initiation of nasal continuous positive airway pressure during sleep, the arrhythmia was abolished. The etiology and possible mechanisms responsible for the supraventricular tachycardia are discussed.

Obstructive sleep apnea and related disorders.

OSAS, a common cause of disrupted sleep and EDS, result from repetitive closure of the upper airway during sleep. It probably represents the most severe syndrome related to obstruction of the upper airway; less severe forms include UARS, a syndrome characterized by the need for increased effort to breath but no prominent apneas or hypopneas, and primary snoring. Initial clues to the presence of OSAS and related disorders are derived from the history and include loud snoring, EDS or insomnia, and witnessed apneas. Some patients, especially women, may complain mostly of tiredness or fatigue, and

Pharyngeal fat in obstructive sleep apnea.

Although most patients with obstructive sleep apnea (OSA) are obese, it is not known how obesity contributes to airway collapse during sleep. The purpose of this study was to determine whether the volume of adipose tissue adjacent to the pharyngeal airway in humans is related to the degree of OSA. We studied 30 subjects, nine without OSA and 21 with OSA; two subjects were studied before and after weight loss. Adipose tissue was detected with magnetic resonance imaging using T1-weighted spin echo sequences. The volume of adipose tissue adjacent to the upper airway was determined by measuring th

Complications of pediatric obstructive sleep apnea.

Obstructive apnea, especially OSA, is a common problem in pediatric practice. In young infants, craniofacial anomalies may be the most common contributor to OSA. Complications include failure to thrive, metabolic alkalosis, and respiratory distress which can be life-threatening. In children over the age of 2 years, adenoidal and tonsillar hypertrophy is more commonly the cause of upper airway obstruction. The complications of growth failure, cor pulmonale, and adult respiratory distress syndrome were discussed. Obstructive sleep apnea can cause significant complications in both infants and chi

Cardiopulmonary consequences of obstructive sleep apnea.

During sleep, oxygen consumption and systemic blood pressure decrease in normal subjects; during rapid eye movement sleep, irregular ventilation can be accompanied by brief periods of apnea. In patients with obstructive sleep apnea, alveolar ventilation during an apneic episode is immediately reduced to zero, and the metabolic demands for oxygen must be met from oxygen stores within the body. As the stores of oxygen within the lung are diminished, the rate of arterial oxyhemoglobin desaturation increases. The development of alveolar hypoventilation during wakefulness seems to be based on a bal

Cine CT in obstructive sleep apnea.

The upper airway was evaluated in eight patients with obstructive sleep apnea by using a rapid sequential CT scanner (Imatron C-100). Four patients also had simultaneous polysomnograms to determine the onset of sleep and apnea. The upper airway was scanned while the patient was awake (eight patients), asleep (four patients), and asleep and apneic (eight patients). Measurements of the cross-sectional area of the upper airway were correlated with the findings on sleep studies in four patients. During the awake state the airway was narrowed and showed increased collapsibility in all eight patient

Medical therapy of obstructive sleep apnea.

Guidelines for the medical therapy of obstructive sleep apnea are difficult to define precisely. While some elegant investigations have been completed, most study populations have been small. Also, the long-term effects of most forms of therapy are not known. Some patients will respond to a given form of therapy or combination of therapies while others will not. In most instances the responders cannot be recognized prior to the institution of therapy and a cycle of trial and error ensues. One of the best nonsurgical approaches appears to be weight loss, albeit unsuccessful in most cases. Almos

Nonhypersomnolent patients with obstructive sleep apnea.

Until recently, snoring had been considered both a medical enigma and a psychosocial problem. Snoring is now considered to be an acoustic phenomenon produced by vibration of the soft palate and the tonsillar pillars. We describe 20 patients with a clinical complaint of excessive snoring who were referred to rule out obstructive sleep apnea. All patients were without symptoms of daytime sleepiness and failure of the right heart. Twenty subjects were studied, 18 of whom were males. All subjects were monitored for one full night in the Sleep Laboratory. The apnea rate ranged from 9.0 to 94.0 inci

Obstructive sleep apnea risk factors and symptoms in children with a known parental obstructive sleep apnea diagnosis.

OBJECTIVE: To document symptoms and risk factors of obstructive sleep apnea (OSA) in children who have a parent diagnosed with OSA and compare them to an age and sex matched sample where parents are low risk for OSA. METHODS: We recruited 25 children with a parent diagnosed with OSA (P-OSA) and 29 age and gender matched children from the community whose parents scored low risk for OSA (P-NOSA). Comparisons were made using the OSA-18 questionnaire, anthropometric measurements, and mallampati score. Statistical analysis included t-tests for OSA-18 score and BMI measures and non parametric analy

The sleep EEG spectral profile of narcolepsy with comorbid obstructive sleep apnea more closely resembles narcolepsy than obstructive sleep apnea.

BACKGROUND: Narcolepsy (NT) and obstructive sleep apnea (OSA) frequently coexist, yet the sleep electroencephalographic (EEG) spectral profile of narcolepsy with comorbid OSA (NT-OSA) remains unclear. We compared spectral power among NT-OSA, NT, and OSA across polysomnography (PSG) and Multiple Sleep Latency Test (MSLT). METHODS: We retrospectively analyzed 224 treatment-naïve adults with excessive daytime sleepiness (37 NT-OSA, 77 NT, and 110 OSA); 39 subjectively sleepy controls (SSC) were included in a secondary analysis. Relative EEG spectral power was calculated in central and occipital
