The function of 'brain waves': a cybernetic model of electroencephalography.

Electroencephalograms recorded from sleeping subjects are reinterpreted, from first principles, to explain the relationship between the observed wave-forms and cerebral processes. The manifestations of narcolepsy, namely, sleep paralysis, hypnopompic hallucinations and cataplexy are consistent with a hypothesis that the disorder is the result of entrainment of cerebral waves, causing almost instantaneous descent into a REM sleep state.

REM alpha rhythm: diagnostic for narcolepsy?

A young man, in whom narcolepsy was subsequently diagnosed, had the simultaneous onset of quadriparesis and a rapid eye movement (REM)-sleep polysomnographic pattern. During this REM-sleep pattern, a waking alpha EEG rhythm, appearing when he was asked to close his eyes, immediately attenuated when he was instructed to open his eyes, after which the REM-sleep pattern persisted. The juxtaposition of REM sleep patterns and reactive alpha rhythms are likely unique to sleep paralysis and may prove valuable in diagnosing narcolepsy.

Unique sleep disorders profile of a population-based sample of 747 Hmong immigrants in Wisconsin.

Concerns regarding sleep disorders in Hmong immigrants in the US emerged when an astonishingly high mortality rate of Sudden Unexplained Nocturnal Death Syndrome (SUNDS) was documented in Hmong men. Stress, genetics, and cardiac abnormalities interacting with disordered sleep were hypothesized as contributing factors to SUNDS. Most recently, sleep apnea has been implicated in nighttime deaths of Brugada Syndrome. This syndrome is thought to comprise a spectrum of sudden cardiac death disorders, including SUNDS. However, little research since has placed SUNDS in its context of Hmong cultural be

Narcolepsy. Diagnosis, treatment, and management.

Narcolepsy is a syndrome of unknown origin characterized by the irresistible urge to sleep. Other important features are disturbed nocturnal sleep and abnormal manifestations of REM sleep such as cataplexy, sleep paralysis, and abnormal sleep-onset REM periods. Narcolepsy is not a rare condition. With a prevalence between 2 and 10 per 10,000 individuals, it is about as common as multiple sclerosis. Like multiple sclerosis, narcolepsy can be disabling and have profound consequences for job capability, public safety, sense of self-worth, and social image.

[HTLV-I associated myelopathy with bilateral abductor vocal cord paralysis--case report].

We have reported a 50-year-old woman with HTLV-I associated myelopathy (HAM) who had bilateral abductor vocal cord paralysis. The symptoms and signs were slowly progressive spastic paraplegia, dysuria, inspiratory stridor, and snoring during sleep. She had no hoarseness. Titers of anti-HTLV-I antibody were elevated in both the serum and cerebrospinal fluid. FEV1.0% on the spirogram was reduced to 66%. The fiberscopic examination demonstrated the abductor limitation of the vocal cords during the inspiratory phase. During induced sleep after the intravenous administration of thiopental sodium, t

Natural history, symptoms and treatment of the narcoleptic syndrome.

This study describes the clinical features, natural history and treatment of 100 patients with narcolepsy. Over half had one or more affected relatives. Symptoms commenced in adolescence or early adult life in most patients, and remissions were uncommon. Narcolepsy occurred several times each day, often in unusual circumstances and sometimes with little warning. The mean total sleep time of narco-leptics was a little over 9 hours in each 24 hour period, as compared with under 8 in normal subjects. Cataplexy occurred in 93 patients, most commonly when subjects were tired. Attacks were similar i

Frequencies and Associations of Narcolepsy-Related Symptoms: A Cross-Sectional Study.

OBJECTIVES: Narcolepsy is a disabling disease with a delayed diagnosis. At least 3 years before the disorder identification, several comorbidities can be observed in patients with narcolepsy. The early recognition of narcolepsy symptoms may improve long-term prognosis of the patients. Thus, we aimed to investigate the prevalence of the symptoms associated with narcolepsy and its social and psychological association in a sample of Sao Paulo city inhabitants. METHODS: We performed a cross-sectional evaluation with 1,008 individuals from the Sao Paulo Epidemiologic Sleep Study (EPISONO). Excessi

Parasomnias and Sleep-Related Movement Disorders in Older Adults.

Parasomnias and sleep-related movement disorders are important problems in older adults. Sleep paralysis is rare, but may occur in families. In a minority of patients with disorders of arousal, the episodes persist until the age of 70. Zolpidem and other medications may induce sleepwalking and sleep-related eating. Most patients with idiopathic rapid eye movement (REM) sleep behavior disorder eventually develop Parkinson's disease or dementia with Lewy bodies. Anti-IgLON5 disease includes abnormal behaviors in NREM and REM sleep. Restless legs syndrome prevalence increases with age. A severe f

Neurochemical perspectives of the narcoleptic syndrome.

Narcolepsy has been defined as a disorder of excessive sleep often associated with cataplexy, sleep paralysis and hypnagogic hallucinations. Although the pathophysiology of the narcoleptic syndrome is not well understood, derangement in the functions of CNS catecholamines and serotonin (5-HT) have been implicated. In the present paper we summarize evidence to suggest a role for the endogenous opioids in the regulation of normal sleep and in the pathophysiology of the narcoleptic syndrome.

Current pharmacologic management of narcolepsy.

Narcolepsy is an incurable sleep disorder characterized by attacks of sleepiness and a series of auxiliary symptoms: cataplexy, sleep paralysis and hypnagogic hallucinations. Classic treatment has included stimulants to control sleepiness and tricyclic antidepressants to control the auxiliary symptoms. Polysomnography is necessary to confirm the diagnosis and to detect other sleep disorders. Recent developments in treatment include the use of codeine for sleepiness and gamma-hydroxybutyrate for auxiliary symptoms.

[Narcolepsy--new implications of molecular biology].

Narcolepsy is a common but underdiagnosed sleep disorder characterized by excessive daytime sleepiness and abnormal manifestations of REM sleep: cataplexy, sleep paralysis and hypnagogogic hallucinations. Sleep onset REM periods (SOREMPs) are diagnostic. Within the last five years the pathogenesis has become clearer: narcolepsy is associated with almost total absence of the neuropeptides hypocretin-1 and -2 in the hypothalamus and CSF. A low level of CSF hypocretin is now recognized as a new diagnostic tool. This review provides updated knowledge of narcolepsy.

A case of narcolepsy with strictly unilateral cataplexy.

I report a case of a 50-year-old African American woman who presented for a sleep medicine evaluation complaining of excessive daytime sleepiness of greater than 10 years' duration. She reported falling asleep while driving, talking with friends, and eating. Her Epworth Sleepiness Scale score was 21/24. Her history was positive for the presence of sleep paralysis. She denied visual or auditory hallucinations in the peri-sleep period.

[Parasomnias].

Parasomnias are frequent. They usually represent either the exaggeration of a physiological phenomenon (e.g. sleep starts) or a non-disturbing, idiopathic and usually benign sleep disorder (e.g. sleep talking and bruxism), which need only counseling and improvement of sleep hygiene. However, occasionally parasomnias are of clinical relevance. They can cause insomnia or hypersomnia (e.g. 'myoclonus nocturnus'), psychosocial stress (e.g. sleep-related enuresis and sleep walking) and injuries to oneself and others (e.g. REM-parasomnia). Finally, they can be symptomatic of neurological and medical

Narcolepsy in Singapore: is it an elusive disease?

INTRODUCTION: The aims of the study were to determine the demographic, clinical, and polysomnographic characteristics of narcolepsy, and to address the difficulties in diagnosing narcolepsy and cataplexy, which is a cardinal symptom. We also ventured to investigate the differences between narcolepsy with and without cataplexy. MATERIALS AND METHODS: Data were collected retrospectively from patients diagnosed with narcolepsy at the Sleep Disorder Unit of Singapore General Hospital over 5 years. Each patient had had a detailed clinical evaluation and overnight polysomnography (PSG) followed by

[Narcolepsy: a new perspective on diagnosis and treatment].

The 5 classic symptoms of narcolepsy are excessive daytime sleepiness, cataplexy, sleep paralysis, hypnagogic hallucinations and disturbed nocturnal sleep. The presence of cataplexy is strongly associated with a deficiency of the neuropeptide hypocretin. This discovery has led to new diagnostic subclassifications: narcolepsy without cataplexy, which can be demonstrated by a multiple sleep latency test, and narcolepsy with cataplexy, which can be confirmed with a multiple sleep latency test or a cerebrospinal fluid deficiency of hypocretin I. Various treatment options are available, including p

Clinical and sleep laboratory study of narcoleptic symptoms in multiple sclerosis.

Seventy white patients with a diagnosis of MS and typed for their HLA-A, B, C, and DR were studied. A clinical interview and a questionnaire were used to evaluate the presence of narcoleptic symptoms. The prevalence of sleep attacks, cataplexy, and sleep paralysis was significantly elevated among these patients. However, no difference was seen between DR2 and non-DR2 subjects with regard to the incidence of narcoleptic symptoms. Nine DR2 patients complaining of both sleep attacks and cataplexy were studied in the sleep laboratory for five consecutive naps, but no polygraphic evidence of narcol

A Nationwide Cohort Study of Parasomnias Among Adolescents.

Objective: To elucidate the incidence rates and predictive factors for parasomnias (disorders of arousal, nightmare, and sleep paralysis) in adolescents. Methods: This was a prospective cohort study of high school students. In 2010, we conducted a baseline survey of first-year students enrolled in randomly selected Japanese schools (10 junior high schools and 14 senior high schools); 2 years later, a follow-up survey of the same participants was conducted. A self-administered questionnaire inquiring about parasomnias and lifestyles was provided to the students for both surveys. The incidence o

Anxiety and sleep problems: emerging concepts and theoretical treatment implications.

The high prevalence and comorbidity of anxiety and sleep problems, especially insomnia, suggest an important underlying relationship between these disorders. In this article, we highlight two theoretical models explaining this co-occurrence, provide a brief update on the association between anxiety-insomnia and anxiety-cataplexy in general, and review more specifically sleep problems in generalized anxiety, post-traumatic stress disorder, and panic disorder. We also explore sleep paralysis as an anxiety-sleep event. Our goal with this examination of selective anxiety-sleep problems is to provi
