Is albumin administration in the acutely ill associated with increased mortality? Results of the SOAP study.

INTRODUCTION: Albumin administration in the critically ill has been the subject of some controversy. We investigated the use of albumin solutions in European intensive care units (ICUs) and its relationship to outcome. METHODS: In a cohort, multicenter, observational study, all patients admitted to one of the participating ICUs between 1 May and 15 May 2002 were followed up until death, hospital discharge, or for 60 days. Patients were classified according to whether or not they received albumin at any time during their ICU stay. RESULTS: Of 3,147 admitted patients, 354 (11.2%) received albu

Current best practice for the health surveillance of enzyme workers in the soap and detergent industry.

This study defines current best practice for the health surveillance of workers who are potentially exposed to enzymes in the manufacture of enzymatic detergent products. It is recommended that health surveillance is performed 6-monthly for the first 2 years and annually thereafter. The health surveillance programme should include a respiratory questionnaire to detect symptoms, assessment of lung function to detect pre-symptomatic changes and an immunological test to detect specific immunoglobulin E (IgE) to enzymes. The impact of skin washing with water and soap during breast irradiation: a randomized study.

BACKGROUND: The effect of washing the irradiated skin during radiotherapy for breast cancer is uncertain. The purpose of this study was to evaluate the impact of washing the breast skin with water and soap during radiotherapy on the intensity of acute skin toxicity. MATERIALS AND METHODS: Ninety-nine patients treated for breast cancer were prospectively randomized prior to receiving radiotherapy to the breast into two groups: (1), no washing was allowed during radiotherapy (49 patients); and (2), washing was allowed with water and soap (50 patients). Acute toxicity was recorded according to t

Assessment of the teratogenic potential of surfactants. Part III--dermal application of LAS and soap.

The surfactant linear alkylbenzene sulphonate (LAS) was examined for embryotoxic and teratogenic potential following percutaneous administration. Solutions containing, 0.03, 0.3 and 3% LAS were applied to shaved skin during pregnancy days 2-13 in mice, 2-15 in rats and 1-16 in rabbits. Dosages employed were 0.5 ml/rat or mouse/day and 10 ml/rabbit/day. For comparison further groups of rats and mice were similarly treated with concentrations of 0.3, 3 and 30% of a standard soap solution. Skin feel in the brain: Oscillatory dynamics of active touch are modulated by commercial soap products.

Previous research has demonstrated changes in neural oscillations associated with varying levels of roughness during active-touch exploration of surfaces. In the present study, we aimed to investigate changes in neural oscillatory activity and softness perception during touch exploration of skin. Two experiments were conducted. Study 1 evaluated active touch of artificial skin samples mounted to a purpose-built touch sensor, whereas Study 2 investigated active stroking of one's own forearm. In both experiments, the substrates were treated with commercially available bar soaps to deliver either

Exploration of different statistical approaches in the comparison of dopamine and norepinephrine in the treatment of shock: SOAP II.

BACKGROUND: Exploring clinical trial data using alternative methods may enhance original study's findings and provide new insights. The SOAP II trial has been published more than 10 years ago; but there is still some speculation that some patients may benefit from dopamine administration for shock management. We aimed to reanalyse the trial under different approaches and evaluate for heterogeneity in treatment effect (HTE). METHODS: All patients enrolled in SOAP II were eligible for reanalysis. We used a variety of methods including the win-ratio (WR), a Bayesian reanalysis stratified accordi

Soap Suds Enemas Are Efficacious and Safe for Treating Fecal Impaction in Children With Abdominal Pain.

OBJECTIVES: Constipation is a common cause of pediatric abdominal pain and emergency department (ED) presentation. Despite the high prevalence, there is a dearth of clinical information and wide practice variation in childhood constipation management in the ED. The objective of the study was to assess the efficacy and safety of soap suds enema (SSE) in the therapy for fecal impaction in children with abdominal pain within the pediatric ED setting. The primary outcome was stool output following SSE. Secondary outcomes were adverse events, admissions, and return visits within 72 hours. METHODS:

Effect of a behaviour-change intervention on handwashing with soap in India (SuperAmma): a cluster-randomised trial.

BACKGROUND: Diarrhoea and respiratory infections are the two biggest causes of child death globally. Handwashing with soap could substantially reduce diarrhoea and respiratory infections, but prevalence of adequate handwashing is low. We tested whether a scalable village-level intervention based on emotional drivers of behaviour, rather than knowledge, could improve handwashing behaviour in rural India. METHODS: The study was done in Chittoor district in southern Andhra Pradesh, India, between May 24, 2011, and Sept 10, 2012. Eligible villages had a population of 700-2000 people, a state-run

The use of a measure of acute irritation to predict the outcome of repeated usage of hand soap products.

BACKGROUND: Healthcare-associated infection is an important worldwide problem that could be reduced by better hand hygiene practice. However, an increasing number of healthcare workers are experiencing irritant contact dermatitis of the hands as a result of repeated hand washing. This may lead to a reduced level of compliance with regard to hand hygiene. OBJECTIVES: To assess whether a measure of acute irritation by hand soaps could predict the effects of repeated usage over a 2-week period. METHODS: In a double-blind, randomized comparison study, the comparative irritation potential of four

Tailored, interactive soap operas for breast cancer education of high-risk Hispanic women.

BACKGROUND: While Hispanic women have lower rates of breast cancer than do women of other ethnic groups, they are the least likely to undergo screening examinations. This study evaluated a culturally sensitive and linguistically appropriate, tailored, computer-based, educational program for early detection of breast cancer aimed at high-risk Hispanic women. METHODS: Spanish-speaking Hispanic women from an inner-city community health clinic were recruited and randomly assigned either to a computer intervention with an interactive soap-opera format (n = 118) or to a comparison group (n = 60). Failure of bland soap handwash to prevent hand transfer of patient bacteria to urethral catheters.

OBJECTIVE: The study was designed to compare the efficacies of bland soap handwash and isopropyl alcohol hand rinse in preventing transfer of aerobic gram-negative bacilli to urinary catheters via transient hand colonization acquired from direct patient contact. Glove juice recovery of gram-negative bacteria was considered transient colonization; catheter recovery was considered transfer colonization. DESIGN: The contact source for gram-negative bacteria was a single "high burden" groin skin carrier (greater than or equal to 10(4)/ml cup scrub fluid). Using a two-period cross-over design, 6 h

Prevention of dryness and eczema of the hands of hospital staff by emulsion cleansing instead of washing with soap.

The acceptability of two handwashing/cleansing methods, cleansing with emulsion or washing with liquid soap and water, were compared in a randomized, prospective long-term study. Thirty-eight members of hospital staff who had a history of hand skin dryness or eczema and a need for frequent handwashing used either liquid soap or emulsion for handwashing or cleansing. The clinical assessment of skin condition was done blindly by the same dermatologist at the onset of the study, and after 2 and 4 months with a predetermined scoring system. The median skin dryness index of persons in the liquid so

Skin irritancy of commercially available soap and detergent bars as measured by water vapour loss.

The irritancy on human skin of 13 toilet soaps was assessed by means of water vapour loss measurements. The tests were performed on healthy skin of the volar side of the forearm in a panel of 33 subjects. The exposures were done using the Finn chamber method. Skin water vapour loss was measured by means of an "evaporimeter". All soaps--tested in 2 g/v% solutions--were found to lead to a statistically significant increase of water vapour loss as compared to the control sites. Synthetic detergents based on alkyl sulfates, particularly Lactacyd, generally had a relatively greater effect on skin v

Spontaneous Pneumomediastinum Following Ingestion of Bleach, Dish Soap, and Laundry Detergent and Inhalation of Methamphetamines.

INTRODUCTION: Spontaneous pneumomediastinum (SPM) is a rare condition defined by the presence of air in the mediastinum in patients without an observable traumatic cause. Pneumomediastinum is a somewhat rare condition that occurs 1 in every 25 000 individuals, aged 5 to 34 years old, with 76% occurring in men. Pneumomediastinum can further be divided into 2 categories: SPM and traumatic pneumomediastinum. Traumatic pneumomediastinum is most commonly seen with blunt force trauma or any iatrogenic procedures to the chest wall. Spontaneous pneumomediastinum frequently occurs due to air leaking th
