Biology, Epidemiology, Clinical Aspects of Hepatocellular Carcinoma and the Role of Sorafenib.

Sorafenib is a small molecular inhibitor of intracellular tyrosine and serine/threonine protein kinases (VEGFR, PDGFR, CRAF and BRAF), and is thought also to induce autophagy, a chief mechanism influencing tumor growth. Sorafenib shows efficacy in the management of non-resectable hepatocellular carcinoma (HCC), which is refractory to other chemotherapeutic drugs. HCC represents a major end point of chronic liver diseases and the third leading cause of cancer-related death. In HCC patients Sorafenib increases overall survival compared to placebo. The most common chronic liver disease affecting

Carotid and brachiocephalic arteries stenosis with long term use of sorafenib.

The risk associated with arterial thromboembolism (ATE) increases with the presence of anti-vascular endothelial growth factor (VEGF). We are reporting a case of transient ischemic attack (TIA) due to stenosis of the carotid and brachiocephalic arteries following long-term treatment with sorafenib for renal cell carcinoma (RCC). The patient is a non-smoker with no known comorbidities and had no history of cardiovascular disease. The patient underwent a right endarterectomy with angioplasty, aortic arch, and brachiocephalic artery angiogram with a stent placed in the brachiocephalic artery.

Splenic infarction associated with sorafenib use in a hepatocellular carcinoma patient.

Sorafenib, a multitargeted tyrosine kinase inhibitor, has been shown to improve survival in patients with advanced hepatocellular carcinoma (HCC). As the clinical use of sorafenib increases, many adverse effects have been reported, such as hand-foot skin reaction, diarrhea, anorexia, asthenia, alopecia, weight loss, hypertension and arterial thromboembolism. However, there are no prior reports of splenic infarction as an adverse effect of sorafenib. Here, a case of splenic infarction in a patient with HCC who was treated with sorafenib is reported. The patient had no other predisposing factors

Mutant FLT3: a direct target of sorafenib in acute myelogenous leukemia.

BACKGROUND: Internal tandem duplication (ITD) mutations in the juxtamembrane domain-coding sequence of the Fms-like tyrosine kinase 3 (FLT3) gene have been identified in 30% of acute myeloid leukemia (AML) patients and are associated with a poor prognosis. The kinase inhibitor sorafenib induces growth arrest and apoptosis at much lower concentrations in AML cell lines that harbor FLT3-ITD mutations than in AML cell lines with wild-type FLT3. METHODS: The antileukemic activity of sorafenib was investigated in isogenic murine Ba/F3 AML cell lines that expressed mutant (ITD, D835G, and D835Y) or

Sorafenib-loaded porphyrin-phospholipid liposomes for enhanced chemophototherapy in hepatocellular carcinoma.

Sorafenib (SF) remains a frontline chemotherapeutic agent for treating hepatocellular carcinoma (HCC). However, its clinical utility is severely hampered by systemic toxicity and the rapid emergence of drug resistance. To overcome these challenges, we developed a liposomal nanoplatform co-encapsulating SF and a porphyrin-phospholipid conjugate (2HPoP) for combined chemo-photodynamic therapy. The synthesized SF-loaded 2HPoP liposomes (2HPoP lip@SF) exhibited favorable physicochemical characteristics, including high encapsulation efficiency, colloidal stability, and prolonged storage stability.

Severe Reversible Heart Failure Enhanced by Sorafenib Treatment for Hepatocellular Carcinoma.

The multitarget oral tyrosine kinase inhibitor sorafenib is an effective first-line treatment option in unresectable hepatocellular carcinoma. Through its mechanism of action, it has been associated with cardiotoxicity, mainly hypertension, which is usually low-grade and well-managed with behavioral changes and antihypertensor treatment adjustment, if needed. Acute, symptomatic heart failure is rarely described. We present the case of a patient with priors of arterial hypertension, dyslipidemia, type 2 diabetes mellitus, and non-alcoholic steatohepatitis-related cirrhosis of the liver, with th

Sorafenib Alleviates Inflammatory Signaling of Tumor Microenvironment in Precancerous Lung Injuries.

According to population-based studies, lung cancer is the prominent reason for cancer-related mortality worldwide in males and is also rising in females at an alarming rate. Sorafenib (SOR), which is approved for the treatment of hepatocellular carcinoma and renal cell carcinoma, is a multitargeted protein kinase inhibitor. Additionally, SOR is the subject of interest for preclinical and clinical trials in lung cancer. This study was designed to assess in vivo the possible effects of sorafenib (SOR) in diethylnitrosamine (DEN)-induced lung carcinogenesis and examine its probable mechanisms of

Ivermectin synergizes sorafenib in hepatocellular carcinoma via targeting multiple oncogenic pathways.

Источник пометил публикацию как отозванную

Advanced hepatocellular carcinoma (HCC) results in generally poor clinical outcomes and necessitates better therapeutic strategies. Ivermectin, which is an existing anti-parasitic drug, has been recently identified as a novel anti-cancer drug. In line with previous efforts, this work demonstrates the translational potential of ivermectin to treat advanced HCC. We demonstrated that ivermectin at clinically relevant concentrations was active against growth and survival in multiple HCC cell lines. We showed that ivermectin had the potential to inhibit metastasis and target HCC stem cell functions

Orlistat Resensitizes Sorafenib-Resistance in Hepatocellular Carcinoma Cells through Modulating Metabolism.

Sorafenib is one of the options for advanced hepatocellular carcinoma treatment and has been shown to extend median overall survival. However, sorafenib resistance often develops a few months after treatment. Hence, developing various strategies to overcome sorafenib resistance and understand the possible mechanisms is urgently needed. We first established sorafenib-resistant hepatocellular carcinoma (HCC) cells. Then, we found that sorafenib-resistant Huh7 cells (Huh7/SR) exhibit higher glucose uptakes and express elevated fatty acid synthesis and glucose metabolism-related proteins than thei

Fulminant Vascular and Cardiac Toxicity Associated with Tyrosine Kinase Inhibitor Sorafenib.

The use of vascular endothelial growth factor inhibitors such as sorafenib is limited by a risk of severe cardiovascular toxicity. A 28-year-old man with acute myeloid leukemia treated with prednisone, tacrolimus, and sorafenib following stem cell transplantation presented with severe bilateral lower extremity claudication. The patient was discharged against medical advice prior to finalizing a cardiovascular evaluation, but returned 1 week later with signs suggestive of septic shock. Laboratory tests revealed troponin I of 12.63 ng/mL, BNP of 1690 pg/mL, and negative infectious workup. Electr

Transcatheter arterial chemoembolization after stopping sorafenib therapy for advanced hepatocellular carcinoma.

Targeted therapy is currently the standard treatment for advanced hepatocellular carcinoma (HCC), but an effective treatment after the discontinuation of sorafenib therapy remains uncertain. We aim to investigate the survival benefits of transcatheter arterial chemoembolization (TACE) after stopping sorafenib therapy. We retrospectively analyzed all patients with advanced HCC, who had received palliative TACE after terminating sorafenib therapy, from January 2008 to June 2016. Patients who were in the terminal stage (Child-Pugh class C or performance status 3-4), who received a liver transplan

Breast cancer resistance protein and P-glycoprotein limit sorafenib brain accumulation.

Sorafenib is a second-generation, orally active multikinase inhibitor that is approved for the treatment of patients with advanced renal cell carcinoma and patients with unresectable hepatocellular carcinoma. We studied active transport of sorafenib in MDCK-II cells expressing human P-glycoprotein (P-gp/ABCB1) or ABCG2 (breast cancer resistance protein) or murine Abcg2. Sorafenib was moderately transported by P-gp and more efficiently by ABCG2 and Abcg2. Because sorafenib is taken orally, we orally administered sorafenib to wild-type, Abcb1a/1b(-/-), Abcg2(-/-), and Abcb1a/1b;Abcg2(-/-) mice,
