New tracheal sound feature for apnoea analysis.

Sleep apnoea syndrome is common in the general population and is currently underdiagnosed. The aim of the present work was to develop a new tracheal sound feature for separation of apnoea events from non-apnoea time. Ten overnight recordings from apnoea patients containing 1,107 visually scored apnoea events totalling 7 h in duration and 72 h of non-apnoea time were included in the study. The feature was designed to describe the local spectral content of the sound signal. The median, maximum and mean smoothing of different time scales were compared in the feature extraction. The feature was de

A methodological study of hemilaryngeal phonation.

An excised hemilarynx setup was developed. The phonatory characteristics of nine excised canine larynges were examined. The left vocal fold of each larynx was then removed and substituted with a vertical plexiglass plate. The larynges were phonated again. Recordings were made of phonation threshold pressure, sound pressure level, average glottal flow, fundamental frequency, and amplitude of vocal fold vibration as observed with a video stroboscope. Measurements were made over a range of subglottal pressures. For the hemilarynx, simultaneous recordings of intraglottal pressure and vocal fold co

Neural response telemetry and auditory/nonauditory sensations in 15 recipients of auditory brainstem implants.

Auditory brainstem implants (ABIs) provide a means of restoring some hearing sensations to individuals with neurofibromatosis type 2 (NF2) who are deaf after vestibular schwannoma removal. In this study, neural response telemetry (NRT) was used to record electrically evoked neuronal activity near the ABI electrode array in 15 such subjects. Our interest was to investigate whether NRT recordings from the brainstem might be useful in implanting or programming ABIs. We therefore sought relationships between postoperative NRT recordings and the sensations reported by the subjects in response to th

Smartphone Recordings are Comparable to "Gold Standard" Recordings for Acoustic Measurements of Voice.

PURPOSE: The purpose of this study was to assess the relationship and comparability of cepstral and spectral measures of voice obtained from a high-cost "flat" microphone and precision sound level meter (SLM) vs. high-end and entry level models of commonly and currently used smartphones (iPhone i12 and iSE; Samsung s21 and s9 smartphones). Device comparisons were also conducted in different settings (sound-treated booth vs. typical "quiet" office room) and at different mouth-to-microphone distances (15 and 30 cm). METHODS: The SLM and smartphone devices were used to record a series of speech

L-type calcium channels refine the neural population code of sound level.

The coding of sound level by ensembles of neurons improves the accuracy with which listeners identify how loud a sound is. In the auditory system, the rate at which neurons fire in response to changes in sound level is shaped by local networks. Voltage-gated conductances alter local output by regulating neuronal firing, but their role in modulating responses to sound level is unclear. We tested the effects of L-type calcium channels (CaL: CaV1.1-1.4) on sound-level coding in the central nucleus of the inferior colliculus (ICC) in the auditory midbrain. We characterized the contribution of CaL

Exploratory Assessment of Pulsed-Wave Doppler Representations of Lung Sounds Using Deep Learning: An In-Vitro Phantom Study.

The increasing availability of portable ultrasound systems motivates exploration of novel approaches to respiratory signal assessment. In this in-vitro study, we investigate whether pulsed-wave (PW) Doppler ultrasound can capture structured spectral patterns from replayed lung sound recordings. Digitized respiratory sounds were replayed through a tissue-mimicking ultrasound phantom, generating 1,478 PW Doppler spectral images from recordings associated with healthy subjects and several externally labeled disease categories. Exploratory classification experiments using a ResNet-18 architecture

Using High-Speed Nasopharyngoscopy to Quantify the Bubbling Above the Velopharyngeal Valve in Cases of Nasal Rustle.

OBJECTIVE: The loud and severely distorting form of audible nasal emission (commonly known as nasal turbulence or nasal rustle) typically occurs with a small velopharyngeal opening during production of pressure-sensitive consonants. The purpose of this study was to determine whether bubbling of the secretions, which commonly occurs on the superior aspect of the velopharyngeal port when there is a small opening, is a periodic process that can generate sound in the nasal cavity. PARTICIPANTS: Ten pediatric patients were included in the study. All participants had normal articulation and resonan

Noninvasive technique for estimating subglottic pressure and laryngeal efficiency.

The clinical assessment of aerodynamic parameters is important in the physiology and pathophysiology of laryngeal function. Vocal efficiency is among these objective measurements and can be calculated from simultaneous recordings of subglottic pressure, airflow, and sound intensity. Modern techniques allow us to accurately determine sound intensity and airflow. However, methods to determine subglottic pressure are either invasive or laborious. We have evaluated a noninvasive, indirect method to determine subglottic pressure by using a technique based on the interruption of transglottic airflow

Systolic ejection click versus split first heart sound: Are our ears deceiving us?

OBJECTIVE: Bicuspid aortic valve (BAV) disease is associated with potential lifetime complications, but auscultation of a BAV click is commonly missed or mistaken for a benign split first heart sound. Our objective was to determine whether pediatric cardiologists could reliably distinguish between BAV clicks and benign split first heart sounds. DESIGN: Quality evaluation project using de-identified recordings from an outpatient pediatric cardiology clinic. OUTCOME MEASURES: Twenty-one cardiologists listened to five de-identified recordings of pediatric heart sounds (three with BAV clicks, tw

Operator auditory perception and spectral quantification of umbilical artery Doppler ultrasound signals.

OBJECTIVE: An experienced sonographer can by listening to the Doppler audio signals perceive various timbres that distinguish different types of umbilical artery flow despite an unchanged pulsatility index (PI). Our aim was to develop an objective measure of the Doppler audio signals recorded from fetoplacental circulation in a sheep model. METHODS: Various degrees of pathological flow velocity waveforms in the umbilical artery, similar to those in human complicated pregnancies, were induced by microsphere embolization of the placental bed (embolization model, 7 lamb fetuses, 370 Doppler reco

The Ca2+ channel subunit beta2 regulates Ca2+ channel abundance and function in inner hair cells and is required for hearing.

Hearing relies on Ca(2+) influx-triggered exocytosis in cochlear inner hair cells (IHCs). Here we studied the role of the Ca(2+) channel subunit Ca(V)beta(2) in hearing. Of the Ca(V)beta(1-4) mRNAs, IHCs predominantly contained Ca(V)beta(2). Hearing was severely impaired in mice lacking Ca(V)beta(2) in extracardiac tissues (Ca(V)beta(2)(-/-)). This involved deficits in cochlear amplification and sound encoding. Otoacoustic emissions were reduced or absent in Ca(V)beta(2)(-/-) mice, which showed strongly elevated auditory thresholds in single neuron recordings and auditory brainstem response me

Computerized phonoenterography: the clinical investigation of a new system.

We assessed bowel sounds in controls and patients with acute abdominal conditions using a new computer-aided sound analysis system (C.A.S.A.S.). Sixty-three controls and 61 patients with an acute abdomen had 10-min recordings of bowel sounds in a silent environment with computer analysis of the digitized acoustic signal. Mean bowel sound length was 20 +/- 1.3 ms in controls, 32 +/- 30 ms in cholecystitis, and 37 +/- 35 ms in intestinal obstruction (p < 0.05; mean +/- sd, Mann-Whitney U test). Similar significant differences existed between controls and acute abdomens in the number of sounds ge

Maternal vocalisations and other sounds in the fetal lamb's sound environment.

In a first experiment a miniaturised radio hydrophone was implanted inside the amniotic sac in three pregnant ewes and recordings were made from 3 weeks before the lambs were born until they had emerged; these recordings showed that the sound of the mother's voice was slightly louder when picked up by the hydrophone inside the amniotic sac than when picked up by a microphone beside her flank and that sounds recorded during labour included long and loud low frequency sounds associated with contractions, and an increase, compared with before labour, in the incidence of sounds produced by the mat

Ca2+-binding protein 2 inhibits Ca2+-channel inactivation in mouse inner hair cells.

Ca2+-binding protein 2 (CaBP2) inhibits the inactivation of heterologously expressed voltage-gated Ca2+ channels of type 1.3 (CaV1.3) and is defective in human autosomal-recessive deafness 93 (DFNB93). Here, we report a newly identified mutation in CABP2 that causes a moderate hearing impairment likely via nonsense-mediated decay of CABP2-mRNA. To study the mechanism of hearing impairment resulting from CABP2 loss of function, we disrupted Cabp2 in mice (Cabp2LacZ/LacZ ). CaBP2 was expressed by cochlear hair cells, preferentially in inner hair cells (IHCs), and was lacking from the postsynapti

Notched noise reveals differential improvement in the neural representation of the sound envelope.

Precise temporal coding in auditory nerve fibers is crucial for sound localization and listening in noise. However, at higher sound levels, typical of everyday listening situations, temporal coding to the stimulus envelope is poor in fibers of the on-frequency channel, i.e., those tuned to the carrier stimulus. We predict that changes in cochlear gain improve temporal coding of the stimulus envelope differentially across frequency channels. Both auditory nerve fiber recordings (in Mongolian gerbils of either sex) and human psychophysics confirm that weak temporal coding at higher levels is imp

Recurrent vs Non-Recurrent Convolutional Neural Networks for Heart Sound Classification.

Convolutional Neural Network (CNN) has been widely proposed for different tasks of heart sound analysis. This paper presents the results of a novel study on the performance of a conventional CNN in comparison to the different architectures of recurrent neural networks combined with CNN for the classification task of abnormal-normal heart sounds. The study considers various combinations of parallel and cascaded integration of CNN with Gated Recurrent Network (GRN) as well as Long- Short Term Memory (LSTM) and explores the accuracy and sensitivity of each integration independently, using the Phy

Feasibility of an electronic stethoscope system for monitoring neonatal bowel sounds.

OBJECTIVE: Bowel dysfunction remains a major problem in neonates. Traditional auscultation of bowel sounds as a diagnostic aid in neonatal gastrointestinal complications is limited by skill and inability to document and reassess. Consequently, we built a unique prototype to investigate the feasibility of an electronic monitoring system for continuous assessment of bowel sounds. METHOD: We attained approval by the Institutional Review Boards for the investigational study to test our system. The system incorporated a prototype stethoscope head with a built-in microphone connected to a digital r

Clinical and electrophysiological findings in the Tullio phenomenon.

A 55 year old female with idiopathic Tullio phenomenon is presented. Binocular, scleral search eye coil recordings demonstrated a predominantly torsional left-beating and vertical down-beating nystagmus in response to sound intensities over 100 dB HL to the left ear, increasing in amplitude and slow phase velocity with sound intensity and removal of visual fixation. The vertical ocular movement was conjugate, i.e. without skew deviation.
