BACKGROUND: The Eustachian tube and sphenoid spine have been previously described as landmarks for endonasal surgical identification of the most distal segment of the parapharyngeal internal carotid artery (PhICA). However, the intervening space between the sphenoid spine and PhICA allows for error during exposure of the artery. In the present study, we have characterized endoscopic endonasal transmasticator exposure of the PhICA using the sphenoid spine, vaginal process of the tympanic bone, and the "tympanic crest" as useful anatomical landmarks.
METHODS: Endonasal dissection was performed

The authors report a case of isolated congenital orbital myofibroma causing sphenoid dysplasia and presenting as global restriction of extraocular motility and ptosis in a neonate. Sphenoid wing dysplasia is most commonly associated neurofibromatosis 1 but this patient had no evidence of neurofibromatosis on clinical examination and genetic testing. Congenital orbital myofibroma can have secondary effects on bone and likely the lesion was present early in development leading to aplasia of the sphenoid bone. To the best of the authors' knowledge, this is the first reported case of sphenoid wing

This study reported a case of an arachnoid cyst of the sphenoid bone causing orbital signs and symptoms in a 58-year-old man with progressive proptosis and nonspecific discomfort in the OS. Orbital MRI showed a 3-cm homogeneous cyst within the left greater wing of the sphenoid bone. To the best of our knowledge, this is the first report of an intradiploic arachnoid cyst in the sphenoid bone with atypical radiological features, causing clinical symptoms, and managed through an eyelid approach, achieving a complete resolution with no complications.

The sphenoid bone is a complex structure in terms of its embryological origin. At birth, the sphenoid sinus is nonpneumatized. Arrested pneumatization of the sphenoid sinus is considered a normal anatomic variant but may be mistaken for disease in imaging studies. We report 2 cases of arrested pneumatization of the sphenoid sinus, a normal variant commonly misdiagnosed as a serious disease of the skull base. A 29-year-old man with a complaint of dizziness visited a local clinic for assessment. Computed tomography (CT) of the paranasal sinuses (PNS) showed a noneroding, nonexpansile, and nonhom

We present a case of a 29-year-old female complaining of right-sided watery nasal discharge. Radiological investigations identified an intrasphenoidal meningocele. The origin of the meningocele was pinpointed to the right parasellar region and was confirmed surgically. The parasellar bony defect appeared to be due to persistence of the lateral craniopharyngeal canal (Sternberg's canal). Therefore, we assume a congenital origin for the intrasphenoidal meningocele found in the patient. Acquired bony defects of the sphenoid sinus are unlikely at the fusion planes of the different sphenoid bone co

Encephaloceles are considered rare with an approximate incidence of 1 in 35,000, and sphenoid encephaloceles are even more uncommon.2 Two types of sphenoid encephaloceles exist: medial perisellar encephaloceles, and lateral sphenoidal encephaloceles. Surgical correction of the lateral sphenoid recess encephalocele is achieved via one of two endoscopic approaches: extended sphenoidotomy or transpterygopalatine. Extended sphenoidotomy is preferred if the angle between the access door and lateral extension of bone defect is greater than 35°1. Otherwise, the transpterygopalatine approach is used.

The aim of this investigation was the study of the development changes of the osseous nasal septum and para-sphenoid systems in adult pigs, domestic cattle and the comparison with the control skull-group of 10 deer and 15 hares. The methodology was described elsewhere. In paired-hoof mammals was found that the alar bone is biomechanically composed system. It represents four segments: three of septum bone and one as subnasal bone. In examined mammals the degree of integration of sphenoid septum and alar bone ones was various. In fishes, amphibians, birds and paired-hoof mammals there are the ax

Skull base pneumatization is anatomically variable and clinically relevant to temporal bone and transsphenoidal surgical corridors, but manual volumetric segmentation is time-consuming. This retrospective pilot study evaluated a clinician-guided deep learning workflow for mastoid and sphenoid sinus compartment segmentation on bone computed tomography. Images were curated and annotated in 3D Slicer using MONAI Label and separate three-dimensional SegResNet models. The mastoid development dataset comprised 122 side-cases, with 28 reserved side-cases; the sphenoid dataset comprised 117 developmen

Morning glory syndrome is a congenital anomaly of the optic disc in which the disc is enlarged and excavated, with white glial tissue in the center. A case is presented of morning glory syndrome associated with sphenoid encephalocele, median cleft lip, and agenesis of the corpus callosum. A 22-day-old boy was referred to the Wakayama Medical College Hospital for management of dyspnea due to a soft-tissue mass in the oral cavity. Magnetic resonance imaging revealed a mass extending through a bone defect in the sphenoid region and into the oral cavity. Surgical repair was attempted through a bif

BACKGROUND: Early diagnosis of lymphoma involving the central nervous system is sometimes difficult but emergent to avoid the delay of therapeutic initiation. Pituitary insufficiencies are usually associated with lymphoma in the pituitary gland. There have been no cases of lymphoma originating from extra pituitary gland with hypopituitarism that simultaneously presenting unilateral upper cranial nerve palsies and ophthalmalgia. These symptoms are mostly caused by neoplastic involvement of the skull base or benign diseases such as Tolosa-Hunt syndrome (THS). We report a case of lymphoma with un

Plasmacytoma is a rare plasma cell neoplasm. Whether solitary or associated with multiple myeloma (MM), it rarely involves the skull base, particularly the sphenoid bone. We present a unique case of sphenoid bone plasmacytoma secondary to MM, highlighting diagnostic and therapeutic challenges. A 56-year-old female presented with headaches, vomiting, epistaxis, and cranial nerve deficits. Cerebral imaging revealed a 65-mm tumor infiltrating the sphenoid bone and adjacent structures. Subtotal resection was performed using an endoscopic nasal approach. Histopathology revealed plasmacytoma, and di

The aim of these investigations was differentiation and systematization of elements of osseous medial palate and attempt to discover of spheno-orbital bone homologue in fish (os orbito-sphenoideum). 50 adults fish heads of carps, perch , tench , common pikes and trout were examined. The methodology was described elsewhere. It was stated that in examined animals the pair premaxilla was divided into three parts: corps, palate and frontal process. The para-sphenoid bone was also a double biomechanical system, composed of septum homologue and transverse lamina , the same as in other vertebrates. A

The bony septum ossifies from cranial to caudal and from ventral to dorsal, thereby forming the perpendicular plate of ethmoid bone and vomer. A small strip of cartilage from the cartilaginous septum remains between these parts, the so-called sphenoidal process of the cartilaginous septum. This sphenoidal process is usually seen at the top of a deviated nasal septum during septoplasty. This study evaluated the clinical significance of the sphenoidal process of the cartilaginous septum as a cause of nasal septal deviation. We studied 37 patients with septal deviation who underwent septoplasty.

INTRODUCTION: Benign diseases of the sphenoid sinus can be classified as intrinsic, originating from the mucosa and sinus bone walls, and extrinsic, arising from adjacent structures. We analyze the CT features of both intrinsic and extrinsic benign sphenoid conditions and their diagnostic role and impact on treatment planning.
MATERIAL AND METHODS: We reviewed the CT scans of 75 patients with benign sphenoid sinus conditions from our 1993-96 series. The conditions were diagnosed at histology of the surgical specimen or from clinical findings and patient's history. Both axial scans, parallel t

The sphenoid sinus (SS) is one of the four paired paranasal sinuses (PNSs) within the sphenoid bone body. Isolated pathologies of sphenoid sinus are uncommon. The patient may have various presentations like headache, nasal discharge, post nasal drip, or non-specific symptoms. Although rare, potential complications of sphenoidal sinusitis can range from mucocele to skull base or cavernous sinus involvement, or cranial neuropathy. Primary tumors are rare and adjoining tumors secondarily invading the sphenoid sinus is seen. Multidetector computed tomography (CT) scan and magnetic resonance imagin

The computed tomographic (CT) scans of the head of 490 blunt trauma victims were reviewed to establish the frequency of sphenoid fractures. There were 111 patients with craniofacial fractures, 78 of whom had fractures of the sphenoid bone. In 51 of the 78 patients these were associated with complex facial fractures, and in the 27 remaining patients they represented primarily fractures of the skull base. The number of sphenoid fractures not only exceeded those that involved the other bones of the base of the skull, including the temporal bone, but also exceeded the number of fractures of the cr

BACKGROUND: Cholesterol granulomas typically arise in the petrous apex of the temporal bone, and involvement of the sphenoid wing has not been reported. This case is notable not only for its rare sphenoid wing origin but also for the ability to document its growth dynamics over long-term imaging follow-up.
OBSERVATIONS: A 35-year-old man was found to have a mass along the lateral aspect of the left sphenoid wing during evaluation after minor head trauma. Long-term radiological follow-up demonstrated gradual enlargement of the lesion, prompting resection. Histopathological examination revealed

Infection of the sphenoid sinus is an uncommon clinical entity frequently misdiagnosed because of its vague symptoms and paucity of clinical findings. We report a case of sphenoid sinus mucocele complicated by retro-orbital pain and ipsilateral optic atrophy. CT scan and MR image revealed a cystic mass lesion of the sphenoid sinus. Bone around the lesion was eroded. The patient underwent transnasal drainage of the cystic mass. We conclude that transnasal drainage is the safest method of treatment of an infectious lesion in and around the sphenoid sinus.

Epidermoids are blastomatous malformations of abnormal epithelial proliferation. Intradiploic epidermoids are very rare lesions that may occur in all flat bones of the calvarium and in the paranasal sinuses, maxilla, temporal bone, and sphenoid bone. This article describes the unique case of an intradiploic epidermoid tumor located in the greater sphenoid wing. Theories of pathogenesis, clinical behavior, diagnosis, and treatment of diploic epidermoids are reviewed.

Orbital manifestations occur in less than 1% of patients with neurofibromatosis type 1 (NF1). These manifestations are frequently associated with sphenoid wing dysplasia. The typical radiologic feature is partial or total loss of the greater wing of the sphenoid bone, which leads to herniation of the temporal lobe through the orbital cavity resulting in proptosis and pulsating exophthalmos. Traditional reconstruction of this bone defect involves split bone grafting or titanium mesh. However, these techniques have some limitations due to bone resorption and infection risk. We report the use of
