Refractory primary and secondary headache disorders that dramatically responded to combined treatment of ultrasound-guided percutaneous suprazygomatic pterygopalatine ganglion blocks and non-invasive vagus nerve stimulation: a case series.

In 1981, Devoghel achieved an 85.6% success rate in treating patients with treatment-refractory cluster headaches with alcoholization of the pterygopalatine ganglion (PPG) via the percutaneous suprazygomatic approach. Devoghel's study led to the theory that interrupting the parasympathetic pathway by blocking its transduction at the PPG could prevent or treat symptoms related to primary headache disorders (PHDs). Furthermore, non-invasive vagus nerve stimulation (nVNS) has proven to treat PHDs and has been approved by national regulatory bodies to treat, among others, cluster headaches and mig

Thermal radiofrequency of the sphenopalatine ganglion in persistent idiopathic facial pain using a suprazygomatic approach: Case report.

INTRODUCTION: The sphenopalatine ganglion (SPG) is a key structure in the pathophysiology of various craniofacial pain syndromes with autonomic manifestations, integrating parasympathetic, sympathetic, and trigeminal fibers within the trigemino-autonomic reflex. Modulation of these pathways through nerve block or radiofrequency has shown effectiveness in managing pain and associated autonomic symptoms. However, no previous reports have described the use of a suprazygomatic approach for thermal radiofrequency of the SPG. CASE REPORT: We present the case of a 70-year-old man with a one-year his

Ultrasound-Guided Nerve Blocks in the Head and Neck for Chronic Pain Management: The Anatomy, Sonoanatomy, and Procedure.

BACKGROUND: Ultrasound guided nerve blocks have become a popular tool in the armamentarium for pain physicians because of its advantages over fluoroscopy by offering portable, radiation-free and real-time imaging. But ultrasound guided procedures require training and practice to gain the expertise. There is a scarcity of review articles describing ultrasound guided injections techniques for nerve blocks of the head and neck. OBJECTIVE: To elucidate the anatomy, sonoanatomy, indications, techniques, side effects and complications for the most frequently utilized nerve blocks of the head and ne

Intracranial catecholamine secreting paragangliomas.

Three intracranial catecholamine-secreting paragangliomas are described. They involved a glomus jugulare, a sphenopalatine ganglion and the clivus and upper cervical spine respectively. The extent of the tumours was shown by CT and MRI. They were all highly vascular with a substantial blood supply from systemic arteries which was subjected to particulate embolisation, followed in two cases by surgery. The importance of studying any tumour which could possibly be a paraganglioma for hormonal and, especially, for catecholamine secretion prior to any invasive procedure, including angiography and

Clinic-based Procedures for Headache.

PURPOSE OF REVIEW: Headache disorders are common and disabling, and many therapies that are effective and safe are procedural. RECENT FINDINGS: After pivotal clinical trials, onabotulinumtoxinA has become an established preventive therapy for chronic migraine; it is better tolerated than many other treatments and may be useful for other headache disorders. Peripheral nerve blocks, especially greater occipital nerve blocks, have amassed evidence from randomized trials in the acute and short-term preventive treatment of migraine and cluster headache. Trigger point injections and sphenopalatine

Intranasal Lidocaine Spray for Trigeminal Autonomic Cephalgia, Simple and Effective Treatment Modality: A Case Series of Five Patients.

BACKGROUND: Sphenopalatine ganglion block (SPGB) is a potential treatment option for trigeminal autonomic cephalgia (TAC) performed fluoroscopically or endoscopically. We explored the role of noninvasive SPGB via intranasal application of lidocaine (INL) in hemicrania continua (HC) and probable TAC. CASE REPORT: In 4 patients of HC, INL spray decreased baseline acute attacks from Numeric Rating Scale (NRS-11) 8-10/10 to nil acute attacks and baseline continuous NRS-11 from 2-6/10 to 0-2/10 in 3 months. In patient of probable TAC, multiple acute attacks a day of NRS-11 10/10 decreased to NRS-1

Trigeminal autonomic cephalalgias: beyond the conventional treatments.

The trigeminal autonomic cephalalgias include cluster headache, paroxysmal hemicrania, short-lasting unilateral neuralgiform headache attacks, and hemicrania continua. While the majority responds to conventional pharmacological treatments, a small but significant proportion of patients are intractable to these treatments. In these cases, alternative choices for these patients include oral and injectable drugs, lesional or resectional surgery, and neurostimulation. The evidence base for conventional treatments is limited, and the evidence for those used beyond convention is more so. At present,

[New treatments for cluster headache].

Subcutaneous injection and nasal spray of sumatriptan are known to be useful for the treatment of acute phase of cluster headache. Although nasal spry of zolmitriptan is not available in Japan, it is also reported to be effective for the acute phase of cluster headache. In addition, high flow oxygen is also reported to abort the attack of cluster headache. For preventive treatment of cluster headache, calcium channel blocker (verapamil) and steroids are recommended. In addition to these medications, some of refractory chronic cluster headache patients are reported to be respond to the occipita

Minimally-invasive pain management techniques in palliative care.

Pain is a common source of suffering for seriously ill patients. Typical first-line treatments consist of lifestyle modifications and medication therapy, including opioids. However, medical treatments often fail or are associated with limiting systemic toxicities, and more targeted interventional approaches are necessary. Herein, we present options for minimally invasive techniques for the alleviation of pain in palliative patients from a head-to-toe approach, with a focus on emerging therapies and advanced techniques. Head and neck: image-guided interventions targeted to sympathetic ganglia o

Open label experience of repeated OnabotulinumtoxinA injections towards the sphenopalatine ganglion in patients with chronic cluster headache and chronic migraine.

BACKGROUND: A novel technique for injection of OnabotulinumtoxinA (BTA) towards the sphenopalatine ganglion (SPG) has shown promise in refractory chronic migraine (CM) and chronic cluster headache (CCH). Open label safety and efficacy data are presented here. METHODS: Patients with refractory CM or CCH who had received at least one injection and completed headache diaries were included. Efficacy was defined as ≥50% reduction in moderate-to-severe headache days for CM, or ≥50% reduction in attack frequency for CCH, at weeks five to eight. RESULTS: Over 261 injections, there were 123 adverse e

Chronic Bilateral Sphenopalatine Ganglion Stimulation for Intractable Bilateral Chronic Cluster Headache: A Case Report.

UNLABELLED: A cluster headache (CH) is an excruciating pain centered on the periorbital region and is accompanied by autonomic symptoms. Despite the best currently available medical therapy, many patients still suffer from these headaches. Some patients also experience CH with side shift of attacks, which makes treatment more difficult. In light of the pathophysiological mechanism of CH, the sphenopalatine ganglion (SPG) is a promising therapeutic target for its management. SPG ablation and non-ablation techniques have been the preferred treatment for CH in recent decades. However, few article

Paroxysmal hemicrania: an update.

Paroxysmal hemicrania (PH) is an underreported and underdiagnosed primary headache disorder. It usually begins in the third or fourth decade of life. The recent observations indicate that it is equally prevalent in both males and females. PH is characterized by severe, strictly unilateral head pain attacks that occur in association with ipsilateral autonomic features. The attacks in PH are shorter and more frequent compared with cluster headache (CH) but otherwise PH and CH have similar clinical features. The hallmark of PH is the absolute cessation of the headache with indomethacin. However,

A double-blind, placebo-controlled study of repetitive transnasal sphenopalatine ganglion blockade with tx360(®) as acute treatment for chronic migraine.

OBJECTIVE: To determine if repetitive sphenopalatine ganglion (SPG) blocks with 0.5% bupivacaine delivered through the Tx360(®) are superior in reducing pain associated with chronic migraine (CM) compared with saline. BACKGROUND: The SPG is a small concentrated structure of neuronal tissue that resides within the pterygopalatine fossa (PPF) in close proximity to the sphenopalatine foramen and is innervated by the maxillary division of the trigeminal nerve. From an anatomical and physiological perspective, SPG blockade may be an effective acute and preventative treatment for CM. METHOD: This

Effectiveness of Combined Peripheral Nerve Blocks Versus Isolated Greater Occipital Nerve Block in the Management of Cluster Headache.

INTRODUCTION: Cluster headache (CH) is a primary headache disorder classified under trigeminal autonomic cephalalgias, characterized by severe, disabling pain attacks. In treatment-resistant cases, where pharmacological interventions are insufficient, peripheral nerve blocks have emerged as a promising interventional option. This study aimed to compare the efficacy of isolated greater occipital nerve (GON) block with combined GON, supraorbital nerve (SON), and sphenopalatine ganglion (SPG) blocks in reducing attack frequency, pain intensity, and treatment response in patients with CH.
